Approaches to Ventral Hernia Repair: A Narrative Review.

Ventral hernia repair is a significant and evolving challenge in general surgery, encompassing a wide spectrum of primary and incisional abdominal wall defects of varying complexity. The burden of ventral hernia continues to rise globally due to increasing rates of abdominal surgeries, aging populations, and the growing prevalence of risk factors such as obesity and metabolic disorders. Despite advances in surgical techniques, recurrence and postoperative complications remain important concerns, necessitating continuous refinement of management strategies. This narrative review synthesizes con

Incarcerated Spigelian Hernia: A Case Report.

Spigelian hernia (SH) is a rare abdominal wall defect that frequently presents a diagnostic challenge because of its non-specific symptoms and deep anatomical location. We report the case of a 64-year-old woman with adequately controlled hypertension who presented with localized abdominal wall pain and a long-standing lateral abdominal bulge. Physical examination revealed an incarcerated SH without signs of strangulation or bowel obstruction. Computed tomography confirmed a Spigelian fascial defect containing incarcerated ileal loops without evidence of bowel compromise, allowing appropriate p

Suture Technique for Fascial Closure in Midline Laparotomy in Patients at High Risk of Post-surgical Evisceration.

The objective of this study is to describe the modified Maingot keel closure technique as a reinforced fascial closure strategy in patients at high risk of postoperative evisceration and to report the short-term clinical outcomes associated with its application. We present a case series of four patients undergoing midline laparotomy who were considered at high risk for postoperative fascial dehiscence according to a Van Ramshorst score greater than 6. The modified Maingot keel closure technique was performed as primary or secondary fascial reinforcement according to the clinical scenario. Four

Surgical management of large ventral hernias following damage control surgery: a retrospective comparison of laparoscopic IPOM plus and open repair techniques.

Damage control surgery (DCS) has become the standard of care in the management of severe abdominal trauma, particularly in military settings. Although this approach improves survival, it frequently results in significant disruption of the anterior abdominal wall and subsequent development of large ventral hernias. Reconstruction in this context is technically challenging due to adhesions, distorted anatomy, and loss of normal tissue planes. While open retromuscular repair is generally preferred, it is often not feasible following DCS. Laparoscopic intraperitoneal on-lay mesh repair with primar

Small Bowel Herniation Into a Post-bariatric Pannus After Cesarean Delivery Managed With Single-Stage Panniculectomy and Rives-Stoppa Repair: A Case Report.

Redundant pannus after massive weight loss can impair function and obscure abdominal wall pathology. Incisional hernia after cesarean delivery is uncommon, and extension of the small bowel into a post-bariatric pannus is particularly unusual. We report a 42-year-old woman with three prior cesarean deliveries and prior bariatric surgery who presented with a functionally limiting grade 3 pannus and a long-standing, nonreducible lower abdominal bulge. Computed tomography (CT) demonstrated a 12.3 cm × 5.6 cm low ventral incisional hernia with a 6.7 cm neck containing small bowel loops extending in

Minimally Invasive Approaches to Complex Abdominal Wall Reconstruction.

Robotic platforms have made it possible to replicate open retromuscular component separation principles through small incisions in a way that was not possible on the laparoscopic platform. However, it is crucial to keep in mind that the optimal repair (not the platform) is key to achieving the best patient outcomes. Robotic abdominal wall reconstruction offers short-term advantages over open repair, including shorter hospital stay and fewer wound complications, albeit with longer operative times, higher procedural costs, and a steep learning curve. Current evidence suggests comparable hernia r

Botulinum Toxin Type A-assisted Mesh-Free Endoscopic Repair of a Pediatric Ventral Hernia After Giant Omphalocele: A Case Report.

Residual ventral hernia after giant omphalocele presents a complex reconstructive challenge in pediatric surgery because of altered abdominal wall anatomy, muscular retraction, and the need to achieve closure without excessive tension. Botulinum toxin type A has been used as an adjunct to abdominal wall reconstruction by inducing temporary muscular relaxation, but pediatric experience remains limited, particularly in combination with endoscopic preaponeurotic repair (REPA). We report the case of a five-year-old boy weighing 13.4 kg with a residual ventral hernia after a giant omphalocele. Preo

Open Ventral Hernia Repair: A Prospective Comparative Analysis of Onlay Versus Sublay Mesh Placement.

