Morphology of aortic arch pathology: implications for endovascular repair.

PURPOSE: To present a detailed description of aortic arch morphology in patients with aneurysm and dissection undergoing thoracic endovascular aortic repair (TEVAR). METHODS: Morphological assessment of the aortic arch was performed in a consecutive series of patients undergoing TEVAR between November 2003 and April 2009. In the absence of standardized reporting criteria, any patient requiring overstenting of the left subclavian artery (LSA) was considered to have pathology of the aortic arch. In all, the arch morphology of 49 patients (31 men; mean age 70 years, range 43-86) was analyzed (25

Multidetector CT scan findings of a right aberrant retroesophageal vertebral artery with an anomalous origin from a cervical aortic arch.

Various malformations of the aortic arch and great vessels have been reported in the literature, which reflects the complexity of their embryologic development. Most of them are of incidental finding and remain asymptomatic but can be responsible for respiratory or digestive symptoms and be associated with congenital cardiac diseases. We report the case of a patient presenting a right retroesophageal vertebral artery. This malformation was associated with multiple anomalies of the aortic arch. We report a classification of the most common aortic arch abnormalities by recalling Edwards double a

Spinal cord protection with a cerebrospinal fluid drain in a patient undergoing thoracic endovascular aortic repair.

Paralysis and paraparesis are dreaded complications of thoracic endovascular aortic repair (TEVAR) that occur with an incidence of 2%-6%. Risks factors include the type of thoracic aortic pathology treated, coverage of the left subclavian artery origin without revascularization, concomitant infrarenal abdominal aortic aneurysm repair, extent of stent graft coverage of the thoracic aorta, and renal failure. Cerebral spinal fluid (CSF) drains have been advocated as one of several protective strategies to prevent spinal cord ischemia. This case discussion briefly addresses the evidence supporting

Stent-grafting of a dissected aortic arch and descending aorta after debranching of the supra-aortic vessels: case-presentation, indications, technique.

Endovascular repair of thoracic aorta pathology is emerging as the preferred treatment strategy in high risk patients. Hybrid techniques with debranching of the supra-aortic vessels extend indications for endovascular repair of the aortic arch and descending aorta. After a brief case-presentation, we describe these procedures, outline the indications and discuss the outcomes and results available in present literature.

Sildenafil added to sitaxsentan in overcirculation-induced pulmonary arterial hypertension.

Experimental left-to-right shunt-induced pulmonary arterial hypertension (PAH) can be partially prevented by the endothelin-A receptor blocker sitaxsentan or by the phosphodiesterase-5 inhibitor sildenafil. We hypothesized that the combined administration of these drugs would completely prevent shunt-induced PAH, arguing in favor of a major role of endothelial dysfunction in the initiation of the disease. Twenty-four 3-wk-old piglets were randomized to a sham operation or to placebo, sitaxsentan therapy, or sitaxsentan combined with sildenafil after the anastomosis of the left subclavian arter

A large posttraumatic subclavian artery aneurysm complicated by artery occlusion and arteriobronchial fistula successfully treated using a covered stent.

The treatment of posttraumatic aneurysms of peripheral arteries using covered stents is increasingly commonplace. We present the case of a 10-year-old girl with a pseudoaneurysm of the subclavian artery complicated by an arteriobronchial fistula with hemorrhaging into the bronchial tree and distal subclavian artery occlusion. Despite the lack of artery patency, endovascular stent graft implantation was successful. Pseudoaneurysm exclusion and involution was achieved, together with a patent implant and maintained collateral circulation patency.

Branching patterns of the subclavian arteries in German Shepherd dogs.

This study is the first report of the branching pattern of the four major branches of the subclavian artery in German Shepherd dogs. A total 116 subclavian artery casts made of silicon under mean arterial pressure were analysed. The casts were classified according to their branching order and the pattern of the first two branches of the vertebral artery and costocervical trunk. The three subtypes of each major type were based on the branching order or level of the next two branches (the superficial cervical artery and internal thoracic artery). Eleven of 12 possible subtypes were found in the

An occult cause of hemodialysis vascular access dysfunction.

Although hemodialysis vascular access dysfunction is often caused by venous stenosis, stenosis can occur anywhere in the circuit. Herein, we report a 75-year-old woman who received repeated percutaneous angioplasty due to insufficient flow. Finally, a culprit stenosis at the subclavian artery was found and treated. Subclavian artery stenosis is often atherosclerotic in origin and is usually delayed to be diagnosed because of caveats in the care of vascular access. This case highlights important clues that can assist in the early discovery of inflow problems and reminds the physician that inflo

Evaluation of an electromagnetic 3D navigation system to facilitate endovascular tasks: a feasibility study.

