Exploratory topological modeling of reported frequencies of aortic arch branching variations.

Anatomical variations in the branching patterns of the aortic arch affect endovascular access, surgical planning, and cerebral perfusion strategies. However, the quantitative relationship between branching topology and the frequency distribution of these variations remains unclear. The study aimed to explore whether topological differences among final aortic arch branching configurations are associated with their reported frequencies. Seven pooled type-level frequency estimates from a published meta-analysis were analyzed. Node-Shift and Edge-Loss-Change Scores were defined from the final bran

Atypical presentation of subclavian steal syndrome mimicking transient ischemic attack: successful endovascular management with angiographic correlation-a case report.

BACKGROUND: Subclavian steal syndrome is an uncommon vascular disorder caused by stenosis or occlusion of the subclavian artery proximal to the vertebral artery origin. It classically presents with vertebrobasilar symptoms such as vertigo, syncope, or diplopia; however, atypical transient neurological presentations may delay diagnosis. CASE PRESENTATION: A 71-year-old patient presented with recurrent slurred speech and left upper limb weakness without vertigo or diplopia. Examination revealed absent measurable blood pressure and absent radial pulse in the left arm, carotid bruits, and mild di

Differential Peri-Strut Vascular Healing After Stent Deployment in Swine Subclavian and Carotid Arteries.

PURPOSE: Vascular healing after stent implantation is influenced by local biomechanical and anatomical environments. Swine carotid and subclavian arteries are commonly used in preclinical vascular device studies; however, regional differences in peri-strut healing responses between these vascular territories remain incompletely defined. This study compared histopathological vascular healing after stent implantation in swine carotid and subclavian arteries. MATERIALS AND METHODS: Self-expanding stents were implanted in both carotid and subclavian arteries in the same domestic swine according t

From the Thoracic Outlet to the Brachial Plexus: A Rare Presentation of Pediatric Ewing Sarcoma.

Ewing sarcoma of the chest wall (CWES) is a highly aggressive mesenchymal tumor, the incidence of which in children under five years of age remains extremely rare. Primary involvement of the first rib and the apex of the thorax presents specific anatomical challenges, leading to severe compressive complications at an early stage. We report the case of a four-year-and-two-month-old girl admitted for right-sided neuropathic arm pain that had been present for three months and was initially attributed to a minor trauma. The gradual onset of upper extremity muscle atrophy, motor deficit predominant

Physician-modified fenestrated thoracic endovascular aortic repair for a rapidly developing infectious distal aortic arch aneurysm after intravesical bacillus Calmette-Guérin therapy.

An 84-year-old man developed hemoptysis 3 weeks after receiving intravesical bacillus Calmette-Guérin therapy for bladder cancer. Computed tomography revealed a new saccular distal aortic arch aneurysm involving the origin of the left subclavian artery, with periaortic inflammation. Computed tomography performed 3 months earlier had shown no aneurysm. Urine polymerase chain reaction testing was positive for the Mycobacterium tuberculosis complex, supporting the diagnosis of a bacillus Calmette-Guérin-related infectious aortic aneurysm. Given the prohibitive surgical risk, emergency endovascula

Subclavian-to-internal carotid artery bypass with carotid endarterectomy for total occlusion of the common carotid artery under awake neurologic monitoring: a case series.

BACKGROUND: Total occlusion of the common carotid artery (CCA) from its origin is rare, occurring in less than 1% of brachiocephalic stenoses. Surgical revascularization is technically demanding, especially with tandem severe ipsilateral internal carotid artery (ICA) stenosis. Procedures under regional anesthesia with continuous neurologic monitoring are rarely described. OBJECTIVE: To evaluate outcomes of subclavian-to-internal carotid artery bypass with carotid endarterectomy (CEA) in symptomatic patients with total CCA occlusion and severe ipsilateral ICA stenosis under regional anesthesia

Anterior Spinal Artery Aneurysms Associated with Coarctation of the Aorta: A Focused Narrative Review with an Illustrative Case.

Anterior spinal artery (ASA) aneurysms are rare causes of subarachnoid hemorrhage (SAH), and coarctation of the aorta (CoA) may recruit the spinal arterial circulation into a high-flow collateral network. We report the case of a 58-year-old woman who was resuscitated after pulseless electrical activity arrest. Noncontrast CT demonstrated SAH concentrated in the cervical spinal canal with limited intracranial extension. Cervically extended CT angiography identified a C5-6 ventral spinal aneurysm, severe thoracic CoA with extensive collateralization, and diffuse left subclavian artery hypoplasia

Kommerell diverticulum in right aortic arch with aberrant left subclavian artery: Routine resection or selective management?

