Association between non-traumatic vertebral fractures and adjacent discs degeneration: a cross-sectional study and literature review.

BACKGROUND: Previous clinical studies reported that thoracolumbar vertebral fractures (VFs) associated with high energy spine trauma cause adjacent intervertebral disc (IVD) degeneration; however, the effect of non-traumatic VFs on the progression of adjacent disc degeneration remains to be determined. The purpose of this study was to examine the association between non-traumatic VFs and degenerative changes of adjacent IVDs. METHODS: Ninety-eight consecutive patients undergoing spinal surgery were included in this study. VFs were semi-quantitatively evaluated by lateral lumbar radiography. F

Traumatic intradural ruptured lumbar disc with a spinal compression fracture: A case report.

RATIONALE: We present a rare case of a traumatic intradural ruptured disc associated with a mild vertebral body compression fracture along with a review of the relevant medical literature. An intradural ruptured disc often occurs due to chronic degenerative diseases and is rarely due to trauma. It can cause irreversible neurological complications if the appropriate treatment is not planned. PATIENT CONCERNS: A 32-year-old male presented with motor paraparesis (grade 3/5), right ankle dorsiflexion, and great toe dorsiflexion (grade 1/5), along with radiating pain at his right L4 and L5 sensory

Traumatic Lumbosacral Spondyloptosis in a Pediatric Patient: Case Report and Literature Review.

A 4-year-old girl was admitted to the emergency department after having been buried beneath a wall. A computed tomography scan revealed anterior grade V L5-S1 spondylolisthesis, and magnetic resonance imaging showed a traumatic rupture of the fibrous annulus of the L5-S1 intervertebral disc and lesion of the anterior longitudinal and yellow ligaments. The patient underwent anterior and posterior fixation. Four months later she was able to walk independently, despite a persistent left foot drop. Additionally, we conducted a literature review on lumbosacral spondyloptosis in the pediatric popula

Expression levels of IL-17 and TNF-α in degenerated lumbar intervertebral discs and their correlation.

The present study aimed to investigate the expression and roles of interleukin (IL)-17 and tumor necrosis factor (TNF)-α in intervertebral disc degeneration (IDD) and to identify the association between the effects of IL-17 and TNF-α in IDD. This may increase understanding of the pathogenic mechanism underlying IDD, and aid the development of alternative therapies. The experimental group consisted of 40 samples of nucleus pulposus tissue obtained from the intervertebral discs (IVDs) of patients with IDD by surgical intervention, and was further divided into an annulus fibrosus disrupted group,

Relationship between autologous bone graft osteointegration and correction loss after antero-posterior spondylodesis of traumatic vertebral body fracture.

BACKGROUND: A common method to restore the sagittal alignment and stabilize the spinal column is a dorso-ventral spondylodesis. It is assumed that correction loss after posttraumatic spondylodesis results from inadequate incorporation of the autologous iliac crest graft. MATERIALS AND METHODS: Retrospective documentation of patients with unstable vertebral body fractures of the thoracic or lumbar spine with concomitant rupture of at least one adjacent intervertebral disk who received surgical treatment at our institution from 2000 to 2006. Followed by analysis of the computer tomography docum

Challenges and strategies in the repair of ruptured annulus fibrosus.

Lumbar discectomy is the surgical procedure most frequently performed for patients suffering from low back pain and sciatica. Disc herniation as a consequence of degenerative or traumatic processes is commonly encountered as the underlying cause for the painful condition. While discectomy provides favourable outcome in a majority of cases, there are conditions where unmet requirements exist in terms of treatment, such as large disc protrusions with minimal disc degeneration; in these cases, the high rate of recurrent disc herniation after discectomy is a prevalent problem. An effective biologi

Little-known Swiss contributions to the description, diagnosis, and surgery of lumbar disc disease before the Mixter and Barr era.

