Control of airway basal stem cell-mediated lung repair by TGF-β signaling.

Airway basal stem cells (BCs) are recognized as resident stem cells responsible for reconstituting epithelial barriers and differentiating into multiple epithelial cell types following severe lung injury. However, the mechanism that the injured lung microenvironment uses to modulate BC-mediated lung repair remains unclear. Here, we showed that the profibrotic transforming growth factor-β (TGF-β) gradient from the injured lung immune microenvironment can be sensed by BCs. Targeted degradation of Smad2/3 in human BCs via proteolysis targeting chimera (PROTAC) disrupted TGF-β signaling, causing c

Photon FLASH spares radiation-induced changes in cardiac function, remodelling and arrythmia in a preclinical model.

INTRODUCTION: Preclinical studies have demonstrated the ability of FLASH irradiation to expand the therapeutic window by sparing normal tissues. The heart is a critical organ at risk in patients receiving radiotherapy for thoracic cancers. This study aimed to quantify the cardiac sparing effects of photon FLASH delivered as single (FLASH) or FLASH split dose (FSD) exposures. METHODS: Female C57BL/6 mice were irradiated with 18.5 ± 0.6 Gy delivered to the whole heart using a FLASH-SARRP (Xstrahl Life Sciences, UK) at a dose rate of 85.6 ± 1.6 Gy/s (isocentre dose rate 75.9 ± 1.5 Gy/s). Prehospital real-time AI for trauma mortality prediction: a multi-institutional and multi-national validation study.

Early identification of high-risk trauma patients in the prehospital setting is crucial for optimizing resource allocation and improving survival. We developed and externally validated a real-time AI model predicting emergency room mortality using 21 prehospital variables. Model development and internal validation utilized the Korean Trauma Data Bank (KTDB; 204,189 patients), and external validation included four South Korean trauma centers (8,358 patients) and one Australian Level 1 center (3,578 patients). Our Prehospital-AI model, an ensemble of XGBoost, LightGBM, and random forest, achieve

Use of fascial plane blocks for traumatic rib fractures: a scoping review.

BACKGROUND: The primary cause of morbidity and mortality in traumatic rib fractures is respiratory complications due to compromised respiratory mechanics secondary to pain and opioid-related respiratory depression. Thoracic epidural analgesia (TEA) provides effective analgesia but may not be possible in patients due to spinal cord injuries, thoracic vertebral fractures, and coagulopathy. New thoracic fascial plane blocks provide new options for patients with multiple rib fractures (MRFs). OBJECTIVE: Our primary objective was to assess the effectiveness of thoracic fascial plane blocks for pat

Chlorella-derived extracellular vesicle-based nanogels suppress cGAS-STING for treatment of radiation-induced lung injury.

Radiation-induced lung injury (RILI) represents a critical complication associated with radiation exposure, requiring targeted treatment strategies in clinical practice. Here, we pioneered the extraction of extracellular vesicles (EVs) from Chlorella and innovatively developed a multifunctional nanogel platform (RU.521-EVs NPs) by combining them with the cGAS inhibitor RU.521. The Chlorella-derived EVs possess a naturally high abundance of antioxidant enzymes (e.g., SOD2, CAT) and anti-inflammatory lipids (e.g., linolenic acid). RU.521-EVs NPs could synergistically inhibit excessive activation

Re: Small versus large bore chest tube in traumatic hemothorax, hemopneumothorax, and pneumothorax-external validity, endpoint heterogeneity, and trial sequential analysis assumptions : Author.

BACKGROUND: Granieri and colleagues compared small-caliber chest drains (14 Ch or smaller) with large-bore chest tubes (28 Ch or larger) for traumatic hemothorax, hemopneumothorax, and pneumothorax and combined conventional meta-analysis with trial sequential analysis. MAIN POINTS: While the pooled effect estimate suggested no clear difference in treatment failure, clinical translation requires caution. First, the randomized trials largely represent selected, stable thoracic trauma rather than high-risk phenotypes (for example, massive hemothorax, ventilated patients, or severe polytrauma). S

TRIM52 Protects Against Doxorubicin-Induced Cardiac Inflammation, Oxidative Stress and Cardiac Injury.

