Macroscopically detected female genital injury after consensual and non-consensual vaginal penetration: a prospective comparison study.

OBJECTIVE: The aim of this study was to compare the prevalence, type and pattern of macroscopically detected female genital injury after consensual and non-consensual vaginal penetration to further an understanding of the forensic significance of genital injury in women reporting sexual assault. A secondary aim was to identify any effect of a range of possible variables upon the likelihood of genital injury resulting from vaginal penetrative sexual intercourse. STUDY DESIGN: Two groups of reproductive age women (aged 18-45 years) were prospectively recruited within 72 h of a single episode of

Spanish version of Bus Drivers' Job Demands Scale (BDJD-24).

INTRODUCTION: The COVID-19 pandemic has placed an enormous strain on the Swiss healthcare system. This study aims to assess the associations of the pandemic on Switzerland's hospital resource use and in-hospital mortality among both COVID-19 and non-COVID-19 patients. METHODS: In this national cohort study, we analysed administrative claims data for medical inpatients from 1 January 2018 to 31 December 2021, using mixed-effects segmented regression models. Hospitalisations were divided into a control and an exposure group before (January 2018 to December 2019) and during (January 2020 to Dece

[Post-traumatic small-bowel obstruction: two case studies].

Post-traumatic small-bowel obstructions are rare and late complications following blunt abdominal trauma. Timely diagnosis is frequently impeded because of nonspecific associated symptoms, and furthermore, the degree of urgency is frequently underestimated due to the accidental cause. During the last 6 years (2005-2011), we have observed only 2 cases (4 and 10 years old) with post-traumatic small-bowel obstruction. On admission the patients had a contusion on the abdominal wall, duodenal hematoma and a Chance fracture of the lumbar spine on the computed tomography (CT) scan. Although early cli

Handlebar injuries in children.

INTRODUCTION: Handlebar injuries in children may lead to severe organ lesions despite minimal initial signs and without visible skin bruise. We present our experiences applying a diagnostic and therapeutic algorithm for blunt abdominal trauma, and present the history of two selected cases. MATERIALS AND METHODS: We retrospectively assessed the charts of children below 16 years of age, only who were observed for 24 h or more in our institution due to a handlebar injury between 2004 and 2011. All children were treated according to an institutional algorithm. RESULTS: 40 patients with a median

An assessment of the usefulness of routine histological examination in hanging deaths.

A retrospective study was carried out on 100 randomly selected medico-legal autopsies of victims who had committed suicide by hanging. All cases had undergone full police and coronial investigation. Complete external and internal examinations had been carried out including routine histological examination of organs. The age range of victims was 15-94 years (average, 41.7 years) with a male-to-female ratio of 7:1. External and internal injuries were consistent with the reported events. Diagnoses based purely on histology included hepatic steatosis (n = 16), asthma (n = 3), lymphocytic thyroidit

Ileal stenosis occurred 3 months after blunt abdominal trauma.

We present a case of ileal stenosis with delayed presentation 3 months after car accident. Ileal stenosis after blunt abdominal trauma is a rare clinical entity. We present CT and small bowel series 3 months after trauma. Image showed segmental thickening of intestinal wall and proximal bowel dilation. At surgery, a stenotic bowel loop was adjacent to a fibrotic mesentery. Histological examination showed ulcers, inflammatory cells and fibroblasts infiltrated to the muscularis mucosae, submucosa, and mesentery. The most likely cause, supported by most authors, implicates an injury to the mesent

A large spontaneous abdominal bruise in a 60-year-old woman: a case report.

OBJECTIVE: To report a case of severe abdominal bruising successfully diagnosed using systematic hemostatic investigations. CLINICAL PRESENTATION AND INTERVENTION: A 60-year-old woman developed a large spontaneous nontraumatic, painless bruise over the right lower abdominal wall. Computed tomographic scan of the abdomen showed a subcutaneous hematoma beneath the site of the obvious bruise diagnosed as a hematoma. Laboratory tests revealed an activated partial thromboplastin time of 53 s and a factor VIII level of <1%, consistent with the diagnosis of acquired hemophilia A. The patient was tre

A fatal elephant attack.

A rare case of an elephant attack is presented. A 44-year-old man working as an elephant keeper was attacked by a cow elephant when he tripped over a foot chain while the animal was being medically treated. The man fell down and was consequently repeatedly attacked with elephant tusks. The man sustained multiple stab injuries to both groin regions, a penetrating injury to the abdominal wall with traumatic prolapse of the loops of the small bowel, multiple defects of the mesentery, and incomplete laceration of the abdominal aorta with massive bleeding into the abdominal cavity. In addition to t

[Seat-belt syndrome in a pre-school child].

