Beyond IgE: Expanding Perspectives on Multiclass Immunoglobulin Responses in Anisakis Allergy.

Anisakis allergy may involve antibody responses beyond IgE alone. Considering both mucosal and systemic immunoglobulin responses may improve our understanding of sensitization, immune regulation, and host-parasite interactions; although further studies are required before these observations can be translated into clinical practice.

Trophic niche segregation in small pelagic fishes suggests mesopelagic pearlsides as the main transmission pathway of Anisakis larvae in a Patagonian food web.

Many parasites are transmitted through trophic interactions, yet the specific pathways connecting intermediate and definitive hosts often remain poorly understood in marine food webs. In the Southwestern Atlantic, the Argentine anchovy (Engraulis anchoita) has been considered a potential vector of larval Anisakis to higher trophic levels, but low infection levels in San Matías Gulf suggest alternative transmission routes in this gulf. We investigated the role of pearlsides (Maurolicus spp.), abundant mesopelagic fishes in the gulf, in the transmission of anisakid nematodes. Examination of 229

Computed tomographic and endoscopic findings of multiple gastric anisakiasis.

A 36-year-old man presented with sudden onset of severe abdominal pain. A CT scan was performed first, which showed marked thickening of the gastric wall. Gastric anisakiasis was suspected because he had eaten sashimi (a raw fish dish), including salmon and tuna, the night before. An upper gastrointestinal endoscopy revealed three Anisakis larvae invading the gastric wall. All larvae were removed by the biopsy forceps. His symptoms were resolved after the removal. Anisakiasis is a gastrointestinal parasitic infection of humans caused by ingestion of raw or undercooked seafood containing Anisak

Antibody Avidity Profiles as Diagnostic Biomarkers in Differentiating Acute and Chronic Anisakis simplex-Related Allergic Diseases.

BACKGROUND/OBJECTIVES: Allergic features of anisakiasis, caused by ingestion of third-stage larvae of Anisakis simplex via raw or undercooked fish, manifest clinically as acute gastroallergic anisakiasis (GAA) or chronic urticaria with Anisakis sensitization (CU+). Differentiating these clinical phenotypes remains challenging. This study aimed to evaluate the maturation and avidity of specific antibodies (IgE, IgG4, IgG, and IgA) as biomarkers for discriminating between acute and chronic forms of anisakiasis. METHODS: A prospective cohort of 65 patients from Madrid, Spain, was classified into

Acute Superior Mesenteric Artery Embolism With Clinical Scenario Mimicking Anisakiasis.

Acute superior mesenteric artery (SMA) occlusion is a lethal but often misdiagnosed disease. A 66-year-old man presented with marked epigastric pain after eating raw fish. Emergency gastroscopy performed due to suspected anisakiasis revealed intestinal ischemia, leading to the diagnosis of SMA embolism based on the finding of newly detected atrial flutter without anticoagulation. His symptom improved after reperfusion by endovascular therapy using a self-expandable stent, and he was discharged with the ability to eat a normal diet after additional laparoscopic partial jejunectomy. In this inst

High-Pressure Processing for Anisakis Larvae Inactivation: Fish Quality Changes and Near-Infrared Spectroscopy to Verify Its Application.

The increasing consumption of raw and minimally processed fish products has raised concerns regarding the risk of anisakiasis, the infection caused by ingesting larvae of the Anisakis genus. Freezing is currently the standard control measure; however, alternative non-thermal technologies are being explored to preserve product quality while ensuring safety. Several studies have investigated the impacts of high-pressure processing (HPP) on seafood products, but limited information is available about the minimum effective pressure required to achieve complete inactivation of Anisakis larvae while

Marine Nematodes in Seafood: A Review of Food Safety Concerns, Advanced Detection Techniques, and Mitigation Strategies.

Marine nematodes are important food-borne parasites infecting many species of marine fish and cephalopods. This review provides a comprehensive discussion of the food safety concerns associated with marine nematodes in seafood, along with emerging methods for their detection. The potential prevention strategies for these seafood-borne parasites were also explored. According to the results, anisakiasis is the most serious nematode disease humans acquire from marine fish, chiefly due to Anisakis simplex larvae and, to a lesser degree, Pseudoterranova decipiens. Several analytical methods are emp

Anisakis allergy beyond elimination: High cooked-fish clinical tolerability and diagnostic value of two-stage ani sIgE.

