A case of successful catheter removal with ventricular septal defect occluder after accidental misplacement of left subclavian vein catheter into aortic arch.

The subclavian vein is one of the most frequently-used approaches for central venous catheterization. However, it demands high technical skills and accurate puncture positioning. In case of accidental misplacement into the artery, it is impossible to achieve sufficient local hemostasis. Moreover, removing the catheter without assistance may lead to severe complications or even death. This paper reports a case of an elderly patient with multiple underlying diseases. After left subclavian vein catheterization accidentally entered the aortic arch, a ventricular septal defect occluder was used to

Computed Tomography (CT) Guided Implantation of a Totally Implantable Venous Access Port (TIVAP) through Subclavian Vein.

Totally implantable venous access ports (TIVAPs) have been proven to be safe and effective and are commonly used in cancer patients. Here, we introduce the entire procedure of CT-guided implantation of a TIVAP through the subclavian vein. A total of 283 nasopharyngeal carcinoma patients, including 215 males and 68 females, underwent TIVAP through the right subclavian vein, with an average age of 46.4 years (range 13-74). One patient experienced a failed right subclavian vein puncture but was able to successfully undergo a left subclavian vein puncture guided by ultrasound. The average operatio

Central Vein stEnosis screeNing using Ultrasound guided Sniff test: A prospective diagnostic accuracy study (C-VENUS).

BACKGROUND: Central venous stenosis (CVS) is a clinically significant complication in patients who depend on long-term hemodialysis access. While digital subtraction angiography remains the diagnostic gold standard, it is invasive, costly, and exposes patients to contrast and radiation. This study examined the diagnostic value of a simple, non-invasive, radiation-free alternative-the ultrasound-guided "sniff test," which assesses dynamic changes in subclavian vein diameter during a forceful sniff to screen for CVS. Despite its potential advantages, the diagnostic performance of this method has

Central venous versus short midline catheter in difficult intravenous access patients: a randomised clinical pilot trial.

BACKGROUND: Patients with difficult intravenous access (DIVA) are at increased risk of delays, discomfort and complications due to multiple failed intravenous access attempts. However, evidence comparing commonly used alternatives, short midline catheters (SMLs) and central venous catheters (CVCs) in this population is limited. OBJECTIVE: To evaluate the feasibility of a larger randomised controlled trial comparing SMLs with CVCs in DIVA patients using predefined feasibility outcomes. DESIGN: This trial was a pragmatic, open-label, single-centre, randomised controlled pilot trial with 1:1 ra

The risk of central line-associated bloodstream infection between internal jugular and subclavian sites in critically ill patients: a multicenter cohort study.

BACKGROUND: Central venous catheterization is a routine procedure in clinical practice, but it can result in severe and costly central line-associated bloodstream infections (CLABSIs), the risk of which may vary by insertion sites. This study sought to evaluate and compare the incidence of CLABSI in critically ill patients between internal jugular and subclavian sites, and to explore the correlation between insertion site and risk of CLABSI. METHODS: This research encompassed critically ill patients with Central venous catheters (CVCs) placed in the internal jugular or subclavian site from IC

Case Report: Percutaneous portal-central venous bypass: a novel salvage therapy for refractory variceal bleeding in TIPS-ineligible patients with portal vein tumor thrombus.

BACKGROUND: Acute esophagogastric variceal bleeding (EGVB) is a lethal complication in patients with hepatocellular carcinoma (HCC) complicated by portal vein tumor thrombus (PVTT). While transjugular intrahepatic portosystemic shunt (TIPS) is a standard treatment for portal hypertension, it is often contraindicated or technically challenging in patients with extensive main portal vein occlusion (Vp4 type) or severe hepatic dysfunction. CASE SUMMARY: We report the first clinical application of a novel salvage technique, percutaneous portal-central venous bypass (PPCV), in a 56-year-old man wi

Recanalization of superior vena cava occlusion using coronary guidewire and microcatheter techniques in a maintenance hemodialysis patient: A case report.

Superior vena cava (SVC) occlusion is a major complication in hemodialysis patients, impairing access function and requiring timely intervention. A 69-year-old man undergoing maintenance hemodialysis for 8 years presented with a 1-month history of inadequate flow in a left upper-arm arteriovenous graft (AVG). Physical examination revealed prominent chest and abdominal wall veins with a caput-medusae pattern. DSA showed multiple venous stenoses, and complete SVC and right subclavian vein occlusion. Using coronary guidewires and microcatheters, the occluded segments were successfully recanalized

Spontaneous Catheter Fracture Leading to a Retained Fragment After Central Venous Access Port Removal: Should Preoperative Chest X-rays Be Obtained?

Central venous access ports are used to administer chemotherapy, antibiotics, and total parenteral nutrition, and for frequent blood draws. The device consists of a subcutaneous reservoir (port) and an intravascular piece (catheter). Access is achieved via direct venipuncture of the subclavian, internal jugular, or cephalic vein with the tip at the cavoatrial junction. Retained central venous access ports are relatively uncommon. In most cases, an indwelling time greater than two years can cause an adhesive fibrous sheath to form around the catheter, contributing to more difficult removal. Spo

Complications of Central Venous Line Insertion: A Case Series.

