Global distribution, clinical characteristics, and outcomes of human intestinal capillariasis, 2000-2025: a systematic review.

BACKGROUND: Intestinal capillariasis, caused by Capillaria philippinensis, is an uncommon but potentially fatal parasitic infection associated with the consumption of raw or undercooked freshwater fish. Despite its sporadic occurrence, its clinical severity and diagnostic challenges warrant a comprehensive synthesis of available evidence. This systematic review aimed to summarize the geographic distribution, risk factors, clinical manifestations, diagnostic approaches, and treatment outcomes of human intestinal capillariasis. METHODS: A systematic search of six electronic databases (PubMed, S

Intestinal Capillariasis in a South Indian Farmer: A Report of a Rare Case From a Tertiary Care Hospital.

Intestinal capillariasis is an uncommon food-borne helminthic infection caused by Capillaria philippinensis. It is classically reported from Southeast Asian countries, but sporadic cases from non-endemic regions, including India, show that the disease may be overlooked in routine clinical practice. We report the case of a 35-year-old male farmer from South India who presented with six months of chronic watery diarrhoea, abdominal pain, and marked weight loss. Clinical examination suggested malnutrition, and laboratory evaluation showed mild anaemia and eosinophilia. Stool microscopy revealed c

Development of an Immunochromatographic Test to Detect Antibodies in Simulated Whole-Blood Samples for the Diagnosis of Intestinal Capillariasis.

Intestinal capillariasis is a zoonotic parasitic disease caused by Capillaria philippinensis, a fish-borne nematode. Patients present with clinical symptoms that include borborygmus chronic diarrhea, intermittent abdominal pain, weight loss, and several degrees of painless lower-leg edema. Atypical clinical manifestations and diagnostic confusion with diarrhea caused by gastrointestinal cancer, HIV infection, giardiasis, and hyperthyroidism may occur. Misdiagnosis and improper treatment can be fatal. Parasite eggs and larvae are passed intermittently in feces, rendering fecal examination insen

First report of Paracapillaria philippinensis infection in flowerhorn cichlid in India.

The flowerhorn cichlid is a popular ornamental fish in many Asian countries. The present study reports the occurrence of Paracapillaria philippinensis (Chitwood et al., 1968), a parasitic nematode in flowerhorn cichlid (Cichlasoma sp.) from south India. The infected fish demonstrated clinical symptoms viz. dark coloration, poor appetite, lethargy, erratic swimming, and white stringy faeces. Microscopic observation of the intestinal content and faeces revealed the presence of adult worms, larvae, and unembryonated eggs. PCR amplification of eukaryotic 18S rRNA, P. philippinensis-specific SSU rR

High Prevalence of Intestinal Capillariasis in Chronic Diarrhea Patients in Thailand: Serological Screening Using a Rapid Lateral-Flow Immunochromatographic Assay.

Intestinal capillariasis is an emerging fish-borne helminthic disease caused by the round worm Capillaria philippinensis. Chronic infection may lead to death if the disease is misdiagnosed and inappropriate treatment is given. We used a rapid lateral-flow immunochromatographic test for screening of intestinal capillariasis in patients with chronic diarrhea. We screened 292 chronic diarrhea patients who had visited hospitals in Thailand. Sixty-six (22.6%) cases were positive according to the kit. All positive patients received mebendazole at 200 mg twice per day for 30 consecutive days or alben

Albendazole and Mebendazole as Anti-Parasitic and Anti-Cancer Agents: an Update.

The use of albendazole and mebendazole, i.e., benzimidazole broad-spectrum anthelmintics, in treatment of parasitic infections, as well as cancers, is briefly reviewed. These drugs are known to block the microtubule systems of parasites and mammalian cells leading to inhibition of glucose uptake and transport and finally cell death. Eventually they exhibit ovicidal, larvicidal, and vermicidal effects on parasites, and tumoricidal effects on hosts. Albendazole and mebendazole are most frequently prescribed for treatment of intestinal nematode infections (ascariasis, hookworm infections, trichur

Antigen Recognition Patterns of Intestinal Capillariasis Using Immunoblot-Based Serodiagnosis.

BACKGROUND: Diagnosis of intestinal capillariasis depending on microscopic detection of parasitic stages is of low sensitivity, especially in cases with low worm burden. There is a necessity to develop sensitive and specific diagnostic tools of capillariasis for early treatment to avoid complications. Western blot (WB) technique showed promising results for antigen recognition patterns in several parasitic infections. AIM OF THE STUDY: This study is directed to identify and evaluate relevant proteins of intestinal capillariasis crude worm antigens using WB immunodiagnosis. MATERIALS AND METH

Comparison of the diagnostic performance of microscopic examination, Copro-ELISA, and Copro-PCR in the diagnosis of Capillaria philippinensis infections.

Intestinal capillariasis is a parasitic zoonosis caused by the tiny nematode parasite Capillaria philippinensis. It is a major health problem that may lead to death if not diagnosed and treated appropriately. The difficulties in the diagnosis of C. philippinensis highlight the importance of developing accurate, sensitive, and specific methods for early diagnosis. This study aimed to detect the presence of C. philippinensis infection among 42 clinically suspected patients with certain criteria that are highly suggestive of capillariasis and to compare the diagnostic yield of microscopy, copro-E

A Hospital-Based Study of Intestinal Capillariasis in Thailand: Clinical Features, Potential Clues for Diagnosis, and Epidemiological Characteristics of 85 Patients.

