On the basis of anatomic studies, we have devised a new method of transferring the thoracodorsal nerve to reinnervate the paralytic deltoid or biceps muscles in an old traumatic injury of the associated nerve. It has been applied to a case of musculocutaneous nerve injury and two cases of axillary nerve injury with good results. The muscle functions have returned to normal within 1 and 2 years, respectively, as shown by electromyography and the muscular contractility test.

A multi-channel phrenic nerve stimulator developed in Tampere has been implanted into seven patients with C2-tetraplegia and into three patients with central sleep apneas. Six bipolar cuff electrodes were implanted bilaterally into the neck. Two four-pole cuff and 14 four-pole noncuff electrodes were used in seven patients and to replace one bipolar electrode. Four-pole electrodes were implanted within the thorax. Seven patients achieved total independence from conventional ventilators within 4 months of implantation, and one for 18 hours each day. We discuss the case of a premature baby weighing 1,200 g who developed right diaphragmatic paralysis when a venous cutdown was performed on his internal jugular vein. Acquired unilateral diaphragmatic paralysis has been associated with obstetric trauma of the cervical plexus and surgical procedures of the neck and thorax, when the phrenic nerve is injured. We suggest fluoroscopy as the best diagnostic method; early diaphragmatic plication reduces the morbimortality of these patients.

New techniques for internal fixation, employed by neurosurgeons and orthopaedic surgeons, provide reduction and stabilization with early mobilization of patients after injuries of the thoracolumbar spine. It has, however, appeared difficult to prove that the neurological outcome after surgery may be superior to conservative treatment. Nonpenetrating injury to the subclavian vessels is uncommon. During a 6-year period we have treated 167 patients with injuries to the subclavian and superior mediastinal arteries. Fifteen of these injuries (9%) occurred after blunt trauma. In 10 patients the proximal segment (first and second parts) of the artery was involved. No patient had an isolated injury; the most frequent associated injuries were rib fractures (n = 11), with the first rib being involved in four of these. Involuntary activity of transferred intercostal motor units was examined in patients with brachial plexus injury. Since the internal intercostal nerves were detached from the thorax to reinnervate the musculus biceps brachii, it was possible to record pure intercostal motor activity in humans. Respiratory activity was seen in the latter part of the expiratory phase, thus dividing the phase into two substages (E1 and E2) by the onset of the activity. Three children, aged 6-10 years, in whom cervical cord injury at the C1-C2 level resulted in apnoea had bilateral implantation of diaphragm pacemakers. With periods of gradual conditioning of the diaphragm muscle to low frequency stimulation and slow respiratory rates they adapted to continuous ventilatory support by simultaneous stimulation of both hemidiaphragms without evidence of fatigue, so far for periods of 23-47 months.

Vagus reflex death in foreign body bronchoscopy as a rare incident that severely incriminates medical work is described via a personal observation in an infant of 2 3/4 years of age. Typical symptoms are probably "thorax rigidity", sphincter-like stenosis of the bronchial lumina, paravertebral contraction atelectasis and catarrhal mucous membrane alterations of the respiratory tract. The bulb rhythm producing structures between the tractus solitarius and the nucleus ambiguus must be considered as origins of respiratory arrest.

The aim of this study was to define the origin, course, caliber and relations of the left recurrent laryngeal nerve (N Laryngeus recurrens) in order to localize the nerve during surgery for esophageal atresia. Eighteen anatomic specimens were dissected and 12 surgical cases were analysed. Left recurrent laryngeal nerve palsy after this particular form of neonatal surgery is a very real risk. It seemed useful to describe an anatomic technique of identifying the nerve in the mediastinum during such surgery, in order to prevent this complication.

The relative inspiratory augmentations of sternomastoid, scalene, external intercostal, interchondral and diaphragmatic electromyographic activities were examined during the progressive asphyxia induced by rebreathing in pentobarbitone/urethane anaesthetized rabbits. Diaphragmatic activity augmented significantly less than that of the scalene, intercostal and interchondral muscles in response to the asphyxic increase in inspiratory drive (hyperpnoea). Cervical vagotomy significantly increased the levels of inspiratory activity during the asphyxic hyperpnoea but did not abolish these relations.

The clinical findings of nine patients who suffered disruption of the subclavian artery following blunt thoracic trauma were reviewed. Seven patients were men, two were women. A systematic line-sampling method is described for counting and mapping myelinated axons in transverse sections of the spinal cord. Its advantages over random sampling of small areas are considered. The case report of a motor vehicle accident victim with a right arm paralysis, Horner's pupil, and a widened mediastinum is presented. These findings were the result of a traction injury to the brachial plexus. The widened mediastinum and apical capping in this patient were the result of a hemorrhage at the base of the neck which extended into the mediastinum. The clinical manifestations and pathophysiology of traction injuries to the brachial plexus are discussed, and an approach to the diagnosis and emergency management of this problem is reviewed.

Ten cases of isolated, complete paralysis of the serratus anterior muscle were diagnosed in young athletes during a three-year period. Total spinal anesthesia following intrathoracic intercostal nerve blocks with bupivacaine performed for postoperative pain relief during thoracotomy is described. Possible mechanisms for this complication include: (1) inadvertent placement of the needle through an intervertebral foramen, (2) puncture of a long dural cuff, and (3) intraneural injection with central spread. Recognition of this potential complication is important, and facilities for proper support must be available.
