Hemodynamic and metabolic effects of hydrogen sulfide during porcine ischemia/reperfusion injury.

In awake spontaneously breathing mice, inhaling gaseous hydrogen sulfide (H2S) produced a "suspended animation-like" metabolic status with hypothermia and reduced O2 demand, thus protecting from lethal hypoxia. Murine models may be questioned, however, because due to their large surface area/mass ratio, rodents can rapidly drop their core temperature. Therefore, we investigated whether intravenous H2S (Na2S, sodium sulfide) would induce a comparable metabolic response in anesthetized and mechanically ventilated pigs. Because H2S was reported to improve heart function after myocardial ischemia,

Posterior acceleration as a mechanism of blunt traumatic injury of the aorta.

Rupture of the thoracic aorta is a leading cause of rapid fatality in automobile crashes, but the exact mechanisms of this injury remain unidentified. One commonly postulated mechanism is a differential motion of the aortic arch relative to the heart and its neighboring vessels caused by high-magnitude acceleration of the thorax. This paper investigates acceleration as an aortic injury mechanism using nine impact-sled tests with human cadaver thoraces. The test system utilized generates very high posteriorly directed thoracic accelerations with minimal compression of the chest. The sled tests

Structural and functional alterations in the rat lung following whole thoracic irradiation with moderate doses: injury and recovery.

PURPOSE: To characterize structural and functional injuries following a single dose of whole-thorax irradiation that might be survivable after a nuclear attack/accident. METHODS: Rats were exposed to 5 or 10 Gy of X-rays to the whole thorax with other organs shielded. Non-invasive measurements of breathing rate and arterial oxygen saturation, and invasive evaluations of bronchoalveolar lavage fluid, (for total protein, Clara cell secretory protein), vascular reactivity and histology were conducted for at least 6 time points up to 52 weeks after irradiation. RESULTS: Irradiation with 10 Gy re

Arterial trauma during central venous catheter insertion: Case series, review and proposed algorithm.

BACKGROUND: Percutaneous catheterization is a frequently-used technique to gain access to the central venous circulation. Inadvertent arterial puncture is often without consequence, but can lead to devastating complications if it goes unrecognized and a large-bore dilator or catheter is inserted. The present study reviews our experience with these complications and the literature to determine the safest way to manage catheter-related cervicothoracic arterial injury (CRCAI). METHODS: We retrospectively identified all cases of iatrogenic carotid or subclavian injury following central venous cat

The blue man: an unusual happy end of a spontaneous rupture of a coronary artery.

We report the case of spontaneous rupture of a coronary artery. It was that of a 56-year-old man admitted for dyspnoea and anterior thoracic pain. The most striking feature on physical examination was the marked cyanosis of his face, upper part of the thorax and the upper limb. The patient was haemodynamically unstable with tachycardia and hypotension. Cardiac tamponade was confirmed by echocardiography and computed tomography of the thorax. The patient was transferred for surgery. Emergency sternotomy revealed pericardial bloody effusion and a continuous bleeding around the posterior interven

Traumatic rupture of thoracic aorta in real-world motor vehicle crashes.

OBJECTIVE: Traumatic rupture of thoracic aorta (TRA) was reported in the literature to be a major cause of death in motor vehicle crashes. This study aims at evaluating the most relevant risk factors of TRA. It also aims at analyzing the types of TRA as a function of car crash conditions and rib cage fractures. METHODS: In-depth crash data collected from 1998 to 2006 as part of the Co-operative Crash Injury Study (CCIS) were retrospectively investigated to assess frontal, near-side, and far-side injury risks. This database includes 15,074 occupants with individual detailed autopsy reports. Mu

A novel non-sense mutation in the SLC2A10 gene of an arterial tortuosity syndrome patient of Kurdish origin.

Arterial tortuosity syndrome (ATS) is a rare autosomal recessive disorder in which patients display tortuosity of arteries in addition to hyperextensible skin, joint laxity, and other connective tissue features. This syndrome is caused by mutations in the SLC2A10 gene. In this article we describe an ATS girl of Kurdish origin who, in addition to arterial tortuosity and connective tissue features, displays stomach displacement within the thorax and bilateral hip dislocation. Clinical details of this patient have been reported previously. Sequencing of the SLC2A10 gene identified a novel homozyg

An innovative treatment concept for free flap reconstruction of complex central chest wall defects--the cephalic-thoraco-acromial (CTA) loop.

BACKGROUND: Loco-regional flaps are the method of choice for chest wall reconstruction. However there is a selected group of patients who require free flap reconstruction, when all other options are used up. A small subgroup of these patients was identified where the commonly used recipient vessels (Internal mammary A. + V., Thoraco-dorsal A. + V.) were no longer available. PATIENT AND METHOD: This group comprised 16 seriously ill patients in the period from 2000 to 2004. Underlying diseases were sternum osteomyelitis (10x), tumor (2x), and osteo-radionecrosis (4x). There were 10 women and 6

Injuries to the azygous venous system.

BACKGROUND: Azygous venous system injuries are rare. Although not commonly classified as a thoracic great vessel, the azygous system manifests morbidity and mortality that is similar to that of other great vessel injuries. METHODS: Over a 40 year period, data were retrospectively collected and charts reviewed from a vascular injury database. RESULTS: A total of 22 injuries to the azygous venous system were identified with 21 to the azygous vein and 1 to the hemiazygous vein. All were secondary to penetrating trauma, with 19 from gunshot wounds and 3 from stab wounds. Eight of 22 patients die

Risk factors for blunt thoracic aortic injury in children.

