Intradural dorsal arteriovenous fistula: a case report and literature review.

BACKGROUND: Intradural dorsal arteriovenous fistula (AVF) is a rare spinal vascular malformation that primarily affects the blood-supplying arteries of the intradural dorsal. Its clinical manifestations are dominated by bilateral lower limb weakness and a lack of specificity in early symptoms, which can easily lead to underdiagnosis or misdiagnosis. Thus, the successful diagnosis of intradural dorsal AVF is difficult. CASE DESCRIPTION: A 56-year-old woman presented with lower limb weakness for 3 months without significant sensory deficits. The patient has a history of hyperthyroidism, but the

Thoracic Discogenic Syndrome

Symptomatic thoracic discogenic syndrome poses a diagnostic challenge due to its rarity. The relative immobility of the thoracic spine due to its particular orientation, structure, and function in the vertebral column contributes to the rarity of thoracic discogenic syndrome. At birth, the thoracic spine and sacrum exhibit a kyphotic curvature, whereas the cervical and lumbar spine develop full lordosis around puberty. The lordotic nature of the cervical and lumbar spine enables them to bear most of the axial skeleton's weight compared to the thoracic and sacral spine. As a result, the latter

Minimally Invasive Resection of a Foraminal Lumbar Root Schwannoma.

BACKGROUND AND IMPORTANCE: This video article reports a rare case of a right L5/S1 foraminal root schwannoma that presents the natural course of the disease, imaging findings, treatment protocol, operative procedure, and highlights some of the possible surgical complications. CLINICAL PRESENTATION: Magnetic resonance imaging (MRI) revealed an intradural, extramedullary, well-enhanced mass at the right L5/S1 level. The operative procedure involved a right minimal L5/S1 laminotomy/foraminotomy posteriorly to open the right L5 root. The facet joints were preserved to prevent spinal instability.

An Incidental Diagnosis of Conjoined Nerve Root After a Work-Related Back Injury: A Case Report.

Conjoined nerve roots (CNRs) are an uncommon condition often overlooked until surgery, posing significant intraoperative risks. This case report discusses a 21-year-old male diagnosed incidentally with a left lumbosacral CNR involving the fifth lumbar (L5) and first sacral (S1) spinal nerve roots following a work-related back injury, emphasizing the importance of preoperative imaging. Accurate early diagnosis of CNRs can prevent surgical complications and guide appropriate management, highlighting the need for careful preoperative planning and patient education.

Trans-Sacral Epiduroscopic Ho:YAG Laser Ablation of the Ligamentum Flavum in a Live Pig.

INTRODUCTION: For the aging population, surgery for lumbar spinal canal stenosis (LSCS) requires minimally invasive procedures. Recently, trans-sacral epiduroscopic laser decompression for lumbar disc herniation has been reported with good results. In this study, we devised a new method to perform trans-sacral epiduroscopic laser ablation of the ligamentum flavum (LF), known to be the major cause of LSCS. Using a live pig, this study aims to evaluate the efficacy, safety, and drawbacks of this procedure. METHODS: Using an epiduroscope, we observed intra-spinal canal structures and then examin

Determination of a neurologic safe zone for bicortical S1 pedicle screw placement.

BACKGROUND CONTEXT: Lumbosacral fixation is commonly used for the management of lumbosacral instability. As the sacrum mainly consists of cancellous bone, bicortical fixation, in which the pedicle screw penetrates the anterior sacral cortex, can help increase the strength of fixation. However, this method carries a risk to the L5 nerves which lie anterior to the sacrum at this level. PURPOSE: The goal of this study is to determine a safe zone for the placement of S1 pedicle screws to decrease the likelihood of L5 nerve injury. STUDY DESIGN: Retrospective imaging review. PATIENT SAMPLE: This

Restoring bladder function using motor and sensory nerve transfers: a cadaveric feasibility study.

OBJECTIVE: Bladder dysfunction after nerve injury has a variable presentation, and extent of injury determines whether the bladder is spastic or atonic. The authors have proposed a series of 3 nerve transfers for functional innervation of the detrusor muscle and external urethral sphincter, along with sensory innervation to the genital dermatome. These transfers are applicable to only cases with low spinal segment injuries (sacral nerve root function is lost) and largely preserved lumbar function. Transfer of the posterior branch of the obturator nerve to the vesical branch of the pelvic nerve

Misplaced S1 screw causing L5 radiculopathy, rare and unusual presentation: a report of 2 cases.

