Characterizing spinal reflexes evoked by sensory spinal cord stimulation in people with lower-limb amputation.

BACKGROUND: People with lower-limb amputation lack sensory inputs from their missing limb, which increases their risk of falling. We recently demonstrated that spinal cord stimulation (SCS) can restore sensation in the missing lower limb. Previous studies have shown that SCS can affect motor control by exciting spinal reflex pathways after stroke or spinal cord injury. The effects of SCS on spinal reflex activation have not been studied in people with lower-limb amputation. Furthermore, it is unknown if SCS-evoked spinal reflex activation would perturb walking. Therefore, the goal of this stud

Combining cortical and spinal stimulation maximizes the improvement of gait after spinal cord injury.

Most spinal cord injuries (SCI) spare descending and sublesional circuits that can be targeted through motor cortex and spinal cord stimulation to mitigate locomotor deficits. However, the potential synergy between cortical and spinal stimulation remains unknown. Here, we first investigated the phase-locked electrical stimulation of the motor cortex and lumbar spinal cord at 40 Hz in a rat model of unilateral SCI. Combining cortical and lumbar stimulation around the anticipated lift synergistically enhanced leg movements. When integrated into rehabilitation training, cortical stimulation prove

Optimization of Spinal Cord Perfusion Pressure for Improvement of Neurological Outcome in Traumatic Spinal Cord Injury: A Systematic Review and Meta-Analysis.

The optimal hemodynamic management of traumatic spinal cord injury (tSCI) is not well established. We performed a systematic review and meta-analysis of patients with acute tSCI to assess the role of lumbar cerebrospinal fluid drainage (CSFD) and to identify factors predictive of neurological improvement. Three studies involving 46 patients with acute tSCI, lumbar drain placement with CSFD, and available pre- and post-intervention American Spinal Injury Association (ASIA) Impairment Scale or International Standards for Neurological Classification of Spinal Cord Injury (ISNCSCI) motor scores we

Clinical Studies Examining Intrathecal Drainage for Acute Traumatic Spinal Cord Injury: A Living Systematic Review.

Traumatic spinal cord injury (TSCI) can cause significant and permanent disability. For over 20 years, lumbar cerebrospinal fluid (CSF) drainage has been routinely performed during the surgical repair of thoracoabdominal aortic aneurysms and descending thoracic aortic aneurysms. However, intrathecal CSF drainage has not been adequately evaluated in the setting of acute TSCI. This living systematic review sought to evaluate whether intrathecal catheter CSF drainage to reduce intrathecal pressure (ITP) in the acute postinjury phase was safe and feasible and could improve clinical indices and neu

Neurologic Deterioration in Acute Traumatic Central Cord Syndrome Without Bone Injury Caused by Traumatic Cerebrospinal Fluid Rhinorrhea: A Case Report.

Whether to perform surgery for acute traumatic central cord syndrome (ATCCS) without bone fracture or dislocation but with spinal cord compression due to preexisting cervical canal stenosis remains controversial. We present a case of ATCCS without bone injury in which neurologic deterioration was possibly caused by traumatic cerebrospinal fluid rhinorrhea (TCFR). An 82-year-old man with ATCCS (American Spinal Injury Association (ASIA) Impairment Scale (AIS) C) and an anterior skull base fracture was urgently transported to our hospital and was initially treated conservatively. However, on the

Association between inpatient kyphoplasty and vertebroplasty and improved short-term outcomes following acute thoracolumbar compression fractures: a nationwide study.

OBJECTIVE: Vertebral compression fractures (VCFs) are associated with significant pain and disability. The current standard of care is expectant medical management; however, there is emerging data encouraging the use of early kyphoplasty or vertebroplasty. The goal of this nationwide study was to investigate the outcomes of patients with acute thoracolumbar VCF who undergo very early (inpatient) kyphoplasty compared with those managed medically. METHODS: This was a retrospective cohort analysis of the Nationwide Readmissions Database from 2016 to 2022. Adult patients admitted nonelectively fo

Safety of spinal anaesthesia in the East African population: assessment of spinal cord termination level at and above L2 vertebra in adults.

BACKGROUND: While spinal anaesthesia is a widely used technique, failure to accurately identify the L3-L4 intervertebral space (IVS) during the procedure may expose a patient to a needlestick injury to the spinal cord. OBJECTIVES: This study aimed to determine the proportion of Conus Medullaris Terminations (CMT) at or above the L2 vertebra in a black East African population and identify any age or gender-based variations. METHODOLOGY: A prospective observational study of lumbar spine MRI was conducted on 331 patients who had no vertebral deformities or prior spine surgeries. Two radiologist

Pediatric spine injuries- a review on current concepts of evaluation & management.

Pediatric spine injuries are distinct from adult spinal injuries due to the unique anatomical and physiological features of the developing spine. These differences, such as increased ligamentous laxity, the elasticity of interspinous ligaments, and the wedge-shaped vertebral bodies, make children more susceptible to certain types of spinal injuries, particularly in the cervical spine. Pediatric spine injuries, including cervical and thoracolumbar (TL) spine trauma, result in significant morbidity and mortality, with motor vehicle accidents being the leading cause of injury. This narrative revi

Traumatic spinal injuries: A retrospective epidemiological analysis following the 2018 driving policy reform in Saudi Arabia.

