Myocardial Perforation From a Fractured Rib During Cardiopulmonary Resuscitation.

Posterior left ventricular perforation from fractured posterior rib. Mid-posterior left ventricular perforation with communication between the left ventricular cavity and pericardial space from a fractured posterior rib (yellow arrow) sustained during cardiopulmonary resuscitation (CPR) with bubbles (orange arrow) and a large pericardial effusion. This case highlights the importance of post-CPR echocardiography in the assessment of CPR-associated cardiac injuries.

First Reported Case of Iatrogenic Cardiac Tamponade Following Chest Drain Insertion for Tension Pneumothorax in a Premature Newborn.

Cardiac tamponade is a challenging clinical situation in preterm newborns. We report the first case of an iatrogenic cardiac tamponade secondary to direct myocardial disruption with pericardial penetration following unsuccessful attempts to drain a right tension pneumothorax in a 34-week premature female newborn. The pathophysiologic mechanisms involved are discussed.

Nail Gunshot Induced Hemopericardium, Detected by Point-of-care Ultrasound (POCUS) in the Emergency Department.

Penetrating injuries to the heart often lead to pericardial effusion (PCE) that may result in cardiac tamponade which can be rapidly fatal. Thus, early detection of PCE is extremely important in initial resuscitation. A 37-year-old man without any past medical illness was sent to our emergency department by ambulance due to left chest wall penetrating injury by nail gun 30 minutes ago. Upon presentation, the patient was agitated and diaphoretic. The penetration site was in the region of cardiac box, which raised awareness that possible heart injury has been inflicted. Point-of-care ultrasound

Lead Perforation in Cardiac Implantable Electronic Devices: Incidence, Diagnosis, and Management.

The implantation of cardiac implantable electronic devices (CIEDs) has steadily increased due to an aging population, advancements in diagnostic techniques, and technological improvements. However, the risk of cardiac perforation following lead implantation, though low, remains a critical concern. Understanding the clinical presentations of this complication is essential for effective prevention, recognition, and management. This review explores the prevalence, clinical presentations, risk factors, diagnosis, management strategies, and future perspectives related to cardiac perforation caused

A Case Report of Pericardial Constriction and Left Ventricular Dysfunction in a Breast Cancer Survivor: Late Cardiotoxicity From Radiotherapy or 5-Fluorouracil.

Cardiotoxicity from oncologic treatments, including radiotherapy and fluoropyrimidine-based chemotherapy, can manifest years later, leading to pericardial constriction and left ventricular dysfunction in cancer survivors. We report the case of a 53-year-old breast cancer survivor, treated with surgery, radiotherapy (>25 Gy), and 5-fluorouracil (5-FU) chemotherapy in 2015, who remained cancer-free until a pleural recurrence in 2024, managed with pleurodesis and capecitabine. Six months later, she developed cardiac tamponade requiring pericardial drainage. Transthoracic echocardiography showed a

Pattern, characterization and outcomes of blunt and penetrating cardiothoracic injuries. A medico-legal comparative study.

Cardiothoracic injuries are crucial aspect of traumatology, resulting in significant morbidities and mortalities. Little is understood about the interrelationship between mechanism of trauma, and the associated outcomes. This work aimed to investigate the association between the mechanism of trauma and the induced pattern of injuries, as well as their association with adverse outcomes, namely mortality and the need for mechanical ventilation (MV). The current study is a prospective cohort study involving 229 patients diagnosed with acute cardiothoracic injuries. This work reveals an overall mo

Bullet to the Heart: A Case Report.

Penetrating cardiac injuries (PCI) from gunshot wounds are among the most fatal forms of trauma, with prehospital mortality rates exceeding 90%. While the right ventricle is most commonly affected due to its anterior location, retained intracardiac projectiles are rarely encountered and pose significant management challenges. Cardiac tamponade, though potentially fatal, can sometimes provide a protective mechanism by limiting hemorrhage. The decision to surgically remove retained bullets remains controversial, particularly in hemodynamically stable patients. We report the case of a 51-year-old

Incidence and predictors of temporary pacemaker implantation-induced cardiac perforation.

