The effect of sciatic nerve block on sciatic nerve depth and pain level in patients with ankle fracture based on different approaches under ultrasound guidance.

To investigate the clinical efficacy of using different approach sciatic nerve blocks on the sciatic nerve depth and pain degree in patients with ankle fracture. A retrospective analysis was conducted on 151 patients with ankle fractures who were admitted to the hospital from May 2020 to May 2023. The patients were divided into a control group (n=76) using the greater trochanteric plane approach (GTA) and an observation group (n=75) using the suprapopliteal approach (PA). Perioperative related indexes, pain levels, muscle strength, hemodynamic indexes, and adverse reactions were compared betwe

Peripheral nerve regeneration using a bioresorbable silk fibroin-based artificial nerve conduit fabricated via a novel freeze-thaw process.

While silk fibroin (SF) obtained from silkworm cocoons is expected to become a next-generation natural polymer, a fabrication method for SF-based artificial nerve conduits (SFCs) has not yet been established. Here, we report a bioresorbable SFC, fabricated using a novel freeze-thaw process, which ensures biosafety by avoiding any harmful chemical additives. The SFC demonstrated favorable biocompatibility (high hydrophilicity and porosity with a water content of > 90%), structural stability (stiffness, toughness, and elasticity), and biodegradability, making it an ideal candidate for nerve rege

Electrically conductive and photocurable MXene-modulated hydrogel conduits for peripheral nerve regeneration: In vitro and in vivo studies.

Electroconductive biomaterials, as advanced nerve guidance conduits (NGCs), have shown great promise to accelerate the rate of peripheral nerve repair and regeneration (PNR) but remain among the greatest challenges in regenerative medicine because of frail recovery. Herein, we introduce injectable nanocomposite nerve conduits based on gelatin methacrylate (GelMa) and MXene nanosheets (MX) for PNR. Microstructural studies determine that the addition of MX increases the mean pore size of GelMa NH from 5.8 ± 1.2 μm to 8.4 ± 1.6 μm for the hydrogel containing 0.25 mg/mL MX, for example, leading to

Liquid Metal-Based Conductive Nerve Guidance Conduit Combined With Electrical Stimulation Boosts Peripheral Nerve Repair.

The combination of nerve guide conduits (NGCs) and electrical stimulation (ES) is an effective treatment for peripheral nerve injury (PNI). Flexible conductive materials with mechanical properties similar to those of biological tissues have been shown to have better long-term biointegration and functionality than rigid conductive materials. In this study, liquid metal (LM)-based conductive polycaprolactone/gelatin/polypyrrole/LM (PCL/Gel/PPy/LM, PGPL) NGC was combined with exogenous ES to repair PNI. PGPL membranes had good hydrophilicity, degradability, and mechanical properties, and its cond

SARM1 Inhibition Maintains Axonal Integrity After Rat Sciatic Nerve Transection and Repair.

PURPOSE: Sterile alpha and TlR motif containing-1 (SARM1) protein has been demonstrated to play a critical role in the initiation of Wallerian degeneration after nerve injury. The goal of this study was to assess whether blockade of SARM1 activity inhibits Wallerian degeneration following nerve transection, potentially promoting more rapid recovery of axonal function. METHODS: An adeno-associated virus plasmid encoded with a dominant-negative SARM1 protein fused with green fluorescent protein to impair SARM1 function, was injected into 24 juvenile rats to create a SARM1 dominant-negative (SAR

Harmine promotes axon regeneration through enhancing glucose metabolism.

Axon regeneration requires a substantial mitochondrial energy supply. However, injured mature neurons often fail to regenerate due to their inability to meet these elevated energy demands. Our findings indicate that harmine compensates for the energy deficit following axonal injury by enhancing the coupling between glucose metabolism and mitochondrial homeostasis, thereby promoting axon regeneration. Notably, harmine facilitates mitochondrial biogenesis and enhances mitophagy, ensuring efficient mitochondrial turnover, and energy supply. Thus, harmine plays a crucial role in enhancing glucose

Electrical stimulation of the sciatic nerve restores inspiratory diaphragm function in mice after spinal cord injury.

