Diagnoses and mechanisms of musculoskeletal injuries in an infantry brigade combat team deployed to Afghanistan evaluated by the brigade physical therapist.

UNLABELLED: Musculoskeletal injuries are the most common cause for disability in deployed environments. Current research is limited to body region affected by the injury. OBJECTIVE: To determine the prevalence of musculoskeletal diagnoses and mechanisms of injury (MOI) as well as associations to specific Military Occupational Specialties (MOS) in a deployed Brigade Combat Team (BCT). METHODS: Data collected on 3,066 patient encounters by the Brigade Combat Team physical therapist over 15 months were analyzed using descriptive statistics and X2 tests. RESULTS: Mechanical low back pain was th

Reduced thoracolumbar fascia shear strain in human chronic low back pain.

BACKGROUND: The role played by the thoracolumbar fascia in chronic low back pain (LBP) is poorly understood. The thoracolumbar fascia is composed of dense connective tissue layers separated by layers of loose connective tissue that normally allow the dense layers to glide past one another during trunk motion. The goal of this study was to quantify shear plane motion within the thoracolumbar fascia using ultrasound elasticity imaging in human subjects with and without chronic low back pain (LBP). METHODS: We tested 121 human subjects, 50 without LBP and 71 with LBP of greater than 12 months du

Neuromuscular manifestations of viscoelastic tissue degradation following high and low risk repetitive lumbar flexion.

Cumulative lumbar disorder is common in individuals engaged in long term performance of repetitive and static occupational/sports activities with the spine. The triggering source and of the disorder, the tissues involved in the failure and the biomechanical, neuromuscular, and biological processes active in the initiation and development of the disorder are not known. The hypothesis is forwarded that static and repetitive (cyclic) lumbar flexion-extension and the associated repeated stretch of the various viscoelastic tissues (ligaments, fascia, facet capsule, discs, etc.) causes micro-damage

Acute repetitive lumbar syndrome: a multi-component insight into the disorder.

PURPOSE: Repetitive Lumbar Injury (RLI) is common in individuals engaged in long term performance of repetitive occupational/sports activities with the spine. The triggering source of the disorder, tissues involved in the failure and biomechanical, neuromuscular, and biological processes active in the initiation and development of the disorder, are not known. The purpose is, therefore, to test, using in-vivo feline model and healthy human subjects, the hypothesis that RLI due to prolonged exposure to repetitive lumbar flexion-extension is triggered by an acute inflammation in the viscoelastic

Spine injuries in dancers.

Care of a dancer calls for a unique balance between athlete and artist. The physician must familiarize himself or herself with dance terminology, common moves, correct technique, and dancer's mentality. The goal is to work intimately with the dancer to care for the injury and, if possible, continue to participate in portions of dance class to limit anxiety and increase compliance to treatment. The spine is the second most injured area of the body in dancers, and many issues stem from poor technique and muscle imbalance. This often leads to hyperlordosis, spondylolysis, spondylolisthesis, lumba

Injury profile in junior tennis players: a prospective two year study.

The aim of this study was to prospectively make a survey of injuries in junior players from a Swedish local tennis club during a 2-year period in relation to gender, anatomic location, month of the year when injured, injury type and injury severity. All 12-18 years old members in a tennis club playing more than twice weekly were asked to participate. Fifty-five junior tennis players, 35 boys and 20 girls accepted to participate. All tennis-related injuries were prospectively registered and evaluated. Time of exposure for playing tennis was recorded. Thirty-nine players sustained 100 injuries,

Validation of the cat as a model for the human lumbar spine during simulated high-velocity, low-amplitude spinal manipulation.

High-velocity, low-amplitude spinal manipulation (HVLA-SM) is an efficacious treatment for low back pain, although the physiological mechanisms underlying its effects remain elusive. The lumbar facet joint capsule (FJC) is innervated with mechanically sensitive neurons and it has been theorized that the neurophysiological benefits of HVLA-SM are partially induced by stimulation of FJC neurons. Biomechanical aspects of this theory have been investigated in humans while neurophysiological aspects have been investigated using cat models. The purpose of this study was to determine the relationship

Continuous assessment of low back loads in long-term care nurses.

