Mapping adverse events and monitoring priorities for trastuzumab deruxtecan use in metastatic breast cancer.

INTRODUCTION: Metastatic breast cancer (MBC) remains a major clinical challenge despite recent advances in HER2-targeted therapies such as trastuzumab deruxtecan (T-DXd). While clinical trials have demonstrated its efficacy, concerns about treatment-emergent toxicities persist, highlighting the need for real-world pharmacovigilance to capture a broader spectrum of adverse events (AEs). METHODS: We analyzed reports from the US FDA Adverse Event Reporting System (FAERS, 2004-2024), including trastuzumab deruxtecan reports recorded since its market introduction in 2019. Disproportionality analys

CXCR2 antagonism as a promising therapeutic approach for pulmonary fibrosis therapy.

Pulmonary inflammation is a central feature of several chronic lung diseases. The chemokine receptor CXCR2 plays a key role in regulating recruitment and activation of leukocytes and other cell types involved in acute and chronic inflammation of the lungs. In this study, we evaluated and compared the anti-inflammatory effects of four CXCR2 antagonists - DF2755A, AZD-5069, SX-682, and SCH527123 - in a murine model of bleomycin-induced lung injury. C57BL/6 mice received intranasal bleomycin and were treated once a day with known active doses of the antagonists by oral gavage. Following disease i

Bibliometric Analysis of the Lung Organoids: Trends, Hotspots, and Future Development.

INTRODUCTION: Organoids are three-dimensional (3D) culture models derived from selforganizing stem cells or progenitor cells, capable of recapitulating the cellular growth microenvironment and interactions within the human body to a certain extent. These models play significant roles in biomedical research, drug screening, and related fields. Research on lung organoids has provided insights into lung development and the cellular and molecular processes underlying diseases, while studies on their transplantation in animals hold promise as novel strategies for post-lung injury therapies. However

Multi-modal data to identify key factors influencing lung injury in ARDS patients undergoing invasive mechanical ventilation: A prospective multi-center observational study protocol.

BACKGROUND: Patients with moderate to severe acute respiratory distress syndrome (ARDS) exhibit extremely poor prognoses following mechanical ventilation, with mortality rates as high as 40% to 55%. Despite extensive research into ARDS classification and prognostic assessment, the disease's pathogenesis remains incompletely understood, and there remains a critical lack of specific biomarkers and effective therapeutic targets for its prevention and management. The core challenges lie in two key areas. First, ARDS demonstrates marked heterogeneity in etiology, pathophysiology, and pathogenesis.

β3-adrenoceptor agonism exerts lung protection in a rat model of bronchopulmonary dysplasia.

BACKGROUND AND PURPOSE: Bronchopulmonary dysplasia (BPD) affects premature newborns, particularly those receiving supplemental oxygen therapy. Exposure of underdeveloped lungs to oxygen levels higher than the intrauterine environment causes oxidative stress, impairing alveolarisation, vascularisation and causing fibrosis. The β3-adrenoceptor could represent a promising target for BPD treatment because of its oxygen-dependent regulation, ability to reduce oxidative stress and protection against intestinal hyperoxia-induced damage. EXPERIMENTAL APPROACH: In this study, neonatal rat pups were re

A tri-scale in silico framework integrating pharmacovigilance and mechanistic modeling suggests tepotinib-associated acute kidney injury risk.

Proactive safety evaluation of molecularly targeted therapies requires generalizable frameworks that integrate real-world evidence with mechanistic insights. As a case in point, tepotinib, a mesenchymal-epithelial transition (MET) inhibitor for non-small cell lung cancer, has a known pulmonary toxicity profile, but its link to acute kidney injury (AKI) and underlying mechanism remain unclear. We devised a tri-scale in silico strategy encompassing population-scale pharmacovigilance signal extraction from the FDA Adverse Event Reporting System (FAERS) (2020-2025) via disproportionality analysis

Toxicity and health effects of delta-8, delta-9, and delta-10-tetrahydrocannabinol and unregulated cannabinoids in vaping products.

Hemp-derived cannabinoids (CBDs) such as Δ8- and Δ10-tetrahydrocannabinol (THC) in cannabis e-cigarettes have been growing in popularity among youth, causing great concern for their health effects. Previous novel lung injury outbreaks, such as E-cigarette or Vaping Use-Associated Lung Injury (EVALI), were associated with the rising use of e-cigarettes and vaping products. Toxicological studies have revealed that chronic exposure to cannabis vapor can cause adverse brain and pulmonary effects. Hemp products are classified as cannabis and set a limit of no more than 0.3 % Δ9-THC, while products

Curcumin Protects against Arsenic and Cadmium-Induced Lung Toxicity in Rats by Modulating Bax/Bcl-2/caspase-3 Pathway: In Vivo and In Silico Approaches.

The present study aims to investigate the putative protective role of curcumin (CUR) against oxidative stress and lung damage caused by a heavy metal mixture (Cd+As) with cadmium (Cd) + arsenic (As). Wistar albino rats were randomly divided into five experimental groups: Control, 0.44 + 5.55 mg/kg Cd+As, 100 mg/kg CUR, 100 CUR + Cd+As, and 200 CUR + Cd+As groups. The Cd+As increased malondialdehyde levels and oxidative stress. However, it also led to a decrease in levels of glutathione (GSH), superoxide dismutase (SOD), catalase (CAT), glutathione peroxidase (GPx) in lung tissue homogenates. D

Acute Lung Injury Induced by Hyperbaric Oxygen or Other External Factors, with a Focus on Exosomes.

