Ligamentum flavum hematoma in the rigid thoracic spinal segments: case report.

Ligamentum flavum hematoma, a rare cause of spinal nerve root and canal compression, typically occurs in the mobile lumbar spine segments. A thoracic ligamentum flavum hematoma is extremely rare--only one such case of a thoracolumbar (T11-12) lesion has been reported. The thoracolumbar region with its floating ribs, however, is structurally and biomechanically similar to the lumbar spine and its mobility is greater than the higher thoracic levels. To the best of their knowledge, the authors report the first case of a ligamentum flavum hematoma in the region of the rigid thoracic spinal segment

Complications of percutaneous vertebroplasty and their prevention.

Vertebroplasty is an efficient treatment of vertebral collapses of malignant or benign origin but also carries a risk of complications. Cement extravasation is a frequent occurrence in vertebroplasty. It is well tolerated in the large majority of cases but is also the main source of complications especially nerve root compression in case of cement leakage into the intervertebral foramen and pulmonary embolism of cement complicating venous cement leakage. Rate of these complications are much higher in malignant than in osteoporotic collapses. The risk of neurological complication also increases

Limitations of dorsal transpedicular stabilization in unstable fractures of the lower thoracic and lumbar spine: an analysis of 133 patients.

The optimal treatment of thoracic and lumbar fractures remains controversial. While many authors recommend dorsal instrumentation with an internal fixator, others favour an anterior approach. To evaluate the posterior approach and to identify conditions under which an anterior approach should be preferred, 133 patients with unstable thoracic and lumbar fractures of the spine who underwent dorsal instrumentation with an internal fixator were analyzed. Clinical data were recorded prospectively with respect to fracture type, neurological findings, operative complications, spinal deformation corre

Mild hypothermia, blood loss and complications in elective spinal surgery.

BACKGROUND CONTEXT: Spinal surgery carries risks of incidental spinal cord and nerve root injury. Neuroprotection, to minimize the extent of such injuries, is desirable. However, no neuroprotective strategies have been conclusively validated in nonvascular spinal surgery. Mild hypothermia resulting from general anesthesia is a readily achievable potential neuroprotective strategy. Mild hypothermia, however, has been associated with wound infection, increased operative blood loss and other complications. No previous studies have specifically evaluated whether mild hypothermia is associated with

Safety and efficacy of Isola instrumentation and arthrodesis for adolescent idiopathic scoliosis: two- to 12-year follow-up.

STUDY DESIGN: Retrospective case series including patient outcome assessment. OBJECTIVE: To study the safety and efficacy of Isola instrumentation in comparison with similar series. SUMMARY OF BACKGROUND DATA: Both the technique and technology used in the surgical treatment of adolescent idiopathic scoliosis continue to evolve, the common theme since the 1980s being provision of instrumentation stable and strong enough to eliminate the need for postoperative immobilization. The purpose of this study is to determine the safety and efficacy of a system deliberately integrating hook, wire, and

The effects from lumbar nerve root transection in rats on spinal somatosensory and motor-evoked potentials.

STUDY DESIGN: Spinal somatosensory-evoked potentials (SSEPs), elicited by mixed nerve and dermatomal stimulation, and compound evoked muscle potentials (CMAPs), elicited by lower thoracic interspinous space stimulation, were recorded in rats that underwent single nerve root transection. OBJECTIVES: To investigate and compare the effects of single nerve root transection using various electrophysiological models and to define a monitoring strategy that can easily and accurately predict compromise caused by a single lumbosacral nerve root injury. SUMMARY OF BACKGROUND DATA: Neuromonitoring is a

Safety of supplemental endplate screws in thoracic pedicle hook fixation.

OBJECT: The AO Universal Spine System thoracic pedicle hook design includes a fixation screw that passes obliquely through the inferior facet into the pedicle to engage in the posterior portion of the superior vertebral body endplate. This endplate screw provides additional purchase at the hook-bone interface. To determine the safety of this fixation system the authors reviewed the operative notes, radiographs, and outcomes of patients who underwent placement of endplate screws. METHODS: Thirty-six patients (16 male and 20 female patients) who required posterior thoracic instrumentation for s

[First experience of intraoperative nerve root monitoring with the INS-1-device on the lumbosacral spine].

AIM: The goal of intraoperative nerve root monitoring on the lumbosacral spine is to minimize the risk of nerve root injuries during surgery by reducing the incidence of misplaced pedicle screws. In this study we hypothesized that the INS-1 device (Nuvasive) may provide more accurate information about screw placement. METHOD: The INS-1 device provides a means for intraoperative EMG stimulation through the inside of the pedicles or the implanted screws. The readings from relevant muscles serve as a monitoring technique to detect perforated pedicles and thus can be used to prevent or detect mis

Magnetic resonance image findings and surgical considerations in T1-2 disc herniation.

OBJECTIVE: To report a case of disc herniation at T1-2. CLINICAL PRESENTATION: A 57-year-old man presented with hand weakness, Horner's syndrome, and pain radiating along the medial aspect of one upper extremity. Magnetic resonance imaging demonstrated extruded T1-2 disc herniation with upward herniation of a sequestrated fragment. INTERVENTION: An anterior approach was used to excise the disc, that was compressing the spinal cord and the T1 nerve root. All the patient's symptoms resolved completely, including Horner's syndrome. CONCLUSION: Anterior discectomy may be the simplest and most e

A comparison of biomechanical mechanisms of whiplash injury from rear impacts.

Several hypotheses have been proposed to explain the mechanism of injury in whiplash including, pressure on nerve root ganglia, stretching of facet capsules, or damage to facet articular cartilage. These injury mechanisms have not been directly compared in the same study. A comparison could provide insight into the most likely mechanism of whiplash injury. Twenty eight volunteers underwent rear impacts with head and chest acceleration data collected. The same apparatus was used to test 11 cervico-thoracic human cadaveric spines with an instrumented headform attached. Head acceleration, individ

Regeneration of primary sensory axons into the adult rat spinal cord via a peripheral nerve graft bridging the lumbar dorsal roots to the dorsal column.

This study investigated the feasibility of using a peripheral nerve autograft (NAG) to promote and guide regeneration of sensory axons from the caudal lumbar dorsal roots to the rostral dorsal column following a lower thoracic cordotomy in adult rats. After a left hemicordotomy at the T13 vertebra level and ipsilateral L3 and L4 rhizotomies, a peripheral NAG (peroneal nerve) was connected to the distal roots stumps, then implanted into the left dorsal column 10 mm rostral to hemicordotomy site (n = 12). After surgery, all animals of the experimental group experienced complete anesthesia in the
