Analgesic Mechanism of Emodin in Neuropathic Pain Through Inhibiting P2X4 Purinoceptor Signaling.

Neuropathic pain (NeP) is a most intractable health problem due to its unsatisfactory treatment effect. Emodin, a natural anthraquinone derivative extracted from Rheum palmatum and Polygonam cuspidatum, exhibits the analgesic effects in various NeP models. However, the underlying mechanisms remain elusive. This study employed whole transcriptome sequencing and metabolomics to elucidate emodin's analgesic mechanism in the spinal cord of chronic constriction injury (CCI) rats. Fifteen-day emodin treatment reversed hyperalgesia and deficit of sciatic nerve function induced by CCI and significantl

Chitosan conduits for peripheral nerve repair: a systematic review of animal studies.

Peripheral nerve injuries are major causes of disability worldwide. The current standard treatment, autologous nerve grafting, puts the donor region at risk and has limited availability. A systematic search of MEDLINE, EMBASE, and Web of Science databases for studies published between January 1, 2008, and September 04, 2024, was performed to compare interventions using chitosan tubes with non-intervention or autologous nerve grafts in rats with artificially injured sciatic nerves. Twenty-one experimental studies including 738 animals were selected. Nerve repair using chitosan conduits resulted

Induced Pluripotent Stem Cell-Derived Exosomes Promote Peripheral Nerve Regeneration in a Rat Sciatic Nerve Crush Injury Model: A Safety and Efficacy Study.

Peripheral nerve injury (PNI) remains a significant clinical challenge, often leading to long-term functional impairment. Despite advances in therapies, current repair strategies offer unsatisfactory clinical outcomes. Exosomes derived from induced pluripotent stem cells (iPSC-Exos) have emerged as a promising therapeutic approach in regenerative medicine. This study assesses the efficacy and safety of iPSC-Exos in a rat model of sciatic nerve crush injury. Briefly, iPSCs were generated from peripheral blood mononuclear cells (PBMCs) of healthy donors using Sendai virus vectors and validated f

ARA290, an alternative of erythropoietin, inhibits activation of NLRP3 inflammasome in schwann cells after sciatic nerve injury.

The challenge of repairing peripheral nerve injury is a critical issue that needs to be addressed urgently. Previous research has shown that erythropoietin (EPO) and its prolonged peptides exhibit beneficial effects in neurological disorders. In our study, we demonstrated that both EPO and pyroglutamic acid helix B surface peptide (pHBSP, also known as ARA290) inhibit the early inflammatory response and promote functional recovery after sciatic nerve crush injury in rat models. Our experimental results demonstrate that significant inflammatory response occurred in Schwann cells after sciatic n

Efficacy of Quality and Quantity media-cultured mononuclear cells for promoting peripheral nerve regeneration in mouse model.

This study aimed to assess the efficacy of Quality and Quantity media-cultured mononuclear cells (QQ-MNCs) for promoting nerve regeneration in a mouse sciatic nerve transection model. Human peripheral blood mononuclear cells (PB-MNCs) and QQ-MNCs derived from healthy volunteers were used/compared. The left sciatic nerve was surgically transected in 27 mice. After complete nerve transection was confirmed, end-to-end direct epineurial nerve repair was performed using 9-0 nylon. Fibrin glue was applied to the tissue around the injury site to limit diffusion of the study treatment followed by appl

In situ piezoelectricity induces M2 polarization of macrophages to regulate Schwann cells for alleviating neuropathic pain of CCI rats.

Peripheral nerve injuries often lead to neuropathic pain (NP), which is influenced by macrophage polarization, impacting Schwann cell function. Bioelectric signals, particularly from piezoelectric materials like polyvinylidene fluoride (PVDF), play a pivotal role in modulating macrophage polarization. In this study, we demonstrate that PVDF's piezoelectric properties enhance M2 polarization during hypoxia macrophage model, as indicated by increased CD206 (M2 marker) expression and decreased CD8 (M1 marker). This shift in polarization is linked to enhanced secretion of Arg-1 and IL-10 from M2 m

Smooth muscle extracellular matrix modified small intestinal submucosa conduits promote peripheral nerve repair.