Considerable effort has been spent evaluating aspects of low back injury risk in nursing yet comprehensive evaluation of all work tasks has been limited. The purpose of this study was to evaluate peak and cumulative lumbar spine loads experienced by personal support workers. A total of 20 female long-term care workers were observed and had trunk posture monitored via an inclinometer throughout their shift. When adjusted for an 8-h workday, workers experienced cumulative loads of 21.3 +/- 4.6 MNs, 1.8 +/- 0.6 MNs and 2.9 +/- 1.4 MNs for compression, lateral and anterior shear, respectively. Pat

High-frequency loading of lumbar ligaments increases proinflammatory cytokines expression in a feline model of repetitive musculoskeletal disorder.

BACKGROUND CONTEXT: Cumulative (repetitive) lumbar disorder is common in the workforce, and the associated epidemiology points out high risk for lifting heavy loads, performing many repetitions, and performing movements at high velocity. Experimental verification of viscoelastic tissue degradation and a neuromuscular disorder exist for cyclic work under heavy loads. Experimental validation for a disorder because of cyclic loads under high-velocity movement is missing. PURPOSE: Obtain experimental verification that high-velocity lumbar flexion-extension results in significant increase of proin

Musculoskeletal injuries description of an under-recognized injury problem among military personnel.

INTRODUCTION: Although injuries are recognized as a leading health problem in the military, the size of the problem is underestimated when only acute traumatic injuries are considered. Injury-related musculoskeletal conditions are common in this young, active population. Many of these involve physical damage caused by micro-trauma (overuse) in recreation, sports, training, and job performance. The purpose of this analysis was to determine the incidence of injury-related musculoskeletal conditions in the military services (2006) and describe a standardized format in which to categorize and repo

Kümmell' s disease: A rare spine entity in a young adult.

Over 100 years ago, Dr Hermann Kümmell described a rare clinical entity in which patients, after a trivial trauma and an asymptomatic period, developed a progressive vertebral body collapse and a painful kyphosis. We present the case of a 31years old heavy labourer, fitting Kümmell's criteria. The patient referred to us in an incapacitated state, due to persistent back pain. Radiographic examination revealed a body collapse of L1 vertebra. The patient had no previous medical record, other than a prolonged history of transient back pain episodes, related to heavy-weight lifting. Last attack was

Imaging of triathlon injuries.

Injuries in triathletes are common and are mostly overuse injuries. Rotator cuff tendinitis is the most common complaint from swimming, but the incidence of tendinopathy and rotator cuff tears on magnetic resonance imaging is comparable in triathletes without and with shoulder pain. Cycling injuries are mainly to the knee, including patellar tendinosis, iliotibial band syndrome, and patellofemoral stress syndrome, and to the Achilles tendon and the cervical and lumbar spine. Running is associated with most injuries in triathletes, during both training and racing, causing the athlete to discont

Methods of model calibration: observations from a mathematical model of cervical cancer.

Continuing with comparative genomic exploration of worldwide butterfly fauna, we use all protein-coding genes as they are retrieved from the whole genome shotgun sequences for phylogeny construction. Analysis of these genome-scale phylogenies projected onto the taxonomic classification and the knowledge about butterfly phenotypes suggests further refinements of butterfly taxonomy that are presented here. As a general rule, we assign most prominent clades of similar genetic differentiation to the same taxonomic rank, and use criteria based on relative population diversification and the extent o

Musculoskeletal injuries in break-dancers.

BACKGROUND: Since no epidemiologic studies have been reported about musculoskeletal injuries in break-dancers, there are no data on the rates and patterns of musculoskeletal injuries in this population that clinicians can use to find ways to decrease injury rate. HYPOTHESIS: We believe that the incidence of injuries in break-dancers is higher than assumed and that injury rates and patterns differ between professional and amateur dancers. STUDY DESIGN: Descriptive epidemiologic study. MATERIALS AND METHODS: Of a total of 42 study subjects, 23 were professional dancers and 19 were amateur dan

High magnitude cyclic load triggers inflammatory response in lumbar ligaments.

BACKGROUND: Cumulative trauma disorder is commonly reported by workers engaged in prolonged repetitive/cyclic occupational activities. Recent experimental evidence confirms that relatively short periods of cyclic lumbar flexion at high loads result in substantial creep of viscoelastic tissues, prolonged periods of its recovery to baseline together with a neuromuscular disorder and exposure to instability. The biochemical process associated with the creep and neuromuscular disorder are not well explored. The purpose of the study is to identify the ligaments as one of the organs of failure and a