BACKGROUND: Anisakis allergy (AA) is a major cause of adult food allergy in Japan. We evaluated the safety of seafood broth, cooked fish, and thawed frozen fish in AA, and the diagnostic value of early Anisakis-specific IgE (Ani sIgE) dynamics. METHODS: We conducted a single-center retrospective cohort study of 44 AA outpatients. Based on clinical tolerability to cooked fish and broth, patients were classified as gastroallergic anisakiasis-like type (GAA-type; asymptomatic with cooked fish/broth; clinically tolerant) or classical anisakis allergy-like type (classical AA-type; symptomatic). Genome assembly and SNP resources of Anisakis simplex enable cost-effective population assessment and discrimination of species and hybrids.

Anisakis spp are parasitic nematodes that can infect fish during larval stages, and, if consumed by humans, can cause anisakiasis and severe allergic reactions, including anaphylaxis. Here, we generated a 168 Mb genome assembly of A. simplex, the most abundant species in the Northeast Atlantic, which was used to develop SNP resources using a whole genome pooling resequencing strategy. 1,824,098 SNPs and 151,767 short structural variants were identified, showing the high polymorphism and complexity of A. simplex genome. A 481 SNP panel was developed for population screening using the Agriseq te

Wood creosote pretreatment of anisakid larvae attenuates extract-induced epithelial dysfunction in vitro.

Anisakiasis and parasite-associated hypersensitivity remain important food-safety concerns where raw marine fish is consumed. Although wood creosote (WC) has been studied mainly for effects on anisakid larval motility, its influence on host-relevant epithelial responses to larva-derived products is unknown. We examined whether WC pretreatment of thawed anisakid third-stage larvae alters the biological activity of crude larval extracts in Caco-2 cells. Crude extract from WC-pretreated larvae (CEWC) was compared with paired control extract (CE). CE and CEWC had similar protein concentrations and

The impact of image-enhanced endoscopy for the detection of parasites in the gastrointestinal tract.

I read with great interest the article by Wong Becerra et al., presenting vivid images of Anisakis larvae captured using blue-laser imaging (BLI) and magnification. I would like to congratulate the authors on providing novel insights into the identification and extraction of parasites in the stomach. I would like to comment on the utility of image-enhanced endoscopy (IEE) in cases other than neoplasms. Conventional white light endoscopy can sometimes have difficulty distinguishing the larvae from white mucus threads, light reflections, and the inflamed or edematous background mucosa. Narrow-ba

A case of extra-gastrointestinal anisakidosis mimicking peritoneal carcinomatosis.

We report a rare case of extra-gastrointestinal anisakidosis in a patient with abdominal pain. Initial computed tomography scan showed lesions concerning for peritoneal carcinomatosis. Endoscopy confirmed anisakidosis as the source of radiographic findings. Clinicians should consider anisakidosis in patients with gastrointestinal symptoms, omental lesions, and dietary risk factors.

First molecular identification of the zoonotic parasite Anisakis pegreffii (Nematoda: Anisakidae) in Albania.

For the first time, Anisakidae larvae in commercially caught fish from the southwestern Ionian Sea off the Albanian coast were identified using molecular techniques. Atlantic horse mackerel (Trachurus trachurus) from the Vlora district were examined for parasitic infections. Enzymatic digestion revealed the presence of Anisakis spp. larvae, which were subsequently analysed at ISS Rome by multiplex PCR and PCR-RFLP. Molecular results confirmed the larvae as Anisakis pegreffii. This study provides the first molecular confirmation of Anisakis species in Albanian waters. The detection of Anisakis

Immunological effects of coinfections with Toxoplasma gondii, Helicobacter pylori, and Epstein-Barr virus on anisakiasis associated phenotypes.

The hygiene hypothesis proposes that infections acquired through the fecal-oral route or contaminated food, such as Toxoplasma gondii, may protect against IgE-mediated allergic disorders. Epstein-Barr virus (EBV) has been negatively associated with atopy, while Anisakis simplex and Helicobacter pylori are associated with acute and chronic urticaria, respectively. This study aimed to explore the impact of coinfection status on anisakiasis-associated phenotypes. A total of 112 human subjects were categorized by Anisakis sensitization status and clinical phenotypes: 25 with gastroallergic anisaki

Determinants of Ascaridoid Nematode Infection and Anisakis-Related Zoonotic Exposure Risk in Eastern Mediterranean Gadiformes Fishes.

Larvae of ascaridoid nematodes, particularly Anisakis spp., are common parasites of commercially important marine fishes and may represent a zoonotic hazard following ingestion of raw or undercooked seafood. We investigated the ascaridoid fauna of the sympatric European hake (Merluccius merluccius) and greater forkbeard (Phycis blennoides) from the Ionian Sea (Eastern Mediterranean), integrating host biometric and seasonal drivers with molecular identification and quantitative risk assessment (QRA) for the zoonotic Anisakis pegreffii. Anisakis pegreffii was the dominant species in both hosts,
