Short-term sacral neuromodulation sustained long-term efficacy in the management of postoperative urinary retention after pelvic surgery: a case report.

BACKGROUND: Sacral neuromodulation (SNM) serves as an effective therapeutic option for lower urinary tract dysfunction, and long-term intervention is generally required to sustain clinical benefits. Reports regarding durable therapeutic outcomes achieved with short-term SNM remain uncommon. This case indicates that short-term SNM can bring about sustained improvement of refractory non-obstructive voiding dysfunction after resection of large pelvic teratoma. CASE PRESENTATION: A 19-year-old female developed refractory urinary retention following excision of a huge mature cystic teratoma of the

Reflex and functional responses to distal tibial/medial plantar nerve electrical stimulation in the treadmill-locomoting spinal cat.

Low-intensity electrical stimulation of the distal tibial nerve innervating the plantar foot with pulse trains promotes extensor activity in reduced animal preparations when delivered during the extensor phase. Since enhancing extensor activity could potentially improve weight support after spinal cord injury (SCI), we evaluated whether electrical stimulation of the distal tibial nerve or its medial plantar branch enhances/prolongs extensor activity in a locomotor-trained feline model of SCI. With stimulation trains at levels activating large-diameter afferents (low-threshold cutaneous/muscle

Burden and health-system challenges of diabetes mellitus in urban slum populations of Iran: a scoping review.

Multiple myeloma (MM) is a clonal plasma cell disorder characterized by aberrant immunoglobulin deposition causing end-organ damage (Jones et al. in Hum Pathol 30:1441-1448, 1999). Rarely, paraproteins crystallize within histiocytes, resulting in crystal-storing histiocytosis (CSH) (Jones et al. in Hum Pathol 30:1441-1448, 1999). The coexistence of MM, CSH, and amyloidosis is exceedingly uncommon (Takahashi et al. in Pathol Int 37:141-154, 1987). A 56-year-old male presented with chronic low back pain and elevated creatinine (~ 2.05 mg/dL). Myeloma workup revealed elevated beta-2 microglobulin

The role of secretome from mesenchymal stromal cells in promoting nerve regeneration after neurotmesis.

BACKGROUND: Neurotmesis, remains a significant clinical challenge due to limited intrinsic regenerative capacity and suboptimal outcomes of current therapies. Mesenchymal stromal cells (MSCs) secretome has emerged as a promising cell-free alternative, providing neurotrophic and immunomodulatory factors to support nerve repair. This study aimed to evaluate the regenerative efficacy of primed adipose-derived MSC secretome in a rat model of sciatic nerve neurotmesis. METHODS: Human and rat adipose-derived MSCs were cultured and primed under hypoxic and inflammatory conditions. Secretomes were ch

BDNF-Enriched Wharton's Jelly-Derived Secretome Combined With 3D Biodegradable Chitosan-PCL Conduits Enhances Peripheral Nerve Regeneration in a Rat Model.

INTRODUCTION: Peripheral nerve injuries often lead to significant functional impairment. While autografts remain the gold standard for repairing critical-sized nerve defects, donor site morbidity and limited graft availability have prompted the exploration of alternative strategies. Although studies investigating nerve regeneration using nerve conduits and biological agents are present in the literature, research investigating the effect of neurotrophic factors enriched secretome with biocompatible 3D conduits combination is insufficient. The aim of this study is to evaluate the regenerative p

Ultrasound-guided HUC-MSCs transplantation alleviates neuropathic pain in CCI rats: a mechanistic study based on microglia/macrophage polarization and the NLRP3 inflammasome.

This study investigates the therapeutic effects of ultrasound-guided transplantation of human umbilical cord mesenchymal stem cells (HUC-MSCs) on nerve injury and neuronal recovery in chronic constriction injury (CCI) rats. Behavioral analysis showed that HUC-MSCs treatment significantly improved Paw Mechanical Withdrawal Threshold (PMWT) and Thermal Paw Withdrawal Latency (TWL), with faster recovery in the HUC-MSCs groups. Histological analysis revealed enhanced repair and regeneration, reduced inflammation, and improved results in the dual-treatment group (CCI + U2). Ultrasound and histologi

Eel-Inspired Self-Powered Hydrogel Nerve Conduit: A Fully Degradable Scaffold for Peripheral Nerve Repair.

Electrical stimulation effectively promotes nerve regeneration and functional recovery, but its clinical application faces challenges such as energy supply limitations, long-term stability issues, and implantation safety concerns. Inspired by the bioelectrogenic mechanism of electric eels, this study developed an electric-eel-inspired ionogel battery (EE-iHB) using chitosan (CS), chondroitin sulfate (CSA), and hydroxyethyl cellulose (HEC). The battery exhibits not only excellent biocompatibility but also outstanding ionic conductivity. By mimicking the intricate multilayer structure of electri

Extracorporeal Shock Wave Therapy Promotes Peripheral Nerve Regeneration Through ERK1/2-Mediated Autophagy.

