Harrington instrumentation was used to treat 23 injuries of the thoracic and lumbar spine accompanied by neural damage encountered during an 8 1/2-year period. The indication for the use of Harrington instrumentation is instability of the thoracic or lumbar spine due to trauma, laminectomy, or both. A study of 60 children with thoracic and lumbar spine injuries indicates that the child's response to trauma is unique. Eight cases of fractures in haemophiliac patients are presented. The clinical and radiological features, behaviour and response to treatment indicate that they are normal, not pathological fractures. If fractures in haemophiliac patients are treated early and correctly both from a haematological and orthopaedic point of view, the results are comparable with those in normal subjects. Twenty patients treated for degenerative spondylolisthesis with an intact neural arch principally at the L4-5 interspace had neural compression caused by dislocation of the vertebral bodies and intrusions of lamina and enlarged, arthrotic facets into a stenotic spinal canal. The authors describe a technique whereby a portion of the lamina removed during exposure of an intervertebral lumbar disc protrusion is implanted in the intervertebral disc space following disc excision. An analysis of 456 consecutive cases operated on by this technique and followed from 1 to 10 years is presented. Of the 418 patients followed, 92 per cent indicated they were able to return to their normal activities and were satisfied with the result. It is obvious from the above that there are several independent mechanisms in the production of the various types of lumbar fractures. Thirty-nine patients with post-poliomyelitic scoliosis and pelvic obliquity were treated and followed for up to four years. The dominant pathological mechanism causing the pelvic obliquity was trunk-muscle imbalance. The natural history of untreated pelvic obliquity is progression, leading ultimately to dislocation of the hip. The functional abilities of the patient are then severely restricted. Observation over clinical features distinguishing the course of traumas involving the sacro-lumbar vertebral column and spinal cord in 251 patients permitted it to recognize a number of points determining the orientation of a complex restorative treatment.