BACKGROUND: Ventral hernia repair is among the most frequently performed general surgical procedures worldwide. Mesh-based repair has become the standard of care, but the optimal plane for mesh placement remains debated. This study compares outcomes of onlay and sublay techniques in open ventral hernia repair. METHODS: A prospective observational study enrolled 62 patients (31 per group) undergoing elective open ventral hernia repair at a tertiary care institute in India over 18 months. Postoperative wound complications (seroma, surgical site infection (SSI)), operative duration, hospital sta

Efficacy and safety of ovine reinforced tissue matrix for ventral hernia repair: a systematic review and meta-analysis.

INTRODUCTION: OviTex® mesh is a reinforced tissue matrix (RTM) that combines biologic and synthetic components, aiming to reduce inflammatory response while providing durable abdominal wall support. Comparative analyses indicate it performs similarly to synthetic mesh regarding hernia recurrence and surgical site infection rates, and may be preferable in patients with increased risk factors or prior mesh infection. This systematic review and meta-analysis evaluated the efficacy and safety profile of OviTex, particularly in patients undergoing ventral hernia repair (VHR). METHODS: Following PR

Hybrid robotic transversus abdominis release (hrTAR) for complex ventral hernias: a systematic review and preliminary synthesis.

Hybrid robotic transversus abdominis release (hrTAR) combines robotic posterior component separation with a planned limited open phase. Evidence supporting this strategy is sparse. Following PRISMA 2020, we searched PubMed, Embase, Scopus, and the Cochrane Library and registered the protocol in PROSPERO (CRD420261303285). Eligible studies reported hrTAR outcomes for ventral or incisional hernia repair. Findings were synthesized narratively; no pooled effect estimates were calculated. Three reports contained 85 hrTAR cohort entries. Because two reports came from the same institution and overlap

Multicentric pragmatic randomized controlled trial of mesh fixation system in primary and recurrent ventral hernia repair.

Controversy persists regarding the ideal type of fixation for hernia mesh. Transfixing methods have been linked to tissue trauma and to the development of chronic pain. In ventral hernia repair (VHR), retrospective studies showed a potential for fixation with cyanoacrylate of being as effective as conventional suture or tacks methods but with less postoperative pain and complications. A pragmatic randomized trial was conducted on patients undergoing primary and incisional VHR, analyzing mesh fixation by cyanoacrylate (experimental group) versus sutures or reabsorbable tacks (control group), st

Concomitant Panniculectomy During Ventral Hernia Repair: Outcomes From a High-Volume Abdominal Wall Reconstruction Unit.

BACKGROUND: Concomitant panniculectomy during ventral hernia repair (VHR) remains controversial due to concerns about wound morbidity. Potential benefits include improved operative exposure and reduced abdominal wall strain. This study evaluated outcomes of VHR with and without panniculectomy in a specialised abdominal wall reconstruction unit. METHODS: A retrospective analysis of a prospectively maintained database of consecutive adult patients undergoing VHR with or without panniculectomy was performed. The primary outcome was hernia recurrence. Secondary outcomes included surgical site occ

ReTAPP for recurrent inguinal hernia after laparoscopic repair: extended experience and comparative analysis.

PURPOSE: Current international guidelines recommend an open anterior approach for inguinal hernia recurrence after a previous laparo-endoscopic repair. However, repeated laparoscopic repair (reTAPP) has emerged as a potential alternative in highly specialized centers. We aimed to compare the safety, feasibility, and outcomes of reTAPP versus open anterior repair in patients with recurrence after previous laparoscopic inguinal hernia repair. METHODS: Retrospective analysis of all transabdominal preperitoneal (TAPP) repairs performed between January 2010 and December 2023. Patients with recurre

Minimally Invasive Resection of Intralobar Pulmonary Sequestration Following Same-Day Interventional Radiological Embolisation of the Feeding Vessel: A Case Series From a Single Tertiary Centre.

Pulmonary sequestration (PS) is a congenital abnormality of the lung where a region of pulmonary parenchymal tissue is supplied by an aberrant systemic artery, which may arise from the thoracic aorta, abdominal aorta, celiac trunk, or intercostal arteries. They are further sub-classified as intralobar or extralobar, depending on whether the sequestered lung has an independent visceral pleural covering. Hybrid minimally invasive treatment of intralobar PS with preoperative embolisation of feeding vessels followed by video-assisted thoracoscopic surgery (VATS) resection has become more widesprea
