Pedicle screw instrumentation for unstable thoracolumbar fractures.

We evaluated the results of short-segment pedicle screw instrumentation in 54 patients with unstable thoracolumbar fractures. Follow-up averaged 25 months (range, 11 to 36 months); 42 patients completed the study. Kyphosis was corrected by an average of 7 degrees at surgery and loss of correction averaged 5 degrees at the end of follow-up. On computed tomography, canal compromise averaged 57% preoperatively and 33% postoperatively. Complications included nerve root irritation due to screw penetration (1/42), screw breakage (2/42), and screw bending (6/42). Solid fusion was achieved in all case

Excision of hemivertebrae and wedge resection in the treatment of congenital scoliosis.

The results of anterior and posterior excision or wedge resection of a hemivertebra and arthrodesis of the spine were reviewed retrospectively for thirty-seven patients. The degree of correction that was obtained and maintained, the balance and alignment of the trunk, changes in pelvic obliquity, and associated complications were evaluated. The average age at the time of the operation was twelve years (range, six months to forty-two years). The average duration of follow-up was six years (range, two to nineteen years). The resection was at the mid-thoracic level in six patients, at the thoraco

Neurological outcome after surgery for thoracolumbar fractures. A retrospective study of 93 consecutive cases, treated with dorsal instrumentation.

Surgical reconstruction and fusion form the treatment of choice for unstable thoracolumbar fractures. It remains difficult, however, to prove that surgical treatment provides an increased potential for neurological recovery. Also, the role of a decompressive laminectomy is still unclear. To address these issues, 93 consecutive cases of thoracolumbar fractures treated with dorsal instrumentation were reviewed. The neurological status at the time of admission and at a mean of 26 months postinjury was graded according to a modified Frankel scale. By using preoperative radiographs and computed tom

[Value of vertebroplasty combined with surgical decompression in the treatment of aggressive spinal angioma. Apropos of 3 cases].

Vertebral hemangiomas can cause difficult-to-treat neurological complications. We report our experience with three such cases. Patients no. 1 and 2 were females aged 64 and 71 years, respectively; patient no. 1 had a two-year history of weakness in the lower limbs and patient no. 2 had a five-month history of back pain. Both these patients had a pyramidal syndrome in the lower limbs. Patient no. 3 was a 61 year old male with a one-year history of left sciatica. Roentgenograms were suggestive of a hemangioma occupying the entire T8 (cases 1 and 2) or L5 (case 3) vertebra. Computed tomography an

Operative spinal endoscopy: stereotopography and surgical possibilities.

The polyprojective microstereotopography of spinal canal structures at the cerebello-spinal, cervical, thoracic, lumbosacral and cauda equina levels on 20 fresh cadavers is presented using flexiscopes 3.7-3.9 mm diameter. This is possible due to the space between spinal cord-vertebral canal which is about 10 mm at all levels. This also allows one to insert the endoscopic tube by posterior or interradicular approach. The subdural and subarachnoid endoscopic examinations have been performed through small foraminotomic openings with resection of the base of the spinous process. The anterior and p

[Clinical value of the posterolateral approach, supplemented with pedicle excision of the arch, to the anterior wall of the spinal canal within the thoracolumbar (Th11-L1) and lumbar spine].

UNLABELLED: The author validates clinically his own lateral approach to the lumbosacral (Th11-L1) and lumbar spine. In this approach an enlargement of the intervertebral foramen is achieved by the partial excision of adjacent pedicles or excision of one or several pedicles. The access to the anterior canal wall is gained in this manner. This surgical approach has been used in 48 patients with neurological injury (spinal cord, conus medullaris, cauda equina or the roots). An active tuberculosis was the causative factor in 12 cases, the sequelae of tuberculous infection in 10 cases, infection in

[Old traumatic lesions of the dorsal and lumbar spine].

The authors present 71 cases of malunion or old traumatic lesions. They result from an unadapted initial surgical or functional treatment. The pain symptoms are in the form of vertebral pain or nerve root pain, often associated with signs of neurological deficit. The authors emphasize the stability or instability of the lesion in order to assess its reducibility. The radiographic exploration (dynamic views, MRI, CT, medullary arteriography) would serve as a guide for the treatment strategy. The authors do not report any case of permanent postoperative neurological aggravation. Three surgical o

Case report 691. Radiation-induced osteochondromas (RIO) arising from the neural arch and producing compression of the spinal cord.

A case of a 16-year-old girl with a RIO of the T11 neural arch compressing the spinal cord is presented. The radiation therapy had been given after removal of a Wilms' tumor at 1 year of age. RIOs usually occur in the long bones and are diagnosed about 8 years following irradiation. Spinal cord or nerve root compression is usually associated with multiple cartilaginous exostoses.

Lateral parascapular extrapleural approach to the upper thoracic spine.

The upper thoracic vertebrae are difficult to approach surgically because of the narrowing of the thoracic inlet, the proximity of the brachial plexus, and the parascapular shoulder musculature. A novel lateral parascapular extrapleural approach to the upper thoracic vertebrae is described. The parascapular shoulder musculature (trapezius, levator scapulae, and rhomboid muscles) is reflected off the spinous processes to the scapula as a musculocutaneous flap, preserving the neurovascular supply. The paraspinal musculature is mobilized and retracted, and the upper dorsal ribs are removed with c

Thoracic spine fractures.

