CLAD: translating basic science to clinical practice.

Chronic lung allograft dysfunction (CLAD), a fibrotic manifestation of chronic rejection, remains the major barrier to long-term survival after lung transplantation. CLAD is increasingly recognised as the final common pathway of diverse alloimmune injuries, augmented by non-alloimmune insults, that culminate in irreversible airway and parenchymal remodelling. Recent advances in molecular profiling, single-cell technologies, and biomarker discovery, with corroboration by novel animal and in vitro models, are redefining CLAD pathogenesis. Emerging data implicate distinct pathways involving cytot

Intrathoracic rib fixation in flail chest: first South American experience.

Intrathoracic rib fixation represents a technical innovation with advantages over traditional methods through minimally invasive approach. We present the first South American case in a 21-year-old patient with flail chest from polytrauma. Four intrathoracic fixation plates (two 90 mm anterior and two 60 mm posterior) were used through video-assisted anterolateral and posterior approaches. The technique facilitates reduction through guide cables, intrathoracic positioning with minimal muscular disruption and automatic adaptation to costal anatomy. Total surgical time was 155 minutes with 250 mL

Degloving in Disguise: Managing a Delayed Spinal Morel-Lavallée Lesion after Thoracic Fracture-Dislocation.

INTRODUCTION: Back-degloving injuries are rare but severe consequences of high-energy blunt trauma. These injuries involve the separation of skin and subcutaneous tissue from the underlying fascia, muscle, or bone, and may present as open or closed lesions. Closed injuries, or Morel-Lavallée lesions, can be particularly insidious due to intact dermis concealing deep fluid accumulation, necrosis, and infection. CASE REPORT: We present the case of a 61-year-old male who sustained extensive polytrauma following a motorcycle-versus-car collision, with emphasis on his spinal injuries and delayed-o

Pre-hospital fluid resuscitation and return of spontaneous circulation in earthquake victims with cardiac arrest during the first 24 hours after the 2023 Kahramanmaraş earthquakes: Implications for disaster risk management.

BACKGROUND: Earthquakes can cause crush injuries and post-extrication deaths. Hyperkalemia, metabolic acidosis, hypovolemia, and circulatory failure are key mechanisms of cardiac arrest in victims at risk of crush syndrome. Although intravenous fluid resuscitation is recommended in disaster care, evidence on resuscitation outcomes remains limited. This study evaluated earthquake victims with cardiac arrest during the first 24 h after the 2023 Kahramanmaraş earthquakes and examined the association between pre-hospital fluid resuscitation, admission metabolic parameters, and return of spontaneou

Burking as a cause of homicidal asphyxia: A forensic case report.

Burking a rare and distinct form of homicidal asphyxia is when the perpetrator(s) obstruct all the external respiratory orifices while simultaneously applying thoracic compression. It is historically associated with the 19th-century serial killers Burke and Hare. It is a complex form of 'complicated asphyxia' which can present significant diagnostic challenges due to its potential for subtle or overlapping postmortem findings. This report describes the autopsy and forensic investigation of a 35-year-old male victim of a suspected homicide. External examination revealed periorbital ecchymosis,

Integrated Analysis of Metabolomics and Proteomics Reveals the Potential Mechanism of Lung Injury Induced by Di(2-Ethylhexyl) Phthalate.

Di(2-ethylhexyl) phthalate (DEHP) has been widely detected in environmental compartments and is highly carcinogenic to humans, but the toxic effects of DEHP on lung injury and its mechanisms are still not clear. Herein, the effects of lung injury were evaluated both in vivo and in vitro; meanwhile, the metabolomics and proteomics methodologies were employed to elucidate the intracellular mechanisms of BEAS-2B cells. The results showed that MEHP treatment significantly decreased cell viability and caused inflammation and fibrosis changes in vivo and in vitro, manifested as abnormal expression o

Hybrid Technique for a Rare Complication of Cardiac Devices Implantation.

Cardiac perforation is a rare complication of cardiac devices implantation. In this case report, a case of a 90-year-old man with early cardiac lead perforation is presented. The combination of capture failure on the electrocardiogram with the malposition of ventricular lead on the chest radiogram raised the suspicion for this complication, while the chest computed tomography confirmed the diagnosis. A hybrid management approach was applied for the removal of the initial ventricular lead. Although cardiac perforation is considered a potentially lethal complication, the early diagnosis and the

Acute type B aortic dissection after ruptured thoracoabdominal aortic aneurysm repair resulting in thoracoabdominal branch endoprosthesis collapse and malperfusion.

We report the case of a patient who developed an acute Stanford type B aortic dissection 4 days after thoracoabdominal branch endoprosthesis repair for a ruptured thoracoabdominal aortic aneurysm. The dissection caused complete endograft collapse and profound malperfusion. Emergent salvage endovascular repair successfully re-expanded the graft; however, the patient ultimately died because of the severity of the systemic injuries. This case highlights the potential for acute type B aortic dissection to occur in a recently stented thoracic aorta segment following thoracoabdominal branch endopros

Optimization of chest deflection definition in human body models: Implication for pedestrian injury assessment.

