Berberine attenuates methotrexate-induced renal and sciatic nerve toxicity via modulation of the cGAS-STING/NF-κB inflammatory pathway.

AIMS: Methotrexate (MTX) is an extensively used chemotherapeutic and anti-inflammatory agent, but its therapeutic application is limited by toxicity affecting multiple organs, primarily the kidneys and sciatic nerve. Berberine (BBR) is known for its potent antioxidant, anti-inflammatory, and cytoprotective properties. This study aims to explore the protective effects of BBR on MTX-induced nephrotoxicity and neurotoxicity. MATERIALS AND METHODS: Twenty-four male Wistar rats (180-200 g) were randomly assigned to four groups (n = 6): Control, BBR (50 mg/kg/day), MTX (single intraperitoneal dose

Iatrogenic risk and anatomical variability of the posterior tibial nerve bifurcation relative to the Dellon-McKinnon line: Assessing safety in medial ankle procedures.

BACKGROUND: Precise knowledge of the posterior tibial nerve (PTN) branching pattern is critical to avoid iatrogenic injury during medial foot and ankle procedures. This study aimed to perform a morphometric assessment of the PTN terminal branching in relation to the Dellon-McKinnon Line (DML) to identify safe surgical zones. METHODS: Forty-three formalin-fixed adult cadaveric limbs were dissected. The bifurcation points of the PTN, its level and distance were measured relative to the DML. Bifurcation angle, and foot length were also measured. Statistical analysis of the measured parameters wa

A Thermal Injury Model by Monopolar Radiofrequency on Sciatic Nerve in Mice.

Peripheral nerve injury often results in neuropathic pain and triggers nerve repair processes. Whilst controllable physical injury models are now well-established, existing thermal injury models remain exploratory and exhibit poor controllability. In clinical practice, radiofrequency ablation (RFA) systems are frequently employed to induce thermal injury to tissues and nerves. RFA is a therapeutic modality that has been demonstrated to effectively block pain transmission by inducing thermal damage to nerves. The extent of the ablation can be modified by adjusting specific parameters, as well a

Hypoxia-induced Schwann cells-derived extracellular vesicles carrying LncRNA TNXA-PS1 promote recovery post sciatic nerve injury.

Peripheral nerve injury is a salient clinical problem but lacks successful treatment schemes. Here we show the protective mechanism of hypoxia-induced Schwann cells-derived extracellular vesicles (H-EVs) carrying lncRNA TNXA-PS1 in peripheral nerve injury. EVs isolated from RSC96 cells undergo hypoxia (H) induction. Sciatic nerve injury is induced in rats, and the animals are evaluated by Sciatic Nerve Function Index, gastrocnemius muscle mass ratio, hematoxylin & eosin stain, and sensory recovery tests. LncRNA TNXA-PS1, miR-338-3p, and EGFL7 expression is tested by RT-qPCR and Western blot. P

Effects of FGF4/HIF-1α axis-mediated neuronal glycolysis on neuropathic pain.

The pathogenesis of neuropathic pain is complex, and effective treatment methods are lacking in clinical practice. Recent studies have shown that glucose metabolism reprogramming may be involved in the process of neuropathic pain, but its role and molecular regulatory mechanisms in neuropathic pain are still unclear. In this study, a rat model of chronic constriction injury of the sciatic nerve (CCI) was established, and the pain threshold was evaluated through behavioural analysis. Morphological staining, transmission electron microscopy, transcriptome sequencing, Western blotting, immunofluo

7-Hydroxyflavone mitigates partial sciatic nerve ligation-induced neuropathic pain via modulation of oxidative and inflammatory pathways.

Chronic neuropathic pain is a common and widely recognized pain syndrome that remains difficult to manage clinically. Although several therapeutic options exist, many are limited by inadequate efficacy and adverse effects which underscore the need for safer and more effective alternatives. 7-Hydroxyflavone (7-HF), a natural occurring flavonoid, has demonstrated diverse biological properties, and limited evidence suggests its antinociceptive potential in chemotherapy-induced neuropathic pain. However, its therapeutic effects and mechanistic involvement in partial sciatic nerve ligation (PSNL)-i

Hyperoside accelerates myelin debris clearance by inhibiting signal transducer and activator of transcription 3 phosphorylation in peripheral nerve injury.

BACKGROUND: Neuroinflammation resulting from myelin debris impedes axonal regeneration. Pharmacological modulation of myelin clearance and inflammatory responses is a potential strategy to enhance nerve regeneration. Hyperoside, a flavonoid with established anti-inflammatory and neuroprotective properties, has not been explored in peripheral nerve injury or in the context of myelin clearance. PURPOSE: This study aims to investigate the effects and underlying mechanisms of hyperoside in promoting nerve regeneration. METHODS: An in vitro phagocytosis model was established in RAW264.7 macrophag

Cryo-shocked neural stem cells as factor pool-functionalized nerve guidance conduits for enhanced peripheral nerve regeneration.

Peripheral nerve injuries (PNI) often induce severe motor and sensory impairments, subsequent muscle atrophy, and even permanent disability. Autologous nerve grafting, the clinical gold standard for PNI repair, is limited by donor site morbidity and graft shortage, while conventional nerve guidance conduits (NGCs) only provide physical guidance and fail to reconstruct the bioactive microenvironment essential for nerve regeneration. To address these limitations, we fabricated chitosan NGCs functionalized with cryo-shocked neural stem cells (Cryo-NSC NGCs) as a tissue-specific factor pool via se

Peripheral Nerve Reconstruction in Lower Extremities: Institutional Experience of Chang Gung Memorial Hospital.