BACKGROUND/PURPOSE: Because blunt thoracic aortic injury is rare in children, a high index of suspicion is needed to identify this injury. The purpose of this study was to use a large national trauma database to define the risk factors for blunt thoracic aortic injury in children. METHODS: Using the National Trauma Database, the authors compared patient demographics, mechanism of injury, and associated injuries between children sustaining blunt trauma with and without a thoracic aortic injury. Factors independently associated with this injury were identified using multivariate methods. RESUL

The widened mediastinum in trauma patients.

Mediastinal widening is a frequent radiological finding in the emergency department patient. The causes of mediastinal widening can be divided into traumatic and nontraumatic mediastinal widening. An important association of moderate to high velocity trauma is the mediastinal haematoma. It may be the result of traumatic transsection of the aorta, or it may be due to bleeding from other mediastinal vessels. Before the era of multidetector spiral CT, angiography was the gold standard for the evaluation of patients with a widened mediastinum. Meanwhile, angiography as a risk-carrying invasive exa

Distal variations of the neurovascular pedicle of the serratus anterior muscle as a flap.

The serratus anterior muscle has recently been suggested as a versatile and reliable flap for reconstruction of complex craniofacial and neck lesions, extremity and sacroiliac region injuries, as well as intrathoracic and extrathoracic reconstruction procedures. The muscle has been used as a microvascular flap or a pedicled transfer and has been transferred in combination with other muscles, bones, and skin. We performed 15 dissections of adult axilla regions that were examined under x3.5 loupe magnification to collect anatomic data regarding the neurovascular pedicle of the serratus anterior

Assessment of a caudal external thoracic artery axial pattern flap for treatment of sternal cutaneous wounds in birds.

OBJECTIVE: To assess the use of a caudal external thoracic artery axial pattern flap to treat sternal cutaneous wounds in birds. ANIMALS: 16 adult Japanese quail. PROCEDURE: A cutaneous defect in the region of the mid-sternum was surgically created in all quail. In 6 quail (group I), an axial pattern flap was created from the skin of the lateral aspect of the thorax and advanced over the sternal defect. In 8 quail (group II), a flap was similarly created and advanced but the flap vasculature was ligated. All quail were euthanatized at 14 days after surgery and had necropsies performed. Secti

[Angina pectoris as initial clinical manifestation of ruptured abdominal aortic aneurysm with aortocaval fistula].

CASE REPORT: A 75-year-old patient was hospitalized for angina pectoris and tachycardia. Neither ECG nor laboratory examinations were diagnostic for myocardial infarction. Because of an abdominal thrill and a history of an aortic aneurysm a thoracic and abdominal CT scan was performed. It revealed a rupture of the infrarenal aortic aneurysm into the inferior vena cava. In an emergency operation the aortocaval fistula was closed and the infrarenal aorta including both Aa. iliacae was replaced by a prosthesis. The patient recovered rapidly and was discharged home on the 11th postoperative day.

Post mediastinal radiation coronary artery disease and its effects on arterial conduits.

Therapeutic mediastinal radiation predisposes patients to an increased risk of developing premature coronary artery disease. Other structures surrounding the heart within the radiation field are also susceptible to injury. This case report highlights the importance of recognizing concurrent damage to the internal mammary artery that, in this particular group of patients, may not be the ideal arterial conduit for coronary artery bypass surgery.

Contrast-enhanced helical computerised tomography in the investigation of thoracic aortic injury.

Aortic angiography is widely considered the 'gold standard' for the diagnosis of traumatic thoracic aortic injury. Unfortunately, thoracic aortic angiography has many disadvantages: the technique is invasive but, more importantly, it is not routinely available in all hospitals, necessitating transfer of critically ill patients. Contrast-enhancement helical computerised tomography (CEHCT) of the thorax is rapidly becoming available, especially in more district general hospitals, and has been shown to be as sensitive and specific in detecting aortic trauma as angiography. This technique has the

Mediastinal impalement with survival: a case report.

Mediastinal impalement injuries are rare and often fatal. Very few instances of survival after mediastinal impalement have been reported. We present the unusual case of an 18-year-old man who was involved in a motor vehicle crash in which a wooden fencepost intruded through the windshield and impaled him through the superior mediastinum. The patient remained hemodynamically stable and had no other significant injuries except a left pneumothorax. Arteriogram revealed a bovine aortic arch with the wooden piece passing over the aortic arch between the two brachiocephalic arteries at the precise p

Pathophysiology of vascular endothelium and circulating platelets: implications for coronary revascularisation and treatment.

Constant vasodilatation, inhibition of platelet and leukocyte adhesion, and local thrombolysis are the mechanisms through which an intact endothelial layer exerts its protective action on coronary circulation. A loss in these features is not only the first step in the development of atherosclerosis, but also a potent trigger for complications after revascularisation procedures. Percutaneous coronary interventions, particularly in the course of stenting, induce endothelial injury that can last up to months after the procedure. On the other hand, the preservation of endothelial function appears
