With the deepening of global population aging, the incidence of hip fractures in the elderly has been increasing year by year. Elderly patients with hip fractures often experience severe resting and movement-related pain, and effective perioperative pain management is critical to shortening hospital length of stay, reducing medical costs, and lowering the risk of postoperative complications and all-cause mortality. However, elderly patients are typically frail with multiple comorbidities, which is associated with a significantly higher incidence of adverse events related to non-steroidal anti-

Perioperative peripheral nerve injury (PPNI) is rare but potentially disabling. In patients with diabetic peripheral neuropathy (DPN), the threshold for nerve injury is lowered due to microvascular compromise and impaired myelin repair. We report a 48-year-old male with type 2 diabetes and chronic foot ulcers who developed immediate left foot drop following low spinal anesthesia for debridement. Electrophysiology revealed conduction block and focal demyelination of the peroneal nerve at the fibular head; high-frequency ultrasound showed no structural abnormality, while lumbar magnetic resonanc

Sciatic nerve injury results in motor dysfunction and muscle atrophy, with limited effective therapies. Umbilical cord-derived mesenchymal stem cells (UC-MSCs) may promote neuromuscular recovery, but their effects on functional and muscle recovery remain unclear. This study aimed to evaluate the effects of UC-MSC therapy on functional and muscle recovery in an animal model of sciatic nerve injury. An animal experimental study with a post-test-only control group was conducted using adult male Wistar rats. Rats were randomly allocated into three groups: sham operation, saline control with sciati

Perineural adhesions resulting from surgery, trauma, or chronic inflammation can impair nerve mobility, promote fibrosis, and contribute to neuropathic pain and autonomic dysfunction. Effective minimally invasive therapeutic strategies for established neural adhesions remain limited. This study aimed to establish reproducible rat models of nerve adhesion and neuropathic pain and to evaluate the therapeutic efficacy of acupotomy in alleviating adhesion-related pathological and inflammatory changes. Sciatic nerve chronic constriction injury (S-CCI) and vagus nerve adhesion (V-CCI) models were es

Peripheral nerve injuries severely impair motor function. Although neurorrhaphy is the gold-standard treatment, recovery is often incomplete. The heterologous fibrin biopolymer (HFB), a nonhuman-derived and biocompatible sealant, has shown regenerative potential in neural tissues, but its effects on the spinal cord microenvironment after peripheral nerve injury remain unclear. This study evaluated the chronic regenerative effects of HFB combined with neurorrhaphy after sciatic neurotmesis in rats. Forty adult male Wistar rats were divided into four groups: control, denervated, neurorrhaphy, an

BACKGROUND AND AIMS: Nonsystemic vasculitic neuropathy classically presents as a painful asymmetric axonal neuropathy, multiple mononeuropathies or radiculoplexus neuropathy. Demyelinating electrophysiologic features are rare and may lead to misdiagnosis. We describe a patient with biopsy-proven vasculitic neuropathy presenting as a severe symmetric demyelinating polyradiculoneuropathy.
METHODS: Clinical, electrophysiologic, imaging, laboratory, and histopathologic findings are presented.
RESULTS: A 57-year-old man developed acute ascending weakness, sensory loss, facial weakness, and arefle

BACKGROUND: Groin flap surgery carries a risk of iatrogenic nerve injury, most commonly involving the lateral femoral cutaneous nerve and presenting as meralgia paresthetica. In contrast, femoral nerve injury is rare and typically associated with motor deficits. Pure sensory femoral neuropathy is exceptionally uncommon and may closely mimic meralgia paresthetica, creating a diagnostic challenge.
CASE REPORT: We report a 24-year-old man who developed severe neuropathic pain over the anterolateral thigh following groin flap donor-site closure. Initial clinical suspicion was meralgia paresthetic

Sciatic nerve injuries result in profound motor and sensory deficits, and when direct repair is not feasible, autologous sural nerve grafting remains the gold standard for reconstruction. In this context, distal sciatic neuroma excision followed by sural nerve autografting illustrates the surgical nuances and technical challenges of bridging large distal nerve gaps. The procedure involves careful neuroma excision and reconstruction of the tibial and peroneal divisions using multiple reversed sural nerve cable grafts, with tension-free epineural coaptation performed under magnification to optim

Peripheral nerve stimulation offers a promising alternative to pharmacological treatments for chronic pain, and recent advances in direct current stimulation enable selective inhibition of nociceptive activity. However, the safety of direct current delivery systems, particularly soft, metal-free nerve cuffs, remains poorly characterized under pathological conditions. This study evaluated the safety and biocompatibility of a silicone tripolar cuff for ionic direct current delivery in naïve rats and rodent models of neuropathic (spared nerve injury) and inflammatory (complete Freund's adjuvant)

