Preoperative Intravenous Indocyanine Green Injection Demarcates Tumor Border and Adjacent Nerves in Surgical Resection of Posterior Mediastinal Neurogenic Tumors.

OBJECTIVES: To assess the application of preoperative indocyanine green (ICG) near-infrared (NIR) fluorescence imaging in video-assisted thoracoscopic surgery (VATS) for posterior mediastinal neurogenic tumors. METHODS: We present a case of a 66-year-old female with a T3-adjacent posterior mediastinal tumor. She received intravenous ICG (5 mg/kg) 24 hours before surgery. Intraoperative NIR imaging was used to identify the tumor and nearby nerves during VATS. RESULTS: ICG fluorescence clearly delineated the tumor margins and visualized the adjacent sympathetic and intercostal nerves. This gui

Impact of acute high thoracic spinal cord injury on in vivo vascular function in rats.

Cardiovascular dysfunction is a major cause of morbidity and mortality following high-thoracic spinal cord injury (SCI), yet the acute impact of SCI on integrated neurovascular control remains poorly defined. In the current study, we aimed to investigate the acute effects of complete T3 transection (T3-SCI) on in vivo hemodynamic function, superior mesenteric artery (SMA) autoregulation, sympathetic baroreflex sensitivity (sBRS), and sympathetic vascular transduction (SVT) in anesthetized rats. Nineteen male Wistar rats were randomized into naïve control (n = 10) or T3-SCI (n = 9) experimental

Ion channel and receptor-mediated regulation of axonal conduction reliability in sympathetic preganglionic neurons.

UNLABELLED: Sympathetic preganglionic neurons (SPNs) provide the sole spinal output to the peripheral sympathetic nervous system. Although sympathetic control is traditionally attributed to synaptic integration within the spinal cord and ganglia, the reliability of spike propagation along SPN axons themselves has received little attention. Here, and in companion papers, we show that axonal conduction in adult mouse thoracic SPNs is highly modifiable and constitutes a critical site of sympathetic gain control. Using an ex vivo preparation preserving intact paravertebral and splanchnic pathways

Frequency-Domain Analysis Links Autonomic Disruption to Renal Autoregulatory Failure after Spinal Cord Injury.

Spinal cord injury (SCI) disrupts supraspinal autonomic pathways that regulate cardiovascular function, producing marked blood pressure instability and contributing to secondary injury in peripheral organs. The kidney is particularly vulnerable to these disturbances because renal blood flow (RBF) depends on tightly regulated interactions between neural, myogenic, and vascular control mechanisms. However, how SCI level and chronicity alter dynamic renal autoregulation remains poorly defined. Here, we investigated the effects of high- and low-thoracic SCI on renal hemodynamic control using in vi

Revisiting thoracic splanchnic nerve anatomy: implications for thoracoscopic splanchnicectomy and pain management.

PURPOSE: The thoracic splanchnic nerves-greater (GSN), lesser (LSN), and least (lSN)-are important targets in thoracic surgery and image-guided interventions, yet detailed quantitative characterization of their anatomy remains limited. This study aimed to define the morphometry, segmental origins, and anatomical variability of these nerves to improve understanding of clinically relevant anatomy. METHODS: Eighty sets of thoracic splanchnic nerves from 42 formalin-embalmed donors were dissected. Segmental contributions were recorded and classified, and morphometric measurements were obtained re

Neural Mechanisms of Heart-Lung Crosstalk in Chronic Heart Failure.

BACKGROUND: The cardiac sympathetic afferent reflex and pulmonary spinal afferent reflex amplify sympathetic activity and may contribute to chronic heart failure. We hypothesized that cardiac injury triggers a cardiac nerve-mediated cytokine uptake cascade that propagates neural inflammation in thoracic dorsal root ganglia (DRGs) and drives reciprocal cardiopulmonary afferent sensitization through suppression of voltage-gated potassium channels after myocardial infarction (MI). METHODS: MI was induced in rats by coronary ligation. Molecular profiling, immunofluorescence, tissue clearing, and

Vascular nexus glia regulates vascular homeostasis and undergoes dynamic remodelling in deep vein thrombosis.

AIMS: The cardiovascular system is tightly regulated by the nervous system, yet while cardiac nexus glial cells are known modulators of physiology, the identity and functional significance of glial populations within the vasculature remain poorly defined. We hypothesized that a specialized population of vascular glial cells exists in the adventitia and undergoes molecular and morphological remodelling under pathological stress. METHODS AND RESULTS: By analysing human and murine single-cell RNA sequencing (scRNA-seq) datasets, we identified a conserved population of glial cells in large vessel

Radiotherapy Targeting the Causative Lesion of Horner's Syndrome in Metastatic Breast Cancer: A Case Report and Literature Review.

INTRODUCTION: Horner's syndrome (HS) is a rare manifestation of metastatic breast cancer resulting from disruption of the sympathetic pathway. Only 6 cases have previously been reported in the literature, almost all of which were managed with systemic therapy. Detailed reports focusing on the role of local treatment, such as radiotherapy directed at the causative lesion of HS in breast cancer, are lacking. CASE PRESENTATION: We report the case of a 60-year-old Japanese woman with HER2-positive, hormone receptor-positive invasive ductal carcinoma who developed left-sided HS due to lymph node m

Anatomy, Imaging, and Clinical Significance of the Cervicothoracic (Stellate) Ganglion.