PURPOSE: Pedestrian chest injuries remain a significant global concern, ranking as the second leading cause of death in traffic accidents. Although human body model (HBM) provides a strain-based methodology for predicting pedestrian chest injuries, conventional deflection-based injury metrics remain valuable due to their hard validation of HBM biofidelity at the tissue response level. Consequently, achieving accurate chest injury assessment through HBM technology represents a critical biomechanical research challenge. This study aims to identify the optimal method for defining thorax deflectio

Heatwave exposure impairs airway epithelial repair via the Lkb1-Glut1 axis in juvenile mice.

Heatwaves (HW), intensified by global warming, pose significant risks to children's respiratory health because their lungs are still developing. Club cells, as key airway epithelial stem cells, are pivotal for repairing airway epithelium after lung damage. However, the role and repairing capacity of club cells following HW-induced lung injury in juvenile mice has not been evaluated. In this study, 4-6-week-old male C57BL/6J wide-type mice and conditional club cell-specific Lkb1 and Slc2a1 (the gene of Glut1) knockout mice were exposed to HW (36°C, 3 h/day for 21 days) and normal temperature as

Focused performance assessment predicts ability to perform open thoracic trauma procedures.

BACKGROUND: Extensive surgical expertise is required to manage blunt and penetrating thoracic trauma. Because opportunities to acquire and maintain these operative skills are limited during training and routine operative practice, trauma focused short courses help bridge potential procedural performance gaps. Integrating procedural assessment into these courses improves surgeon readiness by identifying and closing performance gaps. However, detailed assessment can increase cognitive load and fatigue for assessors. The purpose of this study was to reduce the assement burden by identifying the m

Venovenous Extracorporeal Membranous Oxygenation for Tracheoparenchymal Trauma: A Case Report.

Penetrating tracheal injuries present unique challenges for airway management, ventilation, and surgical repair due to the risk of rapid airway decline. We present a case of a 34-year-old male who sustained an arrow injury to the neck. Given the risk of airway compromise with induction or movement, venovenous extracorporeal membrane oxygenation (VV ECMO) was initiated prior to anesthesia. Awake bronchoscopy confirmed tracheal injury with an embedded arrow. The patient underwent successful tracheal repair and robotic arrow extraction under ECMO support. He was weaned off ECMO intraoperatively a

Delayed Recognition of a Cuff-Level Tracheal Rupture After Double-Lumen Endotracheal Tube Intubation for Thoracic Surgery: A Case Report.

BACKGROUND: Double-lumen endotracheal tubes (DLTs) are widely used to facilitate one-lung ventilation during thoracic surgery and are generally considered safe. However, tracheobronchial rupture is a rare but potentially life-threatening complication. Injuries occurring at the tracheal cuff level may initially remain clinically occult because the inflated cuff can obscure the injured area during bronchoscopic examination. CASE PRESENTATION: An 81-year-old man underwent left upper lobectomy for lung adenocarcinoma using a left-sided DLT. The procedure was converted from VATS to an anterolatera

Penetrating cardiac injury with no initial radiological findings and negative E-FAST: The critical role of clinical judgment.

We report the case of a man in his forties admitted to hospital after being stabbed in the precordial region. The initial chest X-ray and FAST examination revealed nothing abnormal. Despite apparent haemodynamic stability, the patient developed progressive dyspnoea, diaphoresis and signs of circulatory compromise. Due to clinical deterioration, an emergency exploratory thoracotomy was performed. Intraoperative findings revealed haemothorax, haemopericardium, and a perforation of the right atrium associated with pericardial rupture. The cardiac injury was successfully repaired, and the patient

Resveratrol Alleviates Chronic Intermittent Hypoxia-Induced Lung Injury by Inhibiting Ferroptosis via SIRT1-Dependent Modulation of the EGR1/GLS2 Axis.

AIMS: Obstructive sleep apnea (OSA)-associated chronic intermittent hypoxia (CIH) induces lung injury, partly through ferroptosis. Although resveratrol (Res) exhibits antioxidant activity, its mechanism in mitigating ferroptosis under CIH remains unclear. This study aimed to determine whether Res attenuates CIH-induced lung injury by suppressing ferroptosis and to elucidate the role of Sirtuin 1 (SIRT1)-dependent modulation of the early growth response factor 1 (EGR1)/glutaminase 2 (GLS2) axis. RESULTS: CIH models were established using Sprague-Dawley rats and human bronchial epithelial BEAS-

Assessment of Lung Injury Based on the Decompression Scheme of Lung Toxicity Dose Unit and K Value.

INTRODUCTION: This study aimed to assess hyperoxic lung injury (HLI) during a heliox dive based on the unit pulmonary toxic dose (UPTD), K index, and equivalent surface oxygen time (ESOT). METHODS: The UPTD, K index, and ESOT values of each schedule in the Helium-Oxygen Decompression Table of China were calculated. Based on the recommended thresholds, the risk of lung injury was analyzed, and suggestions were proposed to modify certain schedules. RESULTS: The K index and ESOT were more conservative than UPTD in predicting HLI. After a deeper dive, the UPTD, K index, and ESOT values were gene
