The sacroiliac joint (SIJ) constitutes the predominant pain source following lumbar or lumbosacral fusion. Although studies have investigated the biomechanical patterns of SIJ behaviors after lumbosacral fusion, the relationship between ligament strain and SIJ pain following lumbosacral fusion remains unclear. The present study developed a three-dimensional finite element model including L4, L5, sacrum, ilium, SIJ, and seven mainly ligaments. After successful validation, the model was used to investigate the biomechanics of SIJ and ligaments in simulating lumbosacral fusion process. Our result

OBJECTIVES: Evaluating treatment outcomes of local corticosteroid injections for work-related lower back pain (LBP) as the current evidence for the American College of Occupational and Environmental Medicine guidelines is considered insufficient to recommend this practice.
MATERIAL AND METHODS: The authors conducted a retrospective study involving the patients who were treated with peri-articular and lower lumbar corticosteroid injections for work-related LBP at their occupational medicine clinic.
RESULTS: Sixty-four patients met the inclusion criteria. The average pain level was reduced fro

OBJECTIVE: The purpose of this case report was to describe a patient presenting with back pain subsequently diagnosed with thoracic schwannoma.
CLINICAL FEATURES: A 45-year-old male presented to a chiropractic practice with mid-back, low back, and sacroiliac pain. Orthopedic examination revealed joint dysfunction in the thoracic-sacroiliac region with normal findings on neurologic examination. An initial diagnosis of thoracic spinal pain and sacroiliac sprain and strain was made. At approximately 7 weeks of treatment, because of the patient's persistent pain, chest magnetic resonance imaging

It may be assumed that the stability is affected when some ligaments are injured or loosened, and this joint instability causes sacroiliac joint pain. Several physical examinations have been used to diagnose sacroiliac pain and to isolate the source of the pain. However, more quantitative and objective information may be necessary to identify unstable or injured ligaments during these tests due to the lack of understanding of the quantitative relationship between the physical tests and the biomechanical parameters that may be related to pains in the sacroiliac joint and the surrounding ligamen

This case report describes a 10-year-old boy who presented with radiating pain (Visual Analog Scale score of 7.2 cm) down his left groin and was eventually diagnosed to have osteitis pubis. History revealed that he was exceeding the workload guidelines of bowling for a fast bowler. Examination findings were left sacro-iliac joint dysfunction, reduced left internal rotation of the hip, tightness of bilateral hip flexors and poor motor control of the lumbo-pelvic muscles. Physical therapy was aimed at correcting the sacro-iliac joint dysfunction, increasing the hip range of motion and muscle len

Back pain is common in athletes and a source of missed time from practice and competition. Pain generators include muscle (strain), ligament (myofascial sprain and strain), intervertebral disc (herniation and degeneration), nerve (radiculopathy), joint (facet and sacroiliac (SI) joint), and bones (pars interarticularis defect). The goal of treatment of an athlete with back pain is to relieve symptoms and facilitate safe but rapid return to play with no change in performance. Initial conservative treatment includes relative rest, anti-inflammatory medications, and physical therapy. The use of i

OBJECTIVE: To clarify the guidelines and responsibilities of expert witnesses and independent medical evaluators (IMEs).
DESIGN: Literature review and personal opinion.
SETTING: There have been concerns about the objectivity of expert witnesses and IMEs due to potential financial conflicts of interest.
RESULTS: Medical-legal work such as expert witness testimony and independent medical evaluations are a recognized part of the practice of medicine. As such, the opinions and testimony of expert witnesses and IMEs should be held to the same scientific and ethical standards as clinical practice

OBJECTIVE: To explore the relationship between human immunodeficiency virus (HIV) infection and soft tissue rheumatic lesions in HIV-positive black Zambians.
METHODS: We performed a prospective study of all patients over 18 years of age attending a rheumatic clinic in a teaching hospital. All patients underwent routine blood tests, and radiographs were performed when indicated. HIV status was determined by ELISA, and clinical staging was determined by World Health Organization criteria. Patients with isolated sacroiliac pain, enthesitis, or a soft tissue lesion were selected for analysis. For

The runner is especially at risk for development of injury to the hip and pelvis secondary to chronic repetitive microtrauma. The key to treatment is establishing complete and accurate diagnosis, and, in particular, identifying the functional biomechanical deficits in the kinetic chain that contribute to this repetitive microtrauma. A long-term successful outcome and prevention of reinjury are more likely if the focus of rehabilitation is on the restoration of the functional kinetic chain, rather than on a specific injured tissue. For example, the typical treatment of "iliotibial band syndrome

The sacroiliac (SI) joint is a common source of low back pain in the general population. Because it is the link between the lower extremities and the spine, it sustains even higher loads during athletic activity, predisposing athletes to a greater probability of joint dysfunction and pain. The diagnosis and treatment of SI joint dysfunction remains controversial, due to complex anatomy and biomechanics, and a lack of universally accepted nomenclature and terminology, consistently reliable clinical tests and imaging studies, and consistently effective treatments. This article clarifies these is

BACKGROUND: Evidence suggests that spinal manipulation is an effective treatment for mechanical neck and low-back pain (LBP). Treatment efficacy is important to establish for these symptoms because combined they account for a considerable amount of disability and substantial associated direct and indirect costs to society.
OBJECTIVE: The purpose of this study was to examine the outcome of patients undergoing chiropractic treatment for mechanical neck or LBP.
DESIGN AND SETTING: A retrospective, outcome-based analysis was done for patients seeking care at a private chiropractic practice over

OBJECTIVE: To present the case of a 19-year-old collegiate distance runner diagnosed with a unilateral sacral stress fracture.
BACKGROUND: Low back and sacroiliac joint pain are common in female athletes but are often difficult to differentiate. Although sacral stress fractures in young female athletes are rarely reported, they are a potential cause of low back pain.
DIFFERENTIAL DIAGNOSIS: Acute lumbosacral strain, disc disease, gluteus maximus strain, idiopathic low back pain, low back strain, sacroiliac joint sprain, or congenital anomaly.
TREATMENT: Six weeks of active rest and nutritio

A case of the external derangement-type temporomandibular disorder (TMD), temporarily relieved following chiropractic sacro-occipital technique (SOT) treatment, including SOT category II blocking to reduce sacroiliac sprain, is presented. Symptom exacerbation midway through the course of treatment followed additional dental work; symptom remission followed additional SOT treatment. Freedom from symptoms is maintained with a 3-wk treatment interval. There appears to be a cause-effect relationship between external derangement-type TMD and sacroiliac sprain. Concurrent, coordinated chiropractic a

While the incidence of injury in cross-country skiing remains relatively low, overuse problems affecting the lumbosacral region may be on the rise, particularly among elite athletes. In this study, a certified "Part A" (Canadian Physiotherapy Association) manual therapist performed lumbosacral physical assessments on 18 elite cross-country skiers and 15 normal subjects. Results indicated sacroiliac (SI) joint dysfunction occurred significantly more often in the skier population (p < 0.007). Incidences of SI joint asymmetry and lumbar spine dysfunction did not differ between groups. The predomi