Right aortic arch with aberrant left subclavian artery (RAA-ALSA) is one of the most common complete vascular rings encountered in pediatric cardiovascular practice. The associated Kommerell diverticulum (KD), a remnant of the primitive fourth dorsal aortic arch from which the aberrant subclavian artery arises, forms part of the posterior ring and may contribute importantly to persistent airway and esophageal compression. Historically, repair consisted of division of the ligamentum arteriosum. Recognition that posterior compression can persist after ligamentum division has led many surgeons to

Surgical repair of traumatic right subclavian artery pseudoaneurysm after war shrapnel injury: A case report.

A rare right subclavian artery pseudoaneurysm developed after a war-related shrapnel injury in a 46-year-old female with a prior thyroidectomy. There was no initial bleeding or neurologic deficit, and initial noncontrast computed tomography findings were negative. One month later, the patient presented with chest pain, hoarseness, and dysphagia. A chest X-ray showed tracheal compression, Doppler ultrasound revealed leakage from the posterior wall of the right subclavian artery, and computed tomography angiography confirmed a 6 × 7 cm pseudoaneurysm causing 70% tracheal narrowing. Open repair w

[Subclavian steal syndrome: a case report].

An 80-year-old female presented to the emergency department with dizziness, generalized weakness, and exertional dyspnea. Physical examination revealed a significant inter-arm systolic blood pressure discrepancy. Computed tomography angiography (CTA) of the chest demonstrated high-grade stenosis at the origin of the left subclavian artery, correlating with both the clinical symptoms and the hemodynamic findings. This case underscores the diagnostic utility of CTA in ensuring a prompt and precise evaluation of subclavian artery stenosis in patients presenting with inter-arm blood pressure diffe

A snare-assisted pull-up technique for side-branch component delivery during thoracic endovascular aortic repair for distal aortic arch aneurysm using Gore Thoracic Branch Endoprosthesis.

We report a case of thoracic endovascular aortic repair using a thoracic branch endoprosthesis for aneurysm enlargement caused by an endoleak after prior fenestrated repair. A 75-year-old man with previous left subclavian artery stenting presented with aneurysm growth. Advancement of the side-branch component through the pre-existing stent was difficult because of severe angulation, and standard push-up techniques failed. A snare-assisted pull-up technique via a pre-existing 6F brachial sheath enabled direct capture and controlled pull-and-push advancement of the device. This method may serve

Contemporary Management of Subclavian and Axillary Artery Aneurysms in Connective Tissue Disease: Navigating Therapeutic Uncertainty in a Rare Vascular Disease Without Dedicated Guidelines.

BACKGROUND: Subclavian artery aneurysms (SAAs) represent one of the rarest forms of peripheral arterial aneurysmal disease. Despite their low incidence, management in patients with underlying connective tissue disorders poses specific challenges related to intrinsic vessel fragility. Because of their rarity, the available literature is largely confined to case reports and small, heterogeneous retrospective series, and management remains largely experience-based rather than evidence-based. Explicit guidance from the Society for Vascular Surgery and the European Society for Vascular Surgery is l

Mechanical Thrombectomy of the Left Brachial Artery for Graft Occlusion after Debranching Thoracic Endovascular Aortic Repair: A Case Report.

In patients who have undergone debranching thoracic endovascular aortic repair, the access route and system for mechanical thrombectomy must be carefully selected on a case-by-case basis. We report an 82-year-old man in whom mechanical thrombectomy was successfully performed via the left brachial artery for occlusion of a bypass graft. He was admitted with impaired consciousness and was diagnosed with left common carotid artery occlusion on magnetic resonance angiography. The patient had previously undergone debranching thoracic endovascular aortic repair with a right subclavian artery-left co

Percutaneous Transaxillary Access for Large-Bore Cardiovascular Interventions: Evidence, Technique, and Complication Management.

Percutaneous transaxillary access is an established alternative to transfemoral large-bore arterial access for transcatheter structural heart interventions and mechanical circulatory support (MCS). Although useful for overcoming hostile iliofemoral anatomy and providing prolonged support and patient mobilization in case of MCS, the axillary/subclavian territory poses distinct technical and anatomical challenges that demand meticulous pre-procedural imaging, patient selection, accurate puncture under ultrasound and angiographic guidance, preservation of bailout access, and familiarity with sutu

Excision of a giant cervical-thoracic aggressive fibromatosis: a case report and review of literature.

BACKGROUND: Aggressive fibromatosis is a rare, locally invasive tumor with a marked tendency to recur, and it is often diagnosed only after the lesion has already reached a considerable size. Although histologically benign, these tumors can behave aggressively at the local level. CASE DESCRIPTION: We report the case of a male patient with a large mass occupying the entire left hemithorax and extending into the left side of the neck. The tumor compressed the surrounding organs and produced corresponding symptoms. It encircled the left subclavian artery and vein. The patient underwent a complet
