Efficacy evaluation and parametric study of the novel concave triple branched stent graft system based on hemodynamic analysis of first-in-man cases.

BACKGROUND: Concave supra-arch triple branched stent-graft system (CS system) offers a new option for treating aortic arch pathologies. However, the efficacy of the innovative device still lacks objective evaluations. Patient-specific CS system design and treatment strategies remain unknown. This study aims to assess the effectiveness and to inform the patient-specific CS system design by evaluating the hemodynamic effects of key parameters. METHODS: Simulations were conducted via pre- and post-operative computed tomography angiography datasets from five first-in-man study cases. Parametric s

Acute pancreatitis following digital subclavian artery angiography: a case report and literature review.

Subclavian artery (SA) angiography has many well-documented complications. Acute pancreatitis (AP) is a rarely described complication with potentially life-threatening repercussions. This article reports the case of a man with AP that occurred the second day after SA angiography. Contrast agent toxicity and cholesterol emboli are the two mechanisms involved in the occurrence of AP after SA angiography. We searched previous literature using PubMed databases during the same period as comparison.

Anatomical study of the dorsal scapular artery in cadavers and its clinical significance.

The dorsal scapular artery (DSA) is one of the key arteries supplying the muscles of the upper back, along with the Suprascapular artery and the Subscapular artery. It lies close to the brachial plexus, making its variations important during regional blocks and flap surgeries. Variations in its origin, course, and relationships in different population groups, as well as its surgical and anaesthetic significance, emphasise its need for study. This descriptive study was performed following a standard dissection manual on 40 bilateral sides of embalmed human cadavers. The origin of the DSA was id

Synchronous arch arterial occlusion - A clinical impasse?

Peripheral artery disease is considered as an extra-cardiac manifestation of atherosclerosis, and its clinical presentation is equated to lower limb ischemia. A similar pathogenetic mechanism plays a role in luminal stenosis or obliteration of one or more of the aortic arch arteries. This is a presentation of an uncommon case of synchronous athero-thrombotic obliteration of the right brachio-cephalic and left subclavian arteries in a hypertensive male, who had been symptomatic for only 6 hours.

Endovascular repair of descending aorta lesions using a second-generation unibody single-branched stent-graft: a technical note.

BACKGROUND: This report describes the second-generation unibody Cratos single-branched stent-graft for left subclavian artery (LSA) revascularization during thoracic endovascular aortic repair (TEVAR). METHODS: The Cratos device represents the second-generation evolution of the Castor single-branched stent-graft, featuring an outer unibody branch configuration that allows simultaneous exclusion of the primary entry tear and revascularization of the LSA. It incorporates an adjustable proximal stent segment that optimizes sealing and reduces the risk of bird-beak formation at the proximal landi

Single-fenestrated TEVAR for distal aortic arch pathologies using the Zenith® platform: a scoping review.

BACKGROUND: Single-fenestrated thoracic endovascular aortic repair (1f-TEVAR) using custom-made devices (CMDs) has emerged as a less invasive strategy for managing distal aortic arch diseases. METHODS: A scoping review was conducted to identify studies reporting clinical outcomes of 1f-TEVAR with fenestration for the left subclavian artery (LSA) or left common carotid artery (LCCA), with or without a proximal scallop for the innominate artery (IA), were included. RESULTS: Five studies were included, comprising 207 patients. The target vessel was most frequently the LSA, with LCCA fenestratio

A rare anomaly of the cervicobrachial plexus: penetration of the anterior scalene muscle tendon by the T1 nerve root.

The cervicobrachial plexus is known to exhibit anatomical variation, particularly the brachial plexus, which maintains a close anatomical relationship with the subclavian artery and the scalene muscles. Knowledge of these structural variations is critical for clinicians involved in the diagnosis and management of brachial plexus injuries. This case report describes a rare case involving multiple anomalies in the root formation of the brachial plexus, observed in a Japanese male cadaver. In this case, both brachial plexuses were formed from C4 to T1, and the both subclavian arteries passed betw

Arterial thoracic outlet syndrome due to a first-rib anomaly causing brachial artery embolic occlusion: a case report.

Arterial thoracic outlet syndrome is a rare condition caused by compression of the subclavian artery at the thoracic outlet, which often leads to delayed diagnosis and potentially severe outcomes such as upper limb necrosis. We report a case of a man in his 20s who presented with coldness in the right fingers, exacerbated by cold environments. Chest radiography revealed abnormal bone structures, and contrast-enhanced computed tomography confirmed stenosis and post-stenotic dilation of the right subclavian artery, thrombus formation, and occlusion of the brachial artery. Although bilateral firs

The Society for Vascular Surgery clinical practice guideline on the management of blunt thoracic aortic injury: Focused update.

