Transport of the Open Abdomen: A Review.

OBJECTIVE: An open abdomen (OA) is a critical surgical condition in which the abdominal wall is intentionally left open to prevent or manage intra-abdominal hypertension or abdominal compartment syndrome or facilitate staged re-exploration. Although OA management is well described in surgical literature, the safe transport of patients with an OA-particularly in air medical and critical care transport settings-presents unique challenges that are less frequently addressed. METHODS: This review summarizes current evidence and best practices for the transport of patients with an OA, focusing on p

Hyperbaric oxygen therapy attenuates urethral stricture formation after urethral injury: an experimental rabbit study.

PURPOSE: Urethral stricture (US) is a frequent and challenging urological entity characterized by spongiofibrosis and luminal narrowing, often leading to recurrent infections, urinary retention, and upper tract deterioration. Despite various endoscopic and open surgical options, high recurrence rates remain a major problem, and there is no established medical therapy to prevent stricture formation. To investigate the protective effect of hyperbaric oxygen (HBO) therapy on fibrosis and stricture formation in an experimental urethral injury model in rabbits using objective endoscopic, radiologic

Avulsion Fracture of the Lesser Trochanter and the Use of Conservative Treatment.

Avulsion fractures of the lesser trochanter of the femur are extremely rare injuries in the pediatric population, most commonly resulting from physical activity. The typical mechanism involves a sudden contraction of the iliopsoas muscle, which inserts on the lesser trochanter. We present a rare case of such an injury in a pediatric patient, emphasizing the importance of appropriate imaging and the effectiveness of conservative management in achieving full recovery. An 11-year-old boy presented to the emergency department with perineal trauma sustained after losing balance while riding a bicyc

Emergency Management of Crushing Perineal Avulsion with Complete Urethral Disruption: A Case Report.

High-energy machinery entrapment can result in catastrophic perineal injuries involving complete urethral disruption, extensive soft tissue avulsion, and associated orthopedic trauma. We report a 24-year-old man who suffered a crushing roller injury, resulting in total perineal avulsion with bilateral testicular exposure, complete bulbar urethral transection, right pubo and ischial ramus avulsion, and an open ipsilateral tibiofibular fracture. Given the patient's hemodynamic stability following resuscitation and the intraoperative finding of cleanly transected urethral ends without rectal or b

Management of retained bullet fragments from posterior urethra after gunshot wound: a case report.

BACKGROUND: Retained bullet fragments in the genitourinary tract following gunshot wound is rare and require removal. CASE DESCRIPTION: We present the case of a retained bullet in the prostate following a gunshot wound to the pelvis, which was endoscopically removed. A urethral catheter and suprapubic tube were placed. The patient then had migration of a second bullet fragment into the prostatic urethra six weeks later, requiring open removal. CONCLUSIONS: This case shows that several approaches can be considered to remove retained bullet fragments from the urinary tract. Furthermore, access

Multiple ventral hernias in a 30-year-old female with Down syndrome: a case report and review of the literature.

BACKGROUND: Ventral hernias are common abdominal wall defects, usually occurring as isolated entities related to prior surgery, trauma, or increased intra-abdominal pressure. The occurrence of multiple synchronous ventral hernias in a single patient is rare, and reports in individuals with Down syndrome are particularly scarce. Down syndrome is associated with connective tissue abnormalities, hypotonia, and ligamentous laxity, which may predispose patients to hernia formation. However, multiple simultaneous ventral hernias in this population have not been widely described. This case adds to th

A Translational Surgical Porcine Model for Postoperative Intra-Abdominal Adhesion Formation.

A porcine model of postoperative intra-abdominal adhesion formation was established using Yucatan mini pigs. The protocol combines midline laparotomy, small bowel resection with two-layer primary anastomosis, and a unilateral, parietal peritoneal abrasion in the format of an open abdominal surgical procedure. Adhesion formation was assessed four weeks postoperatively using established gross and histologic scoring criteria, with evaluations performed by blinded observers. Adhesions developed in all animals using this model and were multifocal, involving bowel loops, between the bowel and abdomi

Incidence and outcome of acute kidney injury in open abdomen treatment following open surgery for ruptured abdominal aortic aneurysm.

BACKGROUND AND AIMS: Acute kidney injury (AKI) is a key contributor to multiorgan dysfunction and mortality in ruptured abdominal aortic aneurysm (rAAA). Intra-abdominal hypertension and abdominal compartment syndrome (ACS), complications potentially resulting in AKI, can be prevented or managed with open abdomen treatment (OAT). Using consensus-based definitions, this study characterizes AKI in rAAA patients managed with OAT and examines the impact of abdominal wall closure on renal function. METHODS: A single-center observational cohort study including all consecutive patients treated with

Perineal Verrucous Carcinoma Associated With Hypercalcemia: First Clinical Report of Parathyroid Hormone-Related Protein-producing Tumor.

Cancer-related hypercalcemia typically indicates poor prognosis and occurs most commonly in squamous cell carcinoma cases. However, its occurrence in patients with verrucous carcinoma, a clinically indolent subtype, has never been reported. We present the first documented case of verrucous carcinoma-related hypercalcemia caused by parathyroid hormone-related protein (PTHrP) production. A 49-year-old man presented with weight loss, fatigue, and severe hypercalcemia. He had a 3-year history of a scrotal mass that was diagnosed as perineal verrucous carcinoma based on histopathologic examination.

"Simultaneous bulge repair and recontouring after abdominally based breast reconstruction: A well-fixed open transversus abdominis release approach".