The authors report on a pre-school child with a typical presentation of the trauma caused by a car safety belt and termed the seat-belt syndrome. The deceleration mechanism which throws the upper body forward in the presence of a rigid belt over the abdominal region results in large contusions of the abdominal wall and trauma to the spine, retroperitoneum, and intra-abdominal organs, or even the chest. In making the diagnosis a thorough examination of the spine is necessary as well as a repeated surgical examination of the abdomen, because seat-belt syndrome is often associated with small inte

Predictive indicators for bowel injury in pediatric patients who present with a positive seat belt sign after motor vehicle collision.

PURPOSE: Abdominal wall bruising (AWB) is a frequent finding in children wearing seat belts involved in motor vehicle collision (MVC) and is highly suspicious but not indicative of intestinal injury. The aim of this study was to find objective clinical and radiologic predictors for the need of an abdominal exploration in these children. MATERIALS AND METHODS: A retrospective chart review of children admitted from 1998 and 2008 with AWB after MVC was conducted. Demographics, vital signs, physical examinations, radiologic investigations, associated injuries, management, and outcome were extract

Partial avulsion of common bile duct and duodenal perforation in a blunt abdominal trauma.

Complete or partial avulsion of common bile duct is a very rare injury following blunt abdominal trauma in children. A 7-year old boy presented to ER following blunt abdominal trauma by a moving motorcycle. X ray abdomen revealed free air under diaphragm and CT scan showed pancreatic contusion injury. At operation anterior wall of common bile duct (CBD) along with a 2mm rim of duodenal tissue on either side of anterior wall of CBD were found avulsed from the duodenum. The avulsed portion of CBD and duodenum were reanastomosed and a tube cholecystostomy performed. The patient had an uneventful

Rib fracture patterns predict thoracic chest wall and abdominal solid organ injury.

Blunt trauma patients with rib fractures were studied to determine whether the number of rib fractures or their patterns were more predictive of abdominal solid organ injury and/or other thoracic trauma. Rib fractures were characterized as upper zone (ribs 1 to 4), midzone (ribs 5 to 8), and lower zone (ribs 9 to 12). Findings of sternal and scapular fractures, pulmonary contusions, and solid organ injures (liver, spleen, kidney) were characterized by the total number and predominant zone of ribs fractured. There were 296 men and 14 women. There were 38 patients with scapular fracture and 19 p

Lower abdominal wall pain and contusion in single-incision laparoscopic cholecystectomy.

There is a surge in interest in single-incision laparoscopic surgery in the recent years. Due to entry of all the ports and instruments from the same incision and obliquity of the instruments, the lowermost port exerts repeated pressure on the infraumbilical abdominal wall. We are reporting 2 cases of single-incision laparoscopic surgery cholecystectomy who presented with lower abdominal pain and contusion in the post operative period.

Bruising frequency and patterns in children with physical disabilities.

OBJECTIVE: We obtained normative data on bruising in children with physical disability in functioning and evaluated factors associated with bruising in this population. METHODS: We studied children with physical and/or cognitive disabilities who attend a school that provides comprehensive services. Over a 15-month period, the children had skin examinations, including external inspection of the genitalia and anus. For each child, we gathered information on demographics, medications, growth measures, medical conditions, equipment used, and muscle tone. Functional independence in basic mobility,

[Acute traumatic abdominal wall hernia after blunt abdominal trauma].

Traumatic abdominal wall hernias are rare injuries despite the high incidence of blunt abdominal traumas. The mechanism of this injury includes a sudden increase in intra-abdominal pressure and extensive shear forces applied to the abdominal wall. The typical location is found at anatomic weak areas in the lower abdomen. Often, significant intra-abdominal injuries or injuries of the pelvis and chest are associated. We describe a case of an abdominal contusion trauma leading to a traumatic abdominal wall hernia beside the rectal sheath. In this case, parts of the small bowel penetrated through

Isolated Gallbladder Injury after Blunt Abdominal Trauma: a Case Report and Review.

We describe a case of isolated gallbladder contusion after deceleration trauma. On admission, no evident signs of intra-abdominal injury were present. However, after 24 h observation an explorative laparotomy was performed because of persistent abdominal complaints. A contusion of the gallbladder wall was found with intraluminal haematoma and a cholecystectomy was performed. Isolated injury of the gallbladder after blunt trauma is extremely rare. Risk factors are distention of the gallbladder, deceleration trauma and the presence of a relatively mobile gallbladder. Clinical signs often are ver

Osteochondroma developing from the xyphoid appendix into an abdominal wall scar from a previous laparotomy.

We describe herein the case of a 45-year-old man who developed an osteochondroma from the xyphoid appendix into an abdominal wall scar from a laparotomy performed 4 years previously. To our knowledge, rare cases of osteochondroma of the xyphoid bone have been documented in the literature. As shown by the tumor's rapid development during a period of only 4 years, osteochondromas arise from, or grow well under, inflammatory and cicatricial conditions. Periosteal contusion causing growth-plate cartilage migration and enchondral ossification seems to be the first presentation of osteochondroma.