Blunt thoracic aortic injury (BTAI) remains a devastating and potentially fatal complication of major trauma that requires expedient diagnosis and management by a collaborative team of trauma and vascular specialists. Trauma patients with BTAI commonly have multiple associated injuries, including intracranial or truncal hemorrhage, that increase the complexity of factors related to timing and conduct of repair. Optimal outcomes for these patients require consideration of these factors and updated knowledge of the available evidence by all stakeholders of care in a collaborative fashion. In 201

Idiopathic thyrocervical trunk aneurysm managed with endovascular coiling.

Aneurysms of the thyrocervical trunk are rare and often associated with trauma or underlying conditions such as neurofibromatosis type 1. Management guidelines do not exist for this aneurysm and there remains practice variation with some performing open ligation and repair and others performing endovascular coiling. We report a case of idiopathic thyrocervical trunk aneurysm which presented as a painful right-sided pulsatile neck mass. Diagnosis of the right thyrocervical trunk aneurysm was initially made via ultrasonography and subsequently confirmed with CT angiography. The patient was manag

First Reported Use of the AMDS Hybrid Prosthesis for Secondary Type A Aortic Dissection After Prior TEVAR.

Type A aortic dissection represents one of the most life-threatening cardiovascular emergencies, with management strategies evolving toward hybrid and endovascular approaches, particularly in high-risk patients. The Ascyrus Medical Dissection Stent (AMDS) is an emerging adjunctive technology designed to promote true lumen expansion and facilitate favorable aortic remodeling during open repair of acute Type A dissection. We present the first reported case of AMDS deployment in secondary Type A dissection following prior thoracic endovascular aortic repair (TEVAR). An 83-year-old female with ext

Castor stent reconstruction of left subclavian artery reduces the incidence of postoperative cerebral infarction of type B aortic dissection with insufficient proximal landing area: a propensity score matched analysis.

BACKGROUND: Compared with conventional treatment of type B aortic dissection (TBAD) with insufficient proximal anchoring by covering the left subclavian artery (LSA), reconstructing LSA with Castor branch stent is a promising strategy. Prior studies lacked comparisons with partial LSA coverage, so we used propensity score matching analysis (PSMA) to retrospectively evaluate both approaches. METHODS AND RESULTS: We studied 377 patients with thoracic aortic dissection (TBAD) who underwent thoracic endovascular aortic repair (TEVAR) at one center. Of these, 262 had partial LSA coverage, and 115

Outcome Comparisons of Direct Coverage Versus Fenestration for an Isolated Left Vertebral Artery in Zone 2 TEVAR: A Retrospective Study.

BACKGROUND: Thoracic endovascular aortic repair (TEVAR) in Zone 2 frequently necessitates coverage of the isolated left vertebral artery (ILVA), a congenital vascular anomaly, to ensure adequate proximal sealing. However, the clinical requirement of ILVA revascularization remains uncertain. Thus, this study aimed to compare the outcomes between ILVA coverage and fenestration during Zone 2 TEVAR. METHODS: We retrospectively analyzed the clinical records of patients with ILVA who underwent Zone 2 TEVAR between September 2010 and August 2023. Patients were divided into two groups: Coverage Group

Modified Subclavian Artery-to-Right Atrium Bypass Technique in a Patient on Chronic Hemodialysis With Central Venous Occlusion.

It is not uncommon for all of the usual upper and lower extremity autogenous access sites to fail, often in patients for whom neither peritoneal dialysis nor transplantation is an appropriate option or is difficult to access, and who have chronic central venous occlusion. Right atrium bypass may be used as a last resort. We describe our technique through this clinical report. The patient is 40 years of age and has a history of type 1 diabetes. The bypass was performed via a minimally invasive approach (right anterior mini-thoracotomy), using a polytetrafluoroethylene (PTFE) prosthesis as a con

Combination of Albumin and Cilostazol Improves Outcomes in a Rodent Model of Aneurysmal Subarachnoid Hemorrhage.

BACKGROUND AND OBJECTIVES: Patients recovering from aneurysmal subarachnoid hemorrhage (aSAH) often suffer secondary neurologic injury owing to delayed cerebral ischemia (DCI)-a multifactorial phenomenon. Both albumin and cilostazol have shown efficacy for treating DCI in preclinical and early clinical studies. However, the utility of combining these treatments has not yet been rigorously tested. METHODS: A total of 83 mice, male and female, were randomly divided into five groups as follows: sham; vehicle (0.9% saline); 25% human albumin; cilostazol; and combination 25% human albumin with cil
