Traumatic pneumatoceles in adulthood: A case highlighting the limitations of supine chest radiography in trauma.

Traumatic pneumatoceles are uncommon pulmonary pseudocysts that may occur following blunt thoracic trauma and can be entirely occult on chest radiography, potentially mimicking other cavitary lung pathologies. We present the case of a 52-year-old male transferred to a tertiary trauma center following a motorcycle accident. On presentation, the patient was hypoxic with extensive right-sided chest wall injuries. Initial supine chest radiography demonstrated multiple right-sided rib fractures, patchy airspace opacities, a veiling opacity, and a suspected pneumothorax, raising concern for a large

Anesthetic management of bronchopleural fistula repair with severe pneumonia in a 74-year-old man using one-lung ventilation combined with high-flow veno-venous extracorporeal membrane oxygenation without continuous systemic heparinization: a case report.

BACKGROUND: Bronchopleural fistula (BPF) is a serious complication following lung resection, often leading to respiratory failure. In one study of 6,239 patients, 43 (0.65%) developed early BPF, and cases complicated by contralateral pneumonia have rarely been reported. CASE DESCRIPTION: We present the case of a 74-year-old man with chronic obstructive pulmonary disease (COPD), atrial fibrillation, hypertension, and pneumonia who developed BPF after thoracoscopic left upper lobectomy. The patient was scheduled to undergo BPF repair, in which includes an intercostal muscle flap was sutured in

CDK4/6 inhibitor-related interstitial lung disease in breast cancer: a single-center case series and literature-based patient-level review.

BACKGROUND: Cyclin-dependent kinase 4/6 (CDK4/6) inhibitors are widely used in the treatment of breast cancer. CDK4/6 inhibitor-related interstitial lung disease (ILD) is uncommon but potentially serious, and its clinical manifestations, radiologic patterns, and outcomes remain incompletely characterized. This study aimed to characterize the clinical presentation, radiologic patterns, diagnostic evaluation, treatment, and outcomes of CDK4/6 inhibitor-related ILD using a Chinese single-center case series and an updated literature-based patient-level review. METHODS: Seven patients with clinica

MMP-7 as a non-invasive biomarker in interstitial lung diseases and other chronic respiratory diseases: a narrative review.

BACKGROUND: Interstitial lung diseases (ILDs) are a heterogeneous group of disorders in which epithelial injury, inflammation and fibrosis lead to progressive loss of lung function. Several ILDs other than idiopathic pulmonary fibrosis (IPF), including connective tissue disease-associated ILD (CTD-ILD), fibrotic hypersensitivity pneumonitis and fibrotic nonspecific interstitial pneumonia, may develop a progressive fibrosing phenotype, and distinguishing fibrotic from non-fibrotic disease is clinically important. Matrix metalloproteinase-7 (MMP-7), a circulating biomarker of extracellular matri

Monitoring multiple rib fractures using pulmonary electrical impedance tomography technology: correlation analysis of pulmonary ventilation changes.

BACKGROUND: Multiple rib fractures are a common and severe form of chest trauma, which can lead to pulmonary ventilation disorders. Assessment primarily relies on imaging examinations and conventional pulmonary function tests. Electrical impedance tomography (EIT) technology can detect ventilation dysfunctions that are difficult to identify through conventional means. This study utilizes EIT technology to quantify global and regional pulmonary ventilation inhomogeneity and to analyze the correlation between these ventilation characteristics and various factors associated with multiple rib frac

Systemic Sclerosis Presenting With Long-Standing Peripheral Neuropathy and Delayed Multisystem Manifestations: A Case Report.

Systemic sclerosis (SSc) is a rare autoimmune connective tissue disorder characterized by immune dysregulation, microvascular injury, and progressive fibrosis involving the skin and internal organs. Although Raynaud phenomenon and cutaneous sclerosis are the hallmark manifestations, atypical presentations may significantly delay diagnosis. Peripheral neuropathy is an uncommon neurological manifestation and rarely precedes the diagnosis of SSc by several years. We report the case of a 62-year-old female who presented with a 20-year history of progressive distal paresthesia involving both upper

Redefining mechanical ventilation in thoracic anesthesia: flow-controlled ventilation from bench to bedside-a narrative review.

One-lung ventilation (OLV) remains a cornerstone of thoracic anesthesia but continues to challenge perioperative respiratory management. Restriction of ventilation to a reduced functional lung volume promotes ventilation-perfusion mismatch, alveolar instability, and exposure of the dependent lung to increased mechanical stress. Although contemporary lung-protective ventilation strategies have evolved considerably, postoperative pulmonary complications remain common, highlighting the need for innovative approaches to respiratory support. Flow-controlled ventilation (FCV) introduces a distinct p

Matrix Metalloproteinase-8 Deficiency Alters the Balance Between Viral Clearance and Immunopathology in H1N1 Infection.

Matrix metalloproteinase-8 (MMP-8) regulates inflammatory responses, but its contributions to the pathogenesis of influenza A viral (IAV) disease are not known. Plasma MMP-8 levels were increased 30-fold in subjects infected with H1N1 or seasonal IAV and were inversely associated with the PaO2/FiO2 ratio, suggesting potential value as a candidate marker of lung injury severity that requires validation in larger cohorts. MMP-8 levels were elevated in the lungs of H1N1-infected wild-type (WT) mice and localized to airway epithelial cells, macrophages, and polymorphonuclear neutrophils (PMNs). H1

The impact of ventilation mode on systemic inflammation and organ dysfunction in a porcine model of abdominal sepsis-associated acute respiratory distress syndrome.

