Multidisciplinary management of a thoracic aortic-penetrating esophageal foreign body: a case report.

BACKGROUND: Thoracic aortic penetration by an esophageal foreign body (EFB) is a rare, fatal emergency (> 80% mortality). Rapid multidisciplinary coordination with endovascular hemorrhage control may be lifesaving. CASE PRESENTATION: A 57-year-old woman developed retrosternal pain after eating a recently cooked fish meal. Computed tomography confirmed a fish bone penetrating the esophagus into the left subclavian artery and on the medial wall of the initial segment of the descending aorta. Preoperatively, the patient reported no tobacco or alcohol use and no long-term oral medications. Under m

"Migrant" Ductal Tissue in the Aortic Arch: A Substrate for Prostaglandin-Responsive Coarctation.

BACKGROUND: Neonatal aortic arch obstruction is typically related to ductal insertion and usually improves with prostaglandin E1 (PGE1). However, functional constriction caused by ectopic ductal tissue at atypical arch locations is rare and may mimic fixed coarctation. CASE SUMMARY: A one-day-old neonate with suspected coarctation was found to have a right aortic arch with transverse arch hypoplasia, an aberrant left subclavian artery arising from a Kommerell diverticulum, and a large patent ductus arteriosus. Withdrawal of PGE1 led to progressive distal arch constriction with left ventricula

Atypical cardiovocal syndrome associated with right aortic arch and Kommerell diverticulum.

UNLABELLED: Vocal cord palsy (VCP) can be caused by laryngeal and extra-laryngeal pathologies affecting the recurrent laryngeal nerves (RLNs). Many cases of left VCP related to cardiovascular pathologies have been documented. However, right VCP associated with cardiovocal (Ortner) syndrome is an atypical and rare presentation. A case of right VCP caused by a right aortic arch (RAA) and Kommerell diverticulum (KD), diagnosed on CT angiography, and its association with the variant course of the right RLN, is described. CONTRIBUTION: This case report highlights the rarity of cardiovocal syndrome

Right-Left Differential Cyanosis in a Newborn With a Circumflex Retroesophageal Left Aortic Arch.

BACKGROUND: Differential cyanosis typically results from right-to-left shunting through the patent ductus arteriosus (PDA), supplying highly oxygenated blood from the left ventricle to the upper body, whereas the lower body receives desaturated blood from the right ventricle. CASE SUMMARY: A term newborn presented with cyanosis of the right arm, right chest wall, and adjacent right-sided upper abdominal wall. Echocardiography and computed tomography revealed a circumflex retroesophageal left aortic arch with bilateral PDA, nonconfluent pulmonary arteries, and aberrant right subclavian artery

Predictors of Delayed Ischaemic Stroke following Thoracic Endovascular Aortic Repair for Blunt Thoracic Aortic Injury.

OBJECTIVE: Delayed ischaemic stroke (DIS) remains a feared complication following thoracic endovascular aortic repair (TEVAR) for blunt thoracic aortic injury (BTAI), particularly when the left subclavian artery (LSCA) is covered. This study aimed to understand the impact of LSCA revascularisation and other peri-operative factors on delayed stroke risk. METHODS: This was a retrospective cohort study querying the Aortic Trauma Foundation registry to identify patients with BTAI undergoing TEVAR from 2014 to 2025. Patients with DIS after TEVAR were identified, and patient level factors, intra-op

Initial Outcomes After Trans-Right Subclavian Transcatheter Aortic Valve Implantation: Analysis of Device Coaxiality.

BACKGROUND: Trans-subclavian access transcatheter aortic valve implantation (TAVI), typically from the left side, is feasible. However, right subclavian artery access is technically challenging because of the anatomical orientation, resulting in malalignment of the transcatheter heart valve within the aortic annular plane. METHODS AND RESULTS: We aimed to evaluate procedural outcomes, device-annulus alignment, and clinical efficacy of right trans-subclavian (RtTS) TAVI. Of a consecutive 423 patients who underwent TAVI, 32 cases performed via right and left subclavian access were analyzed. Imp

Malposition of the Selective Antegrade Cerebral Perfusion Catheter Identified by Carotid Ultrasonography: A Case Report.

Cerebral hypoperfusion can occur during aortic surgery. We present a case in which malposition of the selective antegrade cerebral perfusion (SACP) catheter was identified using carotid ultrasonography. A 66-year-old man underwent total arch replacement. SACP was initiated via the brachiocephalic artery following deep hypothermic circulatory arrest. However, right forehead regional oxygen saturation (rSO2) remained unchanged. Carotid ultrasonography revealed no blood flow in the right common carotid artery, suggesting catheter malposition into the right subclavian artery. After a slight cathet

Bailout Maneuvers to Avoid Brain Infarct During Aneurysm Exclusion Using the Nexus™ Endograft: A Case Report.

The aim of this study is to report our experience with a complication that occurred during a single-branch aortic arch endograft deployment for a symptomatic aortic aneurysm exclusion. An 85-year-old patient was diagnosed with an aortic arch and descending aortic aneurysm associated with chest pain and dyspnea. After a multidisciplinary discussion, we selected an endovascular approach using an off-the-shelf device for the arch (Nexus™ endograft; Endospan) with a single branch for the brachiocephalic trunk (BCT). Supra-aortic trunks debranching (right common carotid-left subclavian bypass and l

Management of recurrent ascending aortic aneurysm with bovine arch and aberrant right subclavian artery utilizing bilateral carotid to subclavian transposition followed by thoracic endovascular aneurysm repair.

