BACKGROUND: The adductor canal block effectively targets the saphenous nerve and nerve to vastus medialis, providing pain relief after total knee arthroplasty while preserving quadriceps strength. Despite its widespread use, the ideal needle insertion point remains uncertain. This study aims to determine the optimal segment for adductor canal block using anatomical dissections and morphometric analysis.
METHODS: Cadaveric dissections of 30 limbs measured the morphometric data including the thigh length, adductor canal length, and distances from bony landmarks. The Saphenous nerve and nerve to

Persistent inflammation impedes peripheral nerve regeneration, in which infiltrating macrophages play pivotal roles by regulating the M1/M2 phenotypic balance. M2 macrophages facilitate repair, yet factors driving M1-to-M2 transition remain unclear. Lactate, a key metabolic byproduct in injured tissue, dynamically regulates microenvironmental signaling. Here, we observed a correlation between M2 macrophage accumulation and lactate elevation within 5 days post-sciatic nerve injury. In vitro, both Schwann cells and macrophages were identified as important lactate producers. In vivo, low-dose lac

Electrical stimulation from electroactive bioma0terials is indispensable for peripheral nerve regeneration, but traditional invasive methods carry infection risks. Non-invasive techniques based on ultrasound and piezoelectric materials provide new solutions to this challenge. In this study, polydopamine (PDA) was used to surface-modify barium titanate nanoparticles (BTNPs), which were then combined with chitosan (CS) microcarriers to successfully fabricate composite microcarriers (BTNP@PDA/CS) with piezoelectric-conductive dual electroactivity for sciatic nerve defect repair. BTNP@PDA/CS piezo

Lumbar plexus blockade can be performed using both the classical landmark technique and ultrasound guidance. Reports suggest that ultrasound-guided blockade may enhance the safety of the procedure. Various approaches can be utilized for ultrasound-guided lumbar plexus blockade, including the paravertebral paramedian transverse scan, the Shamrock approach, and Trident imaging. In these imaging methods, needle guidance can be accomplished using either an in-plane or out-of-plane technique. Each of these application methods has limitations concerning needle imaging and guidance. In this case repo

Peripheral nerve blockade is commonly employed as perioperative pain management for posterolateral corner (PLC) knee reconstructions. However, common peroneal nerve (CPN) analgesia limits immediate postoperative assessment (within 0.5-2 hours of surgery) for iatrogenic injury and may further cause a "second hit" injury in cases of preoperative CPN trauma. Meanwhile, lack of lateral knee analgesia may result in intractable pain and the need for postoperative "rescue block." The incidence of iatrogenic peroneal nerve injury (from nerve block and/or surgical exploration) and frequency of "rescue

BACKGROUND: Excessive traction stress on the femoral nerve during total hip arthroplasty (THA) may cause femoral nerve palsy. This study aimed to evaluate the tension changes on the femoral nerve during the surgical processes in the modified Watson-Jones approach using spring devices.
METHODS: We examined 11 lower extremities from 7 fresh cadavers. The femoral nerve was identified in the ilioinguinal approach and partially replaced with a tension spring (free length, 18.7 mm). Then, a modified Watson-Jones approach was employed in the lateral decubitus position. To assess the tension on the f

Denervation results in reduced bone quality, largely attributed to reduced loading due to concurrent muscle atrophy. However, sensory and sympathetic nerves also directly innervate bone, suggesting additional potential inputs into bone remodeling. A quantitative baseline of bone morphological and functional changes after nerve injury is lacking. We investigated structural, biomechanical, and histological outcomes for time points up to three months following peripheral nerve injury. Our primary objective was to establish timelines over which bone and muscle structure and biomechanical function

BACKGROUND AND AIMS: Severe peripheral nerve injury (PNI) remains a major clinical challenge, and functional recovery after conventional neurorrhaphy is often unsatisfactory due to fascicular mismatch, suture tension, and limited Schwann cell viability. To address these limitations, we previously developed a small-gap chitosan-based conduit that provides a controlled microenvironment for regenerative interventions. This study aimed to investigate whether SOX5 overexpression enhances Schwann cell regenerative potential and, when combined with this conduit, synergistically promotes peripheral ne

BACKGROUND: Peripheral nerve injuries frequently lead to incomplete functional recovery, largely due to secondary oxidative stress and neuroinflammation that impede regeneration. Pycnogenol®, a standardized French maritime pine bark extract rich in procyanidins, exhibits potent antioxidant and anti-inflammatory activities, suggesting potential neuroregenerative benefits. This study aimed to comprehensively evaluate the neuroregenerative potential of Pycnogenol® in a rat sciatic nerve crush model using an integrated assessment of histopathological, biochemical, and functional electrophysiologic

This study aimed to elucidate the relationship between the spatiotemporal behavior of macrophages during exercise and axonal regeneration following peripheral nerve injury. A sciatic nerve crush model was created using male C57BL/6J mice, which were divided into sedentary and exercise groups. The exercise group performed low-intensity treadmill running (10 m/min, 60 min/day, 5 days/week) starting 3 days post-injury. Immunohistochemical analyses of Neurofilament 200 kDa (NF200), Growth Associated Protein-43 (GAP-43), F4/80, and arginase-1 (Arg-1), a representative marker of M2 macrophages and a

