Clinical instability of the spine is an intensely controversial subject, and its diagnosis, especially in the aging, is difficult. Yet success in its management rests on accurate diagnosis. Because both clinical presentation and radiographic manifestations are nonspecific, the diagnosis of clinical instability lies in understanding the biomechanics involved, in recognizing the relevant radiographic manifestations, and, most importantly, in correlating those observations with the patient's clinical history and physical examination. Stabilization is the treatment of choice for clinical instabili

In this series, the patients with disturbance of small articulations of the lumbar vertebrae were treated by puncturing the effective points and reduction by manual traction with satisfactory results, the cure rate being 39.3%, the markedly effective rate 28.6%, with a total effective rate 98.3%. This method is simple and easy to master, and highly effective. Therefore it is very acceptable to the patients. It is a very excellent therapeutic method for disturbance of small articulations of the lumbar vertebrae, sometimes complicated by acute lumbar sprain and chronic lumbar strain.

30 competitive volleyball attackers of the German third league, 15 with shoulder pain (group I) and 15 without shoulder pain (group II) were examined clinically and sonographically. Anamnesis was also carried out. The results were compared with those of a control of 15 non-volleyball players not subject to stress caused by overhead use of their arms. On the visual analog scale the average pain was 5.1 (SD 1.4). The average history of pain was 2.7 (SD 2.7) years. Cause for the pain was "spiking without warming up" (n = 7) or "Нет данных" (n = 8). Spiking respectively serving caused new pain sensat

A comparative study of cervical spines from 16 subjects who died of major trauma and 16 control subjects who died of natural causes, showed clefts in the cartilage plates of the intervertebral discs in 15 of 16 spines from the trauma victims. These were quite distinct from the uncovertebral clefts and central disc fissures that are a normal feature of aging in cervical discs. Posterior disc herniation through a damaged anulus fibrosus and hemarthrosis in facet joints were also observed. No directly comparable lesions were found in the control subjects, but two discs in this group showed "rim l

Low back pain is a large and increasing cause of workers compensation claims in all industrialized nations. In the United States, 23% of all compensation claims involved back pain. Almost 90% of these were self limiting sprains and strains. It has been estimated that the remaining 10% account for approximately 75% of lost work days, medical costs and permanent disability payments. It is the goal of the spine surgeon to provide the patient with relief from his symptoms and return him or her to the work place. Advances in lumbar skeletal fixation have increased the fusion rate, decreased hospita

While the incidence of injury in cross-country skiing remains relatively low, overuse problems affecting the lumbosacral region may be on the rise, particularly among elite athletes. In this study, a certified "Part A" (Canadian Physiotherapy Association) manual therapist performed lumbosacral physical assessments on 18 elite cross-country skiers and 15 normal subjects. Results indicated sacroiliac (SI) joint dysfunction occurred significantly more often in the skier population (p < 0.007). Incidences of SI joint asymmetry and lumbar spine dysfunction did not differ between groups. The predomi

A total of 395 male infantry recruits were evaluated in a prospective study of possible risk factors for overexertional back pain and the efficacy of drug treatment regimens for this syndrome. Recruits were classified into subgroups of lumbar or thoracic, and paraspinal or spinous process pain. Recruits were divided into three treatment groups: Ibuprofen, Paracetamol, and no drug treatment. Of the recruits, 18% were diagnosed as having overexertional back pain during the course of 14 weeks of training. By multivariate analysis low body mass index was found to be a risk factor for overexertiona

The action of fast bowling in the game of cricket is known to cause injuries to the lumbar spine. We studied a group of 16- to 18-year-old fast bowlers, selected for special training in Western Australia. All 24 had MR scans of the spine, 22 had radiographs and CT scans; in 20 the bowling technique was analysed biomechanically. There was a high incidence of back pain and this was always associated with a radiological abnormality. Pars interarticularis defects were diagnosed in 54% and intervertebral disc degeneration in 63%. Bowling actions which involved counter-rotation were associated with