Intestinal capillariasis caused by Capillaria philippinensis, a fish-borne nematode, is an important, emerging zoonotic helminthiasis. Cases may be fatal if suitable treatment is not administered in time. We reported a hospital-based study of 85 cases in Thailand, most of which were in the northeast. All patients had a history of eating raw or insufficiently cooked fresh water fish or prawns. The clinical manifestations are characterized by chronic diarrhea, borborygmi, abdominal pain, marked weight loss, muscle weakness, fatigue, dizziness, anorexia, and edema, as well as protein and electrol

Development and evaluation of a rapid diagnostic immunochromatographic device to detect antibodies in sera from intestinal capillariasis cases.

Intestinal capillariasis, a fish-borne nematodiasis, is an important emerging zoonotic disease. Patients present clinical symptoms of borborygmus chronic diarrhea, intermittent abdominal pain, weight loss, and several degrees of painless lower-leg edema. Death may occur in cases of misdiagnosis and improper treatment. Diagnosis is difficult because of the atypical clinical symptoms and diagnostic confusion with diarrhea caused by gastrointestinal cancer, opportunistic infections in human immunodeficiency virus patients, and hyperthyroidism. In addition, parasite eggs are not always found in st

Experimental Assessment of the Efficacy of Five Veterinary Broad-Spectrum Anthelmintics to Control the Intestinal Capillariasis in Zebrafish (Danio rerio).

Zebrafish in research facilities are frequently infected with capillarids. Since the health status (as a nonprotocol source of variation) of zebrafish can affect the validity of experiments, it is important to develop therapies for common zebrafish diseases. Regarding the likelihood of (1) the development of drug resistance and (2) the loss of the efficacy of a drug for laboratory zebrafish, the availability of alternatives for treatment is of direct importance. The efficacy of five dewormers from the same or different therapeutic groups was assessed in the current study. The exposure to each

Some studies on Capillaria philipinensis and its mysterious trip from Philippines to Egypt (review article).

Capillaria philippinensis is a mysterious parasite and intestinal capillariasis is a mysterious disease. It is now more than half a century since the discovery of the first case in Philippines without answering many questions concerning the parasite's taxonomy, morphology, life cycle, diagnosis, pathology, clinical symptoms, mode of transmission as well as how it was transported to Egypt and how it started to spread and progressed in most Egyptian Governorates; particularly those of Middle Egypt. This article is a trial to overview all these aspects of the parasite.

Intestinal capillariasis in the 21st century: clinical presentations and role of endoscopy and imaging.

BACKGROUND: Intestinal capillariasis is one of the common causes of malabsorption in the East. Reports emphasizing the roles of clinical, endoscopic and radiologic findings of intestinal capillariasis are limited. METHODS: Retrospective review of medical records of 26 patients diagnosed with intestinal capillariasis at Siriraj Hospital, Bangkok, Thailand between 2001- 2013. RESULTS: Clinical manifestations were chronic watery diarrhea (93%), chronic abdominal pain (70%), significant weight loss (92%), hypoalbuminemia (100%; 85% lower than 2.0 g/dL), and anemia (50%). The median duration of s

Endoscopic modified medial maxillectomy for odontogenic cysts and tumours.

The persistent hyperglycemic microenvironment in diabetic wounds causes dysfunction of repair cells, resulting in impaired angiogenesis and disorganized extracellular matrix (ECM) deposition. Small extracellular vesicles (sEVs) derived from hypoxia-challenged chorionic plate mesenchymal stem cells (CP-MSC-sEVsHypo) have been explored as novel acellular therapeutics. However, the effects of oxygen tension during culturing parent cells on the pro-regenerative efficacy and mechanisms of CP-MSC-sEVs have not been systematically investigated. In addition, their rapid clearance and limited interacti

Intestinal capillariasis diagnosed by endoscopic biopsy.

Many new parasitic infections have emerged in Korea, with >35 new species appearing since the 1980s. Among them, Capillaria species are unique for contributing to morbidity in many countries as well as in Korea. Since the first reported case of a 41-year-old male patient diagnosed with intestinal capillariasis in 1991, a total of six cases have been reported thus far. In this case report, we present another imported case of intestinal capillariasis in Korea, in which a 42-year-old male patient presented with intractable diarrhea and weight loss. The diagnosis was confirmed by biopsy of the ile

An indigenous case of intestinal capillariasis with protein-losing enteropathy in Korea.

We encountered an indigenous case of intestinal capillariasis with protein-losing enteropathy in the Republic of Korea. A 37-year-old man, residing in Sacheon-si, Gyeongsangnam-do, admitted to the Gyeongsang National University Hospital (GNUH) due to long-lasting diarrhea, abdominal pain, anasarca, and weight loss. He recalled that he frequently ate raw fish, especially the common blackish goby (Acanthogobius flavimanus) and has never been abroad. Under the suspicion of protein-losing enteropathy, he received various kinds of medical examinations, and was diagnosed as intestinal capillariasis
