Hepatic migration of metallic wire from gastrointestinal tract: a rare case report.

INTRODUCTION AND IMPORTANCE: The translocation of gastrointestinal (GI) foreign bodies to hepatic tissue is exceptionally rare, occurring in less than 1% of foreign body ingestion cases. Most ingested objects either remain within the GI tract or are naturally eliminated. CASE PRESENTATION: We present a 57-year-old man with 24-hour acute upper abdominal pain. Despite denying intentional foreign object ingestion, computed tomography (CT) revealed a linear metallic structure (3.5 cm) in the second hepatic segment. Laboratory analysis showed mild leukocytosis and elevated interleukin-6 levels. Th

Interparietal hernias after posterior component separation.

BACKGROUND: Incisional ventral hernia repair is a complex problem. Rives-Stoppa technique is the most common procedure for patients with large incisional hernias. However, in cases of incisional hernias with a large defect width and/or the rectus to defect ratio (RDR) of less than 2, component separation is often required. At the same time, posterior component separation (PCS) is a technically challenging procedure. Currently, the evaluation of postoperative outcomes after PCS is primarily based on analysis of wound morbidity and recurrence rate. The aim of our study was to evaluate long-term

Open, complex abdominal wall reconstruction with synthetic versus biologic mesh: Outcomes with a minimum of 5-year follow-up.

INTRODUCTION: Reported outcomes for synthetic mesh versus biologic mesh in open abdominal wall reconstruction vary widely and are influenced by mesh type, location, fascial closure, wound complications, and patient characteristics. Little comparative data controlling for these variables or long-term results exists. This study compared outcomes between synthetic mesh and biologic mesh with >5 years of follow-up. METHODS: A prospectively maintained database was queried for open abdominal wall reconstruction with >5 years of follow-up. A 1:1 propensity score match was performed based on age, bod

Transversus abdominis release in incisional hernia repair: a comprehensive review.

Incisional hernia remains a significant challenge following abdominal surgery, necessitating robust repair strategies to mitigate recurrence and wound complications. While the introduction of synthetic mesh has improved durability, complex defects with significant loss of domain or lateral extension often exceed the limitations of traditional repair techniques. The transversus abdominis release (TAR) has emerged as a cornerstone technique in abdominal wall reconstruction, offering a physiological solution by mobilizing the posterior rectus sheath and creating a wide retromuscular plane for ext

Safety of Combined Deep Inferior Epigastric Artery Perforator Flaps and Gynecologic Procedures.

BACKGROUND: Multidisciplinary breast cancer treatment is essential. For some patients, risk-reducing gynecologic procedures are a necessary addition. The deep inferior epigastric artery perforator (DIEP) flap for autologous breast reconstruction shares a common abdominal surgical site with open gynecological procedures, but the impact of concurrent surgery remains to be seen. METHODS: A retrospective chart review of all patients undergoing DIEP flap breast reconstruction with and without gynecologic procedures at our institution from 2017 to 2022 was performed. RESULTS: Patients undergoing c

Does (patient) size matter? the impact of body mass index on outcomes for patients undergoing minimally invasive transversus abdominis release.

BACKGROUND: Obesity increases the rate of postoperative wound complications for patients undergoing open abdominal wall reconstruction (AWR) and predisposes patients to hernia recurrence. While minimally invasive surgery (MIS) techniques minimize wound morbidity in obese patients, the exact degree to which this occurs remains unknown. METHODS: A prospectively maintained single-institution hernia database was queried for patients undergoing robotic-assisted (rTAR) from 2018 to 2024. Basic demographics, operative characteristics and postoperative outcomes were reviewed. A univariate analysis wa

Optimizing robotic approach to ventral hernia repair: an updated systematic review and meta-analysis between preperitoneal versus retromuscular repair.

BACKGROUND: The Rives-Stoppa retromuscular repair remains the reference standard for open ventral hernia repair. Advances in robotic surgery have expanded the minimally invasive options for extraperitoneal mesh placement, enabling both retromuscular and preperitoneal approaches. The robotic ventral transabdominal preperitoneal repair (Rv-TAPP), has seen increasing adoption, but comparative evidence with the robotic retromuscular techniques remains limited. METHODS: A systematic review and meta-analysis were reported in accordance with PRISMA and AMSTAR II guidelines (PROSPERO 2025: CRD4202511

Clinical Evaluation of Rectus Sheath Block Versus Local Anesthetic Infiltration in Laparoscopic Inguinal Hernia Repair: A Retrospective Comparative Study.

Background Laparoscopic inguinal hernia repair is frequently associated with early postoperative pain due to somatic nociceptive input from the anterior abdominal wall. Ultrasound-guided rectus sheath block (RSB) provides targeted analgesia by blocking terminal branches of the thoracoabdominal nerves within the rectus sheath. Although RSB is increasingly used in minimally invasive surgery, its comparative effectiveness against conventional local anesthetic (LA) wound infiltration in adult hernia repair remains inadequately characterized. Objectives The objective of this study is to evaluate th

Hemodynamic Collapse and Hidden Pathology: A Case of Perforated Duodenal Diverticulum Identified During Trauma Laparotomy.