Neuropathic pain is a condition that results from nerve injury. There is a relationship between neuropathic pain and mood disorders. Citicoline, when used as a dietary supplement, exhibits neuroprotective, antidepressant, and anxiolytic properties. Moreover, bupropion is an atypical antidepressant with unique pharmacologic properties. We sought to investigate the effects of citicoline and bupropion as well as their possible interaction on the control of anxiety‑ and depression‑like behaviors in nerve‑ligated mice. Unilateral sciatic nerve ligation was performed on the right hind limb. Anxiety‑

BACKGROUND: Lumbosacral plexopathy (LSP) is characterized by severe neuropathic pain, motor weakness, and sensory deficits in the lumbosacral plexus region, often leading to significant functional impairment and reduced quality of life. Post-traumatic LSP is particularly challenging to treat due to its neuropathic nature and limited response to conventional pharmacologic therapies. Repetitive transcranial magnetic stimulation (rTMS) is a non-invasive neuromodulation technique that has shown therapeutic potential for chronic neuropathic pain.

BACKGROUND: Spared nerve injury (SNI) is a widely used rodent model in the study of peripheral neuropathic pain. The injury, that involves ligation and resection of the common peroneal and tibial branches of the sciatic nerve, results in a robust and prolonged nociceptive response. The model is characterised by measurable mechanical and thermal hypersensitivity in the innervation area of the spared sural nerve of the lateral hind paw, that persists for several months. Since its description, the model has been used to further the understanding of a diverse range of mechanisms contributing to th

Peripheral nerve injury (PNI) remains a major clinical challenge due to the limitations of autologous nerve grafting. Herein, a multifunctional composite nerve conduit was developed by integrating an oriented collagen-grafted polylactic acid (PLA) electrospun scaffold with a conductive and anti-inflammatory hydrogel to enhance peripheral nerve regeneration. The hydrogel was fabricated by chemically cross-linking carboxymethyl chitosan (CMCS) and sodium carboxymethyl cellulose (CMC-Na), incorporating polyaniline (PANI) to impart electrical conductivity and loading rhein (RH) for sustained anti-

PURPOSE: To explore the mechanism by which alpha-lipoic acid (ALA) regulates mitochondrial axonal transport to protect the sciatic nerve in diabetic rats.
METHODS: Among 55 healthy male Sprague‒Dawley rats, 40 were randomly selected, fed a high-carbohydrate/high-fat diet and intraperitoneally injected with streptozotocin (STZ) to induce diabetes. Diabetic rats were randomly divided into diabetic peripheral neuropathy (DPN) and alpha lipoic acid (ALA) groups, with 15 rats in each group, excluding the rats in which diabetes failed to be induced and the dead rats. The other 15 rats were used as

Supercharge end-to-side (SETS) nerve transfer enhances motor recovery in proximal nerve injuries by providing early reinnervation. However, the optimal indications and mechanisms remain unclear. This study examined the role of donor nerves using rat models of varying injury severity to clarify the clinical indications for SETS. Eighty female Sprague-Dawley rats were assigned to five groups: Control, Mild-SETS(-), Mild-SETS(+), Severe-SETS(-), and Severe-SETS(+). The tibial nerve was transected, decellularized, and reconstructed with a 10 mm (mild) or 20 mm (severe) graft. SETS consisted of end

During the development of the peripheral nervous system, Schwann cells rely on intricate interactions among extracellular matrix proteins, integrins, and growth factor signaling pathways to ensure survival, axonal sorting, and the myelination of axons. Central to these interactions is the IPP complex, a heterotrimeric assembly of Pinch ('particularly interesting new cysteine-histidine-rich') proteins, integrin-linked kinase (Ilk) and parvins, which serves as a crucial signaling hub linking integrins to diverse cellular pathways. Through Schwann cell-specific conditional gene ablation in mice,

This study explores the therapeutic potential and mechanisms of Hesperidin, a natural flavonoid, in promoting peripheral nerve injury (PNI) repair. We focused on its regulation of autophagy and apoptosis, and examined whether these effects are mediated through the STAT3/Bcl-2 signaling pathway. In vitro, a TBHP-induced RSC96 Schwann cell injury model was used to assess cell viability, cytotoxicity, and mitochondrial membrane potential, alongside autophagy and apoptosis analysis by electron microscopy, immunofluorescence, and Western blot. STAT3 and autophagy inhibitors were used to clarify pat

Peripheral nerve injuries pose significant clinical challenges, leading to sensory and motor impairments, neuralgia, and socioeconomic burdens due to suboptimal recovery. This study evaluated the efficacy of electrospun polylactic acid (PLA) conduits combined with leupeptin and methylprednisolone for nerve regeneration in a rat sciatic nerve defect model. Fifty adult male Sprague-Dawley rats were randomized into five groups (n = 10/group): control, saline, leupeptin (0.5 mg/mL), glucocorticoid (30 mg/mL methylprednisolone), and combined (leupeptin + methylprednisolone).
