Static load magnitude is a risk factor in the development of cumulative low back disorder.

Occupations requiring frequent periods of static lumbar flexion are known epidemiologically to be risk factors for the development of cumulative low back disorder. The impact of the load magnitude sustained during a series of short static lumbar flexions followed by an equally long rest period on the development of a cumulative low back disorder was addressed in an in vivo feline model. Static loads of 20, 40, and 60 N were applied over 10 min of flexion followed by 10-min rest sessions that were repeated six times (for a total of 2 h) while monitoring lumbar viscoelastic creep (laxity) and re

Human lumbar facet joint capsule strains: I. During physiological motions.

BACKGROUND CONTEXT: The lumbar facet joint capsule is innervated with nociceptors and mechanoreceptors, and is thought to play a role in low back pain as well as to function proprioceptively. PURPOSE: In order to examine the facet capsule's potential proprioceptive role, relationships between intracapsular strain and relative spine position were examined. STUDY DESIGN/SETTING: Lumbar facet joint capsule strains were measured in human cadaveric specimens during displacement-controlled motions. METHODS: Ligamentous lumbar spine specimens (n=7) were potted and actuated without inducing a momen

Human lumbar facet joint capsule strains: II. Alteration of strains subsequent to anterior interbody fixation.

BACKGROUND CONTEXT: In cases of low back pain associated with biomechanical lumbar instability, anterior interbody fixation can be used as a surgical treatment, but its affect on facet joint capsule strains is unknown. PURPOSE: To determine the effect of a single-level anterolateral interbody fixation, the changes in lumbar facet joint capsule strains at the level of and adjacent to the fixation were evaluated. STUDY DESIGN/SETTING: Human cadaveric lumbar spine specimens were tested under displacement control before and after the addition of a single anterior thoracolumbar plate (ATLP) on th

Frequency-dependent changes in neuromuscular responses to cyclic lumbar flexion.

Repetitive lifting in the workplace has been identified to be a cause of low back disorders. Epidemiologic data further supports an hypothesis that higher repetition rate (i.e. frequency) is an added risk factor. The objective of this study was to provide experimental data testing the above hypothesis. An in vivo feline model was subjected to 20-min of cyclic lumbar loading at frequencies of 0.1 Hz and 0.5 Hz while monitoring the EMG from the L-3/4-L-5/6 multifidus muscles and the creep at the L-4/5 level. Seven hours of rest were allowed after the cyclic flexion/extension was terminated. Duri

Static load repetition is a risk factor in the development of lumbar cumulative musculoskeletal disorder.

STUDY DESIGN: In vivo feline model subjected to variable number of repetitions of a short static lumbar flexion followed by an equally long rest period. OBJECTIVES: The purpose of this study was to determine the influence of the number of repetitions as a risk factor in promoting a cumulative low back disorder in the feline model. SUMMARY OF BACKGROUND DATA: Epidemiologic data point out that the increased number of repetitions of static lumbar loading is a major risk factor in the development of cumulative low back disorder. Biomechanical and physiologic confirmation of the epidemiology is l

The influence of slouching and lumbar support on iliolumbar ligaments, intervertebral discs and sacroiliac joints.

OBJECTIVE: To investigate lumbopelvic kinematics when moving into a slouch. DESIGN: A biomechanical model was developed. Load tests in vitro verified the model. BACKGROUND: The precise mechanism causing disc herniation and back sprain is still debated. Most biomechanical studies have focused on lifting in a stooped posture. Previous studies address instability situations due to Euler buckling of the spine under axial load. However, no studies address lumbosacral, iliolumbar and sacroiliac kinematics in slouching, i.e. flexing the spine in situations with negligible compressive spinal load.

Interspinous bursitis in an athlete.

We present a case of L2/3 interspinous bursitis treated with extraspinal injections. No previous investigations have used fluoroscopically guided spinal injections to confirm the clinical relevance of the MRI features of this type of bursae. Autopsy studies have revealed an increased incidence of interspinous lumbar bursal cavities with advancing age. Afflicted patients present with localised, midline lower lumbar pain exacerbated by extension. In young athletes these symptoms can mimic spondylolysis. MRI is useful in detecting soft-tissue injury of the posterior elements. Fluoroscopically gui

Lumbar spine region pathology and hamstring and calf injuries in athletes: is there a connection?