INTRODUCTION: Spinal cord injury in the high cervical cord can impair breathing due to disruption of pathways between brainstem respiratory centers and respiratory motor neurons in the spinal cord. Electrical stimulation of limb afferents can increase ventilation in healthy humans and animals, but it is not known if limb afferent stimulation can improve breathing following a cervical injury. METHODS: We stimulated the sciatic nerve while using electromyography to measure diaphragm function in anesthetized mice following a cervical (C2) hemisection spinal cord injury, as well as in uninjured c

Enhancing nerve repair using artificial nerve conduits: A bridging technique without relying on the weak material property-A biomechanical study.

INTRODUCTION: Peripheral nerve injuries with extensive defects are often challenging to treat. Although second-generation collagen nerve conduit is a promising alternative to autologous nerve grafts, there are concerns about the limited early postoperative range of motion due to the conduit's weak material properties. This biomechanical study demonstrates the potential weakness of conventional suturing techniques in a nerve defect model using a collagen nerve conduit. We also investigated the effectiveness of a novel suturing technique, the suspension bridge method (SBM), which directly connec

The cannabinoid CB2 receptor mediates the analgesic effects of Cannabis sativa extract in a rat model of neuropathic pain.

Neuropathic pain (NP) is a complex and debilitating condition that is often refractory to currently available analgesic medications. Cannabis sativa extract (CSE) has been reported to exhibit analgesic properties across various pain models; however, the underlying mechanisms of action are not fully understood. This study aimed to investigate the involvement of the cannabinoid CB2 receptor in mediating the analgesic effects of CSE in a rat model of NP, where NP was induced in male Wistar rats through chronic constriction injury (CCI) of the sciatic nerve. Rats were randomly allocated into four

Mesenchymal Stem Cells Expressing Baculovirus-Engineered Brain-Derived Neurotrophic Factor Improve Peripheral Nerve Regeneration in a Rat Model.

BACKGROUND: Peripheral nerve injuries are a major clinical challenge because of their complex nature and limited regenerative capacity. This study aimed to improve peripheral nerve regeneration using Wharton's jelly mesenchymal stem cells (WJ-MSCs) engineered to express brain-derived neurotrophic factor (BDNF) via a baculovirus (BV) vector. The cells were evaluated for efficacy when seeded into acellular nerve grafts (ANGs) in a rat sciatic nerve defect model. METHODS: WJ-MSCs were transfected with recombinant BV to upregulate BDNF expression. Conditioned medium (CM) from these cells was util

Effect of traction force during surgery on physical integrity and histological changes in peripheral nerves: experimental study on rabbits.

Sensory and motor nerve damage is a common complication of maxillofacial surgery and trauma. Procedures such as orthognathic surgery, tumor resection, and salivary gland interventions can damage peripheral nerves when the surrounding soft tissue or the nerve itself is manipulated. The purpose of this study was to evaluate the histological changes in the sciatic and median nerves of albino rabbits following traction-induced nerve injury. Nine albino rabbits were included in the study and divided equally into three groups, with three rabbits per group. In each rabbit, four peripheral nerves were

Hyaluronic Acid-Based Therapy for Alleviating Early Lipid Peroxidation in Peripheral Nerve Compression Injury Repair.

BACKGROUND: Peripheral nerve injuries compromise sensory and motor functions, severely affecting patients' quality of life. Early lipid peroxidation drives oxidative stress, disrupting the regenerative microenvironment. Hyaluronic acid (HA), an essential extracellular matrix component, shows promise in mitigating oxidative damage and fostering repair. METHODS: In a rat sciatic nerve crush model, HA hydrogel was applied to enhance retention at the injury site. Transcriptomic analysis at 24 hours postinjury identified key pathways. In vitro assays examined HA's protective effects on Schwann cel

Antihypertensive Drug Amlodipine Besylate Shows Potential in Alleviating Diabetic Peripheral Neuropathy.