INTRODUCTION: The early and accurate assessment of peripheral nerve injury in patients with Guillain–Barré syndrome (GBS) is crucial for clinical diagnosis and treatment. Recently, there has been growing interest in using MR neurography especially diffusion-weighted imaging with background suppression (DWIBS) to evaluate peripheral nerves and associated lesions. However, this technique has not yet been applied to GBS patients. The aim of this study was to investigate the potential value of MR neurography in assessing the tibial nerve (TN) and common peroneal nerve (CPN) in individuals with GBS

Understanding the multiple mechanisms underlying peripheral nerve regeneration in humans is crucial for developing effective therapies for peripheral axonal neuropathies. Although in vitro and in vivo studies have provided significant insights, human studies exploring the composition of proteome remain limited. The ECM plays a crucial role in nerve regeneration, influencing cell adhesion, proliferation, migration, and finally differentiation. In this study, we used a high-sensitivity, label-free liquid chromatography-mass spectrometry (LC-MS) method to investigate the global proteomic profile

Neurotmesis, the most severe form of peripheral nerve injury, involves complete transection and loss of motor and sensory function. Surgical repair, typically via end-to-end neurorrhaphy (NRR), is often required. A key pathological feature is Wallerian degeneration (WD) in the distal stump, which, along with slow axonal regeneration, leads to muscle atrophy and poor functional recovery. Polyethylene glycol (PEG)-mediated axonal fusion has emerged as a promising strategy to bypass WD by rapidly reconnecting severed axons and restoring conduction. Methylene blue (MB) has previously enhanced PEG-

PURPOSE: Polyethylene glycol (PEG) fusion has been reported to restore axonal continuity after nerve repair if initiated before Wallerian degeneration (WD). Sterile α and Toll/Interleukin-1 Receptor motif-containing 1 (SARM1) inhibition can potentially delay WD. We hypothesized that PEG fusion in SARM1 knockout (KO) rats would allow for improved strength and nerve regeneration following nerve transection.
METHODS: A total of 32 wild-type (WT) and 32 SARM1-KO rats underwent sciatic nerve transection, followed by standard neurorrhaphy or PEG fusion. Baseline compound nerve action potentials (CN

Segmental-loss peripheral nerve injuries (SL-PNIs) often produce severe deficits in sensory/motor functions and voluntary behaviors. The current gold standard for repair is neurorrhaphy of a cable autograft that (1) produces donor site morbidity; (2) results in rapid Wallerian degeneration (WD) of severed distal nerve segments; (3) relies on slow (1-2 mm/day) axonal regeneration; (4) does not prevent atrophy of denervated muscles and sensory structures; and (5) results in poor to non-existent recovery of sensory/motor functions and voluntary behaviors, especially with longer segmental-loss gap

INTRODUCTION: Radiation-induced lumbosacral plexopathy (RILSP) is a rare and debilitating complication of pelvic radiotherapy, classically presenting months to decades after treatment. Very early onset within weeks is exceedingly rare and may be misattributed to tumor recurrence or other etiologies. We report a case of rare early-onset RILSP that showed a remarkable response to corticosteroid therapy, highlighting crucial diagnostic and therapeutic considerations. CASE PRESENTATION: A 46-year-old cervical cancer patient, who developed symptoms nine weeks after radiotherapy (a total dose of 45

BACKGROUND: Congenital anomalies of kidney and urinary tract (CAKUT) are the leading causes of chronic kidney disease (CKD) in childhood. Determining the clinical course, outcome, and prognostic factors of this heterogeneous disease group is important to provide appropriate management and follow-up. Therefore, we aimed to identify the risk factors of CKD in CAKUT and the differences in clinical courses between disease subgroups.
METHODS: Three hundred patients (M/F: 203/97) divided into 16 CAKUT categories were enrolled in the study. Logistic regression and survival analyses were performed to

BACKGROUND: Peripheral nervous system injury (PNI) is a leading cause of long-term disability worldwide, with traumatic damage to major nerves such as the sciatic nerve, facial nerve, and brachial plexus severely impairing motor function, reducing life expectancy, and predisposing to mood disorders, thereby imposing a substantial socioeconomic burden. Effective therapies to limit PNI-induced neural damage, particularly in sciatic nerve injury (SNI), remain inadequate, largely due to an incomplete understanding of the underlying molecular mechanisms. OBJECTIVE: To evaluate the efficacy of the C

L-Periaxin is a PDZ-domain protein localized to the plasma membrane of myelinating Schwann cells and plays a key role in the stabilization of mature myelin in peripheral nerves. Mutations in L-periaxin have recently been described in some patients with demyelinating peripheral neuropathy, suggesting that disruption of L-periaxin function may result in nerve injury. In this study, we report the presence of autoantibodies to L-periaxin in sera from two of 12 patients with diabetes mellitus (type 2)-associated neuropathy and three of 17 patients with IgG monoclonal gammopathy of undetermined sign