Diagnosis of thoracic spine fractures in patients with multiple trauma requires careful evaluation of the chest radiograph. Primary findings, consisting of a fracture line through the vertebral bodies, decreased height of the vertebral body, an increase in the interpediculate distance and misalignment of the vertebral bodies, may all be noted on the anteroposterior supine chest radiograph. Early diagnosis and intervention may prevent a catastrophic neurologic event, such as spinal cord or nerve root damage.

[Spiroid fractures of the thoracic and lumbar spine].

The review of our observations of fractures of the thoracic and lumbar spine (588 files could be used from 1969 to 1989) allowed us to demonstrate fractures not included in the usual classification. In our opinion, the fracture line is spiral. The mechanism therefore includes a very likely axial rotation. The fracture line may be confined to the vertebral body (this is the type called S1) or extends to the posterior arch as well (S2 type). The fractures often cause nerve root lesions. However, no complications involving the cord were noted in our series, even in the few cases showing considera

Spinal pedicle finder for transpedicular screw fixation--design and early clinical result.

Spinal transpedicular fixation has gained widespread popularity in the past 5 years. In biomechanical studies, the deeply-inserted transpedicular screws withstood the largest number of cycles in the cephalad-caudad and medial-lateral direction before failure. However, in clinical practice, the risk of screw placement which is too far medially or too far laterally do exist. The optimization of increasing screw depth to avoid complication is of significant clinical importance. A Spinal Pedicle Finder (S.P.F) has been designed for transpedicular screws and a prototype has been completed. It is co

Methods of surgical stabilization for metastatic disease of the spine.

A variety of surgical techniques is available for treatment of metastatic disease of the spine. Prior emphasis on the use of these procedures has been on their benefit as a palliative measure to relieve signs and symptoms of spinal cord and nerve root compression not aided by radiation therapy and corticosteroid administration. More recently, development of surgical techniques that combine neural decompression with restoration of spinal stability has brought about consideration of additional indications for surgery in the treatment of metastatic disease of the spine. The present scope of such

Thoracic intervertebral disc protrusion with spinal cord compression.

Four patients presenting with spinal cord compression and spinal nerve root irritation are described. The clinical and radiological features are described and the contribution of CT scanning to accurate diagnosis of this condition is highlighted. Review of the literature indicates that the natural history of this disorder is one of steadily progressive neurological dysfunction. Surgical decompression of the lesion, although hazardous, is the only therapeutic option. Brief comment is made on the neurosurgical approaches currently in vogue.

The spectrum of neurological manifestations in myeloma.

Thirty-two patients with myelomatosis (3 with a solitary plasmocytoma and 29 with multiple myeloma) and neurological complications, from a group of 110 unselected patients with myelomatous disease were recorded. Spinal cord compression was the most frequent complication seen in 12 cases. In 9 subjects, it was the presenting symptom, and in 11 cases was caused by extradural plasmocytoma of the thoracic spine. Early decompressive laminectomy was curative in 3 cases, but in the rest, delay in diagnosis resulted in only partial recovery even with radiotherapy. Of the 6 patients with nerve root inv

Computed tomography of thoracic and lumbar spine fractures that have been treated with Harrington instrumentation.

Twenty patients with fractures of the thoracic and lumbar spine underwent computed tomography (CT) following Harrington distraction instrumentation and spinal fusion. CT was done to search for a cause of persistent cord or nerve root compression in those patients who failed to improve and completely recover their partial neurologic deficit (14 cases). In 6 patients seen in the late recovery period, CT was performed to evaluate the causes of pain and instability at the fracture site. Even though the image was degraded due to the artifacts generated by the metallic rods, significant information

Computed tomography of thoracic and lumbar spine injuries.

CT scans of 73 patients with acute thoracic/lumbar spine injuries from T3 to L5 were reviewed. Injuries were classified as burst fractures (48), fracture-dislocations (ten), wedge compression fractures (11), and seatbelt-type injuries (four). Thirty-one (42%) had motor deficits due to spinal cord or nerve root damage. Such neurologic deficits were present in all patients with fracture-dislocations, and 60% of those with burst fractures. Seven patients, four initially normal, developed progressive neurologic impairment early after injury. Burst fractures, one with dislocation, were the spinal i

Dislocations at the cervicothoracic junction.

Dislocations of the cervicothoracic junction are frequently missed. Experience of this rare injury over 27 years at the Spinal Injuries Unit in Sheffield confirms that nearly two-thirds were not properly diagnosed on admission. Only two of the 14 dislocations studied were reduced by conservative methods and these were both associated with fractures of the posterior bony elements. Open reduction is necessary to replace pure dislocations at the C7-T1 level. It seems likely that the spinal cord lesion is not influenced by reduction of the displacement. The three patients who had an incomplete les

High resolution CT of thoracolumbar fractures.

Twenty-one patients with thoracolumbar spinal fractures were studied with computed tomography (CT). A vertebral body was involved in 20; 12 had additional fractures of the posterior elements. Five had multiple level injuries. Neurologic deficits occurred in nine and were more common in combined vertebral body and posterior element injury. Five patients had surgical exploration of the spinal canal. CT provided more information than plain films, which missed a vertebral body fracture in one of 20, spinal canal compromise in four of 17, and posterior element fracture in one of 12. CT showed the p