UNLABELLED: Diabetic peripheral neuropathy (DPN) is a common diabetes complication with no currently available curative treatments. Here, we demonstrated that the protein level of G-protein-coupled receptor 40 (GPR40) is significantly repressed in the sciatic nerves (SNs) of DPN patients, as well as in the peripheral nerves, including dorsal root ganglia (DRG) and SNs, of streptozotocin-induced type 1 diabetic mice and BKS Cg-m+/+Lepr db/J (db/db) type 2 diabetic mice. We identified that amlodipine besylate (AB), a first-line clinical antihypertensive drug, is a GPR40 agonist capable of allevi

Brain-Derived Extracellular Matrix (B-ECM)-Based Aligned Electrospun Fibers for Sciatic Nerve Regeneration.

Nerve injuries pose numerous challenges and adversely affect the quality-of-life (QOL) of patients. Artificial nerve guidance conduits (aNGCs) are fabricated using natural and synthetic polymers alongside bioactive cues. The objective of this study is to simultaneously leverage biophysical and biochemical cues to develop aNGCs. The study fabricates core/shell aligned electrospun fibers using polycaprolactone (PCL) and brain-derived extracellular matrix/gelatin (B-ECM/Gel) as core and shell components, respectively, and then integrates them with PCL/Gel-based tubular scaffolds to fabricate aNGC

Selective distal block of the infrapatellar branch of the saphenous nerve: anatomical and ultrasonographic insights.

PURPOSE: The infrapatellar branch of the saphenous nerve (IPBSN) plays a critical role in cutaneous innervation of the anterior knee and is frequently implicated in both acute and chronic post-surgical pain. Due to its anatomical variability, precise localization is essential for effective nerve blocks and minimizing iatrogenic injury. This study evaluates the feasibility of two distal ultrasound-guided approaches for selective IPBSN blocks, with the aim of addressing the limitations of proximal techniques. METHODS: Anatomical layer-by-layer dissections of two paired lower limbs and transvers

Induced Pseudomembrane Enrichment in Long Nerve Allograft Reconstruction.

INTRODUCTION/AIMS: Long nerve defects are typically reconstructed with autograft or processed acellular nerve allograft (PNA). PNA is convenient and avoids donor morbidity but lacks the neurotrophic environment of autograft. Increased levels of neurotrophic factors have been identified in pseudomembranes induced around silicone implanted between nerve ends. This study aimed to determine if pseudomembrane can be reliably induced around silicone implanted between nerve ends, and if this enhances regeneration of PNA inset within using a staged technique. METHODS: Lewis rats (n = 24) underwent re

The effect of 807-nm light-emitting diode photobiomodulation on peripheral nerve injury.

Photobiomodulation (PBM) has demonstrated potential in promoting peripheral nerve regeneration. However, there is a limited and inconclusive study on the application of light-emitting diode (LED) for nerve injury repair. In this study, we designed an 807-nm LED device with high luminous uniformity to investigate the effects of LED-based PBM on peripheral nerve injury repair. RSC96 cells were utilized as the target cells and optimal light parameters were selected based on ATP concentration, CCK-8 assays, PCR, and immunofluorescent staining. One-way analysis of variance and Student's t-test was

Amniotic membrane hydrogel as novel injectable platform in combination with metformin for treatment of sciatic nerve injury.

Peripheral nerve tissue engineering is a field that uses cells, growth factors and biological scaffold material to provide a nutritional and physical support in the repair of nerve injuries. The specific properties of injectable human amniotic membrane-derived hydrogel including growth factors as well as anti-inflammatory and neuroprotective agents make it an ideal tool for nerve tissue repair, and metformin may also aid in nerve regeneration. The aim of this study was to investigate the effects of hydrogel derived from amniotic membrane (AM) along with metformin (MET) administration in the re