Acute lung injury (ALI) is in part precipitated by hyperbaric oxygen or other mechanical insults. It constitutes the fundamental pathological process underlying acute respiratory distress syndrome (ARDS). The manifestation of the condition is characterized by an uncontrolled inflammatory response and alveolar edema, consequent to the disruption of the alveolar-capillary barrier. This phenomenon is associated with elevated morbidity and mortality rates. The current therapeutic interventions for ALI are not well researched or articulated. However, recent studies have indicated that stem cells ma

A Comparative Study of Glucocorticoids Efficacy in Acute Respiratory Distress Syndrome.

Background: Acute respiratory distress syndrome (ARDS), recognized as an inflammatory and life-threatening lung injury, is typified by severe hypoxaemia, lack of heart-related pulmonary edema, and bilateral lung infiltrates. Glucocorticoids are anti-inflammatory and immunoregulatory agents that are considered a viable treatment for ARDS. This study sought to contrast the effects of methylprednisolone, hydrocortisone, and dexamethasone at equivalent doses in ARDS. Methods: About 195 ARDS patients were allocated at random to take methylprednisolone (1 mg/kg/day), hydrocortisone (350 mg/day), or

The effects of marine fatty acid omega-3 supplements on incident fractures and bone mineral density in generally healthy adults.

Although preclinical studies suggest that omega-3 fatty acids may benefit skeletal health, there are few randomized controlled trials investigating effects of supplemental omega-3 on bone outcomes. This VITamin D and OmegA-3 TriaL (VITAL) ancillary study investigated effects of marine omega-3 (1 g/d; EPA+DHA in a 1.2:1 ratio) vs placebo supplements on fracture risk and bone density/structure. VITAL is a 2 × 2 factorial randomized placebo-controlled trial that studied effects of supplemental marine omega-3 fatty acids and/or vitamin D3 vs placebo on cancer and cardiovascular events. Longer Duration of Donor Ventilation is not Associated With Long-term Lung Function After Lung Transplantation.

BACKGROUND: Prolonged mechanical ventilation increases the risk of developing ventilator-induced lung injury, but the relationship between the duration of donor ventilation (DDV) and post-lung transplant lung function is unknown. METHODS: We analyzed adult patients undergoing double lung transplantation in our program between January 2007 and December 2020. The risk factor of interest was DDV in days before organ retrieval. The primary outcome was forced expiratory volume in 1 s % predicted at 1-y posttransplant (1yFEV1%), and secondary outcomes, including lung function at 3 mo, short and lon

Berberine Alleviates Lipopolysaccharide-Induced Acute Lung Injury by Modulating the AMPK-HMGB1-NF-κB Signaling Axis.

BACKGROUND: The activation of adenosine-monophosphate-activated protein kinase (AMPK) by berberine (BBR) benefits various inflammatory diseases. Conversely, high mobility group box-1 (HMGB1), a prototypical damage-associated molecular pattern (DAMP), typically exerts opposing effects. This research aims to investigate the relationship between AMPK and HMGB1, elucidating the functions and underlying mechanisms by which BBR alleviates acute lung injury (ALI) caused by lipopolysaccharide (LPS). METHODS: Male C57BL/6J mice were intragastrically administered BBR twice daily for three days with a t

Osimertinib activates TFEB to trigger hepatocyte cytoplasmic vacuolation-associated cell death.

Osimertinib is a first-line treatment for advanced non-small cell lung cancer with epidermal growth factor receptor (EGFR)-sensitive mutations; however, the severe hepatotoxicity of osimertinib is a challenge that has attracted attention. In this study, we found that osimertinib directly caused hepatocyte damage, with cells showing morphological features of vacuolization after treatment. Using transmission electron microscopy and other analytical techniques, we found that excessive activation of the autophagy-lysosome pathway was the main cause of cell vacuolation. Mechanistically, osimertinib

Interstitial pneumonias of undetermined etiology in foals in California, 1990-2020.

Interstitial and bronchointerstitial pneumonias of undetermined etiology in young foals are relatively common in autopsy services with an equine focus. Unknown viruses, toxins, hyperthermia, surfactant or alveolar macrophage function deficiency, certain antibiotics, and aberrant responses to Rhodococcus equi or other bacteria have been proposed as causes. We performed a retrospective study of autopsies on foals with a diagnosis of interstitial or bronchointerstitial pneumonia with an unidentified etiology. Forty-one foals (median age: 3-mo-old) were included. Most were received in summer (n =

Functional and clinical significance of novel SERPINA1 variants on alpha-1 antitrypsin deficiency.

BACKGROUND: Mutations in the SERPINA1 gene can result in alpha-1 antitrypsin deficiency (AATD), which may be associated with lung or liver injury. Although the S and Z alleles account for over 95% of cases of AATD, a wide variety of rare variants have been linked to deficiency and dysfunction, while other variants are associated with normal alpha-1 antitrypsin (AAT) levels and activity. Here, we present the identification and characterization of thirteen rare SERPINA1 variants discovered during the genetic diagnosis of AATD by the Progenika diagnostic network. METHODS: The new variants were i

Mechanisms associated with progression from lobar inflammation to bilateral lung injury: an experimental study.

BACKGROUND: Localized lung insult, such as pneumonia, can increase respiratory drive and effort. The interplay between the initial injury, the inflammatory reaction and increased drive and effort could damage other areas and progress to bilateral lung injury. METHODS: Localized pulmonary inflammation was induced through instillation of 0.5 mg/kg lipopolysaccharide (LPS) in the left lower lobe of 18 pigs, while 5 animals served as sham controls. After 24 hours of spontaneous breathing, the severity of right lung injury was assessed by the validated histological score and correlated with potenti