Lower-extremity nerve reconstruction, most commonly involving the lumbosacral plexus (LSP), sciatic nerve (including common peroneal and posterior tibial nerves), and the femoral nerve, remains one of the most demanding challenges in peripheral nerve surgery, with outcomes primarily determined by injury mechanism, anatomical level, and timing of reconstruction. Over the past 35 years (1987-2022), the Peripheral Nerve Team at the Department of Plastic and Reconstructive Surgery, Chang Gung Memorial Hospital, Linkou Medical Center, Taiwan treated 3 patients with LSP injuries, 6 patients with fem

Calcium-alginate microspheres for sustained curcumin delivery attenuate neuroinflammation and enhance nerve regeneration via suppressing NF-κB/MAPK pathways.

Peripheral nerve regeneration is hindered by persistent neuroinflammation. Curcumin, a promising anti-inflammatory agent, is limited by poor bioavailability. To enable localized and sustained delivery, we developed curcumin-loaded calcium-alginate microspheres (Cur@SA-MS), with an encapsulation efficiency of 75.93%, a loading capacity of 7.78%, and a stable two-week release profile, with cumulative release reaching approximately 80% by day 14. In vitro, Cur@SA-MS protected Schwann cells from LPS-induced inflammation by improving cell viability from ∼28% to ∼79% and reducing oxidative injury an

Kif15 orchestrates neuronal-microglial communication via CX3CL1 to impede nerve regeneration.

Kinesins, a class of microtubule (MT)-dependent molecular motors, regulate MT dynamics and MT-mediated transport. We previously identified Kif15 (kinesin-12) as a key player in axonal growth by modulating MT remodeling during neuronal development, and more recently, its involvement in protein localization. In this study, we observed that Kif15 knockout (Kif15 KO) mice exhibited accelerated functional recovery after sciatic nerve injury. To investigate the cellular responses underlying this enhanced recovery after axotomy, spinal cord tissues from the injured regions were collected for single-n

Multifunctional Electrospun Nerve Conduits Enriched With Stem Cells and Bioactive Molecules for Sciatic Nerve Regeneration.

Peripheral nerve injuries are a major clinical challenge, interrupting neural connectivity between the central nervous system (CNS) and peripheral organs. Traumatic events, natural disasters, and accidents often cause these complex injuries. Recent studies highlight the neurogenic potential of dexamethasone, retinoic acid, and umbilical cord blood stem cells in promoting nerve regeneration. Therefore, the aim of this study was to design electrospun conduits based on PLCL, gelatin, and collagen, along with dexamethasone, retinoic acid, carboxymethyl chitosan, and umbilical cord blood cells to r

Stiffness-engineered hydrogels drive neural regeneration via the Man2b1- lysosomal mechanotransduction axis.

Peripheral nerve repair requires biomaterials capable of dynamically guiding neural cells. While stiffness-tunable hydrogels hold promise, how their mechanical properties translated into pro-regenerative intracellular signals remains poorly understood. To address gap, we developed a library of polyacrylamide/chitosan hydrogels with tunable stiffness (1.5∼39 kPa) to mimic the native neural microenvironment. Using this platform, we identified an optimal stiffness (∼13.75 kPa) that maximizes PC12 cell adhesion, spreading, neurite outgrowth, and migration. When fabricated into nerve guidance condu

Low-intensity pulsed ultrasound promotes peripheral nerve regeneration by alleviating Schwann cells pyroptosis.

Peripheral nerve injury (PNI), a common clinical condition, heavily relies on the functional condition of Schwann cells (SCs) for effective repair. Emerging evidence shows that excessive pyroptosis of SCs hinders neural regeneration. Low-intensity pulsed ultrasound (LIPUS), a non-invasive physical therapy, shows potential in tissue repair; however, its role in regulating SCs pyroptosis remains unclear. This study explores the mechanism by which LIPUS improves PNI recovery by reducing SCs pyroptosis. We find that LIPUS significantly enhances motor function recovery, promotes axonal regeneration

Therapeutic potential of M2 macrophage-derived exosomes and hyperbaric oxygen in sciatic nerve crush injury: Insights from histological, molecular, and biochemical analyses.

Peripheral nerve injuries often lead to long-term functional deficits due to limited intrinsic regeneration and secondary inflammatory and oxidative damage. This study evaluated the therapeutic efficacy of M2 macrophage-derived exosomes (M2-Exosomes), hyperbaric oxygen therapy (HBO), and their combined administration in a rat model of sciatic nerve crush injury. Animals were randomly divided into five groups: sham, control (injury without treatment), exosome, HBO, and Exosome+HBO. HBO was administered in a pressurized chamber, while exosomes were injected immediately post-injury and at schedul

Modulation of S100β and Inflammatory Signalling by Isorhamnetin Enhances Peripheral Nerve Regeneration.

Peripheral nerve injury is a leading cause of disability, which can result in partial or complete loss of motor, sensory, and autonomic function, and currently, there is no effective treatment for this incapacitating condition. It is important to identify new compounds that enable rapid and complete functional recovery. This study evaluated the effects of isorhamnetin (ISO) on functional rehabilitation in a mouse model of sciatic nerve injury. A total of 30 BALB/c mice, aged 8-10 weeks, were randomly assigned to three groups: sham, control, and treatment (n = 10/group). The mice in the ISO and
