[Diagnosis and tactics in multiple trauma].

It is difficult to establish the diagnosis in combined injuries because of the peculiar pathogenesis leading to unusual symptomatology and diagnostic errors. This is associated with some peculiarities of a combined injury: it is marked by the syndrome of mutual aggravation; signs of less severe injuries often come to the forefront; the usual complex of symptoms may be indistinct or even absent. For instance, in internal hemorrhage combined with contusion of the brain arterial pressure may be normal or even increased, while the signs of "acute abdomen" may not be manifested as the result of dim

[Syndrome of "Pseudo acute abdomen" in minor craniocerebral injury].

In 112 sufferers with light craniocerebral trauma, laparocentesis, or microlaparotomy was performed for "pseudoacute abdomen". The main causes of the development of the syndrome were distinguished: fracture or contusion of the bones, contusion of the abdominal wall and organs of the abdominal cavity, exacerbation of chronic disease of the abdominal cavity, injury to the organs adjacent to the diaphragm.

Geriatric trauma.

Elderly individuals not only live longer but are also more active than in the past. Accompanying this increase in activity is the number of older trauma victims. The effect of aging on response to injury is reviewed, and the initial evaluation and treatment of geriatric trauma are delineated in this article. Specific injuries of note include head and chest wall injury; pulmonary and cardiac contusion; abdominal trauma; and aortic, spinal, and musculoskeletal injury.

Seat-belt injuries of the spine in young children.

Seat-belt fractures of the lumbar spine in adolescents and adults are well recognised but there are few reports of these injuries in young children. We reviewed all seat-belt injuries in skeletally immature patients (Risser 0), seen at a tertiary referral centre between 1974 and 1991. There were ten cases, eight girls and two boys, with an average age of 7.5 years (3 to 13). Four distinct patterns of injury were observed, most commonly at the L2 to L4 level. Paraplegia, which is thought to be uncommon, occurred in three of our ten cases. Four children had intra-abdominal injuries requiring lap

Rupture of the duodenum with avulsion of the papilla of Vater due to blunt trauma in a child, and review of the literature.

Duodenal rupture with avulsion of the papilla of Vater is an uncommon injury due to blunt abdominal trauma in children. This report describes a 9-year-old boy who sustained this injury and was treated successfully two days delayed by a Roux-en-Y choledocho-pancreaticojejunostomy and temporary implantation of a zipper for abdominal wall closure. The rarity of this complex injury and its initial presentation as a posttraumatic pancreatitis warrant its description. A review of the literature, modes of diagnosis and surgical technique for repair are described.

Sigmoid colon obstruction due to blunt abdominal trauma: a case report.

Post-traumatic colonic stenosis (obstruction) is rare. We experienced a case of sigmoid obstruction due to blunt abdominal trauma. A 75-year-old man was hit on the lower abdomen 3 days before admission and gradually developed abdominal pain and distension. Laboratory data showed severe inflammation and a barium enema disclosed obstruction of the sigmoid colon. Conservative treatment was carefully carried out, because there was no sign of peritoneal irritation and there were passages of normal stool and flatus. The sigmoid obstruction gradually improved and the stenosis was almost undetectable

Delayed cardiac rupture due to blunt chest trauma.

A 7-year-old boy fell from a 2.5-m height and suffered contusion of the thorax with hematoma in the soft tissues of the lateral chest wall. The rib cage itself remained intact. After 3 days, there was only local pressure sensitivity of the left hemithorax. On day 8 after the trauma, the child, who up to then had seemed to be in good health, suddenly died during lessons at school. Autopsy revealed a full-thickness rupture of the left ventricle with resultant cardiac tamponade. The macroscopic and histopathologic findings are presented and discussed.

[Blunt injuries of the gastrointestinal tract].

This study represents our experience with severe blunt gastrointestinal injury in 29 polytrauma and 3 solitary trauma patients. The primary diagnosis was missed in intestinal perforation (4x) and mesenteric disrupture (1x). In 2 other patients contusion of the gastrointestinal wall led to postprimary perforation. The average mortality was 21.8%; only 2 patients died from abdominal sepsis. Blunt gastrointestinal trauma differs from penetrating injury as far as the diagnostic and therapeutic aspects are concerned. In some cases, explorative laparotomy alone leads to early diagnosis. Prediction of the delayed complications of intestinal and mesenteric injuries following experimental blunt abdominal trauma.

Injuries to the intestine and mesentery are often found in patients undergoing laparotomy for blunt abdominal trauma. Although treatment of perforations is relatively straightforward, the same is not true for contusions. Few guidelines exist at present to aid the surgeon in deciding which injuries require resection in order to avoid the complications of delayed perforation and late stricture formation. The natural history of these non-perforating intestinal and mesenteric injuries has been examined in an experimental model to identify possible criteria on which future management can be based.

Biophysics of impact injury to the chest and abdomen.

