Conventional robotic cardiac approaches generally require multiple intercostal access sites to establish instrument triangulation. This study presents an in-house-developed single-port robotic platform designed around cardiac manipulation requirements and evaluates its initial task-specific feasibility in one pig. The system integrates a common entry guide, three-dimensional endoscopic visualization, articulated instruments, and master-slave teleoperation. Robot-assisted internal mammary artery (IMA) dissection was performed on the beating heart through retractor-assisted intercostal access. M

Hemothorax caused by intercostal artery rupture is a potentially life-threatening disease. A 66-year-old woman presented to our emergency department with left back pain. She had undergone surgery for left hemothorax 14 years ago. Contrast-enhanced chest computed tomography revealed active bleeding from the left dorsal ninth intercostal artery near the descending aorta. Subsequently, she developed hypovolemic shock. Based on her medical history, the patient was suspected of having adhesions in the left thoracic cavity. Surgery was considered a time-consuming approach to control the bleeding. Ou

BACKGROUND: Ankylosing spondylitis (AS) significantly increases vulnerability to severe spinal injuries from minor trauma due to rigid "bamboo spine" morphology and secondary osteoporosis. Isolated intercostal artery rupture without rib fractures is extremely rare in general trauma populations. The concurrent occurrence of multilevel spinal fractures with complete spinal cord injury and occult intercostal artery hemorrhage following low-energy blunt trauma in AS patients has rarely been documented, and the underlying pathophysiological mechanisms remain unclear.

BACKGROUND: The aberrant right subclavian artery (ARSA) is the most common anomaly of the arch of aorta and is significantly associated with a non-recurrent laryngeal nerve (NRLN). Nevertheless, the co-occurrence of ARSA with complex thoracic wall vascular remodeling is scarcely described in the current literature. This exploration describes an unusual group of neurovascular variations with important surgical significance.

Surgical resection constitutes the cornerstone of treatment for lung cancer, a disease with high global incidence. Among postoperative complications, delayed fatal hemorrhage occurring beyond 30 days postoperatively has garnered scant attention in clinical and research settings. Notably, there are no documented cases of vascular rupture and bleeding induced by chronic mechanical friction between the bronchial stump and pulmonary artery trunk to date. Following pulmonary lobectomy, the anatomical proximity between the bronchial stump and pulmonary artery trunk creates a potential risk of delaye

BACKGROUND: Chronic Obstructive Pulmonary Disease (COPD) is characterized by respiratory and extrarespiratory components referring both to systemic complications of COPD, like skeletal muscle myopathy, weight loss and others, and frequently associated comorbidities, interesting various organs and systems (cardiovascular diseases, malignancies, osteoporosis, diabetes, etc.). These comorbidities may increase the rate of hospitalization of COPD patients and have a huge effect on the outcomes of the respiratory disease. Inhalation therapy of COPD with bronchodilators and steroid is primary driven

In response to a recent case report published in the International Journal of Legal Medicine entitled "A case of hemorrhage at the junctions of the posterior intercostal arteries-a vital sign?", we wish to corroborate the findings of periadventitial hemorrhages at the junctions of the posterior intercostal arteries in cases of suicidal hanging and to reveal two other novel aortic lesions associated with hanging. In our ongoing prospective study, we identified six cases of aortic intimal ruptures among 257 suicidal hanging deaths, along with the novel observation of subintimal hemorrhages-both

Chest tube placement is often a lifesaving but also a potentially hazardous intervention, particularly in trauma settings. A male bodybuilder in his 30s presented at the emergency department after a single-vehicle motorcycle accident. On presentation, the left side of the chest showed decreased movement and subcutaneous emphysema. A chest tube was inserted in the left fifth intercostal space. During insertion, the heart was objectively close to the introduction place, and a wide complex tachycardia was observed, which resolved spontaneously. The ECG thereafter showed ST-segment elevation in pr

BACKGROUND: Spinal Cord Injury is a disastrous complication of thoracoabdominal aortic aneurysm surgery. This study is to assess the effectiveness and safety normothermic artery bypass and visceral-anastomosis-first strategy for thoracoabdominal aortic aneurysm repair.
METHODS: Normothermic artery bypass and visceral-anastomosis-first strategy prioritizes the reconstruction of visceral and other distal vessels, with intercostal arteries serving as the physiological blood supply at high pressure during this period. Reconstruction of intercostal arteries and proximal anastomosis is then perform

A 52-year-old woman was referred to our hospital with a chief complaint of difficulty in swallowing and coughing while eating. Enhanced computed tomography (CT) revealed a Kommerell diverticulum with right aortic arch and aberrant left subclavian artery. The diverticulum compressed the esophagus and trachea. We avoided total aortic arch replacement because there were risks of circulatory arrest, selective cerebral perfusion and neurological complication including injury to recurrent laryngeal nerve. Therefore, we scheduled two-stage repair of the diverticulum. First, we performed axillo-axilla