We present a case of a 68-year-old man with variant anatomy and a history of prior Bentall procedure with open ascending aortic aneurysm repair, who presented with a recurrent pseudoaneurysm at the previous surgical distal anastomosis. His management was complicated by an aberrant right subclavian artery and bovine arch anomaly. The patient was managed using a staged approach consisting of a bilateral carotid to subclavian transposition followed by thoracic endovascular aneurysm repair. This technique allowed for the successful placement of an endograft to exclude both the aneurysm sac and the

Transcatheter stent implantation for long-segment coarctation in a young child: technical considerations and vascular safety using a large-diameter covered stent a case report.

BACKGROUND: Severe long-segment coarctation of the aorta in children is traditionally managed surgically; however, transcatheter stent implantation is emerging as a viable alternative in selected cases. The use of large-diameter covered stents in small children raises concerns regarding vascular access, procedural safety, and future reintervention. CASE PRESENTATION: An 8-year-old male child (21.7 kg) with global developmental delay presented with breathlessness. Clinical examination revealed radio-femoral delay, absent lower limb pulses, and upper limb hypertension (130/80 mmHg). Echocardiogr

Case Report: HHS-spectrum distal palmar arterial thrombosis in a volleyball athlete with provocative-position thoracic outlet narrowing.

BACKGROUND: Thoracic outlet syndrome (TOS) is a rare condition (∼1-3 per 100,000) caused by neurovascular compression at the thoracic outlet. Arterial involvement is uncommon and may result from chronic or dynamic subclavian artery compression leading to endothelial injury, thrombosis, and distal ischemia. Hypothenar hammer syndrome (HHS) is an uncommon cause of hand ischemia due to repetitive trauma to the ulnar artery and palmar circulation and has been reported in athletes engaged in repetitive high-impact hand activities (e.g., baseball, karate, golf, volleyball). CASE PRESENTATION: We re

Adenosine-induced Flow Arrest to Facilitate Control of a Basilar Artery Injury during Craniotomy for Petroclival Meningioma: A Case Report.

Intraoperative hemorrhage is a life-threatening complication during neurosurgery, especially in posterior fossa surgery, where critical vasculature and brain structures are present. Adenosine has been used in neurovascular surgery, particularly in the management of intraoperative aneurysm rupture and hemorrhage for its ability to produce transient flow arrest. This case report describes a novel application for adenosine, in which adenosine-induced flow arrest was used to facilitate control of a vascular injury sustained during meningioma surgery. A 46-year-old female diagnosed with a petrocliv

Yasui Procedure with Aberrant Right Subclavian Artery Patch for Interrupted Aortic Arch (Type B).

Reconstructing an interrupted aortic arch requiring a Damus-Kaye-Stansel (DKS) anastomosis can result in "gothic arch" formation due to extensive aortic mobilization, which is linked to late hypertension and bronchial compression. This repair is further complicated in the presence of an anomalous aberrant right subclavian artery (RSCA). We describe the use of a free aberrant RSCA patch (rather than a pedicled flap) to reconstruct the posterior aortic arch wall in 2 infants, both undergoing a staged Yasui procedure for interrupted aortic arch (type B) with hypoplastic left ventricular outflow t

Contrast-Induced Encephalopathy With Concomitant Procedure-Related Microembolic Ischemic Lesions Following Subclavian Artery Stenting.

Contrast-induced encephalopathy (CIE) is a rare and underrecognized neurological complication following exposure to iodinated contrast media. Because its clinical presentation frequently mimics acute ischemic stroke, early differentiation is critical to avoid misdiagnosis and inappropriate management. Although CIE has been described after coronary and cerebral angiography, its occurrence following supra-aortic interventions such as subclavian artery stenting remains exceptionally rare. We report the case of a 68-year-old woman with diabetes mellitus and a prior history of percutaneous coronary

Influence of moderate left subclavian artery stenosis on outcomes after coronary bypass surgery in patients requiring hemodialysis.

OBJECTIVES: Left subclavian artery stenosis in patients requiring hemodialysis can cause coronary steal syndrome via the left internal thoracic artery (LITA) graft to the left anterior descending artery (LAD), potentially leading to graft failure. This study evaluated the influence of moderate left subclavian artery stenosis on long-term outcomes following LITA-LAD coronary artery bypass grafting. METHODS: Among 1744 patients undergoing primary isolated coronary artery bypass grafting, 104 patients requiring hemodialysis with left upper limb arteriovenous fistulas and preoperative contrast-en

Extra-Carotid Arterial Webs: A Single-Center Series and Systematic Review.

BACKGROUND: Carotid webs are increasingly recognized as a cause of ischemic stroke, but less is known about morphologically similar lesions in other arteries. We present the first study characterizing the clinical and radiographic features of extra-carotid arterial webs through a single-center case series and systematic review. METHODS: Patients with possible extra-carotid webs were identified from 2017 to 2025 using a natural language processing search of radiology reports at our institution. Candidate cases underwent imaging review with multiplanar and 3-dimensional reconstructions to disti

Review of chest injuries in a trauma centre without local cardiothoracic surgery.

BACKGROUND AND AIMS: The vast majority of UK Major Trauma Centres either have on site cardiothoracic surgery or are located in proximity to a cardiothoracic centre. One exception is Ninewells Hospital, Dundee, which is located 70 miles from the nearest cardiothoracic department. The nature of this atypical arrangement and its implications are reviewed. METHODS AND RESULTS: The Scottish Trauma Audit group (STAG) was searched for details of patients sustaining a thoracic injury and presenting to NHS Tayside over a 17 month period. Data were analyzed for demographic information, mechanisms of in
