Multiplexed self-adaptable Janus hydrogels rescue epithelial malfunction to promote complete trachea repair.

Epithelial malfunction rescue is the decisive step involved in complete trachea repair; however, this step remains challenging due to the harsh tracheal environment and unclear pathogenesis, which still made current bioengineered trachea transplants receive fatal complications. Herein, bacterial infection-induced neutrophilic oxidative stress imbalance and epithelial stemness loss were identified as the pathogenic factors. Targeting pathogenesis, multiplexed hydrogels with adhesive and anti-fouling Janus sides, anti-swelling and anti-bacteria properties are constructed to adapt in mucous and c

Cervical Incision-Free Technique for R0 Resection in Cervicothoracic Esophageal Carcinoma: A Novel Approach of the Chai's Supra-Thoracic Apex Technique.

Purpose: Esophageal squamous cell carcinoma (ESCC) located at the cervicothoracic junction (CTJ-ESCC), approximately 18-21 cm from the incisors, poses unique surgical difficulties given its proximity to the trachea and recurrent laryngeal nerves. This investigation evaluated a new approach that avoids cervical incisions by combining the EGIARADMT™ curvilinear stapling method with OrVil™-based three-dimensional suspension anastomosis (TriSAS) to achieve R0 resection. Methods: A retrospective analysis was performed on 11 consecutive cases of CTJ-ESCC treated between February 2019 and March 2024,

Tracheal Reconstruction Using a Nitinol Stent for Thoracic Tracheal Rupture.

BACKGROUND/AIM: Endotracheal tube intubation is required for surgery under inhalation anesthesia. In the present study, after inhalation anesthesia in dogs, the tracheal tube was extubated without deflating the endotracheal tube cuff, resulting in thoracic tracheal rupture, subcutaneous emphysema, and pneumomediastinum. The ruptured thoracic trachea was reconstructed using an endotracheal stent to treat intrathoracic emphysema. MATERIALS AND METHODS: A double-wire woven nitinol stent was handmade using a cross-and-hook knitting method. The fabricated endotracheal stent was 2-3 mm larger than

Experience using a free pericardial fat pad to repair an intrathoracic injury during esophageal cancer surgery.

In surgery for thoracic esophageal cancer, repair of surgically damaged organs, such as the trachea or lungs, or a reconstructed gastric conduit may be necessary. The omentum, intercostal myocutaneous flaps, and latissimus dorsi myocutaneous flaps are often used as covering materials. Although these structures are effective dressings with suitable blood flow, the omentum is often unavailable following esophageal surgery, and creating a muscle flap is invasive and laborious. Using a free pericardial fat pad as a sealant is a safe and effective option to prevent postoperative alveolar air leaks

Full-thickness tracheal laceration managed conservatively: a case report.

BACKGROUND: Tracheobronchial injuries, which are rare complications of intubation, can be overlooked without a high degree of clinical suspicion. While traditionally managed surgically, recent literature proposes nonoperative management for low-risk injuries. CASE PRESENTATION: This case illustrates a 56-year old Caucasian Canadian female with a grade IIIA injury successfully treated conservatively using an endotracheal tube and medical therapy. Consequently, it suggests that nonoperative management may be suitable for patients meeting specific criteria indicative of clinical stability. CONC

Rhabdomyolysis After Tracheobronchoplasty: Incidence, Predictors, and Association With Postoperative Outcomes.

INTRODUCTION: Prolonged intraoperative immobility during tracheobronchoplasty (TBP) can cause rhabdomyolysis. We investigated the incidence of rhabdomyolysis after TBP and its association with postoperative outcomes. METHODS: We conducted a single-center review of TBP patients between 2008 and 2024. Patients were dichotomized by the severity of rhabdomyolysis. Outcomes included postoperative acute kidney injury (AKI), complications, readmissions, hospital length of stay, and disposition. The Fisher's exact test, Wilcoxon rank sum test, and stepwise logistic regression were used for analyses.

