We report the case of a patient who developed an acute Stanford type B aortic dissection 4 days after thoracoabdominal branch endoprosthesis repair for a ruptured thoracoabdominal aortic aneurysm. The dissection caused complete endograft collapse and profound malperfusion. Emergent salvage endovascular repair successfully re-expanded the graft; however, the patient ultimately died because of the severity of the systemic injuries. This case highlights the potential for acute type B aortic dissection to occur in a recently stented thoracic aorta segment following thoracoabdominal branch endopros

PURPOSE: Pedestrian chest injuries remain a significant global concern, ranking as the second leading cause of death in traffic accidents. Although human body model (HBM) provides a strain-based methodology for predicting pedestrian chest injuries, conventional deflection-based injury metrics remain valuable due to their hard validation of HBM biofidelity at the tissue response level. Consequently, achieving accurate chest injury assessment through HBM technology represents a critical biomechanical research challenge. This study aims to identify the optimal method for defining thorax deflectio

Heatwaves (HW), intensified by global warming, pose significant risks to children's respiratory health because their lungs are still developing. Club cells, as key airway epithelial stem cells, are pivotal for repairing airway epithelium after lung damage. However, the role and repairing capacity of club cells following HW-induced lung injury in juvenile mice has not been evaluated. In this study, 4-6-week-old male C57BL/6J wide-type mice and conditional club cell-specific Lkb1 and Slc2a1 (the gene of Glut1) knockout mice were exposed to HW (36°C, 3 h/day for 21 days) and normal temperature as

BACKGROUND: Extensive surgical expertise is required to manage blunt and penetrating thoracic trauma. Because opportunities to acquire and maintain these operative skills are limited during training and routine operative practice, trauma focused short courses help bridge potential procedural performance gaps. Integrating procedural assessment into these courses improves surgeon readiness by identifying and closing performance gaps. However, detailed assessment can increase cognitive load and fatigue for assessors. The purpose of this study was to reduce the assement burden by identifying the m

Penetrating tracheal injuries present unique challenges for airway management, ventilation, and surgical repair due to the risk of rapid airway decline. We present a case of a 34-year-old male who sustained an arrow injury to the neck. Given the risk of airway compromise with induction or movement, venovenous extracorporeal membrane oxygenation (VV ECMO) was initiated prior to anesthesia. Awake bronchoscopy confirmed tracheal injury with an embedded arrow. The patient underwent successful tracheal repair and robotic arrow extraction under ECMO support. He was weaned off ECMO intraoperatively a

BACKGROUND: Double-lumen endotracheal tubes (DLTs) are widely used to facilitate one-lung ventilation during thoracic surgery and are generally considered safe. However, tracheobronchial rupture is a rare but potentially life-threatening complication. Injuries occurring at the tracheal cuff level may initially remain clinically occult because the inflated cuff can obscure the injured area during bronchoscopic examination.
CASE PRESENTATION: An 81-year-old man underwent left upper lobectomy for lung adenocarcinoma using a left-sided DLT. The procedure was converted from VATS to an anterolatera

We report the case of a man in his forties admitted to hospital after being stabbed in the precordial region. The initial chest X-ray and FAST examination revealed nothing abnormal. Despite apparent haemodynamic stability, the patient developed progressive dyspnoea, diaphoresis and signs of circulatory compromise. Due to clinical deterioration, an emergency exploratory thoracotomy was performed. Intraoperative findings revealed haemothorax, haemopericardium, and a perforation of the right atrium associated with pericardial rupture. The cardiac injury was successfully repaired, and the patient

AIMS: Obstructive sleep apnea (OSA)-associated chronic intermittent hypoxia (CIH) induces lung injury, partly through ferroptosis. Although resveratrol (Res) exhibits antioxidant activity, its mechanism in mitigating ferroptosis under CIH remains unclear. This study aimed to determine whether Res attenuates CIH-induced lung injury by suppressing ferroptosis and to elucidate the role of Sirtuin 1 (SIRT1)-dependent modulation of the early growth response factor 1 (EGR1)/glutaminase 2 (GLS2) axis.
RESULTS: CIH models were established using Sprague-Dawley rats and human bronchial epithelial BEAS-

INTRODUCTION: This study aimed to assess hyperoxic lung injury (HLI) during a heliox dive based on the unit pulmonary toxic dose (UPTD), K index, and equivalent surface oxygen time (ESOT).
METHODS: The UPTD, K index, and ESOT values of each schedule in the Helium-Oxygen Decompression Table of China were calculated. Based on the recommended thresholds, the risk of lung injury was analyzed, and suggestions were proposed to modify certain schedules.
RESULTS: The K index and ESOT were more conservative than UPTD in predicting HLI. After a deeper dive, the UPTD, K index, and ESOT values were gene