BACKGROUND: Cardiac perforation (CP) and cardiac tamponade (CT) are rare but dreaded complications of transvenous temporary pacing (TTP), with variable reported incidences and impacts on morbidity and mortality. The role of serial echocardiographic imaging in accurately detecting these complications has not been comprehensively investigated. OBJECTIVES: This study aimed to determine the incidence of CP and CT after TTP using serial echocardiographic imaging, assess the timeline of CP and CT development, and identify predictors of CP and CT. METHODS: A single-center, prospective, observationa

Case Report: Complex tricuspid valve repair and entrapped right ventricle foreign body extraction in an IVDU patient-early diagnosis and treatment considerations.

We report the case of a 39-year-old pregnant woman with a history of drug abuse who was admitted to the cardiology department with a diagnosis of tricuspid valve endocarditis. After the multidisciplinary team decided on a conservative treatment with antibiotherapy, the pregnancy was closely monitored. After 4 weeks of treatment, the patient developed extreme thoracic pain and pericardial effusion that was considered infectious and did not require urgent surgery. One month after giving birth to her baby, the patient was admitted to our hospital for the completion of the preoperative protocol. D

CT-Based Pericardial Composition Change as an Imaging Biomarker for Radiation-Induced Cardiotoxicity.

BACKGROUND/OBJECTIVES: No reliable noninvasive biomarkers are available to predict RT-induced cardiotoxicity. Because the pericardial sac is a fast responder to cardiac injury, we investigated whether RT-induced radiographic pericardial changes might serve as early imaging biomarkers for late cardiotoxicity. METHODS: We performed a retrospective study of 476 patients (210 males, 266 females; median age, 69 years; median follow-up, 26.7 months) treated with chemo-RT for small cell and non-small cell lung cancers at one single institution from 2009 to 2020. The heart and its 4 mm outmost layer

Management of Transvenous Leads in Patients With Iatrogenic Lead Perforation.

INTRODUCTION: Iatrogenic lead perforation is a rare but serious complication of cardiac implantable electronic device (CIED) implantation. Evidence on percutaneous management of subacute or delayed cases remains limited. METHODS: We retrospectively reviewed 38 patients treated for iatrogenic lead perforation between January 2012 and October 2024. Acute lead perforation cases, which were managed during the implantation procedure, were excluded. Lead removal was performed under imaging guidance in either the electrophysiology (EP) lab or a hybrid operating room (OR) with surgical backup. Clinic

Characteristics of cardiac toxicity after definitive radiation therapy for thoracic esophageal cancer in Japanese patients.

In recent years, there has been growing interest in cardiac toxicity following radiation therapy (RT) for esophageal cancer; however, detailed incidence and risk factors in Japanese patients remain unclear. The purpose of this study was to clarify the incidence, timing, risk factors, and dose-volume relationships of multiple cardiac toxicities, including pericardial effusion, heart failure, arrhythmia, cardiac valve disease and acute coronary syndrome. We retrospectively analyzed patients of thoracic esophageal cancer without distant metastasis who were treated with curative RT at our hospital

Cardiomyopathies of endocrine origin: A state-of-the-art review.

Endocrine disorders are increasingly recognized as potentially reversible causes of secondary cardiomyopathies, yet they often remain underdiagnosed in clinical practice. These conditions-including thyroid dysfunction, acromegaly, pheochromocytoma, diabetes mellitus, adrenal disorders, among others-can significantly alter cardiac structure and function through hormonal excess, metabolic remodeling, and neurohumoral activation. Hyperthyroidism may lead to high-output heart failure (HF) and atrial fibrillation, while hypothyroidism is associated with diastolic dysfunction, pericardial effusion,
