A Swiss flag through the chest: a case report of a rare transmediastinal impalement.

INTRODUCTION: Management of transmediastinal penetrating injuries (TMI) is challenging. We report an educational flagpole-related case. CASE PRESENTATION: A 61-year-old woman arrived at our trauma center after a boating accident resulting in transthoracic impalement from a fall onto a flagpole. She was hemodynamically stable, fully conscious, and without additional injuries. Chest X-ray showed an approximately 30-cm foreign body penetrating the thorax from the left and ending paramediastinally on the right, suggesting mediastinal and pulmonary involvement. CT confirmed that the flagpole enter

[Tracheal Membranous Wall Injury Secondary to Endotracheal Intubation:Report of a Case].

An 81-year-old woman underwent endovascular treatment for a cerebral aneurysm under general anesthesia with intubation using a 7.0-mm endotracheal tube. Shortly after extubation, she developed hemoptysis. Bronchoscopy revealed a longitudinal laceration and bleeding of the tracheal membranous wall, extending from approximately 4-cm below the vocal cords to near the carina. Hemostasis was achieved with diluted epinephrine. Chest computed tomography (CT) showed no pneumomediastinum or perforation. Given her stable condition and that the injury was confined to the mucosal layer, conservative manag

Bronchoscopic removal of lower airway foreign body: a two-center retrospective cohort study.

BACKGROUND: Tracheobronchial foreign body (FB) aspiration is a life-threatening condition, particularly in older adults. Despite its clinical significance, research on the etiology of adult FB aspiration, as well as real-world outcomes and complications associated with FB removal in lower airways, remains limited. PATIENTS AND METHODS: This was a two-center retrospective cohort study in Japan. We included adult patients aged 18 years or older who underwent bronchoscopic FB removal between April 2010 and March 2025. RESULTS: A total of 28 patients were included in this analysis (mean age, 75.4

Progressive Extension of a Post-intubation Tracheal Laceration Presenting As Massive Pneumomediastinum: A Case Report.

Post-intubation tracheal laceration is rare and is usually managed conservatively, provided the tear is protected from positive pressure, whether by spontaneous ventilation, by a cuff sited beyond the lesion, or by ventilation delivered without loss of tidal volume. We describe a woman in her early 70s intubated at a district hospital for progressive impairment of consciousness. Imaging before intubation already showed a hiatal hernia, right bronchial mucous plugging, lobar consolidation and bilateral effusions, and initial ventilation delivered 500 mL at a rate of 18 per minute, approximately

Thyroid Cancer Management in Contemporary Practice: A Systematic Review and Meta-Analysis of Diagnostic Pathways, Surgical Strategies, Airway Challenges, and Mediastinal Extension.

BACKGROUND: Thyroid cancer is the most common endocrine malignancy worldwide, with a clinically significant subset of patients presenting with locally advanced disease involving the trachea, recurrent laryngeal nerve, esophagus, and superior mediastinum. However, evidence describing the integrated burden of diagnostic limitations, airway complications, surgical outcomes, and mediastinal involvement remains fragmented across small and heterogeneous studies. OBJECTIVES: This study aimed to systematically evaluate and quantitatively synthesize evidence on diagnostic performance, perioperative ai

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

The role of bronchoscopy in benign airway stenosis: literature review of a central procedure guiding multidisciplinary care and therapeutic approach for improving patient outcomes.

BACKGROUND AND OBJECTIVE: Tracheal stenosis is a potentially life-threatening condition whose symptoms including dyspnea, wheezing and cough, are frequently misdiagnosed, leading to a delay in the diagnosis. The more frequent causes of stenosis are iatrogenic: prolonged intubation (producing a circumferential ischemic injury of the mucosa and consequent scarring) and tracheotomy (due to a direct damage of tracheal wall). However, other causes include autoimmune disease, infections, previous radiation therapy. This review aims to highlight the role of bronchoscopy, not only as a diagnostic proc

Extracorporeal Membranous Oxygenation Associated With Tracheal Procedures: An Extracorporeal Life Support Organization (ELSO) Registry Analysis.