Extracorporeal shock wave therapy (SWT) has shown potential in promoting peripheral nerve regeneration, yet its underlying mechanisms remain incompletely understood. In this study, we explored the role of extracellular signal-regulated kinase 1/2 (ERK1/2)-mediated autophagy in SWT-enhanced nerve repair using both in vivo and in vitro models. Application of SWT to transected sciatic nerves in rats led to increased ERK1/2 phosphorylation and upregulation of the autophagy marker LC3B, as confirmed by immunofluorescence and RT-qPCR. In vitro, SWT-treated Schwann cells exhibited enhanced autophagos

Isoform-specific roles of QKI-6 and QKI-7 direct Schwann cell lineage progression and enhance peripheral nerve regeneration.

Peripheral nerve injury presents a major therapeutic challenge owing to limited endogenous repair and incomplete functional recovery. Schwann cells (SCs), the principal glia of the peripheral nervous system, support axonal integrity and regeneration, but the post-transcriptional mechanisms regulating their development and reparative function remain poorly defined. Here, we investigated the isoform-specific roles of the RNA-binding protein Quaking (QKI), which is alternatively spliced into nuclear (QKI-5) and cytoplasmic (QKI-6 and QKI-7) variants, in governing human Schwann lineage progression

In Vivo Evaluation of Conductive Biopolymer-Based 3D Bioprinted Nerve Conduit in Sciatic Nerve Injury Repair.

Peripheral nerve injury (PNI) is one of the most common conditions that occurs due to trauma and accidents. A successful peripheral nerve regeneration can potentially benefit from the use of conduits with adequate physical, mechanical, and biochemical cues to recapitulate the native neural microenvironment. We report an electroconductive 3D bioprinted nerve conduit fabricated using alginate/methylcellulose/reduced graphene oxide (Alg/MC/rGO) hydrogel. The incorporation of rGO rendered the hydrogel with improved electrical conductivity while maintaining good printability and shape fidelity thro

The role of an ultrasound-responsive injectable piezoelectric hydrogel in promoting nerve regeneration and alleviating neuropathic pain.

BACKGROUND: Neuropathic pain (NP) resulting from peripheral nerve injury (PNI) represents a major clinical challenge. Although electrical stimulation (ES) has demonstrated therapeutic benefits, its clinical translation is limited by the invasiveness of electrode implantation, which often requires large surgical incisions and percutaneous wiring, increasing tissue damage and infection risk. METHODS: This study developed an ultrasound-responsive, injectable, and biodegradable piezoelectric hydrogel (sPLLA-Gel) composed of electrospun poly-L-lactic acid (PLLA) and methacryloyl gelatin (GelMA). An axon-intrinsic loop restricts nerve regeneration through axonal protein synthesis.

Injured axons synthesize the RNA Binding Protein KHSRP that promotes mRNA decay and slows nerve regeneration. Axotomy-induced increase in axoplasmic Ca2+ activates axonal Khsrp translation, and while axonal Ca2+ returns to pre-injury levels within 16 hours post-axotomy, axonal KHSRP remains elevated. Alternating translation of Reg3a and Khsrp sustains KHSRP levels in regenerating axons. Axonal Reg3a mRNA and protein increase proximal to the injury site days after sciatic nerve crush. REG3A stimulates ER Ca2+ release in axons to activate PERK, increase eIF2α phosphorylation, and increase Khsrp

Plasma fractionation in Latin America: a path towards sovereignty and sustainability in the supply of plasma-derived medicines.

PURPOSE: Increased reactive oxygen species (ROS) released by NADPH oxidase and inflammation are associated with arterial hypertension and eye diseases associated with high blood pressure, including glaucoma, retinopathies (e.g., age-related macular degeneration), and choroidopathies affecting ocular function; however, the mechanisms underlying these adverse outcomes remain undefined. The present study was designed to highlight the importance of oxidative stress in severe hypertension-related eye damage. METHODS: Male Wistar rats (n = 7, unless otherwise specified for specific experiments) wer

Likely radiation-induced sciatic neuropathy in a dog.

Radiation-induced peripheral neuropathy (RIPN) is a well-known late adverse effect of radiotherapy (RT) in humans, but to date, is not reported in companion animals. An 11-year-old spayed female beagle was presented for evaluation regarding a progressive left sciatic neuropathy. The dog had received stereotactic RT (20 Gy) following excision of a grade 2 soft tissue sarcoma over the proximo-caudal aspect of the left thigh approximately 19 months prior to the onset of signs. Computed tomography and magnetic resonance imaging of the left sciatic nerve was consistent with RIPN, and histopathology

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Efficacy of Nerve Tape-Assisted PEG Fusion in a Rabbit Tibial Nerve Repair Model.

BACKGROUND: Polyethylene glycol (PEG) hydrogel is capable of fusing transected axons and restoring axonal continuity. The technique requires precise nerve coaptation alignment and stability, which is difficult to achieve utilizing conventional microsuture neurorrhaphy. Nerve Tape (NT) is a microhook-based nerve coaptation device engineered to facilitate stable and accurate nerve end alignment. The primary objective was to modify and validate NT-assisted PEG fusion using a rabbit proximal tibial nerve repair model at an intermediate time point. METHODS: Three groups of rabbits underwent mid-th