Central venous catheter (CVC) insertion is a commonly performed procedure in critically ill patients. While generally safe, it carries risks of potentially serious complications, particularly when performed without ultrasound guidance or in emergent settings. We report a series of four patients who developed uncommon but significant complications following CVC placement. The first case describes the right internal mammary artery aneurysm after malplacement of CVC line. The second case outlines the formation of sternocleidomastoid hematoma after internal jugular vein access. The third case invo

A Jugulo-Cephalico-Subclavian Venous Complex Associated With a Double Fenestration of the External Jugular Vein: A Dissection Report.

The purpose of the present cadaveric report is to describe the unique combination of 2 rare variations of the venous system. A rare anatomic variation was discovered during the dissection of a 70-year-old donated male cadaver. This unique case revealed an atypical supraclavicular course and termination of the cephalic vein at the confluence of the external, internal, and subclavian veins, coexisting with the external jugular vein's double fenestration before its confluence. This unusual finding, the jugulo-cephalico-subclavian venous complex, holds significant clinical value.

Using Three-Dimensional Quantitative CT Models to Compare Density Patterns Across Pelvic Fractures.

BACKGROUND: Between 1990 and 2007, the population-adjusted incidence of pelvic ring fractures increased from 27.24 to 34.30 per 100 000 capita. Measuring bone density would aid in surgical planning for patient-specific treatment after traumatic pelvic injuries to decrease the need for repeat surgery. Opportunistic quantitative CT has recently been used to provide bone density data without the need for an additional measurement through dual-energy radiograph absorptiometry, in addition to providing more detailed information about the density distribution. The purpose of this study was to determ

Compression of the Left Brachiocephalic Vein by a Type II Right Aortic Arch: A Rare Vascular Anomaly With Unique Clinical Presentation.

Right-sided aortic arch (RAA) is an uncommon anatomical variation of the thoracic vasculature, occurring in approximately 0.1% of adults. In nearly half of these instances, the left subclavian artery follows an abnormal course. The left subclavian artery typically emerges from a tapered dilation at its origin from the aorta, referred to as Kommerell's diverticulum (KD). Based on available literature, only a few cases have been documented. We describe a 62-year-old female patient who presented with a painless chest bump persisting for four weeks. She reported no symptoms such as cough, shortnes

Surgical Management of Right Innominate Interarteriovenous Lymph Node Metastasis in Thyroid Carcinoma: Clinical Features and Operative Experience.

BACKGROUND: The right innominate interarteriovenous lymph nodes (RIAVLN) represent a distinct nodal group situated between the right innominate artery and vein. This area lies outside the conventional level VI-VII boundaries in thyroid carcinoma surgery and is seldom addressed in standard guidelines. Metastasis in this region is difficult to detect and surgically challenging due to its proximity to major vascular structures. This study aimed to analyze the clinical characteristics and surgical management of RIAVLN metastasis in patients with thyroid carcinoma. METHODS: We retrospectively revi

Anatomical Landmarks for Decompression of Common Peroneal Nerve: A Cadaveric study.

BACKGROUND: Common peroneal nerve (CPN) neuropathy may result from trauma or iatrogenic injury. Complete decompression requires precise incision placement. This study aimed to define surface landmarks and incisional guidelines for optimal CPN decompression. METHODS: Thirty lower limbs from 15 cadavers were studied with knees flexed at 90 degrees. Dissection exposed the CPN, allowing measurement of compression sites between the intermuscular septa and distances from key anatomical landmarks. The CPN angle to the fibular axis and distances from the tibial tubercle and crest were measured by 2 o

Bailout Technique for a Detached Distal Tip of a Tissue-Stabilization Sheath During Transvenous Lead Extraction.

CASE SUMMARY: A rare complication of distal tip detachment of the Evolution Steady Sheath has been reported previously. We describe a case in which a detached distal tip located in the innominate vein was successfully retrieved via the subclavian vein using a balloon-assisted technique during transvenous lead extraction. An 85-year-old man with pacemaker infection underwent lead extraction, during which the distal tip detached. A pull-through system was established, and balloon traction enabled successful retrieval through the pocket access. TAKE-HOME MESSAGE: The balloon-assisted pull-throug

The 10-year course of a complex recurrent venous thoracic outlet syndrome.

Venous thoracic outlet syndrome classically presents with upper extremity venous thrombosis (Padget-Schroetter syndrome). The established treatment pathway includes thrombolysis with early surgical decompression of the thoracic outlet. This case is a 20-year-old woman initially treated erroneously with thrombolysis and subclavian vein stenting, without surgical decompression. Over the next decade, she experienced multiple recurrent complications, requiring several interventions. Despite multiple attempts to overcome the original procedure, her symptoms have reoccurred owing to restenosis and i