Duodenal diverticula are rare extraluminal sac-like protrusions of the duodenal wall. Perforation of the diverticula is a life-threatening but uncommon complication, particularly following blunt abdominal trauma. We present the case of an 82-year-old woman who sustained blunt abdominal trauma after being struck by a motor vehicle, resulting in perforation of a duodenal diverticulum. She was hemodynamically unstable on arrival, necessitating emergent exploratory laparotomy during which a stapled diverticulectomy was performed. This case highlights the rarity of traumatic perforation of duodenal

Open versus laparoscopic techniques for incisional hernia repair: A systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: The choice between open and laparoscopic techniques for incisional hernia repair remains debated in surgical practice. This systematic review and meta-analysis was aimed at comparing the clinical outcomes of open versus laparoscopic incisional hernia repair in adults, with a focus on postoperative complications, recurrence, recovery, and patient satisfaction. METHODS: A comprehensive literature search was performed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Necrotizing Soft Tissue Infection of the Breast: A Unique Presentation of Underlying Invasive Breast Cancer.

Necrotizing soft tissue infections (NSTIs) are life-threatening infections that most commonly affect the extremities, perineum, and abdominal wall. These infections begin with the presence of toxin-producing bacteria that invade through a defect in the skin barrier, such as a wound, laceration, trauma, or recent surgical incision. These bacteria cause subsequent tissue destruction and necrosis that can involve the superficial skin, subcutaneous tissue, fascia, and/or muscle. NSTIs can progress quickly, leading to severe sepsis, shock, and even death. NSTIs associated with the breast are an exc

Importance of Investigating Occult Gallbladder Injury in Hepatic Trauma: A Case Report.

Gallbladder injury secondary to blunt abdominal trauma is a rare condition and may be missed during the initial evaluation. We present the case of a 65-year-old male patient with a history of chronic alcohol and tobacco use who was admitted two weeks after a physical assault, with facial trauma, abdominal pain, and hemodynamic stability. Initial imaging revealed a grade III hepatic injury and mild thickening of the gallbladder wall. The patient was initially managed conservatively. However, within 48 hours, he developed persistent right upper quadrant pain, low-grade fever, leukocytosis, and a

Endovascular treatment of a traumatic aortic transection via implantation of self-expanding nitinol stent with open-cell design: a case report.

BACKGROUND: Traumatic aortic transection is a life-threatening injury with an acute mortality rate approaching 80%, typically resulting from high-energy blunt trauma, such as motor vehicle collisions. Without intervention, the overall mortality rate increases to 90-94%, primarily due to the rupture of pseudoaneurysms or expanding hematoma leading to exsanguination. Although delayed surgical repair may decrease immediate mortality, it is associated with significant perioperative risks. Endovascular techniques are becoming increasingly prevalent and have demonstrated lower morbidity and mortalit

Early results from the use of an innovative vertical fascial traction system for the management of patients with open abdomen.

BACKGROUND: The combination of negative pressure wound therapy (NPWT) with dynamic fascial traction is currently considered the preferred method for temporary closure of the open abdomen (OA). However, this approach often requires repeated returns to the operating room for further fascial approximation. The aim of this study was to present our institution's experience with a vertical fascial traction device (VTD) for OA management and early closure. METHODS: This is a prospective registry of patients treated with the VTD between May 2023 and the present. The system used is commercially named

Delayed Ureteral Obstruction Due to Intramural Endo-Endoscopic Gastrointestinal Anastomosis Staples Following Laparoscopic Sigmoid Resection: A Case Report.

Ureteral injuries are among the most significant urological complications associated with colorectal surgery. Although most iatrogenic ureteral injuries are detected intraoperatively or shortly after surgery, a considerable number are identified only later. Very delayed manifestations that occur months or even years after the initial procedure are uncommon and pose diagnostic challenges. We report a case of a 50-year-old female patient who presented with intermittent abdominal discomfort and was found to have a left-sided mid-ureteral stone with grade I hydronephrosis. Her surgical history inc

Rectus Femoris Flap as a Choice for Complicated Abdominal Wound Defect Following Appendectomy.

Burst abdomen is a difficult case to address, with various risk factors responsible for wound dehiscence, one of which is malnutrition and infection. Several flap options can be used specifically to treat extensive abdominal defects, but there are still few articles discussing the use of the rectus femoris flap, especially in cases of extensive abdominal defects accompanied by infection and malnutrition. This report highlights the use of a rectus femoris flap to resolve the complexity of this issue. We present a case of a 45-year-old man with poor nutritional status who developed an abscess on