INTRODUCTION: Sepsis commonly causes acute respiratory distress syndrome (ARDS), and immune hyperactivation may be a causal link. Ventilator-induced lung injury (VILI) is common in ARDS, and VILI may potentiate organ dysfunction in severe sepsis. We used a porcine model of abdominal sepsis-associated ARDS to test whether ventilation strategy could impact systemic inflammation and organ dysfunction. METHODS: Female Yorkshire pigs (6-8 months) underwent ischemia-reperfusion (I/R) injury of the small bowel and fecal clot implantation or sham laparotomy. The abdominal sepsis animals were ventilat

KLF4 Protects Against Sepsis-Induced Coagulopathy via Inhibition of the AKT/mTOR/STAT3 Signaling Pathway.

PURPOSE: Coagulation dysfunction is an independent predictor of morbidity and mortality in patients with severe sepsis, yet current therapeutic options remain inadequate. This study systematically evaluated the therapeutic potential of Krüppel-like factor 4 (KLF4) in sepsis-induced coagulopathy and explored the underlying molecular mechanisms. METHODS: In vivo, mice were randomized into sham operation group, cecal ligation and puncture (CLP) group, and CLP + APTO-253 groups. The KLF4 agonist APTO-253 was intraperitoneally administered to CLP-induced septic mice to evaluate systemic inflammati

Temporary Left Ventricular Unloading With Impella as a Bridge to Emergency Surgery for Acute Mitral Bioprosthetic Failure.

Structural valve deterioration usually declares itself gradually, but a bioprosthetic leaflet that tears away from a stent post can produce catastrophic regurgitation without warning. In the mitral position, the left atrium has not had time to become compliant, and the resulting rise in pulmonary capillary pressure can flood the lungs within hours. A 68-year-old woman who had undergone mitral valve replacement with a bovine pericardial bioprosthesis 13 years earlier developed sudden dyspnea. Her oxygen saturation was 58%, the chest radiograph showed almost complete bilateral opacification, and

Dual route of polystyrene micro- and nanoplastics exposure increases in vivo leukocyte-endothelium interactions and aligns with organ injury.

Micro- and nanoplastics are pervasive environmental contaminants, yet direct in vivo evidence linking mixed-route exposure to leukocyte-endothelium interactions remains limited. Here, we investigated the effects of 28-day dual-route polystyrene exposure (oral microplastics and inhalational nanoplastics) in a sex-stratified murine model. Using intravital microscopy of mesenteric post-capillary venules, we quantified leukocyte rolling, stationary events and firm adhesion under physiological flow. Exposure increased leukocyte-endothelium interactions across the trafficking cascade compared with c

From microbiome to outcome: the cascading effects of combining acid suppressants with anti-tuberculosis therapy.

Gastrointestinal adverse reactions occur in up to 71.8% of patients receiving anti-tuberculosis drugs (ATDs), and acid-suppressing agents are frequently co-prescribed to alleviate these symptoms. Direct evidence from human studies demonstrates that ATDs alone reduce gut microbial diversity, deplete short-chain fatty acid (SCFA)-producing bacteria, and promote opportunistic pathogen overgrowth. Concurrently, acid suppressants, particularly proton pump inhibitors (PPIs), raise intragastric pH and facilitate oral-to-gut translocation of bacteria, independently disrupting microbiota composition an

Phenotype-Guided Surfactant Therapy in ARDS: From Failed Replacement to Targeted Biological Intervention, Narrative Review.

PURPOSE OF REVIEW: Acute respiratory distress syndrome (ARDS) is associated with profound pulmonary surfactant dysfunction, yet clinical trials of exogenous surfactant have not demonstrated consistent outcome benefits in adults. This review reassesses these failures in light of current understanding of ARDS heterogeneity, surfactant biology and intrapulmonary drug delivery. RECENT FINDINGS: Surfactant dysfunction in ARDS reflects not only quantitative depletion but also inhibition by plasma proteins, altered lipid and protein composition, impaired synthesis and recycling, epithelial injury an

Endothelial cells as regulatory hubs in the fibrotic microenvironment.

Tissue fibrosis is a common pathological response to chronic injury and a major contributor to progressive organ dysfunction. In the lung, liver, heart, kidney, skin, and other organs, persistent inflammation, ischemia, toxic injury, metabolic stress, or immune dysregulation can shift repair from adaptive remodeling toward scar formation. Once extracellular matrix (ECM) deposition becomes self-sustaining, current therapies often slow progression but rarely reverse established fibrosis. Increasing attention has therefore shifted from individual effector cells to the profibrotic microenvironment

From Menarche to Menopause: A Narrative Review of Female Reproductive Milestones and Risk of Chronic Airway Disease.

TOPIC IMPORTANCE: Female reproductive factors are underrecognized determinants of lifelong respiratory health. This narrative review examines relationships between reproductive milestones and chronic airway disease risk. REVIEW FINDINGS: Early menarche is associated with reduced adult lung function and a higher incidence of asthma. Polyendocrine metabolic ovarian syndrome is also associated with increased asthma risk, particularly in obese individuals, through sex hormone dysregulation, metabolic dysfunction, and chronic low-grade inflammation. Adverse pregnancy outcomes are associated with l

Basal Stem Cell Signaling Pathways Regulating Collective Epithelial Cell Migration and Airway Repair.

The airway epithelium serves as a critical barrier against pathogens, allergens, and other environmental insults. Disruption of this barrier contributes to respiratory diseases such as chronic obstructive pulmonary disease, making rapid epithelial repair essential for maintaining lung function. Basal cells (BCs), the primary regenerative cell population of the proximal airway epithelium, drive repair through coordinated collective cell migration rather than isolated cellular behaviors. This process is closely linked to dynamic transitions between jammed and unjammed tissue states that regulate
