Multimodal precision diagnostics in clinical xenotransplantation.

Recent breakthroughs in pig-to-human xenotransplantation have successfully prevented hyperacute rejection of kidney, heart, liver and lung xenografts, marking a milestone long considered a critical barrier. However, these major advances also brought new challenges into focus, as immune-mediated graft injuries including antibody-mediated rejection, T cell-mediated rejection and innate immune activation have been observed in most xenotransplantation procedures to date. Detection and management of these injuries remain a critical bottleneck, as conventional histopathology and standard clinical mo

Impact of hypothermia level on neurological and renal outcomes in emergency aortic surgery: a single-centre experience.

BackgroundTemperature management during emergency surgery for acute aortic syndrome is critical for neurological, renal, and systemic protection. Recent European Association for Cardio-Thoracic Surgery (EACTS) guidelines recommend moderate hypothermia with antegrade cerebral perfusion for hemiarch procedures. Nevertheless, deep hypothermic circulatory arrest (DHCA) remains common. This study evaluates neurological and other postoperative outcomes across different intraoperative temperature ranges.MethodsA single-centre retrospective study of consecutive patients undergoing surgery for acute ao

Inhaled nitric oxide: Expanding indications beyond traditional boundaries.

Inhaled nitric oxide (iNO) is a selective pulmonary vasodilator that improves ventilation-perfusion matching with minimal effect on systemic blood pressure. Although the FDA approval is restricted to neonatal hypoxic respiratory failure associated with pulmonary hypertension, many clinicians now use iNO in adults with conditions where elevated pulmonary vascular resistance, right ventricular dysfunction threaten stability and hypoxic respiratory failure. This review examines both the mechanistic rationale and the available evidence for such off-label use. Examples range from acute RV failure i

Assessment of endothelial glycocalyx degradation in sepsis using direct visualization with sublingual microscopy.

BACKGROUND: The endothelial glycocalyx plays a crucial role in the pathophysiology of sepsis, but assessment of glycocalyx injury in clinical settings remains challenging. Perfused boundary region (PBR) is derived from sublingual intravital video microscopy and has been studied as a surrogate of endothelial glycocalyx barrier function. The goal of this study was to determine whether PBR is associated with circulating heparan sulfate (a biochemical marker of glycocalyx degradation), sepsis status, in-hospital mortality, and intravenous (IV) fluid administration. METHODS: This was an ancillary

Extreme temperatures and the epithelial barrier theory.

Extreme temperatures are now recognized as significant global health threats, driving morbidity and mortality across diverse diseases by compromising epithelial barrier integrity and causing periepithelial inflammation. The epithelial barrier theory posits that environmentally induced barrier dysfunction, driven by modern industrial and urban exposures, leads to compromised surfaces of the exposed tissues accompanied by epithelial activation and barrier dysfunction, microbial dysbiosis, loss of commensal bacteria, opportunistic pathogen colonization, bacterial translocation, inflammation and i

ICU-acquired pneumonia beyond pathogen detection.

Intensive care unit-acquired pneumonia is often treated as a binary diagnosis, although clinical deterioration, imaging and microbial detection describe different aspects of disease. This Perspective proposes an explicit, graded and revisable assessment of whether a detected organism plausibly causes the current lung injury. The framework encompasses pneumonia arising during ventilation, pneumonia that precedes intubation and pneumonia in non-ventilated ICU patients. It places molecular results within clinical, anatomical, microbiological and host context and uses formal reassessment at 48-72

Clinicopathologic Features and Diagnostic Challenges of Mycophenolate Mofetil-Associated Enterocolitis in Lung Transplant Recipients.

Mycophenolate mofetil (MMF) is a commonly used immunosuppressant for lung transplant recipients. Although it efficiently suppresses transplant rejection and improves graft function, MMF has been found to occasionally cause gastrointestinal (GI) injury. The symptoms are nonspecific and include diarrhea, nausea, vomiting, and rare cases of bleeding. Endoscopic findings range from normal appearance to erythema to ulcers. The histopathologic features mimic several different disorders like graft-versus-host disease (GVHD), cytomegalovirus (CMV) colitis, ischemic colitis, or inflammatory bowel disea

Tongsai Granules Ameliorate Airway Mucus Hypersecretion in AECOPD by Remodeling LIPA-Driven Ceramide Metabolism.

ETHNOPHARMACOLOGICAL RELEVANCE: Tongsai Granules (TSG) is a traditional Chinese medicine formula traditionally used for the treatment of acute exacerbation of chronic obstructive pulmonary disease (AECOPD) with phlegm-heat obstructing lung syndrome, a condition characterized by cough, excessive sputum production, and dyspnea; however, its underlying therapeutic mechanisms remain unclear. AIM OF THE STUDY: To investigate the mechanisms by which TSG alleviates airway mucus hypersecretion in AECOPD through regulation of the LIPA/ceramide axis. MATERIALS AND METHODS: A murine AECOPD model and ai

Mechanical ventilation-associated AKI: toward a multimodal and kidney-protective ventilatory framework.

