Polystyrene-associated pulmonary fibrosis: a summary based on the available evidence and prospects.

Microplastics have become a pervasive component of the atmosphere and of indoor and occupational air, and the human respiratory tract is now recognized as a major and largely unavoidable route of exposure. Polystyrene, one of the most abundant commodity plastics, is a dominant model and real-world airborne microplastic, and mounting evidence links inhaled polystyrene microplastics (PS-MPs) to a spectrum of pulmonary injuries that increasingly implicate fibrogenesis. This review synthesizes the current mechanistic understanding of how PS-MPs may contribute to pulmonary fibrosis, drawing primari

Intraoperative hypotension and postoperative acute kidney injury or myocardial injury after liver resection: A protocol for a systematic review and meta-analysis.

BACKGROUND: Liver resection creates a clinical tension between limiting operative bleeding and maintaining organ perfusion. Low central venous pressure, fluid restriction, vascular inflow occlusion and vasoactive treatment can coincide with lower arterial pressure, yet existing reviews have centred on blood loss and transfusion. They have not resolved whether directly quantified intraoperative hypotension is associated with postoperative acute kidney injury or myocardial injury, or whether comparative haemodynamic strategies alter these outcomes. We will address both questions while keeping ex

Self-inflicted thoracoabdominal nail-gun injury presenting as STEMI: diagnostic challenges in a time-critical care pathway.

A 60-year-old male with a history of depression and a prior nail-gun thoracic injury was found in shock at home, with inferior ST-segment elevation on the paramedic electrocardiogram prompting pre-hospital ST-elevation myocardial infarction pathway activation. Initial high-sensitivity troponin was elevated, consistent with the presumed diagnosis. Normal coronary angiography and preserved ventricular function were discordant with the patient's ongoing haemodynamic instability. Four small anterior chest puncture wounds and radiopaque foreign bodies demonstrated on chest radiograph and angiograph

Intelligent thermoregulation strategy versus routine thermal management on myocardial injury after noncardiac surgery during gastrointestinal tumor resection in elderly patients (SAGE): a multicenter, parallel-group, superiority trial.

BACKGROUND: Inadvertent perioperative hypothermia is associated with adverse cardiovascular outcomes, but adequately powered trials using high-sensitivity troponin endpoints is lacking. The SAGE trial evaluates whether an intelligent, closed-loop thermoregulation strategy reduces myocardial injury after noncardiac surgery (MINS) in high-risk elderly patients. METHODS: This prospective, multicenter, parallel-group, randomized controlled superiority trial conducted at seven tertiary hospitals in China. Participants aged ≥65 years scheduled for elective gastrointestinal tumor resection under gen

Fatal thoracic aortic transection following a motor vehicle collision: Medico-legal significance of the seat belt sign.

Traumatic thoracic aortic transection is one of the most lethal consequences of blunt chest trauma; it is commonly associated with high-speed deceleration injuries in road traffic collisions. Despite advances in trauma management, survival rates remain poor due to rapid internal haemorrhage and the presence of multiple associated injuries. We report a fatal case of thoracic aortic transection in a 50-year-old restrained male driver involved in a high-velocity head-on collision with a truck. Following admission, he presented with hypotension, left-sided flail chest, multiple long bone fractures

Measurement properties of pulmonary function measurement and patient-reported outcome measures in patients with multiple rib fracture without flail segment; validation study alongside the multicenter FixCon trial.

BACKGROUND: The reliability and validity of pulmonary function, pain and health related quality of life (HR-QoL) as outcome measures for patients with rib fractures, remain uncertain. Our objective was to evaluate measurement properties in patients with multiple rib fractures by comparing them with generic HR-QoL instruments (EuroQoL-5D-5L [EQ-5D-5L], Short Form-12 [SF-12]). METHODS: A multicenter RCT in adults (n = 180) was performed. Spirometry was performed at 30 days, 3, 6, and 12 months after trauma. Patients completed five questions on pain, EQ-5D-5L, SF-12, and two anchor questions on

The long thoracic nerve as a donor for elbow extension reconstruction in traumatic adult brachial plexus injuries.

BACKGROUND AND AIMS: Elbow extension reconstruction in traumatic adult complete or extended partial brachial plexus injuries (BPIs) is challenging owing to the limited availability of reliable uninjured donor nerves. The intraplexal long thoracic nerve (LTn), offers an underused donor option is selected circumstances. Its segmental innervation of serratus anterior allows selective distal harvesting with minimal functional loss. However, its use has been described only in small, heterogeneous series with limited outcome data. We aimed to evaluate the clinical utility and outcomes of LTn transfe

Successful repair of a penetrating right atrial gunshot injury via right posterolateral thoracotomy in a resource-limited hospital: a case report.

