Successful open repair of blunt traumatic thoracoabdominal aortic injury with concomitant L1/L2 fracture-dislocation.

BACKGROUND: Although rare, blunt traumatic aortic injury occurs in the aortic isthmus and rarely in the thoracoabdominal aorta. CASE PRESENTATION: A 56-year-old man became unconscious after falling from the third floor during a suicide attempt. Computed tomography revealed a thoraco-abdominal aortic injury (pseudo-aneurysm) and a posterior dislocation fracture at L1/L2. Thoraco-abdominal aortic graft replacement was performed 3 d after the injury, and posterior spinal fusion was performed 10 d later. CONCLUSIONS: Thoracic endovascular aortic repair is usually performed for blunt traumatic ao

Radiofrequency echographic multispectrometry assessment of osteoporosis in severe motor and intellectual disabilities: A cross-sectional study.

Conventional bone mineral density (BMD) assessment using dual-energy X-ray absorptiometry is challenging in individuals with severe motor and intellectual disability (SMID) because of scoliosis, hip dislocation, joint contractures, and involuntary movements. Radiofrequency echographic multispectrometry (REMS) is a feasible, radiation-free alternative to bedside BMD assessment. In this cross-sectional study, we aimed to characterize REMS-based BMD profiles of lumbar spine (L1-L4) and femoral neck in individuals with SMID residing at a care facility and identify clinical factors associated with

Complication Rates of Hip-Spine Syndrome Patients With Total Hip Arthroplasty and Lumbar Spine Surgery.

INTRODUCTION: Hip-spine syndrome, defined as the coexistence of degeneration in the hip and lumbar spine, is a common disorder in the elderly patient population. There is much debate on whether the hip or lumbar spine should be prioritized for surgical intervention. Although lumbar spine surgery (LSS) and total hip arthroplasty (THA) are well-investigated with proven success, there is limited knowledge on whether the order of surgical intervention influences outcomes. This study investigates dislocation rates and complications in hip-spine syndrome patients with a history of LSS and the impact

Impact of Operative Order on Postoperative Outcomes in Patients Undergoing Lumbar Fusion and Total Hip Arthroplasty: A Systematic Review and Meta-Analysis.

STUDY DESIGN: Systematic review and meta-analysis. OBJECTIVE: To assess how operative order affects clinical outcomes and complications in patients with concomitant lumbar spinal and hip pathologies. SUMMARY OF BACKGROUND DATA: Concomitant lumbar spinal and hip pathologies are becoming increasingly prevalent in the growing population. Determining the optimal operative order for surgical treatment remains challenging, particularly when comparing spine arthrodesis followed by hip arthroplasty (SAHA) with hip arthroplasty followed by spine arthrodesis (HASA). Prior studies report conflicting re

Association between lumbar-spine trabecular bone score and radiographic severity of low-energy Colles-type distal radius fractures in women aged 50 years or older.

UNLABELLED: We assessed the association of TBS with radiographic severity of Colles-type DRFs in female patients aged ≥ 50 years. The results indicated a significantly higher incidence of intra-articular DRFs and DUFs in low-TBS group, independent of displacement. Deteriorated bone microarchitecture may be associated with the severity of low-energy osteoporotic DRFs. PURPOSE: This study investigated the effect of the trabecular bone score (TBS), which reflects bone microarchitecture, on the radiographic severity of Colles-type distal radius fractures (DRFs). METHODS: 104 patients aged ≥ 50 y

Does the surgical sequence of lumbar spine fusion and total hip arthroplasty affect the dislocation and revision risk? A meta‑analysis and systematic review.

BACKGROUND: Patients who underwent primary total hip arthroplasty (THA) after lumbar spine fusion (LSF) had a higher incidence of complications compared with those who had not previously undergone LSF. This study aimed to determine whether the timing of THA before or after LSF impacts the incidence of THA complications. METHODS: Following the Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines, we searched PubMed, Web of science, Embase, and Cochrane Library from their inception until 1 October 2025. Data from all eligible published studies meeting the inclu

How Is Lumbar Fusion Associated With Compensatory Hip Motion After THA?