The understanding of lumbar spine pathologies made substantial progress at the turn of the twentieth century. The authors review the original publication of Otto Veraguth in 1929 reporting on the successful resection of a herniated lumbar disc, published exclusively in the German language. His early report is put into the historical context, and its impact on the understanding of pathologies of the intervertebral disc (IVD) is estimated. The Swiss surgeon and Nobel Prize laureate Emil Theodor Kocher was among the first physicians to describe the traumatic rupture of the IVD in 1896. As early a

A clinically useful classification of traumatic intervertebral disk lesions.

OBJECTIVE: Lesions of the intervertebral disk accompanying vertebral fractures are the subject of controversy regarding the extent of surgical intervention, in part due to the lack of a comprehensive classification. The purpose of this study is to present a novel and clinically useful classification system for traumatic disk lesions after vertebral fractures. MATERIALS AND METHODS: MRI of 204 disks in 102 patients with trauma-induced single-level thoracolumbar fractures referred to our trauma center between 2007 and 2011 were analyzed retrospectively. Exclusion criteria were malignant vertebr

Low thoracic and lumbar burst fractures: radiographic and functional outcomes.

Twenty patients with thoracolumbar burst fractures (type A3 in the classification of Magerl et al.) were studied prospectively for the evaluation of clinical, radiographic and functional results. The patients were submitted to surgical treatment by posterior arthrodesis, posterior fixation and autologous transpedicular graft. The patients were followed up for 2 years after surgery and assessed on the basis of clinical (pain, neurologic deficit, postoperative infection), radiographic (load sharing classification, Farcy s sagittal index of the fractured segment, relation between traumatic verteb

Lumbar disc herniation in early childhood: case report and literature review.

INTRODUCTION: Lumbar disc herniation is a common condition in adults, uncommon in adolescents and exceptional below the age of 10 years. CASE REPORT: We report on a 6-year-old boy who presented with low-back pain and sciatica after having sustained an injury in a rear-end automobile accident. Due to our limited experience with protruded discs in this age group, we decided to be conservative initially. However, the persistence of incapacitating pain prompted surgical treatment. Operative findings consisted of a torn annulus, attached at one end to the cartilaginous plate, which formed a major

[Double vertebral injury in a calf (case report)].

A double spinal cord trauma of an eight weeks old calf is reported, born on pasture from a heifer as a premature birth at the beginning of month 9. Observations made were rupture of intervertebral disk at the crossing of cervical to thoracic vertebrae followed by syndesmosis or synchondrosis resp., as well as a comminuted fracture of the 1st lumbar vertebra including both the adjoining vertebrae, with succeeding reactive callus formation. It is concluded that this traumatization has occurred during parturition. Motoric or sensory disturbances were observed neither.

[Exertion, spinal fracture, intervertebral disk displacement].

If causes which cannot be clearly defined as traumatic, result in vertebral fractures or disc ruptures, the expert frequently finds himself confronted with considerable problems. He must carefully establish the relation between what is due to the trauma and what must be attributed to the pre-existing damage. This requires the pre-existing nox to be objectively cleared, especially with regard to osteoporosis. The diagnosis "osteoporosis" should be subjected to particularly careful investigation, especially if the patient is a male. X-ray photographs, taken at an early stage, are insufficient. A

Juvenile lumbar discs.

One hundred and two patients, ages 10-18 years, were operated upon for herniated lumbar discs. Until age 17, the male-to-female ratio was even. A significant relationship was found between traumatic etiology and the occurrence of free disc rupture. Disc material was found outside the annular ligament in more than half of the patients. Results of surgical excision have been excellent in 95% of cases. Same level recurrence remains below 3%.

Jejuno-renal injury following traumatic hyperextension.

A 4-year-old boy suffered hyperextension injury with small-bowel obstruction and renal injuries. This unusual constellation of findings is discussed in light of the pathophysiologic mechanisms of the injury. Hyperextension of the spine resulted in rupture of the anterior longitudinal ligament and avulsion of the L1-L2 disc; a loop of bowel was then drawn into the disc space. There also was an internal tear of the renal artery. Two years after uretero-ureterostomy with nephrostomy tube, the child is normotensive with adequate renal function and with normal growth and development.