Challenges still exist to develop an ideal cell-free nerve guidance conduit (NGC) providing a favorable microenvironment for rapid and successful nerve regeneration. Proteomic analysis revealed that extracellular matrix (ECM) derived from smooth muscle cells (SMCs) was abundant in nerve-related active proteins and significantly enriched signaling pathways involved in nerve regeneration. However, whether NGCs based on SMCs-derived ECM modification strategy promote nerve regeneration remains unclear. In the study, we investigated the neuroregenerative effect of SMCs-derived ECM and developed a n

Microhook Nerve Connector-Assisted Polyethylene Glycol Fusion for Immediate Restoration of Axon Continuity in a Rodent Model.

PURPOSE: The purpose of the study was to test the efficacy of polyethylene glycol (PEG) fusion in restoring axon continuity when repaired with a microhook nerve connector (MNC) coaptation aid versus conventional microsutures. Our hypothesis was that the MNC would provide precise nerve alignment and stable coaptation and would support a higher percentage of axon fusion. METHODS: In a rodent sciatic nerve model, transected nerves were either repaired with microsutures only (n = 12), microsutures with PEG (n = 12), or a MNC with PEG (n = 12). At initial surgery, PEG fusion was confirmed using el

Conductive Hydrogels with Topographical Geometry and Mechanical Robustness for Enhanced Peripheral Nerve Regeneration.

Nerve guidance conduits (NGCs) emerge as a promising solution for nerve regeneration; however, conventional NGCs fail to fulfill the requirements for peripheral nerve regeneration, which are subjected to periodical yet vigorous stretching, bending, and compression. Here, we developed a fatigue-resistant conductive hydrogel-based NGC by integrating topographical geometry, enhanced electroactivity, and superior fatigue resistance within one unit. The hydrogel, consisting of a PVA matrix with PEDOT:PSS as a conductive filler, features a topographical alignment that promotes axonal growth and achi

Controlled Magnesium Ion Delivery via Mg-Sputtered Nerve Conduit for Enhancing Peripheral Nerve Regeneration.

Autologous nerve grafting remains the gold standard for treating peripheral nerve injuries; however, it is constrained by limited donor nerve availability, the need for secondary surgeries, and sensory loss at the donor site. Biodegradable material-based nerve conduits have emerged as a promising alternative to address these limitations and enhance nerve regeneration. Among these materials, magnesium stands out due to its exceptional biocompatibility, biofunctionality, and neuroprotective properties. Despite its potential, magnesium's rapid corrosion rate and the need for controlled ion releas

Psoas Quartus and Femoral Nerve Branching: A Case Report and Potential Clinical Implications.

BACKGROUND Psoas quartus is a rare anatomical variant of the psoas major muscle that can alter the course and branching of the femoral nerve as it descends from the posterior abdominal wall. Understanding its anatomical implications is crucial for clinicians and surgeons. CASE REPORT During a routine cadaveric dissection, we identified a unilateral psoas quartus muscle on the left side. This muscle was situated between the psoas major, psoas minor, and iliacus, originating from the transverse process of L5 and the quadratus lumborum. The femoral nerve exhibited an unusual branching pattern as

Predegenerating donor nerve for grafting using focused ultrasound neurotomy.

Early experimental evidence suggests that predegenerated nerve graft enhance axonal growth compared to freshly harvested nerve graft in nerve gap repair. However, the practicality of surgically pre-conditioning nerves for grafting remains a major obstacle. Herein, we aim to investigate the feasibility of focal lesioning of nerve tissue using high-intensity focused ultrasound (HIFU) and compare the regenerative effects of predegenerated nerve grafts to freshly harvested grafts in a rat sciatic nerve transection and repair model. As a proof of principle, sciatic nerves were exposed for HIFU lesi

Mechanistic study of modulating mitochondrial fission and fusion to ameliorate neuropathic pain in mice.

The emergence of neuropathic pain is significantly influenced by the impairment of mitochondrial processes. Ensuring the stability of mitochondrial activity requires a delicate equilibrium between the processes of mitochondrial fission and fusion. However, the specific alterations in mitochondrial activity across different models of neuropathic pain and the underlying mechanisms remain largely unclear. We developed a persistent compression injury (CCI) model targeting the sciatic nerve in mice. CCI induced pain like behaviors in mice, which were associated with increased levels of dynamin rela
