A Millimeter-Scale Implantable Magneto-Mechano-Electric Transducer Based on BTO Piezoelectric Ceramics for Remote Wireless Electrical Stimulation of Injured Sciatic Nerves.

Electrical stimulation is widely recognized as an effective strategy for promoting functional recovery after peripheral nerve injury. However, conventional stimulators rely on wired connections or implanted batteries, severely limiting implantability, biosafety, and clinical translation. Recent wireless, battery-free piezoelectric systems, primarily based on ultrasound or pressure-driven transduction, remain constrained by auxiliary equipment, working distance, and external control. Here, we report a fully wireless, circuit-free neural stimulator built on a millimetric magneto-mechano-electric

Walnut-Derived Extracellular Vesicles Orchestrate a Pre-Regenerative Niche via c-Myc Mediated Metabolic Reprogramming.

Peripheral nerve injury (PNI) remains a major regenerative challenge, in part because the post-injury microenvironment can disrupt Schwann cell (SCs) homeostasis. Walnuts (Juglans regia) have long been used in ethnomedicine for perceived neurotrophic or neuroprotective benefits, a view historically linked to their resemblance to the brain. To examine whether this traditional concept can be leveraged as a nanotherapeutic approach, we isolated walnut-derived extracellular vesicles (WEVs) and evaluated their effects on peripheral nerve repair. We found that WEVs are readily internalized by SCs an

Anatomic feasibility of intercostal-to-femoral and ulnar-to-sciatic nerve transfers for lower extremity neurotization after traumatic spinal cord injury.

BACKGROUND: Traumatic spinal cord injury (SCI) is associated with functional morbidity and limited options for restoring lower extremity motor function. We explored the technical feasibility of intercostal-to-femoral and ulnar-to-sciatic nerve transfers as a potential approach to reinnervate the lower limb following SCI. METHODS: A cadaveric anatomical feasibility study was performed using a fresh-frozen adult human specimen to evaluate two novel nerve transfer strategies: posterior intercostal-to-femoral and long-segment ulnar-to-sciatic nerve transfers. Dissections assessed the ability to a

Paeonia lactiflora-derived extracellular vesicles ameliorate diabetic peripheral neuropathy by modulating chemokine-guided immune trafficking in peripheral nerves.

AIMS: This study investigated the therapeutic potential of Paeonia lactiflora-derived extracellular vesicles (PRA-EVs) for diabetic peripheral neuropathy (DPN), with a particular focus on their immunomodulatory effects on the CC motif chemokine ligand 21 (CCL21) / CC chemokine receptor type 7 (CCR7) axis. METHODS: PRA-EVs were isolated and characterized. In vitro, their effects on high glucose (HG)-induced Schwann cell apoptosis, CCL21 expression, and CD8⁺ T-cell migration were assessed. In vivo, therapeutic efficacy was evaluated in an STZ-induced DPN mouse model using behavioral testing, hi

Identification of potential mechanisms of quercetin in diabetic peripheral neuropathy through integrated network pharmacology and experimental validation.

Diabetic peripheral neuropathy (DPN) is a common complication of diabetes characterized by oxidative stress and neuroinflammation. Quercetin, a natural flavonoid with antioxidant properties, has shown potential in treating various nerve injuries, but its mechanisms in DPN remain unclear. This research aims to explore the potential therapeutic mechanisms of quercetin in DPN through network pharmacology and experimental validation. In this study, streptozotocin (STZ)-induced diabetic rats were treated with quercetin for 12 weeks. Behavioral tests and histopathological examination were performed

Massage Regulates Brain Plasticity in Chronic Sciatic Nerve Compression Injury Rats: A Study Based on Resting-State Functional Magnetic Resonance Imaging.

BACKGROUND: Neuropathic pain (NP) is associated with maladaptive functional reorganization of the brain, yet the central mechanisms through which massage therapy exerts its analgesic effects remain poorly understood. This study aimed to investigate the impact of acupoint massage on spontaneous neural activity in a rat model of chronic constriction injury (CCI) of the sciatic nerve using resting-state functional magnetic resonance imaging (rs-fMRI). METHODS: Male Sprague-Dawley rats (N = 45) were randomly allocated into three groups: control, CCI model, and CCI with massage intervention. The m

Dual CCR2/CCR5 Antagonism with Cenicriviroc Alleviates Nerve Injury-Induced Hypersensitivity and Prevents Morphine Tolerancevia Modulation of Neuroimmune Signaling.

Mounting evidence implicates that chemokine receptors type 2 and 5 play a pivotal role in neuropathic pain. Here, we report for the first time that cenicriviroc, a dual CCR2/CCR5 antagonist, exerts strong, long-lasting, and morphine-potentiating effects in a mouse model of chronic constriction injury (CCI) to the sciatic nerve. The analgesic effect of cenicriviroc was comparable to that of morphine in the initial phase of neuropathy (Day 3) and, rewardingly, superior to the effect of morphine on Days 8 and 16 of twice-daily intraperitoneal administration. Moreover, on Day 16, tolerance to the

A potent and selective P2 × 7R antagonist ameliorates neuropathic pain by suppressing inflammation and oxidative stress in a mouse chronic constriction injury model.

Neuropathic pain is an unpleasant condition that involves neuroinflammation and oxidative stress following injury to peripheral nerves. Recently, P2 × 7R, an ATP-gated channel, has been shown to play a role in the pathogenesis of neuropathic pain. Hence, we investigated the role of a potent and selective P2 × 7R antagonist in the amelioration of neuropathic pain in a mouse model of chronic constriction injury (CCI). Mice were randomly divided into five groups, namely: sham, CCI, CCI + P2 × 7R antagonist (2.5 mg/kg), CCI + P2 × 7R antagonist (0.5 mg/kg), and CCI + pregabalin (5 mg/kg). Behavior

Cdc42-Modified BMSC-Derived exosomes promote acellular nerve allografts to bridge sciatic nerve defects.