The physical mechanism of blunt impact injuries to thoracic and abdominal viscera is often conveniently described simply in terms of "crush"--this is an over-simplification. Any impact to the torso does result in the rapid displacement of the body wall which may lacerate and contuse underlying viscera, but this simple explanation does not account for pathology at sites some distance from the contact point and does not adequately describe the dependence of the severity and location of injury upon the rate of energy transfer. Quite minor displacements of the body wall may produce serious injury

Some observations relating to behind-body armour blunt trauma effects caused by ballistic impact.

Live, anesthetised pigs were used to assess behind-armour blunt trauma effects. The thoraco-abdominal body region was covered with varying thicknesses of Kevlar fabric packets. This soft body armour was applied, either in direct contact with the thoracic wall of the animals, or with different plastic foam sheets, so-called trauma packs, between the armour and the skin. The live animals were surgically evaluated, and then sacrificed. Blocks of soft soap were subjected to equal tests and the behind-armour indentations were measured. The results indicate that serious injury to the body armour-pro

Transverse disruption of the abdominal wall--a tell-tale sign of seat belt related hollow viscus injury.

A pattern of injuries has been observed in five (5) female front seat passengers wearing seat belts. All were involved in high speed frontal impact motor vehicle accidents on country roads. Varying degrees of abdominal wall disruption involving fat, fascia or muscle, were universally associated with hollow viscus injury and right-sided rib fractures. Most patients had mesenteric or omental tears, flail chests and left clavicular injuries. Intimal tears of the distal aorta, right breast injuries and spinal injuries were also observed. The hollow viscus and mesenteric injuries may result from di

Controversies in the management of pulmonary contusion.

Most polytraumatized patients with lung contusion also have injury to the chest wall. Both are serious but the lung contusion is most important in contributing to respiratory failure. A small and localized injury to the lung leads to generalized edema in surrounding lung and even in the opposite lung. The permeability of capillaries is affected and there is an outpouring of the fluid through a leaking pulmonary membrane. Diagnosis is easy when the condition is straightforward and severe, but may be complicated in a confused or unconscious patient with or without head, abdominal and orthopedic

[Rupture of the abdominal wall in highway accident victims wearing seat-belts].

Rupture of the rectus abdominis muscle represents one of the rare specific lesions due to seat-belts. Two cases are presented which illustrate diagnostic difficulties and the value of the warning provided by these lesions, which are very frequently associated with severe contusion of the small intestine or colon.

Management of acute full-thickness losses of the abdominal wall.

Over a 20-year interval, 167 patients sustained acute full-thickness abdominal wall loss due to necrotizing infection (124 patients), destructive trauma (32 patients), or en bloc tumor excision (11 patients). Polymicrobial infection or contamination was present in all but five of the patients. Of 13 patients managed by debridement and primary closure under tension, abdominal wall dehiscence occurred in each. Only two patients survived, the 11 deaths being caused by wound sepsis, evisceration, and/or bowel fistula. Debridement and gauze packing of a small defect was used in 15 patients; the sin

On the basis of two observations of courses is demonstrated that not in the least only hypercoagulability and retardation of the movement of the blood but also contusions of the femoral and hip region by fall and thrown which first of all appear as harmless may become the starting-point for alterations of the venous wall with extended thromboses of the veins of leg and pelvis under inclusion of the infrarenal section of the lower vena cava with a distinct picture of the postthrombotic condition of the lower extremities and a threatening embolism of the lungs. [Diagnosis and management of abdominal injuries (author's transl)].

Prognosis and mortality rate of abdominal injuries are substantially influenced by early diagnosis and thereby early definite operative treatment. Especially in cases of multiple traumata a subtle search for intraabdominal lesions in needed. 242 patients with abdominal traumata were analysed. 142 (= 58%) of them had contusions of the abdominal wall only, 83 (= 36%) had blunt and 17 (= 6%) had perforating abdominal injuries. In two-third of blunt traumata with organ lesions the operation was indicated only by clinical symptoms. The preoperative radiological examinations revealed organ lesions i

[Traumatic myocardial infarct].

Two cases of traumatic, closed chest, myocardial infarction in two young patients, aged 18 and 19 years respectively, are reported. They illustrate the two possible physiopathological mechanisms of this affection: in the first case, myocardial contusion after thoraco-abdominal trauma by crushing, probably complicated by a subendocardial tear of the inferior wall of the left ventricle, with a spontaneous favourable outcome; in the second case, a transmural myocardial infarction was observed secondary to a coronary lesion (? tearing of the adventitia of the left anterior descending artery) compl

The alternative to colostomy for the injured colon.

In 34 patients with penetrating colon wounds which were considered to be liable to dehiscence, the sutured wounds were exteriorized. There were several 'high-risk' factors, i.e. operative delay of more than 6 hours after injury, faecal contamination of the peritoneal cavity, marked contusion of the bowel wall, and severe associated visceral haematoma which is deemed liable to infection. Additional considerations were thoraco-abdominal penetration and combined colonic and renal injuries. The operative technique is described in detail, and the morbidity of the procedure, which is low, is analyse