Cricket fast bowlers are the potent strike force in a multidiscipline team. They subject their spines to repetitive sagittal plane and rotatory movements over many years. The effect of this repetitive stress has not previously been analysed. This study examined 20 former fast bowlers to determine the incidence of spondylolysis, spondylolisthesis and degenerative change. Fast bowlers are noted to have an increased incidence of spondylolysis. A mixed front/side bowling style involving more lumbar hyperextension or rotation has significant association with spondylolysis when compared with side-on

As many as 1.5 million young men participate in American football in the United States. An estimated 1.2 million football-related injuries are sustained annually. Since the 1970s epidemiological studies have shown that the risk of injury is higher in older athletes and lower in teams with more experienced coaches and more assistant coaches. 51% of injuries occurred at training; contact sessions were 4.7 times more likely to produce injuries than controlled sessions. Injury rates were reduced by wearing shorter cleats and preseason conditioning. Overall, lower extremity injuries accounted for 5

Lumbar spine pain accounts for 5 to 8% of athletic injuries. Although back pain is not the most common injury, it is one of the most challenging for the sports physician to diagnose and treat. Factors predisposing the young athlete to back injury include the growth spurt, abrupt increases in training intensity or frequency, improper technique, unsuitable sports equipment, and leg-length inequality. Poor strength of the back extensor and abdominal musculature, and inflexibility of the lumbar spine, hamstrings and hip flexor muscles may contribute to chronic low back pain. Excessive lifting and

The causes of low back pain range from ruptured lumbar disks and spinal stenosis to compression fractures and osteoarthritis. Although surgery may be indicated in some cases, conservative measures are often effective, and preventive regimens may be successful in patients who are obese or otherwise physically unfit. In patients who have been injured on the job or in an automobile accident, the question of compensation may hinder treatment. Hysterical patients may need psychiatric referral.

This study was performed in an attempt to determine the total spinal compressive load during assembly line work to find a possible association with the many complaints of back pain. A flexion analyzer was used to register trunk movements, and analysis of postures and lifted weights was done from video recordings. The load on the spine at the L3 level was calculated through a biomechanical model, meant for analysis of static, sagittally symmetric postures and lifting tasks. Maximum lift tests were performed before and after a full work day. The peak load on the L3-L4 level when lifting correspo

Orthopaedists and other physicians in Maine organized the Maine Medical Assessment Foundation to deal with the problem of variations in the rates of hospitalization for orthopaedic conditions. Five musculoskeletal injuries and five orthopaedic procedures were selected for study. The variation in decision-making by orthopaedists was least for fractures of the ankle and fractures of the hip and was greatest for fractures of the forearm, derangement of the knee, and lumbosacral sprain. The rates in an area tended to be consistently high or low for the same treatments. The major reasons for the va

The authors report the study of a series of 23 partial isthmic lyses which occurred in a young population (15 cases) and in an adult one (8 cases) between 1983 and 1988, in the form of persistent lumbosacral pains. The initial radiological signs, the key to an early diagnosis, are defined: cortical notches more often regarding the lower cortex than the upper one, incomplete fissures reaching the two cortical poles, gradual narrowing of the isthmus. The value of standard radiography is underlined, with the oblique incidences close to the profile regularly ensuring the diagnosis, with the except

A questionnaire, designed to elict information about training programs, experience and injury profile, was administered to 358 bodybuilders and 60 powerlifters. This was followed by a clinical orthopedic and radiological examination. The upper extremity, particulary the shoulder and elbow joint, showed the highest injury rate. More than 40% of all injuries occurred in this area. The low back region and the knee were other sites of elevated injury occurrences. Muscular injuries (muscle pulls, tendonitis, sprains) were perceived to account for 83.6% of all injury types. Powerlifting showed a twi

Stress fracture in the dorsolumbar region is a well-known complication of the rigid spine usually occurring at the end stages of ankylosing spondylitis. When indicated, surgical treatment by anterior spinal fusion is prescribed. Three cases illustrating the appearance of new stress fractures in different parts of the spine after the successful anterior surgical fusion of a previous lesion indicate the possibility of recurrence. This explains the persistence of back pain postoperatively. These stress fractures seem unrelated to spinal deformities. External bracing is useful, but additional surg