Background/Objectives: The stellate ganglion (SG), formed by the fusion of the inferior cervical and first thoracic sympathetic ganglia in approximately 80% of individuals, plays crucial roles in cardiac innervation, pain management, and autonomic regulation. This review examines the anatomical variations, histological structure, clinical applications, and therapeutic implications of the SG and stellate ganglion block (SGB), presenting original high-resolution magnetic resonance imaging (MRI) evidence of SG visualization, an underutilized approach in autonomic nervous system research. Methods:

[Sympathetic Nerve Trunk Preserved by Intracapsular and Transcapsular Resection in a Patient with Superior Mediastinal Schwannoma].

A 52-year-old man was referred to our hospital because of an abnormal shadow on a chest X-ray. He was asymptomatic. Computed tomography (CT) revealed a smooth 32 mm right superior mediastinal mass at the level of the first to third thoracic vertebrae. T2 magnetic resonance imaging (MRI)revealed a cystic mass with a fluid level. Foregut cysts were initially considered. After three years, the mass had enlarged to 39 mm. Thoracoscopic surgery was performed, and the mass was originated from a sympathetic trunk and removed via intracapsular and transcapsular resection. A histological examination co

LncTCONS_00058568 is involved in the pathophysiologic processes mediated by P2X7R in the lower thoracic spinal cord after acute kidney injury.

Acute kidney injury (AKI), a prevalent clinical syndrome, involves the participation of the nervous system in neuroimmune regulation. However, the intricate molecular mechanism that governs renal function regulation by the central nervous system (CNS) is complex and remains incompletely understood. In the present study, we found that the upregulated expression of lncTCONS_00058568 in lower thoracic spinal cord significantly ameliorated AKI-induced renal tissue injury, kidney morphology, inflammation and apoptosis, and suppressed renal sympathetic nerve activity. Mechanistically, the purinergic

Intrathecal Anesthesia Prevents Ventricular Arrhythmias in Rats with Myocardial Ischemia/Reperfusion.

INTRODUCTION: Ventricular arrhythmia is commonly provoked by acute cardiac ischemia through sympathetic exaggeration and is often resistant to anti-arrhythmic therapies. Thoracic epidural anesthesia has been reported to terminate fatal ventricular arrhythmia; however, its underlying mechanism is unknown. METHODS: Rats were randomly divided into four groups: sham, sham plus bupivacaine, ischemia/reperfusion (IR), and IR plus bupivacaine groups. Bupivacaine (1 mg/mL, 0.05 mL/100 g body weight) was injected intrathecally into the L5-L6 intervertebral space prior to establishing a myocardial IR r

Vagal nerve stimulation in myocardial ischemia/reperfusion injury: from bench to bedside.

The identification of acute cardioprotective strategies against myocardial ischemia/reperfusion (I/R) injury that can be applied in the catheterization room is currently an unmet clinical need and several interventions evaluated in the past at the pre-clinical level have failed in translation. Autonomic imbalance, sustained by an abnormal afferent signalling, is a key component of I/R injury. Accordingly, there is a strong rationale for neuromodulation strategies, aimed at reducing sympathetic activity and/or increasing vagal tone, in this setting. In this review we focus on cervical vagal ner

Acute intermittent hypoxia elicits sympathetic neuroplasticity independent of peripheral chemoreflex activation and spinal cord tissue hypoxia in a rodent model of high-thoracic spinal cord injury.

The loss of medullary control of spinal circuits controlling the heart and blood vessels is a unifying mechanism linking both hemodynamic instability and the risk for cardiovascular diseases (CVD) following spinal cord injury (SCI). As such, new avenues to regulate sympathetic activity are essential to mitigate CVD in this population. Acute intermittent hypoxia (AIH) induces a type of neuroplasticity known as long-term facilitation (LTF), a persistent increase in nerve activity post-AIH in spinal motor circuits. Whether LTF occurs within the sympathetic circuit following SCI is largely unknown

Horner's syndrome caused by the first rib fracture sustained during coronary artery bypass grafting: a case report and literature review.

BACKGROUND: Horner's syndrome is a rare complication of cardiovascular surgery. A bone fragment and hematoma due to rib fracture after cardiac surgery may cause injury to the brachial nerve plexus and sympathetic nerve trunk, leading to neurologic disorders and Horner's syndrome. However, few reports have revealed the etiology of Horner's syndrome after cardiovascular surgery based on imaging. Herein we present a case in which a plain CT scan confirmed the etiology of Horner's syndrome after coronary artery bypass grafting (CABG), reviewing 139 CABG cases retrospectively in our hospital and 6

Postmorterm Computed Tomography and Autopsy to Confirm Sudden Death Due to Tracheal Compression by Mediastinal Fat Tissue in a Young Man With Obesity.

A man in his early twenties with obesity was found dead in his apartment. The deceased was found naked and surrounded by empty bottles of electrolytes. An autopsy performed approximately 6 days postmortem and gross inspection revealed the absence of injury and no apparent extrinsic cause of death. It was decided to dissect to investigate the cause of death. The deceased had become morbidly obese (weight, 98 kg; height, 160 cm; body mass index, 38.3). Shortly before his death, he presented at a clinic complaining of gastric discomfort and heartburn, but other than hypertension (155/91 mmHg) no

Sympathetic neural overdrive and diminished exercise capacity in reduced ejection fraction heart failure related to anthracycline-based chemotherapy.

Cardiotoxicity is the most worrying cardiovascular alteration in patients treated with chemotherapy. To improve the understanding regarding the cardiotoxicity, we studied whether 1) patients with cardiac dysfunction related to anthracycline-based chemotherapy have augmented sympathetic nerve activity and decreased exercise capacity and 2) these responses are similar to those observed in patients with heart failure caused by other etiologies. Sixteen patients with heart failure with reduced ejection fraction related to anthracycline-based chemotherapy with or without chest radiation (HFrEFCA),
