Unusual presentation of the urogenital myiasis caused by Luciliasericata (Diptera: Calliphoridae).

INTRODUCTION AND OBJECTIVE: The case report describes the unusual presentation of the urogenital myiasis caused by Luciliasericata in two Slovakian men. MATERIAL AND METHODS: The first patient, aged 66, who suffered from a locally advanced and inoperable urinary bladder dedifferentiated TCC with bilateral ureteral obstruction, chronic renal insufficiency and non-functioning left kidney. After surgical exploration the patient developed a malignant vesico-intestino-cutaneous fistula with stool leakage through the open wound. Because of very poor hygiene, and unsatisfactory attendance by staff,

Surgical treatment of post-traumatic distraction posterior urethral strictures.

Approximately 4-14% pelvic fractures cause a posterior urethral injury. Pelvic fractures associated with straddle injuries or large trauma accidents are more frequently involved with this kind of lesions. Primary open repair of the urethral injury is discouraged in the acute setting. 3-6 months after urinary diversion a formal open reconstruction can be safely attempted. This gives time for scar maturation, reabsorption of pelvic hematomas, and relative restoration of anatomical fascial layers. The complexity of such interventions can be minimized following proper diagnostic and surgical proto

Effect of a 5-HT1A receptor agonist (8-OH-DPAT) on external urethral sphincter activity in a rat model of pudendal nerve injury.

Although serotonergic agents have been used to treat patients with stress urinary incontinence, the characteristics of the external urethral sphincter (EUS) activity activated by 5-HT receptors have not been extensively studied. This study examined the effects of the 5-HT(1A) receptor agonist, 8-hydroxy-2-(di-n-propylamino)tetralin (8-OH-DPAT), on the EUS-electromyography and resistance of the urethra in a rat model with bilateral pudendal nerve injury (BPNI). Two measurements were utilized to assess the effects of the drug on bladder and urethral functions: the simultaneous recordings of tran

Fibroblast expression of α-smooth muscle actin, α2β1 integrin and αvβ3 integrin: influence of surface rigidity.

Open wound contraction necessitates cell and connective tissue interactions, that produce tension. Investigating fibroblast responses to tension utilizes collagen coated polyacrylamide gels with differences in stiffness. Human foreskin fibroblasts were plated on native type I collagen-coated polyacrylamide gel cover slips with different rigidities, which were controlled by bis-acrylamide concentrations. Changes in alpha smooth muscle actin (αSMA), α2β1 integrin (CD49B) and αvβ3 integrin (CD-51) were documented by immuno-histology and Western blot analysis. Cells plated on rigid gels were longe

Urethral foreign body insertion for secondary gain in the incarcerated population.

Not uncommonly, hostile prison environments can lead desperate prisoners to place foreign materials into natural orifices in an effort to gain transfer to an outside healthcare facility. In this article, we present a case series of urethral foreign body insertion of varying burdens and durations requiring transfer to our facility for surgical management. Endoscopic retrieval was the initial management in each case; one case required conversion to open cystotomy for complete removal due the orientation, amount of inserted foreign body, and erosion into the proximal urethral and bladder urotheli

Effect of wound closure on buccal mucosal graft harvest site morbidity: results of a randomized prospective trial.

OBJECTIVE: To determine the effect of closure versus nonclosure of the buccal mucosal graft harvest site in men undergoing bulbar urethroplasty in a randomized prospective study. The optimal postoperative management of the buccal mucosal graft donor site remains unknown. METHODS: A total of 50 consecutive patients were randomized to either donor site closure or leaving the donor site open. Postoperatively, questionnaires assessing pain, diet, salivation, perioral sensation, and mouth opening were completed daily for the first week and then monthly for 6 months. The primary endpoint was postop

Transurethral incision of the bladder neck to treat bladder neck dysfunction and voiding dysfunction in patients with high-level spinal cord injuries.

PURPOSE: Detrusor-sphincter dyssynergia (DSD) and bladder neck dysfunction (BND) may cause voiding dysfunction in patients with spinal cord injury (SCI). We report the surgical outcomes in patients with high-level SCI who underwent transurethral incision of the bladder neck (TUI-BN). PATIENTS AND METHODS: Twenty-two patients with SCI at or above the mid-thoracic level who presented with voiding dysfunction due to BND with different types of DSD received TUI-BN. Surgical outcome was determined by comparing preoperative with postoperative urodynamic parameters and clinical presentations. Satisf

Traumatic complete urethral disruption: the West Virginia experience.

Type III complete urethral disruption is an uncommon injury that occurs primarily in male patients with pelvic trauma. Herein we present our results from management of this condition. Management should initially be conservative with a catheter placement in all cases. Full return of urinary function was noted in all patients managed endoscopically, and in 1 of 3 of patients managed with open urethroplasty. Erectile function was preserved in 2 of 3 of patients managed with endoscopic repair, and in none of the patients managed with open urethroplasty. The most common surgical urologic complicati

Successful removal of a 3.6-cm long metal band used as a penile constriction ring.

INTRODUCTION: Penile constriction rings are either used for autoerotic stimulus or to increase sexual performance. Potentially, they can become irremovable and cause urologic emergencies. AIM: We describe the successful removal of a 3.6-cm long piece of heavy metal tubing used as a penile constriction ring. METHODS: An angel grinder was used to open the metal tubing on both lateral sides. During the cutting procedure, the soft tissue parts were protected by two metal spatulas. Wet towels and cool running water prevented thermal injury. RESULTS: After removal of the band, no iatrogenic injur

[Urethral reconstruction after failed primary surgery].

There are numerous procedures available for surgical management of a urethral stricture. Establishing the correct indication for performing a specific surgical method requires a great deal of experience. Nonetheless, postoperative complications are never avoidable. Secondary intervention after an unsuccessful primary operation poses particular challenges to the surgeon. This article provides an overview of the open surgical techniques currently employed. It also highlights factors that should be considered to minimize postoperative complications. The concluding discussion describes the individ

[Traumatic urethral disruption treated by endoscopic management with transluminal puncture under fluoroscopic guidance: a case report and review of the literature].

A 76-year-old man injured in a car accident was admitted with urinary retention and urethral bleeding. Cystoscopic findings revealed disruption of the bulbous urethra and we performed cystostomy. At sixty days after the injury, we performed endoscopic urethroplasty with a steel needle puncturing from the proximal end of the disruption to the distal end under the guidance of C-Arm fluoroscopy. We removed the urethral catheter at the 37th day after surgery and then performed urethral dilation for postoperative urethral stricture. Retrograde urethrocystography showed no urethral stricture at 5 mo