Many spine surgeons are not optimally acquainted with anatomy anterior to sacrum. Screw malposition injuring these structures can lead to unwanted lethal consequences. We report unusual cases of acute radiculopathy due to misplaced bicortical sacral screw causing L5 nerve root impingement on anterior sacrum. A 39/M patient complained of severe rest pain (VAS 9/10) post TLIF in region of L5 dermatome with sensory deficit along the right lateral leg and straight leg raise less than 30°. X-ray revealed S1 screw protruding beyond the second cortex with a straight trajectory. CT scan revealed a pro

Does the L5 spinal nerve move? Anatomical evaluation with implications for postoperative L5 nerve palsy.

PURPOSE: While palsy of the L5 nerve root due to stretch injury is a known complication in complex lumbosacral spine surgery, the underlying pathophysiology remains unclear. The goal of this cadaveric study was to quantify movement of the L5 nerve root during flexion/extension of the hip and lower lumbar spine. METHODS: Five fresh-frozen human cadavers were dissected on both sides to expose the lumbar vertebral bodies and the L5 nerve roots. Movement of the L5 nerve root was tested during flexion and extension of the hip and lower lumbar spine. Four steps were undertaken to characterize these

Single S1 Dorsal Root Ganglia Stimulation for Intractable Complex Regional Pain Syndrome Foot Pain After Lumbar Spine Surgery: A Case Series.

INTRODUCTION: Intractable complex regional pain syndrome (CRPS)-related chronic foot pain, is a common therapeutic challenge for interventional pain management physicians and patients alike. Dorsal root ganglia (DRG) stimulation is a very target specific dorsal column stimulation technique with very promising clinical outcomes. Patients with CRPS foot pain and previous back surgery can benefit from DRG stimulation but also run a significant risk of epidural trauma from the DRG sheath advancement. Most sensory innervation to the foot is from L5 and S1 dermatomes. Although there is dual modulati

Factors that delay treatment of symptomatic metastatic extradural spinal cord compression.

BACKGROUND: Treatment delays of metastatic extradural spinal cord compression (MESCC) sometimes have been reported, but reasons for its delay have remained unclear. The purpose of this study was to assess which clinical factors are associated with treatment delays in neurologically symptomatic MESCC in the hospital settings. METHODS: We reviewed medical records of MESCC patients in our institute who had at least one progressive neurological symptom (weakness, sensory changes, urinary retention, or nerve root pain), were diagnosed by magnetic resonance imaging (MRI), and were treated with at l

L5 spinal nerve injury caused by misplacement of outwardly-inserted S1 pedicle screws.

PURPOSE: To evaluate L5 nerve root injuries caused by outwardly misplaced S1 pedicle screws. Pedicle screws remain the criterion standard for fixation of L5-S1 to correct lumbosacral instability. When inserting S1 pedicle screws, it is possible to injure the L5 nerve root if screws are inserted outwardly and the tip of the screw perforates the anterior cortex of the sacrum. Despite this risk, to our knowledge this type of injury has never been reported as a case series. METHODS: We experienced 2 cases of L5 nerve root injury caused by outwardly-inserted S1 pedicle screws. In both cases, bilat

Complications of axial lumbar interbody fusion.

OBJECT: Axial lumbar interbody fusion (AxiaLIF) is a novel minimally invasive approach for fusion of L4-5 and L5-S1. This technique uses the presacral space for percutaneous access to the anterior sacrum. The AxiaLIF procedure has the potential to decrease patient recovery time, length of hospital stay, and overall occurrence of surgical complications. It can be used alone or in combination with minimally invasive or traditional open fusion procedures. The purpose of this study was to evaluate complications of the AxiaLIF procedure at the authors' institutions. METHODS: Patients who underwent

Novel concepts in the evaluation and treatment of high-dysplastic spondylolisthesis.

The classification system of spondylolisthesis proposed by Marchetti and Bartolozzi is the most practical regarding prognosis and treatment and includes the description of both low- and high-dysplastic developmental spondylolisthesis (HDDS). Unfortunately, it does not provide strict criteria on how to differentiate between these two subtypes. The accepted treatment for HDDS is surgical. However, there is no consensus on how to surgically stabilize this subtype of spondylolisthesis, and although the concept of reducing spinal deformity before fusion is attractive, the issue of surgical reductio
