The management of thoracic and thoraco-lumbar injuries of the spine with neurological involvement.

The results of treatment of injuries of the thoracic and thoraco-lumbar spine with neurological involvement have been reviewed in a retrospective study of 115 patients, of whom eighty-nine received conservative and twenty-six surgical treatment. Operation was reserved, in general, for patients with irreducible dislocations and incomplete neurological lesions, open reduction and internal fixation being the commonest procedure. Only three patients required a delayed spinal fusion for suspected instability after a period of conservative treatment. On the other hand, ten patients, eight of whom ha

Epidural venography: diagnosis of herniated lumbar intervertebral disc and other disease of the epidural space.

Epidural venography is a simple and safe procedure for the diagnosis of herniated intervertebral disc and other disease of the epidural space. It is especially useful in symptomatic patients with normal or equivocal myelographic findings. The accuracy rate of positive venograms is 88.2% in this series of 58 patients. Five illustrative cases are presented.

Myelographic localization of spinal cord compression in dogs. A comparison between cisternal and lumbar injections of metrizamide "Amipaque" in diagnosing and locating spinal cord compression.

Metrizamide is a new water-soluble contrast medium possessing unique properties, one of which is that even in an isotonic solution it has a radiographic density sufficient for a diagnostic myelogram. Whereas in the past myelography with water-soluble preparations invariably entailed lumbar injections, which are technically difficult in dogs, the fact that isotonic metrizamide causes so little tissue irritation has made it possible to inject the contrast solution into the cisterna magna. The author has compared metrizamide myelograms obtained with both cisternal and lumbar injections. Myelograp

Spinal cord compression caused by osteocartilaginous exostoses of the spine in two dogs.

Two dogs, one 4 months old and the other 1 year old, had histories of progressive posterior weakness. In both cases, there was severe asymmetric proprioceptive loss but appreciable preservation of strength. Benign, cystic expansile lesions involving the facets and lamina of the thoracic spine were observed radiographically; a single lesioons was demonstrated in the other. Examination of biopsy speciments taken at the time of laminectomy revealed the lesions to be osteocartilaginous exostoses (osteochondromas, ecchondromas, exostoses). The progress of dogs 1 and 2 has been monitored for 1 1/2 y

[Evolution of the astroglial reaction in the pyramidal pathway after injury of the rat motor cortex].

Investigation of astrocytes in the pyramidal tract of the rat 2 to 100 days after a lesion of the motor cortex indicates that the glial reaction, with increase in size of astrocytes and in affinity for Cajal's gold-sublimate, begins at the lesion and extends to the distal segments of the spinal cord. But this reaction recedes and disappears in lumbar and dorsal cord about 70 days after the operation. It persists in the cervical cord and in the brain. On the other hand, a characteristic increase in activity of glutamate andglucose-6-phosphate dehydrogenases is restricted to the vicinity of the

Hemivertebra in the dog: clinical and pathological observations.

The clinical and radiographic findings associated with the presence of hemivertebra ("wedge-shaped"vertebra) in small and brachycephalic breeds of dogs are reported together with the results of post mortem examination in the availabel cases. The condition is characterised clinically by progressive hind-leg weakness, spinal pain, abnormalities of the nervous system and evidence of muscle atrophy or other abnormalities of conformation. Confirmation of the clinical diagnosis is by radiography. It is suggested that the condition is congenital in origin. Breed incidences are reported. The occurrenc

The lumbar disc and its imitators.

A review of the differential diagnosis of the lumbar disc syndrome, based on patients seen and treated during the past 10 years, provides discussion of the unusual cases. A survey of 285 patients treated in a consecutive series from 1964 to 1972 was done, which showed that 253 patients had prolapsed lumbar discs, 20 were treated for spondylolisthesis and 62 (23%) required consideration because of unusual features in association with the disc lesion or because of other pathology. The need for lateral and complete exposure of the nerve root in some instances is reaffirmed.

Chronic lumbar extradural haematoma simulating disc syndrome.

Two patients presenting with a clinical picture of nerve root compression suggestive of the herniated disc syndrome are reported. In both the diagnosis proved to be chronic encapsulated extradural haematoma. Recognition of this entity at the time of surgical exposure should render the surgical dissection easier. The postoperative results in both were excellent. One of these patients was on chronic anticoagulant therapy and the unusual location and good prognosis of extradural haematoma in the lumbar spinal canal in such patients is emphasized. In the other, the haematoma developed after a mino

INFLUENCE OF HOST FACTORS ON NEUROINVASIVENESS OF VESICULAR STOMATITIS VIRUS : III. EFFECT OF AGE AND PATHWAY OF INFECTION ON THE CHARACTER AND LOCALIZATION OF LESIONS IN THE CENTRAL NERVOUS SYSTEM.

It will be well to restate the main problem at this point and to examine how far the accumulated data can help to elucidate it. The problem is this: Why are old mice generally resistant to all forms of peripheral inoculation of vesicular stomatitis virus when intracerebral injection is equally fatal for mice of all ages? The results of experiments in which the presence of virus was demonstrated by animal passage suggested that the reason can perhaps be found in (a) the different mechanisms of virus progression after intracerebral and peripheral injection, and (b) the development with age of lo