BACKGROUND: Penetrating cardiac injuries are uncommon but frequently fatal, particularly after gunshot wounds. Diagnosis may be challenging when pleural bleeding predominates, and pericardial fluid is minimal because blood may decompress through a pericardial defect into the thoracic cavity. CASE PRESENTATION: A 25-year-old man sustained a penetrating chest gunshot wound during wartime and presented to a resource-limited hospital in hemorrhagic shock with a massive right hemothorax. Right tube thoracostomy immediately drained approximately 900 mL of blood, but he remained hemodynamically unst

CLAD: translating basic science to clinical practice.

Chronic lung allograft dysfunction (CLAD), a fibrotic manifestation of chronic rejection, remains the major barrier to long-term survival after lung transplantation. CLAD is increasingly recognised as the final common pathway of diverse alloimmune injuries, augmented by non-alloimmune insults, that culminate in irreversible airway and parenchymal remodelling. Recent advances in molecular profiling, single-cell technologies, and biomarker discovery, with corroboration by novel animal and in vitro models, are redefining CLAD pathogenesis. Emerging data implicate distinct pathways involving cytot

Intrathoracic rib fixation in flail chest: first South American experience.

Intrathoracic rib fixation represents a technical innovation with advantages over traditional methods through minimally invasive approach. We present the first South American case in a 21-year-old patient with flail chest from polytrauma. Four intrathoracic fixation plates (two 90 mm anterior and two 60 mm posterior) were used through video-assisted anterolateral and posterior approaches. The technique facilitates reduction through guide cables, intrathoracic positioning with minimal muscular disruption and automatic adaptation to costal anatomy. Total surgical time was 155 minutes with 250 mL

Degloving in Disguise: Managing a Delayed Spinal Morel-Lavallée Lesion after Thoracic Fracture-Dislocation.

INTRODUCTION: Back-degloving injuries are rare but severe consequences of high-energy blunt trauma. These injuries involve the separation of skin and subcutaneous tissue from the underlying fascia, muscle, or bone, and may present as open or closed lesions. Closed injuries, or Morel-Lavallée lesions, can be particularly insidious due to intact dermis concealing deep fluid accumulation, necrosis, and infection. CASE REPORT: We present the case of a 61-year-old male who sustained extensive polytrauma following a motorcycle-versus-car collision, with emphasis on his spinal injuries and delayed-o

Pre-hospital fluid resuscitation and return of spontaneous circulation in earthquake victims with cardiac arrest during the first 24 hours after the 2023 Kahramanmaraş earthquakes: Implications for disaster risk management.

BACKGROUND: Earthquakes can cause crush injuries and post-extrication deaths. Hyperkalemia, metabolic acidosis, hypovolemia, and circulatory failure are key mechanisms of cardiac arrest in victims at risk of crush syndrome. Although intravenous fluid resuscitation is recommended in disaster care, evidence on resuscitation outcomes remains limited. This study evaluated earthquake victims with cardiac arrest during the first 24 h after the 2023 Kahramanmaraş earthquakes and examined the association between pre-hospital fluid resuscitation, admission metabolic parameters, and return of spontaneou

Burking as a cause of homicidal asphyxia: A forensic case report.

Burking a rare and distinct form of homicidal asphyxia is when the perpetrator(s) obstruct all the external respiratory orifices while simultaneously applying thoracic compression. It is historically associated with the 19th-century serial killers Burke and Hare. It is a complex form of 'complicated asphyxia' which can present significant diagnostic challenges due to its potential for subtle or overlapping postmortem findings. This report describes the autopsy and forensic investigation of a 35-year-old male victim of a suspected homicide. External examination revealed periorbital ecchymosis,

Integrated Analysis of Metabolomics and Proteomics Reveals the Potential Mechanism of Lung Injury Induced by Di(2-Ethylhexyl) Phthalate.

Di(2-ethylhexyl) phthalate (DEHP) has been widely detected in environmental compartments and is highly carcinogenic to humans, but the toxic effects of DEHP on lung injury and its mechanisms are still not clear. Herein, the effects of lung injury were evaluated both in vivo and in vitro; meanwhile, the metabolomics and proteomics methodologies were employed to elucidate the intracellular mechanisms of BEAS-2B cells. The results showed that MEHP treatment significantly decreased cell viability and caused inflammation and fibrosis changes in vivo and in vitro, manifested as abnormal expression o

Hybrid Technique for a Rare Complication of Cardiac Devices Implantation.

Cardiac perforation is a rare complication of cardiac devices implantation. In this case report, a case of a 90-year-old man with early cardiac lead perforation is presented. The combination of capture failure on the electrocardiogram with the malposition of ventricular lead on the chest radiogram raised the suspicion for this complication, while the chest computed tomography confirmed the diagnosis. A hybrid management approach was applied for the removal of the initial ventricular lead. Although cardiac perforation is considered a potentially lethal complication, the early diagnosis and the