Tripartite motif 52 (TRIM52) has been identified as a key regulator of inflammatory responses. However, its involvement in doxorubicin (DOX)-induced cardiotoxicity (DIC) and the underlying molecular mechanisms remain poorly understood. To investigate the functional role of TRIM52, we employed an adeno-associated virus serotype 9 (AAV9) delivery system to achieve cardiac-specific Trim52 knockout via tail-vein injection. C57BL/6 mice received intraperitoneal DOX (5 mg/kg, administered once a week, with a total cumulative dose of 15 mg/kg). Myocardial injury was evaluated by histopathological ass

[Traumatic injuries of the thoracic aorta-Role of interventional radiology].

Acute aortic injuries are among the most life-threatening emergencies in vascular and emergency medicine. They are mostly caused by high-velocity trauma with deceleration and shear forces in the region of the aortic isthmus. Due to the anatomical features these particularly result in tears or complete transection of the vascular wall. Without rapid diagnosis and treatment the majority of cases are fatal. Contrast-enhanced computed tomography (CT) angiography is considered the diagnostic gold standard and enables a precise classification of the injury according to the severity and immediate tre

Behind the bruise: unmasking hidden blunt cardiac injuries.

Blunt cardiac injury in children is rare and may present with delayed complications. We report two cases illustrating diverse manifestations of blunt cardiac injury: one boy in middle childhood who developed ventricular tachycardia following chest trauma, while a girl of similar age presented with a right atrial pseudoaneurysm causing dyspnoea. Multimodality imaging guided successful medical and surgical management. These cases highlight the need for a high index of suspicion, ECG and echocardiographic screening in early identification of cardiac injury following blunt trauma to the chest.

Molecular Changes in CSPG and Glial Scar Markers in Response to Subpial Chondroitinase ABC Treatment Following Spinal Cord Injury.

Chondroitin sulphate proteoglycans (CSPGs) are major contributors to the inhibitory microenvironment that hinders regeneration following spinal cord injury (SCI). Chondroitinase ABC (ChABC) mediated degradation of CSPGs is associated with enhanced regenerative potential following SCI. In this study, the molecular responses of Neurocan, NG2 and Phosphacan CSPGs, as well as cellular markers for pro-inflammatory and neuroprotective microglia, astrocytes, oligodendrocytes and neurons, were investigated following a single subpial injection of ChABC (0.2 U in 10 μL) 14 days after thoracic (Th9) spin

Differential expression of apoptosis markers in early-phase sulfur mustard-induced pulmonary injuries.

Sulfur mustard (SM), a highly reactive alkylating agent, remains a significant threat to both military personnel and civilian populations due to its toxic effects. Although substantial research has been conducted, the precise apoptotic mechanisms triggered by SM exposure are not yet fully understood. This study aimed to assess potential differences in the apoptosis following SM-induced acute pulmonary injury via the two routes. An acute pulmonary injury model was developed using SM at an equitoxic dose (1 LD50), administered via either a single intraperitoneal injection or intratracheal instil

PM2.5-induced lung injury in rats: Identification of differential circRNAs and analysis of circRNA-miRNA-mRNA regulatory networks.

Exposure to high levels of fine particulate matter (PM) over a long period is strongly associated with an increased risk of morbidity and mortality from respiratory diseases. Although PM2.5 has been demonstrated to cause lung damage, the exact mechanisms are not fully understood. In this study, six-week-old SPF-grade SD rats were used as an experimental model for PM2.5 exposure via the endotracheal drip method. A library of circRNAs in rat lung tissues was constructed, and 16,020 circRNAs were successfully predicted, with the highest percentage being of the all-exon type. Validation of the scr