IntroductionThere remains a paucity of literature surrounding chest wall injury within military trauma. The purpose of this study was to examine the US Military Trauma database hypothesizing differences in chest wall injury patterns and rates given the different mechanisms of injury within this population.MethodsRecords of injuries that occurred between 2004 and 2020 from the US Military Trauma database were evaluated. Patients' injury characteristics were coded based on mechanism of injury (MOI) and injury severity score (ISS). Individuals with secondary injury types (ITs) were coded as combi

BACKGROUND: Horse-related injuries often involve high-energy blunt trauma that can be fatal. In cases of chest trauma associated with horse kicks, death is typically attributed to structural cardiac injuries such as cardiac rupture or tamponade. Fatal arrhythmia associated with myocardial contusion following a horse kick has rarely been reported in the forensic literature.
CASE PRESENTATION: A 48-year-old man was kicked in the left lower chest by the right hind limb of a Thoroughbred horse while handling the animal. The patient collapsed immediately after the impact. Emergency medical personn

BACKGROUND: Traumatic cardiac arrest (TCA) carries an overall poor prognosis, yet the impact of advanced age on prehospital management and outcomes remains poorly characterised. Whether older patients with TCA present distinct clinical profiles or respond differently to resuscitation compared with younger adults is largely unknown.
METHODS: We conducted a multicenter cohort study using data from the French National Out-of-Hospital Cardiac Arrest Registry (RéAC). All adult patients (≥18 years) with out-of-hospital TCA were included. Patients were categorised as adults (18-64 years) or older ad

A 42-year-old female sustained multiple unilateral rib fractures and a hemopneumothorax following a motor vehicle collision. A paravertebral catheter was placed for analgesia; however, extensive subcutaneous emphysema obstructed ultrasound visualization, necessitating landmark‑guided placement. A continuous ropivacaine infusion produced excellent analgesia; however, the patient subsequently developed a pleural effusion. Computed tomography revealed a segment of the catheter migrated into the pleural cavity, resulting in inadvertent intrapleural local anesthetic infusion. This case highlights t

UNLABELLED: Children with complex congenital heart disease (CHD) are at high risk of brain lesions both before and after cardiac surgery. Preoperative brain injury reflects foetal and perinatal brain development and represents an independent predictor of neurodevelopmental outcome. This study aimed to characterize preoperative brain lesions and their correlation with clinical course and neurodevelopmental outcomes in children with complex CHD. The secondary objective was to compare findings between syndromic and non-syndromic patients, as preoperative brain injury in syndromic children remains

OBJECTIVE: Tracheostomy decisions after operative cervical spine trauma vary widely, and existing prediction tools were developed in heterogeneous populations that included nonoperative patients or rely on variables not consistently available early after injury. No validated score exists specifically for surgically managed cervical trauma patients. The authors sought to develop and internally validate a practical risk score to estimate tracheostomy risk after operative cervical spine trauma.
METHODS: The authors performed a retrospective cohort study of adults (≥ 18 years) who underwent opera

BACKGROUND: Testosterone may assist in traumatic hemostasis given its well-established role in erythropoiesis, platelet activation, and vasoconstriction; however, whether exogenous testosterone improves acute outcomes in traumatic hemorrhage is unknown.
METHODS: Following PRISMA 2020, we registered the protocol in PROSPERO (CRD42024617294; 12 October 2024). MEDLINE, EMBASE (both via Ovid), and PubMed were searched from inception to November 2024 for studies that investigated the acute effects of testosterone administration in adults with hemorrhage. Two reviewers independently screened citati

BACKGROUND Image-guided pericardiocentesis is a diagnostic and potentially life-saving intervention, yet even under guidance, rare complications can occur. Among them, bowel perforation with subsequent fecal pericarditis is an exceptionally uncommon and often under-recognized occurrence. CASE REPORT We describe the case of a 72-year-old man with multiple comorbidities, including acute colonic pseudo-obstruction (Ogilvie syndrome), Parkinson disease, and chronic kidney disease, who underwent urgent fluoroscopy-guided pericardiocentesis for a large pericardial effusion. After the procedure, his

OBJECTIVE: 2011 Society for Vascular Surgery guidelines recommend surgical management for all > grade I blunt thoracic aortic injuries (BTAI). An update to these guidelines in 2026 recommends nonoperative management (NOM) for grade II injury. Previous work from the Aortic Trauma Foundation (ATF) database has combined grade I and II BTAI for analysis. We sought to analyze outcomes specifically for grade II injuries to determine if current outcomes represented in the ATF are in line with guideline updated NOM of grade II injuries.
METHODS: ATF data from 2014 to 2025 was retrospectively analyzed