Clinical insights and management outcomes of tracheoesophageal and bronchoesophageal fistulas: a 20-year case series review.

BACKGROUND: Abnormal connections between the trachea and esophagus, known as tracheoesophageal fistula (TEFs) and bronchoesophageal fistula (BEFs), are uncommon but potentially life-threatening conditions that can result in severe complications. This study aimed to explore the anatomical features, underlying causes of TEF and BEF and evaluate the treatment efficacy. METHODS: This retrospective study reviewed 15 cases of TEF/BEF treated at the University Hospital from 2001 to 2020. It examines the anatomical features, underlying causes, and treatment outcomes of these fistulas and evaluates var

A novel anti-acute lung injury mechanism of astilbin: inhibition of epithelial cells ferroptosis by targeting NRF2 activation via binding Val608 site of NRF2.

OBJECTIVE AND DESIGN: Astilbin (ATB) is a newly discovered natural compound with anti-inflammatory and immunomodulatory effects. However, its effects and underlying mechanisms in acute lung injury (ALI) remain unclear. MATERIAL OR SUBJECTS: An ALI model was established by intratracheal injection of lipopolysaccharide (LPS) into the trachea of C57BL/6 mice. In vitro, MLE-12 cells were stimulated with LPS. ATB was administered as a pretreatment to C57BL/6 mice and MLE-12 cells. RESULTS: ATB significantly alleviated ALI and suppressed the inflammatory response induced by LPS. Further data sugge

Aortoesophageal fistula following emergency thoracic endovascular aortic repair: 2 case reports and brief review.

RATIONALE: Aortoesophageal fistula (AEF) following thoracic endovascular aortic repair (TEVAR) is a rare but highly fatal complication. Despite successful initial repair, factors such as large aneurysms and mediastinal hematoma may lead to persistent esophageal compression and ischemia, culminating in delayed AEF. This report presents 2 fatal cases to highlight the diagnostic challenges and management pitfalls of this condition, underscoring the need for increased vigilance in high-risk patients even after successful procedures. PATIENT CONCERNS: Case 1: A 60-year-old male was hospitalized af

Injuries of the Posterior Tracheal Wall: Insights from a High-Volume Single-Centre Experience.

Background: Major airway injuries, regardless of whether their aetiology is traumatic or iatrogenic, are rare but potentially fatal. In selected cases, surgery plays a key role; however, it has to be performed by highly experienced professionals in emergency settings. Methods: We reviewed all surgical procedures involving the trachea which were performed at our institution in the last 5 years (365 procedures). We report here our experiences with major airway injuries, both traumatic and iatrogenic (19 procedures). All patients, including individuals from within our hospital and from other peri

Bronchogenic Cyst with Degeneration of the Adjacent Membranous Portion of the Trachea: A Case Report.

INTRODUCTION: Bronchogenic cysts are congenital, benign cystic lesions that develop in the mediastinum. Many patients are asymptomatic, and conservative observation is often chosen in clinical practice. However, delayed surgical resection following cyst enlargement and compression of the adjacent membranous portion of the trachea can result in perioperative challenges. CASE PRESENTATION: We report the case of a 53-year-old woman who had been under observation for 10 years for an asymptomatic mediastinal mass. The mass enlarged gradually and caused persistent cough along with obstructive venti

[Two-stage Repair of Kommerell Diverticulum with Right Aortic Arch and Aberrant Left Subclavian Artery:Report of a Case].

A 52-year-old woman was referred to our hospital with a chief complaint of difficulty in swallowing and coughing while eating. Enhanced computed tomography (CT) revealed a Kommerell diverticulum with right aortic arch and aberrant left subclavian artery. The diverticulum compressed the esophagus and trachea. We avoided total aortic arch replacement because there were risks of circulatory arrest, selective cerebral perfusion and neurological complication including injury to recurrent laryngeal nerve. Therefore, we scheduled two-stage repair of the diverticulum. First, we performed axillo-axilla