INTRODUCTION: We describe a novel approach to arterial cannulation using the StealthStation(®) Guidance System (Medtronic, USA). This uses electromagnetic technology to track the guidewire, displaying a 3D image of the vessel and guidewire. METHODS: The study was performed on a 'bench top' simulation model called the Cannulation Suite comprising of a silicone aortic arch model and simulated fluoroscopy. The accuracy of the StealthStation(®) was assessed. 16 participants of varying experience in performing endovascular procedures (novices: 6 participants, ≤5 procedures performed; intermediate:

Massive coronary subclavian steal syndrome.

Coronary subclavian steal syndrome is an unusual cause of angina, secondary to decreased or reversed flow in patients with patent "in situ" internal mammary-to-coronary artery graft. The most frequent cause of coronary subclavian steal syndrome is ipsilateral subclavian artery stenosis. We present a 60-year-old man with cerebrovascular and peripheral artery disease and a documented massive coronary subclavian steal syndrome, which impaired cardiopulmonary bypass weaning after multiple coronary artery bypass with double T-mammary artery graft.

Subclavian artery access for ambulatory balloon pump insertion.

Intraaortic balloon pump insertion is traditionally performed through the femoral artery in the groin. However, this restricts the patient to bed rest, and prolonged implantation can be associated with infections in the groin crease. We describe a technique of insertion of a balloon pump through the subclavian artery, which allows the patient to ambulate. This technique can also be performed under local anesthesia in the cardiac catheterization laboratory.

Percutaneous occlusion of the left subclavian and celiac arteries before or during endograft repair of thoracic and thoracoabdominal aortic aneurysms with detachable nitinol vascular plugs.

PURPOSE: To review an experience with the Amplatzer vascular plug (AVP) for prevention of type II endoleaks during endovascular aneurysm repair (EVAR) of thoracic and thoracoabdominal aneurysms. MATERIALS AND METHODS: Retrospective review was undertaken of 14 patients undergoing transcatheter occlusion of the left subclavian (n = 12) or celiac artery (n = 2) with the AVP as part of EVAR of thoracic and thoracoabdominal aneurysms at a single institution. Procedural criteria evaluated were success at target vessel occlusion, the number of AVPs used, use of adjunctive embolization devices, and e

Combined treatment of traumatic aortic transection and Kommerell diverticulum.

We present the case of a 41-year-old male trauma patient admitted to the emergency department after being struck in a pedestrian versus a motor vehicle accident. Computed tomography revealed a traumatic transaction of the descending aorta with pseudoaneurysm and an aberrant right subclavian artery with Kommerell diverticulum. Surgical correction was accomplished with bilateral subclavian carotid bypass, with occlusion of both subclavian arteries followed by the placement of endovascular stent grafts to repair the aortic injury.

[Sudden/unexpected death due to spontaneous aortic rupture: two unusual manifestation forms].

Sudden unexpected death from a natural (pathological) cause often occurs under suspicious circumstances suggesting foreign intervention, so that a forensic autopsy is deemed necessary. In adults, a significant share of these deaths is due to spontaneous ruptures of the aorta. In figures, dissections of the ascending aorta and the aortic arch (mostly associated with secondary perforation into the pericardium) account for a large percentage followed by arteriosclerotic aneurysms of the abdominal aorta (with retroperitoneal and sometimes intraperitoneal hemorrhage). The authors report on two fata

A mouse model of ischemic spinal cord injury with delayed paralysis caused by aortic cross-clamping.

BACKGROUND: Spinal cord ischemia and paralysis are devastating perioperative complications that can accompany open or endovascular repair surgery for aortic aneurysms. Here, we report on the development of a new mouse model of spinal cord ischemia with delayed paralysis induced by cross-clamping the descending aorta. METHODS: Transient aortic occlusion was produced in mice by cross-clamping the descending aorta through a lateral thoracotomy. To establish an optimal surgical procedure with limited mortality, variable cross-clamp times and core temperatures were tested between experiments. RES

Coexisting of aortic arch variation of the left common carotid artery arising from brachiocephalic trunk and absence of the main branches of right subclavian artery: a review of the literature.

The variations in the aortic arch and its branches are of surgical interest. Many anomalies of the aortic arch and great vessels are uncovered as serendipitous findings on imaging studies, in the anatomy laboratory, or at surgery. A 56-year-old woman had an arch angiogram as part of an evaluation for cardiovascular disease. A two-vessel left aortic arch was identified consisting of the brachiocephalic trunk and left subclavian artery. In this study, we present a case of the left common carotid (LCC) artery arising from the brachiocephalic trunk (BT) and absence of the main branches of right su

A novel approach to the upper anterior thoracic spine: a cadaveric feasibility study.

OBJECT: Surgical approaches to the upper anterior thoracic spine can be a challenge. Various techniques such as transsternal routes have been employed but access to the midthoracic vertebrae is limited due to the position of the heart and great vessels. In the present study the authors' goal was to evaluate in cadavers a novel approach to the upper anterior thoracic spine. METHODS: In 12 adult cadavers the majority of the left first rib was removed following infraclavicular transection of the attachment of the anterior and middle scalene muscles from this bone.
