PURPOSE: Kocher-Langenbeck (K-L) approach is widely used in surgery of posterior wall fracture of acetabulum. However, challenges are frequently encountered in fractures involving the superior dome due to its short of view. We aimed to evaluate the efficacy of digastric trochanteric flip osteotomy (DTFO) in the K-L approach for high posterior wall acetabular fracture (HPWF).
METHODS: From January 2014 to December 2016, 39 patients who suffered high posterior wall fracture (HPWF) were included in this retrospective study. All the patients were divided into two groups according to surgery type

Severe lumbosacral pain, paraparesis or paraplegia, and urinary incontinence are common but frustrating problems in dogs with lumbosacral spinal cord injury (SCI). The surgical interventions including stabilization and decompression may not restore satisfying neurological functions in severe SCI. Adipose tissue-derived mesenchymal stem cells (Ad-MSCs) show benefits in immunomodulation, anti-inflammation, and promotion of axonal growth and remyelination, and also display efficacy in several diseases in veterinary medicine. In this report, four dogs presented with fracture of sacrum vertebrae or

PURPOSE: Pelvic bone segmentation in CT has always been an essential step in clinical diagnosis and surgery planning of pelvic bone diseases. Existing methods for pelvic bone segmentation are either hand-crafted or semi-automatic and achieve limited accuracy when dealing with image appearance variations due to the multi-site domain shift, the presence of contrasted vessels, coprolith and chyme, bone fractures, low dose, metal artifacts, etc. Due to the lack of a large-scale pelvic CT dataset with annotations, deep learning methods are not fully explored.
METHODS: In this paper, we aim to brid

OBJECTIVE: To develop a post-traumatic osteoarthritic model of hip following fracture of acetabulum in rabbit for revealing biochemical mechanism of post-traumatic osteoarthritis.
METHODS: A total of 36 mature male New Zealand white rabbits were equally divided into sham group (n = 12), non-ORIF group (n = 12), and open reduction and internal fixation (ORIF) group (n = 12). Except for the sham group, rabbits had survival surgeries to create acetabular fractures of dorsal wall for simulating dashboard impaction mechanism. The ORIF group received open reduction and internal fixation, while frac

Traumatic bilateral hip dislocation is a rare phenomenon. Simultaneous traumatic asymmetrical bilateral hip dislocation is even more unusual. There is no report in the literature regarding the mechanism of injury in tramcar squeezing. A 49-year-old male who had a serious tramcar accident and developed asymmetric bilateral hip dislocations (left posterior, right anterior) combined with fracture of left acetabulum and multiple injuries was admitted. Right hip dislocation was treated with manual reduction. Left hip dislocation, fracture of acetabulum and bilateral pubis, and open wound of left th

BACKGROUND: The first magnetically controlled growing rod (MCGR) was implanted in 2009. Since then multiple complications have been identified that have helped drive the development of the MCGR and its surgery. The aim of this report is to illustrate how identified complications in the first MCGR helped with developments in the past decade and to report a unique failure mechanism with stud fracture close to the barrel opening.
CASE PRESENTATION: A 5-year old girl with a scoliosis of 58.5 degrees at T1-9 and 72.8 degrees at T9-L4 had a single MCGR inserted and anchored at T3-4 and L3-4. At pos

PURPOSE: This finite element analysis assessed lateral compression (LC-1) fracture stability using machine learning for morphological mapping and classification of pelvic ring stability.
METHODS: Computed tomography (CT) files of LC-1 pelvic fractures were collected. After morphological mapping and producing matrix data, we used K-means clustering in unsupervised machine learning to classify the fractures. Based on these subtypes, we manually added fracture lines in ANSYS software. Finally, we performed a finite element analysis of a normal pelvis and eight fracture subtypes based on von Mise

OBJECTIVE: Traumatic spinal injury (TSI) is a serious trauma-related injury with a significant risk for mortality and morbidity. Road traffic accident (RTA) is the leading cause for the spinal and orthopedic injuries globally. Detailed information on the association of orthopedic fractures with TSI and its influence on outcomes is lacking. In this study, RTA-related TSI with orthopedic associated fractures in terms of demographics, prevalence, patterns, and outcomes were studied.
PATIENTS AND METHODS: This was a single-center retrospective study conducted over ten years in the largest tertiar