An abdominal bulge is an often-underreported complication of abdominally based breast reconstruction (ABBR) with no gold standard treatment. A bulge confers substantial morbidity to patients through compromised core function and poor esthetic contour. Here, we present a new technique for bulge repair that addresses both aspects through an open transversus abdominis release (TAR) with a well-fixed, polypropylene mesh. An electronic data warehouse query was initiated using keywords "bulge" and "TAR" for the senior author's (GAD) patients from 2020-2025 for bulge repair after ABBR. A total of 11

Abdominal Aortic Aneurysm Pathomechanics, Current Understanding and Emerging Directions: A Perspective for 2025 and Beyond.

Abdominal aortic aneurysms (AAAs) are permanent, local expansions of the abdominal segment of the aorta that can potentially be fatal if progressing to rupture. AAAs are rarely found in patients under age 60, but primarily are more common in older age groups, occurring in 2-3% of the whole population. Their rupture produces 10-14,000 deaths annually in the United States alone. Because aneurysmal rupture is a mechanical phenomenon, in recent years there has been a major effort among researchers to investigate the biologic and mechanical processes surrounding AAA progression and rupture. In addi

Anastomotic Dehiscence Secondary to Early Eventration in Abdominal Trauma Caused by a Bull Horn: The Critical Importance of Abdominal Wall Integrity.

Abdominal trauma caused by a bull horn is a rare but severe injury that can lead to multiple intestinal perforations and life-threatening complications. This case report highlights the critical management challenges and emphasizes the importance of multidisciplinary care in a high-risk surgical patient. A 36-year-old male presented with open abdominal trauma and small bowel evisceration following a bull horn injury. An initial exploratory laparotomy revealed three perforations in the ileum, requiring a 30 cm resection and end-to-end anastomosis. Postoperatively, he was managed in the intensive

Parastomal hernia repair using retromuscular 3D funnel mesh: the "Sugar Funnel" technique.

INTRODUCTION: Parastomal hernias are a common and complex problem for patients and their surgeons. Despite multiple established repair strategies, recurrence rates remain high. We present a novel open retromuscular repair technique using a 3D funnel mesh. METHODS: The technique involves retrorectus dissection, transversus abdominis release (TAR), lateralisation of the stomal conduit, and retromuscular placement of a 3D funnel mesh. A second flat mesh reinforces the remaining retromuscular space. Abdominoplasty was performed where excess skin or fat contributed to the parastomal ‘bulge’. RESULT

An Unusual Case of Appendicitis Entrapped Within a Spigelian Hernia.

Acute appendicitis within a Spigelian hernia is a rare clinical entity that may present with atypical symptoms, resulting in diagnostic delay and complex operative decision-making. Previously reported cases have most commonly presented with localized right lower quadrant pain, abdominal wall masses, incarceration, bowel obstruction, or strangulation. Spigelian hernias occur along the semilunar line and may contain intra-abdominal structures, including the appendix, resulting in non-classic symptoms of appendicitis. We report a case of a 52-year-old woman with a history of cerebrovascular accid

Robot-Assisted Surgery Performed Using a Fluorescent Ureteral Catheter for Locally Advanced Sigmoid Colon Cancer: A Case Report.

In recent years, robotic surgery for colorectal cancer has increased and is now being performed even for locally advanced colorectal cancers. However, due to adhesions and tissue fibrosis caused by infiltration and inflammation, the risk of misidentification during dissection is high. In robotic surgery, which lacks tactile feedback, particular caution is required to avoid injuries such as ureteral damage. Navigation surgery using fluorescent ureteral catheters that emit fluorescence upon near-infrared light irradiation has attracted attention. In this report, we describe a case where a safe r

Open abdomen techniques - a herniologist's view.

Open abdomen, laparostomy, or temporary abdominal closure, represent a surgical strategy in which the abdominal cavity is intentionally left unclosed after laparotomy. This approach is indicated primarily in critically ill patients in whom primary abdominal wall closure would result in deterioration of intra-abdominal conditions or prevent adequate control of the underlying pathology. This group mainly includes patients with generalized peritonitis, severe acute pancreatitis, major abdominal trauma, or significant visceral edema. A second group comprises patients undergoing decompressive lapar

The Difficult Cholecystectomy.

IMPORTANCE: Difficult cholecystectomies are associated with a higher risk of severe bilio-vascular injuries. OBSERVATIONS: Obesity, cirrhosis, high American Society of Anesthesiologists score, previous abdominal operations, and presence of acute cholecystitis or common bile duct stones are associated with difficult cholecystectomies. On imaging, thickened gallbladder wall, pericholecystic fluid, and an impacted gallstone are associated with difficult cholecystectomies. The cesarean apron: description, proposed pathophysiology, classification, and prevention through scarpa fascia closure at cesarean delivery.

The "cesarean apron," also known as a cesarean bulge, pouch, or shelf, is a frequent but often overlooked postoperative deformity of the lower abdominal wall that may occur even after uncomplicated cesarean delivery. It is characterized by redundant skin and subcutaneous adipose tissue that overhangs the cesarean scar, producing a contour irregularity that ranges from a mild suprapubic bulge to a large apron extending below the mons pubis. This deformity affects both obese and nonobese patients and is frequently dismissed as a cosmetic concern or attributed to weight gain. However, it can caus

Delayed Recognition of Multiple Small Bowel Perforations and Mesenteric Lacerations Following Combined Blunt and Penetrating Lumbar Trauma: A Case Report.

Small bowel and mesenteric injuries (SBMI) following abdominal trauma are challenging to diagnose, particularly when initial imaging is non-definitive and multiple distracting injuries are present. Bowel injury may remain occult during the initial assessment, and delayed recognition can result in significant morbidity and mortality. A 39-year-old previously healthy male construction worker sustained a penetrating injury to the right lumbar region after a heavy object fell from approximately 2-2.5 meters, with retained steel fragments and associated blunt abdominal trauma. He was hemodynamicall