BACKGROUND: The epidemiology of traumatic spinal injuries (TSI) differs across various regions and is influenced by national income, infrastructure, and culture. TSIs are a source of high morbidity and mortality, requiring considerable resource allocation. Saudi Arabia's 2018 decision allowing women to drive introduced a new demographic element, potentially shifting TSI epidemiology. In this paper, we aimed to study patterns and outcomes of TSIs, including sex-based analyses, and to identify demographic, injury-related, and in-hospital factors associated with length of hospital stay, neurologi

A Novel Retrievable Drug Infusion Stent and Blood-Brain Barrier Portal for Antibody Therapies to the Spinal Cord.

BACKGROUND AND OBJECTIVES: The blood-brain barrier (BBB) is prohibitive to intravascular drug delivery. We hypothesized that isolation of spinal cord perfusion would focus on a higher effective dose while minimizing systemic toxicity and that a histamine portal would increase BBB permeability and facilitate large molecule therapies to the spinal cord. METHODS: A dumbbell-shaped, retrievable drug infusion stentgraft (RDIS) was constructed from nitinol and polytetrafluorethylene. A porcine model under anesthesia underwent percutaneous femoral insertion of the sheathed RDIS and deployment in the

Risk factors for recurrent lumbar disc herniation after unilateral biportal endoscopy: a retrospective study.

PURPOSE: Unilateral biportal endoscopy (UBE) is a predominantly minimally invasive surgical technique for addressing LDH. Nonetheless, recurrent lumbar disc herniation (rLDH) remains the predominant reason for reoperation following UBE. This retrospective study examined the risk factors and reoperation rates for rLDH following UBE. This study aimed to examine the risk factors associated with rLDH and the reoperation rate after single-level UBE. METHODS: We retrospectively analyzed 205 patients who underwent UBE for single-level LDH from 2019 to 2023 to determine reoperation causes and related

Graphene oxide scaffolds promote functional improvements mediated by scaffold-invading axons in thoracic transected rats.

Millions of patients and their caretakers live and deal with the devastating consequences of spinal cord injury (SCI) worldwide. Despite outstanding advances in the field to both understand and tackle these pathologies, a cure for SCI patients, with their peculiar characteristics, is still a mirage. One of the most promising therapeutic strategies to date for these patients involves the use of epidural electrical stimulation. In this context, electrically active materials such as graphene and its derivates become particularly interesting. Indeed, solid evidence of their capacity to closely int

Blockade of IgG Fc receptors reduces pain after intervertebral disc injury.

BACKGROUND: Intervertebral disc (IVD) injury is a common cause of low back pain and disability. Recent evidence suggests that autoantibodies contribute to such pain. We, therefore, explored the hypothesis that a novel IgG Fc receptor blocker could reverse chronic pain-related behaviors in a mouse model of IVD injury. METHODS: These studies used a multi-level lumbar IVD puncture model of low back pain in male C57BL/6 mice. Additional mouse strains evaluated included Fc gamma chain knockouts failing to express excitatory IgG Fc receptors and muMT mice incapable of producing mature B lymphocytes

Spinal Epidural Abscess: A Single-Center Retrospective Review of Incidence, Risk Factors, and Management at a Community Hospital.

INTRODUCTION: While relatively rare, spinal epidural abscess (SEA) is a neurosurgical pathology that can result in life-altering spinal cord injury if not properly managed or if diagnosis is delayed. The textbook presentation of SEA includes the triad of back pain, fever, and neurologic deficits; however, this classic triad is infrequently seen. Due to the nonspecific nature of each individual symptom, the condition is often misdiagnosed, resulting in delayed management and potentially significant consequences for the patient. This study aims to characterize the incidence, risk factors, and ma

Accuracy and Safety of Pedicle Screw Insertion Using Novel Intraoperative Computed Tomography Navigation System for Spinal Fusion Surgery.

INTRODUCTION: Accurate pedicle screw placement is critical in spinal fusion surgery to prevent complications such as neurological and vascular injuries. While conventional intraoperative computed tomography (iCT) navigation systems enhance placement accuracy and reduce radiation exposure compared to fluoroscopic guidance, they can encounter line-of-sight issues that disrupt surgical workflows. The NextAR iCT navigation system aims to overcome these challenges by integrating an infrared camera directly onto surgical instruments, streamlining navigation and improving procedural efficiency. Comprehensive guide to the diagnosis, management, and treatment of metastatic spinal cord compression syndrome.

Spinal metastases are the most common tumors found in the neuraxis. They correspond to 90 % of all tumors at this level. The thoracic region is the most frequently affected area, followed by the lumbar region, with the cervical region being the least frequently affected place. Back pain is the most common symptom, appearing in up to 95 % of patients who present with spinal cord compression syndrome. The evaluation of patients with spinal cord compression syndrome is complex and requires an entire multidisciplinary team for diagnosis, management, and treatment. In the medical part, treatment is