Extracorporeal membrane oxygenation (ECMO) has been primarily used for respiratory and circulatory failure, but its airway-related use has not been investigated well. Tracheal procedures are a situation when ECMO could be used to support patients during anticipated difficult airway management. The Extracorporeal Life Support Organization registry was queried for adult patients treated with ECMO in 2010-2022 during the same admission with types of tracheal procedures. Tracheal procedures were divided into surgical procedure and bronchoscopic procedure groups, and the survival rate was analyzed

Management of the ventilation in intraoperative tracheobronchial injury with manual occlusion of the rent-An unusual case report.

Intraoperative tracheobronchial injury is one of the most serious complications of any thoracic surgery. Its management is really challenging both for the surgeons as well as for the anesthesiologists. We present a rare case of intraoperative tracheobronchial injury during esophagectomy and the management of the ventilation by intermittent manual occlusion of the rent as a rescue method. The left-sided bronchus got injured near the carina, while the patient was on one-lung ventilation in a lateral position and the left lung was being ventilated with a left-sided double-lumen tube (DLT). Radiation-induced Osteosarcoma in the Thoracic Cavity 28 Years after Irradiation for a Benign Disease: An Autopsy Case.

A 60-year-old man with a history of radiation therapy 28 years ago for a benign disease (an aneurysmal bone cyst) was admitted with dyspnea and dysphagia. Computed tomography showed an 8-cm tumor invading the trachea and esophagus. His condition declined rapidly, and he died on day 27 of hospitalization. Autopsy revealed radiation-induced osteosarcoma. Radiation-induced sarcoma can develop even after radiotherapy for benign diseases, in which case the latency period might be longer. Furthermore, the prognosis depends on the location of the resectable tumor. Therefore, longer-term internal imag

Case Report of an Unexpected Cause of Noisy Breathing: Breast Pump Membrane as an Esophageal Foreign Body in an Infant.

Background: Due to their nonspecific symptoms, foreign bodies in the aerodigestive tract (FBA) are often misdiagnosed as respiratory conditions, especially when unwitnessed, posing significant risks for morbidity and mortality in the pediatric population. Case Presentation: We report a case of an otherwise healthy 4-month-old, predominantly breastfed infant with episodic "honking" breathing, choking with feeds, and a 2-month history of recurrent croup episodes requiring emergency services and an hospital admission. After referral to pediatric otolaryngology/respirology, a flexible laryngoscopy

FPPS Downregulation Attenuates Endoplasmic Reticulum Stress in Pulmonary Epithelial Cells.

The airway epithelium is the primary target of the trachea in lung transplant rejection and epithelial cell injury are frequently observed in lung transplants. Farnesyl pyrophosphate synthase (FPPS), a pivotal enzyme in the mevalonate pathway, synthesizes isoprenoid compounds like FPP and GGPP. This study found upregulated expression of FPPS in the epithelial cells of the tracheal transplant rat model and the use of the FPPS inhibitor zoledronic acid reduced the tracheal epithelial cell damage. Using CRISPR/CAS9, FPPS was knocked down in pulmonary-derived epithelial cells, and RNA sequencing a

Perioperative outcomes of median sternotomy for intrathoracic tracheal malignancies: a 15-year retrospective analysis.

BACKGROUND: Tracheal resection and reconstruction is a complex thoracic surgery procedure. Posterolateral thoracotomy (PLT) is the standard approach for intrathoracic tracheal resection; however, research on the median sternotomy (MST) approach is limited. This study evaluated the surgical outcomes of MST in treating intrathoracic tracheal malignancies. METHODS: A 15-year retrospective analysis of the data of patients with primary intrathoracic tracheal malignancies who underwent "trachea + carina" resection and reconstruction at a single center was conducted. RESULTS: A total of 62 patients

Tracheoesophageal Fistulas Unrelated to Malignancy: A Case Series.

Tracheoesophageal fistula (TEF) is a rare, pathological connection between the trachea and esophagus that can be acquired or congenital. Acquired TEF typically occurs due to iatrogenic injuries. There is often a delay in diagnosis due to the rare nature of this condition. These patients have a very high mortality rate, and a multidisciplinary strategy is required for the management of TEF involving specialists from interventional pulmonology, gastroenterology, and thoracic surgery. The clinical features, diagnosis, and management of nine patients with TEF are covered in this article. Eight pat