INTRODUCTION: The authors describe their first experience with virtually navigated pelvic and spine screws based on perioperative CT navigation.
MATERIAL AND METHODS: From 22 October 2012 (launching the device) to 9 January 2013, a total of 15 CT-navigated pelvic and spine operations were performed in 14 patients. Nerve root compression, scoliosis, vertebral fracture and spondylodiscitis were the indications for spine procedures; B-type and C-type fractures according to the AO classification were the indications in pelvic surgical procedures. The preparation and the course of the procedures w

Lumbar artery pseudoaneurysm is a rare adverse event that is presented in reports. It is associated with serious injuries such as in the retroperitoneal cavity and pelvis. Furthermore, infection and iatrogenic etiology were documented as well. This case report presents a patient with a chest stab wound and a fall from a 2-m height, which resulted in multiple spinal fractures that were treated surgically. However, he presented back with lumbar artery pseudoaneurysm 2 weeks postoperatively, which was treated successfully by endovascular embolization.

Frontal impacts with reclined occupants are rare but severe, and they are anticipated to become more common with the introduction of vehicles with automated driving capabilities. Computational and physical human surrogates are needed to design and evaluate injury countermeasures for reclined occupants, but the validity of such surrogates in a reclined posture is unknown. Experiments with post-mortem human subjects (PMHS) in a recline posture are needed both to define biofidelity targets for other surrogates and to describe the biomechanical response of reclined occupants in restrained frontal

INTRODUCTION: Concomitant ipsilateral floating hip with floating knee is a very rare injury pattern. Its co-occurrence with ipsilateral complete separation of ileum from pelvic girdle, which can be termed as "floating ileum," is even rarer. These are extremely high energy injuries associated with significant systemic insult adding to the dilemma and complexity in management. There is no mention in the literature about this potentially life-threatening injury combination; hence, an attempt has been made to provide a pathway of the management of this rare but complex injury pattern.

Acetabular fracture usually occurs as a result of high velocity injury and often affects the young and economically productive population. Previously, treatment of acetabular fracture was grossly inadequate and many patients were left with incapacitating pain, limitation of movement. Proper management should be given in our set-up to save lives and to minimize long term complications and related disabilities. This study was done to evaluate the outcome of open reduction and internal fixation of posterior wall fracture of acetabulum. This prospective observational study was carried out in Dhaka

BACKGROUND: A traumatically shattered lumbosacral junction/pelvis may be difficult to repair. Here the authors offer a pelvic fixation technique utilizing routine pedicle screws, interbody lumbar fusions, bilateral iliac screws/ rods/crosslinks, and bilateral fibular strut allografts from the lumbar spine to the sacrum.
METHODS: A middle aged male sustained a multiple storey fall resulting in a left sacral fracture, and right sacroiliac joint (SI) dislocation. The patient had previously undergone attempted decompressions with routine pedicle screw L4-S1 fusions at outside institutions; these

INTRODUCTION: Multiple fractures are of high clinical relevance, as a significant increase in mortality rate has been described. The purpose of this study was to evaluate differences in age and gender distribution in multiple fractures dependent on severity of trauma. Furthermore, affected anatomic regions and frequently associated fracture regions were investigated.
METHODS: Patients who had sustained multiple fractures between 2000 and 2012 were included in this study. At hospital admission, patients were divided according to trauma severity (high- vs low-traumatic), gender, and age for dem

RATIONALE: Damage control is a staged surgical approach to manage polytraumatized patients. The damage control approach comprises three steps. First, bleeding is controlled and fractures are stabilized temporarily; second, vital parameters are stabilized and the child is rewarmed in the intensive care unit; and third, the child is reoperated for definitive repair of injuries. We aimed to describe the feasibility of the damage control orthopedic approach in a child.
PATIENT CONCERNS: An 8-year-old girl fell from the balcony of the 5th floor onto concrete pavement and was admitted to our accide