This paper discusses the theory that subtle lumbosacral canal impingement of the L5 nerve root may be a relatively common occurrence in older footballers and may in fact be a common underlying basis for the age related predisposition towards hamstring and calf strains.

Retroperitoneal hematoma caused by a ruptured pelvic varix in a patient with iliac vein compression syndrome.

May-Thurner syndrome, or iliac vein compression syndrome (IVCS), is a rare but well-described entity. It refers to the formation of venous thrombus caused by compression of the left iliac vein, most commonly between the right iliac artery and lumbar vertebrae. Several variants of IVCS have been described, including unusual presenting symptoms, etiologies, and complications. The authors describe an unusual case of IVCS in which the patient presented with a left-sided retroperitoneal hematoma arising from a ruptured collateral venous varix shortly after the development of symptomatic left lower-

Neuromuscular dysfunction elicited by cyclic lumbar flexion.

An attempt was made to develop an in vivo model that could explain the neurophysiological and biomechanical processes active in the development of the idiopathic low back disorder common in workers who perform repetitive lifting tasks in industry. Passive cyclic flexion of the feline lumbar spine at 0.1 HZ for 20 min resulted in creep of the supraspinous ligament and other lumbar viscoelastic tissues as well as spasms superimposed on a decreasing electromyogram (EMG) elicited reflexly from the multifidus muscles. Rest for 7 h did not allow full recovery of the viscoelastic creep; the multifidu

Degenerative spondyloarthropathy of the cervical and lumbar spine in jockeys.

This prospective study analyzed the long-term effects of horse riding on the cervical and lumbar spine of jockeys. Thirty-two jockeys were observed for 13 years. All jockeys had clinical and radiographic evaluation of the spine; three consecutive age groups were studied. Results were compared to age-matched, normal population control groups. The incidence of degenerative changes of the spine was higher in the jockeys compared with the control groups and was more prominent in the older age group for both the lumbar and cervical spine. These findings suggest that equestrian sports, particularly

Histology of intervertebral disc protrusion: an experimental study using an aged rat model.

STUDY DESIGN: In vitro experimental intervertebral disc ruptures of aged rats were examined histologically. OBJECTIVES: To clarify the mechanism of intervertebral disc herniations by microscopic investigation of ruptured discs. SUMMARY OF BACKGROUND DATA: Clinically, disc herniations have been classified into two types: extrusion and protrusion. However, the pathogenesis of protrusion type herniations has not yet been demonstrated by any studies. To clarify this issue, it is essential to establish an appropriate model producing disc herniations, and to examine the sequential changes in the s

Bloodletting acupuncture of the engorged vein around Bl-40 (Wei-Chung) for acute lumbar sprain.

Bloodletting acupuncture is one of the most classic methods of acupuncture therapy, and is still popularly used to treat acute lumbar sprain in the oriental world. However, most physicians in the western world are not familiar with bloodletting acupuncture, though they may know ordinary acupuncture well. Furthermore based on the literature reviewed, there have been few studies which have investigated the effect of bloodletting acupuncture upon acute lumbar sprain. In this study, we tried to determine if bloodletting acupuncture is effective for acute lumbar sprain. In total, twelve patients we

Biexponential recovery model of lumbar viscoelastic laxity and reflexive muscular activity after prolonged cyclic loading.

OBJECTIVES: To determine the rest duration required for full recovery of reflexive muscular activity and laxity/creep induced in the lumbar viscoelastic structures (e.g., ligaments, discs, etc.) after 50 min of cyclic loading, and to develop a model describing such recovery. BACKGROUND: It is well established that steady, cyclic or vibratory loading of the lumbar spine induces laxity/creep in its viscoelastic structures. It was also shown that such viscoelastic creep does not fully recover when subjected to rest equal in duration to the loading period. Rest periods of 24 h, however, were more