The spontaneous rupture of a subcostal (12th intercostal) artery is exceptionally rare and could be fatal, requiring early diagnosis and treatment. Only one case of intercostal artery (ICA) bleeding in a patient undergoing hemodialysis (HD) has been reported. We additionally describe a 41-year-old man undergoing HD who presented with a spontaneous hemoperitoneum and shock resulting from a subcostal artery rupture. He initially complained of diffuse abdominal pain and dizziness at the emergency room. His abdomen was bloated, and there was tenderness in the right upper quadrant area. Enhanced co

BACKGROUND: The occurrence of massive retroperitoneal hematoma caused by intercostal artery bleeding is exceedingly uncommon.
CASE PRESENTATION: A middle-aged male presented to the hospital after a fall. Computed tomography scan revealed a massive retroperitoneal hematoma without any evidence of organ or major vessel rupture. The angiogram revealed extravasation from a branch of the twelfth intercostal artery, and successful transcatheter arterial embolization was performed on this specific artery.
CONCLUSIONS: The possibility of intercostal artery rupture should be considered in cases of re

Aortic coarctation is diagnosed in approximately 5% of adult patients with congenital heart disease and is commonly diagnosed through the close examination of hypertension. Various surgical strategies for adult coarctation have been recently reported. Generally, aortic replacement may require blood transfusion in case of injury of the well-developed collateral vessels. Therefore, in order to secure an operative safety, we preoperatively used a medical image viewer to identify the abnormal vessels by three-dimensional computer graphics (3DCG) reconstruction. A 34-year-old male patient was refer

The authors present the case of a 58-year-old man found hanging from a radiator by his shoelaces. The time of death was approximately 6 h before the body was discovered. An autopsy was performed approximately 24 h after the body was found, which revealed hemorrhages in the thoracic aorta at the junctions of the posterior intercostal arteries. Before autopsy, a routine whole-body CT scan was performed. Histologic examination of the aorta and the posterior intercostal arteries revealed a fresh hemorrhage into the tunica adventitia of the aorta. To our knowledge, there is no case description of s

RATIONALE: Subclavian arterial injury due to blunt trauma is rare but can have devastating outcomes. Massive hemorrhage or limb ischemia might develop depending on the extent of damage, and open repair might be necessary to salvage the limb. However, life-saving treatments should be prioritized in critically unstable patients.
PATIENT CONCERNS: A 21-year-old male patient who was transferred to our trauma center following a motorcycle accident. Abdominal and chest computed tomography (CT) revealed right renal injury and massive hemothorax with several rib fractures in the right chest.

OBJECTIVE: To report the use of a pericardial patch graft to repair a post-traumatic caudal vena cava cicatrix in a dog.
STUDY DESIGN: Case report.
ANIMALS: A 1 year-old Rottweiler presenting with ascites following presumed blunt thoracic trauma.
METHODS: Kinking of the caudal vena cava and resultant Budd-Chiari-like syndrome was diagnosed on echocardiography and computed tomographic angiography (CTA). Surgical exploration via right sixth intercostal thoracotomy was performed. Release of the cicatrix was unsuccessful in reducing the pressure gradient and a pericardial patch graft repair of

We report a unique case of cervical anterior spinal artery (ASA) infarction in a 49-year-old male with hypercholesterolemia and sleep apnea. The patient experienced sudden cervical pain, quadriparesis, areflexia, and urinary incontinence after swallowing a large food bolus. Imaging revealed an infarction at the C3-C5 levels and an anomalous right vertebral artery (VA) originating from the thoracic aorta, tightly enclosed between the aorta and a vertebral column with an anterior osteophyte. This aberrant VA was the primary vascular supply to the ASA, with no contribution from the left VA or sup

BACKGROUND: Hemothorax caused by a right intercostal artery (ICA) injury behind the left atrium (LA) is a potentially fatal complication during pulmonary vein isolation. However, their anatomic relationship has not been fully elucidated.
OBJECTIVE: This study aimed to investigate the clinical anatomy of the right ICA in relation to the LA.
METHODS: This retrospective study included 100 patients (70.2 ± 10.6 years; 39.0% female) who underwent cardiac computed tomography. The patients were divided into sinus rhythm and atrial fibrillation groups. We focused on the distance between the LA and r

A woman in her 80s experienced a life-threatening complication of pacemaker implant consisting of subacute right ventricular lead perforation causing iatrogenic injury to an intercostal artery, resulting in a large haemothorax. A CT scan confirmed active bleeding from the fourth intercostal artery. The patient underwent cardiothoracic surgery via a median sternotomy approach, during which the source of the bleeding was sealed, a new epicardial lead was positioned, and the original lead was extracted. This case emphasises the potentially severe consequences of pacemaker lead perforation and sec

A 77-year-old female cadaver was observed to have a rare branching pattern of the right axillary artery (AA). The first part of the AA typically gives off only a superior thoracic artery (STA) but was observed to give off three branches in the case: a lateral thoracic artery (LTA), a thoracoacromial trunk, and a large common trunk (CT). The LTA travelled to provide a variant STA to the 1st and 2nd intercostal spaces. The CT provided an accessory LTA and accessory thoracodorsal artery before bifurcating into a subscapular artery (SA) and posterior humeral circumflex artery. As expected, the SA