BACKGROUND: Although patients with THA may increase their use of hip flexion to compensate for loss of spinal motion after lumbar fusion, no studies have directly examined this relationship. Furthermore, the association between the number of fused levels on postoperative compensatory hip motion is not clear, making it difficult to anticipate these changes prior to lumbar fusion. QUESTIONS/PURPOSES: (1) Is lumbar fusion after THA associated with resting spinopelvic alignment or compensatory changes at the hip? (2) Is the number of fused spinal segments associated with an increase in hip compen

Traumatic lumbosacral instability: part 2-indications and techniques for surgical management.

Traumatic lumbosacral instability (TLSI) refers to a traumatic disruption of the axial skeleton at the level of the lumbosacral motion segment and/or sacrum, resulting in mechanical separation of the caudal spinal column from the posterior pelvic ring. Managing TLSI and its four underlying conditions poses unique challenges among spinal and pelvic injuries. This second part of a two-part series focuses on treatment strategies and decision making in TLSI, with an emphasis on surgical stabilization techniques. The primary objectives of this article are to: (1) elucidate factors influencing clini

Successful ultrasound-guided epidural anaesthesia following failed spinal anaesthesia in a patient with neurofibromatosis 1.

Neurofibromatosis type 1 (NF1) is a genetic disorder characterised by neurofibromas and skeletal anomalies, including lumbar kyphoscoliosis and dysmorphic sacral vertebrae. We report a rare case of a male in his late 30s with NF1 presenting with a left-sided hip dislocation and an unusual response to spinal anaesthesia. Spinal anaesthesia was attempted using 3 mL of 0.5% hyperbaric bupivacaine (dextrose 8%), resulting in a unilateral block. Pulmonary function tests did not favour general anaesthesia; hence, an MRI was performed that showed extensive abnormalities in the spine. The next day, ul

Evaluating cervical dislocation methods, without using tension on the tail, for humanely killing adult laboratory mice.

Cervical dislocation (CD) is a widely used method worldwide for humanely killing adult laboratory mice in accordance with national legislation, such as the Animals (Scientific Procedures) Act 1986 in the UK. However, concerns have been raised regarding the limitations and potential risks associated with CD, including a reported failure rate of up to 20% and the risk of injury to the thoracic or lumbar spine region. To address these concerns, we have adopted a CD method that avoids the use of tension on the tail or any additional tools. In this study, we detail our process of validation through

Fueling Prostate Cancer: The Central Role of Glutamine/Glutamate Metabolic Reprogramming.

Metabolic reprogramming induced by the glutamine/glutamate (Gln/Glu) metabolic pathway is a key mechanism in ATP production, precursor biosynthesis, and redox homeostasis, promoting prostate cancer (PCa) growth and proliferation. This evolutionarily acquired hallmark of cancers enables malignant cells to adapt their bioenergetic and biosynthetic pathways in response to microenvironmental stresses. Therefore, Gln/Glu metabolism orchestrates epigenetic regulation, metastatic capacity, and oxidative homeostasis in PCa, supporting the survival of PCa tumors. Fluctuations in Glu metabolite levels a

Bilateral trans-scaphoid perilunate fracture-dislocation: A case report.

BACKGROUND: Bilateral trans-scaphoid perilunate fracture dislocation is an infrequent injury that usually occurs following either high-energy trauma or a fall on an outstretched hand. It can be easily overlooked in patients with polytrauma and may lead to an unfavorable prognosis with pain and arthritis. CASE PRESENTATION: We present the case of a 26-year-old man with a history of a fall from a two-story building, diagnosed with bilateral trans-scaphoid perilunate fracture-dislocation, bilateral calcaneal fractures, and a burst fracture of the third lumbar vertebra (L3). In the emergency depa

Early revision rate after posterior spinal fusion surgery increases with every additional BMI point.

PURPOSE: Revision surgery has a profound impact on patient outcomes and is a crucial consideration in the assessment of healthcare burden following spine surgery. In this context, obesity is a significant factor influencing the rate of revision surgery. To elucidate the impact of obesity on the risk of early revision surgery after posterior fusion of the thoracic and lumbar spine, we conducted a prospective single-institution cohort study. METHODS: Over a 24-month period a total of 227 consecutive patients who underwent posterior thoracolumbar spinal fusion surgery involving up to four segmen