Peripheral nerve injury (PNI) often results in persistent functional deficits, and current treatments remain suboptimal. This study developed a tissue-engineered graft by integrating Cdc42-modified bone marrow-derived mesenchymal stem cell (BMSC)-derived exosomes (Exos-Cdc42) with an acellular nerve allograft (ANA) and evaluated its therapeutic potential for nerve regeneration and functional recovery. Exosomes were isolated from BMSCs, and Exos-Cdc42 were generated by transfecting these cells with Cdc42 overexpression vectors. In vitro, Exos-Cdc42 significantly enhanced Schwann cell proliferat

A biomimetic supramolecular hydrogel enabling sequential dual-agent release reprograms the inflammatory microenvironment for sciatic nerve regeneration.

Peripheral nerve injury (PNI) repair is fundamentally hindered by a dysregulated inflammatory microenvironment and insufficient bioactive components. Conventional hydrogels fail to concurrently integrate dynamic responsiveness, neural biomimicry, and inflammation regulation. Herein, we engineered a biomimetic supramolecular hydrogel capable of reprogramming the immune microenvironment. This hydrogel was constructed through the reversible self-assembly of carboxymethyl chitosan (CM-CTS) and oxidized chondroitin sulfate (OCS), enabling the sequential release of a vascular endothelial growth fact

Effect of Ultrasound-Guided Sciatic and Saphenous Nerve Blocks on Analgesia During General Anaesthesia for Ankle Fracture Surgery: A Randomised Controlled Trial.

Background and Objectives: Ultrasound-guided sciatic and saphenous nerve blocks are widely used as analgesic alternatives for multimodal analgesia. However, how to objectively quantify the opioid- and anaesthetic-sparing effects of an additional peripheral nerve block in patients undergoing ankle fracture surgery under general anaesthesia remains unclear. This study aimed to evaluate their efficacy in managing postoperative pain and opioid-sparing effects following ankle fracture surgery. Materials and Methods: Patients scheduled for ankle fracture surgery were randomly assigned to the nerve b

Evolution of Achilles Tendon Repair: From Open Surgery to Percutaneous Techniques.

This article examines the evolution of Achilles tendon repair, focusing on minimally invasive and percutaneous techniques that reduce complications and improve recovery. These approaches emphasize preserving anatomic structures like the paratenon and sural nerve while optimizing biomechanical repair strength. Modern suture configurations enhance stability, enabling early mobilization and better functional outcomes. Clinical evidence shows minimally invasive methods offer comparable or superior results to open surgery, with fewer wound complications and faster return to activity, though sural n

Autonomic dysfunction and hemodynamic management after acute spinal cord injury: blood pressure targets, perfusion strategies, and emerging therapies.

OBJECTIVE: Acute spinal cord injury (SCI) produces profound cardiovascular instability that exacerbates secondary damage, emphasizing the need for timely blood pressure management and hemodynamic support. While stabilizing hemodynamics is central to acute SCI management, evidence guiding optimal mean arterial pressure (MAP) targets, vasopressor selection, and management strategies remains limited. We conducted a narrative, comprehensive review of peer-reviewed clinical and preclinical studies addressing hemodynamic management after SCI, defined here as the first 7 days after injury, including

Lumbar enlargement spinal cord stimulation for severe spasticity and motor function improvement after traumatic brain injury: a case report.

This case report describes the successful use of epidural spinal cord stimulation (SCS) in managing severe, refractory spasticity in a 58-year-old male following traumatic brain injury. Despite nearly 8 months of conventional pharmacotherapy and rehabilitation for his tetraplegia, his lower-limb spasticity persisted at Modified Ashworth Scale (MAS) grade 3, severely impeding functional recovery. After implantation of a trial and subsequently permanent SCS system at the lumbar enlargement, muscle tone decreased to MAS grade 2 within 48 h, alongside improvements in muscle strength. Over 6 months

Protocol of a multi-centre randomized controlled trial to compare pericapsular nerve group block, fascia-iliaca compartment block and femoral nerve block for pain management in patients with a hip fracture in the emergency department (CPFF-ED).

BACKGROUND: Patients presenting in the emergency department (ED) with a hip fracture are often in significant pain. Opioids are frequently prescribed as part of standardized pain protocols, but are associated with many adverse events, especially in the elderly. Regional anaesthesia with a femoral nerve block (FNB), Fascia Iliaca compartment block (FICB) or Pericapsular nerve block (PENG) may provide a reasonable alternative to these patients. We designed a prospective multicentre study to compare the efficacy and safety of these blocks in the ED setting, which has been registered in the Centra

Sublytic pyroptosis blocks Schwann cell remyelination through caspase-1-mediated Tyro3 cleavage.

BACKGROUND: Inflammasomes canonically trigger pyroptotic cell death through GSDMD-mediated membrane rupture. This study investigates a distinct sublytic pyroptosis in Schwann cells following peripheral nerve injury, where inflammasome activation causes functional impairment without extensive cell death. METHODS: Using a mouse sciatic nerve transection-repair model combined with in vitro and in vivo analyses, we examined NLRP3 inflammasome activation and cell fate. Primary Schwann cell cultures and RSC96 cells were used for biochemical assays, while ultrastructural analyses were performed on sc