Mechanical ventilation-associated acute kidney injury (AKI) represents a complex manifestation of lung-kidney cross-talk driven by hemodynamic alterations, neurohumoral activation, systemic inflammation and disturbances in gas exchange and acid-base homeostasis. Beyond static ventilatory settings, increasing evidence suggests that dynamic fluctuations in airway and perfusion parameters, including static driving pressure (ΔP), positive end-expiratory pressure (PEEP), mean airway pressure (Pmean), and effective renal perfusion pressure (eRPP) may substantially influence renal vulnerability durin

Global Protective Ventilator Settings Are Not Regional Protection: A Compartment-Resolved Framework for Personalizing Mechanical Ventilation in Acute Respiratory Distress Syndrome.

Background: Lung-protective ventilation in acute respiratory distress syndrome (ARDS) is guided by whole-lung measures, although the same acceptable breath may stabilize regions undergoing repeated alveolar collapse and expansion (RACE) while overdistending tissue already open. Fatigue-Aware Alveolar Stress and Strain Tracking (FAAST) is a clinician-in-the-loop framework designed to make this regional tradeoff observable. Methods: We first constructed a deterministic, twenty-compartment, phenotype-conditioned model from published observations of acute lung injury. With FAAST excluded, the test

Pharmacological Enhancement of O-GlcNAcylation Attenuates Lung Ischemia-Reperfusion Injury.

Lung ischemia-reperfusion injury (LIRI) contributes substantially to primary graft dysfunction after lung transplantation and also occurs in other clinical settings involving transient interruption and restoration of pulmonary blood flow. O-linked β-N-acetylglucosamine (O-GlcNAc) modification is rapidly altered by cellular stress, yet its role in acute pulmonary ischemia-reperfusion remains incompletely understood. We investigated this question using a murine model of transient left pulmonary hilar occlusion followed by reperfusion and an oxygen-glucose deprivation/reoxygenation (OGD/R) model

Guillain-Barré Syndrome (GBS) as a COVID-19 Complication: A Multicenter Study Conducted Throughout the 2-Year Pandemic.

BACKGROUND: Several studies have been published on the coexistence of Guillain-Barré syndrome (GBS) and COVID-19, raising the question of whether SARS-CoV-2 plays a role in GBS pathogenesis or is just a coincidence. In this research, we discuss GBS characteristics in COVID-19 patients during the pandemic. MATERIAL AND METHODS: This cross-sectional, multi-center study was performed on patients who were hospitalized in one of two GBS referral hospitals in Isfahan Providence between March 2020 to March 2022. Development of Versatile Nanoparticle System for Efficient Drug Delivery into Pulmonary Endothelium.

Drug delivery precisely to the pulmonary vasculature represents a formidable challenge. Pediatric Acute Respiratory Distress Syndrome (PARDS) is an urgent public health concern accounting for high mortality and long-term morbidity due to severe damage to lung endothelial cells (ECs). Advanced target-specific therapeutic interventions to protect lung vasculature in PARDS are in dire need. FOXF1 protein is a critical mediator of lung angiogenesis and vascular repair after injury. However, whether FOXF1 is a useful pharmacological target for PARDS is unknown. Herein, we developed a universal nano

Immobilized Neuroglobin Scavenges Carbon Monoxide From Circulating Carboxyhemoglobin.

Carbon monoxide (CO) poisoning is responsible for around 50,000 emergency department visits per year in the United States alone. Currently, there is no available antidote for CO poisoning. We have previously developed an antidotal therapy based on an engineered hemeprotein, human neuroglobin (Ngb-H64Q-CCC). Intravenous infusion of Ngb-H64Q-CCC removes CO from circulating red blood cells and improves survival in a lethal CO poisoning mouse model. However, the infusion of heme-containing proteins has inherent heme toxicity risks that may limit the dose that can be used safely without liver or ki

Traumatic pneumatoceles in adulthood: A case highlighting the limitations of supine chest radiography in trauma.

Traumatic pneumatoceles are uncommon pulmonary pseudocysts that may occur following blunt thoracic trauma and can be entirely occult on chest radiography, potentially mimicking other cavitary lung pathologies. We present the case of a 52-year-old male transferred to a tertiary trauma center following a motorcycle accident. On presentation, the patient was hypoxic with extensive right-sided chest wall injuries. Initial supine chest radiography demonstrated multiple right-sided rib fractures, patchy airspace opacities, a veiling opacity, and a suspected pneumothorax, raising concern for a large
