Sixty consecutive patients, except for one 7-year-old patient, their ages varying from 1 day to 12 months received total intravenous hyperalimentation (TIH). Out of 116 cases of abdominal trauma observed, 6 (23%) consisted of hepatic lesions. In 2 cases (33.3%) the hepatic lesion was isolated. In the other cases, it was associated with lesions to the diaphragm, duodenum, pancreas or thorax. After emphasis on the gravity of such lesions and the high mortality rate (33.3% in the cases observed) the factors contributing to the extremely gloomy prognosis are discussed and the basic therapeutic approach to injuries of varying gravity is outlined.

The problems of surgical management of injury to blood vessels of the thoracic outlet are illustrated by a case report of clavicular excision to gain direct access to the damaged vessels. The article reviews the surgical procedure and documents its efficacy and the lack of subsequent deformity or dysfunction. It recounts the hazards of unsuccessful attempts to repair the clavicle. Last, it discusses the beneficial effects of clavicular resection carried out for reasons other than vascular injury.

Survival of patients with posttraumatic thoracic aortic rupture depends on early diagnosis. It is frequently stated that fracture of the first or second ribs and mediastinal widening are findings suggestive of thoracic aortic rupture. We found that the probability of sustaining thoracic aortic rupture is the same for patients with upper rib fractures as for those with other rib fractures (1/64 versus 5/149; p = 0.85). During 10 years of serious strife the Royal Victoria Hospital, Belfast, has dealt with trauma inflicted by a wide variety of missiles. Injuries involving 316 vessels were sustained by 192 patients. Fourteen male patients with chronic bronchitis and hypoxia had a lateral radiograph of the pituitary fossa. Nine of the 14 had definite or probable abnormalities, a significantly higher frequency (p less than or equal to 0.01) than is represented by the two out of 14 age-matched men from a control group with head injuries. During an 8-year period, 191 consecutive patients were treated for arterial injuries. Rupture of the aorta frequently occurs in major blunt trauma to the thorax, but few patients survive long enough to develop a chronic aneurysm. Trauma to the thorax represents a significant portion of injuries seen in an inner-city emergency room. Blunt injuries to branches of the aortic arch are not unusual and must be considered in any patient surviving deceleration or crush injury. Acute RDS is a clinical and pathologic phenomenon with many causes. Fat labeled with triolein-(131)I was introduced through a burr hole into single tibial marrow cavities and the hole was sealed. The radioactivity over the thorax was monitored for 2-5 hours.
