Self-Assembling Biohybrid Bands of Büngner for Axonal Regeneration in the Peripheral Nervous System.

There is broad consensus that successful repair of severe peripheral nerve injuries requires recreating key structural and cellular features of the natural regenerative process, particularly the action of Bands of Büngner (BoB), longitudinal Schwann cell (SC) structures that guide regenerating axons. Current biomaterial-based strategies have shown limited efficacy, in part because they do not sufficiently reproduce the anisotropic and cellular microenvironment established by BoB, resulting in disorganized axonal growth and reduced regenerative efficiency across long gaps. To address this limit

Pharmacological mechanisms of Yuxuebi tablet in alleviating neuropathic pain: Insights from network pharmacology, molecular docking, 16S rRNA sequencing, and metabolomics.

BACKGROUND: Yuxuebi Tablet (YXBT) is a traditional Chinese patent medicine and is mainly used to treat joint swelling and pain associated with rheumatic arthritis and osteoarthritis. However, to date, no definitive clinical studies have confirmed the efficacy of YXBT for treating neuropathic pain (NP). PURPOSE: To assess the analgesic effects of YXBT on chronic constriction injury (CCI)-stimulated NP in rats and to elucidate the underlying molecular mechanism. METHODS: Network pharmacology and molecular docking were employed to predict the key targets of YXBT in the treatment of NP. A CCI-st

Web-based Cellpose‑SAM segmentation enables quantification of cross-sectional area in formalin-fixed paraffin-embedded skeletal muscle sections stained with hematoxylin and eosin.

Cross‑sectional area (CSA) of skeletal myofibers is a primary morphometric indicator of atrophy and regeneration, yet it is most commonly quantified on cryosections after immunofluorescent labeling of basal lamina proteins, which requires specialized reagents and equipment and limits reuse of routine histology slides. Here we present a practical, fully web‑based workflow that leverages Cellpose‑SAM on a public browser interface to segment myofibers directly from formalin-fixed paraffin-embedded (FFPE) skeletal muscle sections stained with hematoxylin and eosin (H&E) or toluidine blue (TB), fol

Angelica dahurica alleviates peripheral nerve injury-induced muscle atrophy via the Phlpp2-Akt-FoxO3α axis independently of analgesia.

BACKGROUND: Peripheral nerve injury frequently causes refractory neuropathic pain and muscle atrophy. While clinical care focuses on analgesia, effective therapies for muscle wasting are lacking. Angelica dahurica (BZ) is known for its analgesic properties, but whether it also ameliorates muscle atrophy remains unclear. PURPOSE: This study evaluates the dual analgesic and anti‑atrophic effects of BZ, with a focus on determining whether its muscle‑protective action is separable from pain relief and elucidating the underlying mechanism. METHODS: Following sciatic nerve chronic constriction inj

Acupoint electrical stimulation with implantable carbon nanotube electrodes mitigates sciatic nerve injury-induced muscle atrophy.

Sciatic nerve injury (SNI) is a common form of peripheral nerve injury that often leads to persistent atrophy of denervated skeletal muscle. To address this issue, we developed an implantable electrode made of carbon nanotube fibers (CNTFs) for targeted acupoint electrical stimulation (AES) at the Huantiao acupoint (GB30) in SNI rats. CNTFs offer high tensile strength, excellent electrical conductivity, and proven biocompatibility. Following 21 consecutive days of AES intervention, we evaluated therapeutic effects through behavioral tests, morphological analysis, and biochemical assays. Compar

The miR-206-3p/Cpeb1 axis delays acetylcholine receptor degradation and preserves neuromuscular junction stability in denervation-induced muscle atrophy.

Peripheral nerve injury leads to progressive neuromuscular junction (NMJ) destabilization and acetylcholine receptor (AChR) degradation, which are critical drivers of denervation-induced muscle atrophy and impaired motor recovery. However, the post-transcriptional mechanisms regulating AChR stability during denervation remain poorly understood. Here, we investigated the role of miR-206-3p in NMJ maintenance and muscle preservation after denervation, with a focus on its interaction with the RNA-binding protein cytoplasmic polyadenylation element binding protein 1 (Cpeb1). Using C2C12 myoblasts

Inhibition of leucine-rich kinase 2 (LRRK2) promotes peripheral axon regeneration via phosphorylation-network remodelling.

BACKGROUND AND PURPOSE: In contrast to neurons in the central nervous system, neurons in the peripheral nervous system can regenerate axons after injury via activation of a pro-regenerative transcriptional programme. Pathogenic mutations in leucine-rich repeat kinase 2 (LRRK2) are the most common genetic cause of Parkinson's disease, and several small-molecule LRRK2 kinase inhibitors have been developed, with some in clinical trials. However, the physiological role of endogenous, non-pathogenic LRRK2 remains largely unknown. EXPERIMENTAL APPROACH: LRRK2 expression was examined in murine dorsa

Understanding femoral nerve injuries during extreme lateral lumbar interbody fusion using transabdominal muscle action potential and an event-based protocol.

OBJECTIVE: Extreme lateral lumbar interbody fusion (XLIF) carries a significant risk of femoral nerve injury, particularly at L4-5, where neural structures are close to the transpsoas corridor. While neuromonitoring techniques such as triggered electromyography and somatosensory evoked potentials are widely used, they lack the real-time sensitivity and specificity necessary for reliably detecting nerve compromise. The absence of real-time neuromonitoring data in XLIF has limited the understanding of when and how femoral nerve injuries occur, leaving surgeons without a clear strategy to mitigat

Foot Drop: An Anatomical, Clinical, and Electrodiagnostic Approach to Localization.

Foot drop, characterized by weakness of ankle and toe dorsiflexion, is a clinical sign rather than a diagnosis and requires accurate neuroanatomic localization. Potential sites of injury include the L5 nerve root, lumbosacral plexus, sciatic nerve, and common fibular (peroneal) nerve. Because lesions at different levels can produce overlapping motor and sensory findings, incorrect localization may lead to inappropriate diagnostic testing or treatment. This narrative review presents an anatomical, clinical, and electrodiagnostic framework for localizing neurogenic foot drop, with particular emp

L5/S1 anterior lumbar interbody fusion: anatomical and technical considerations.

Anterior lumbar interbody fusion (ALIF) at the L5/S1 level is a well-established surgical technique used in the management of degenerative, deformity, and instability-related pathologies of the lumbosacral junction. This region represents a biomechanically significant transition between the mobile lumbar spine and the pelvis, contributing substantially to overall lumbar lordosis while being subjected to considerable shear forces. Successful execution of L5/S1 ALIF requires a comprehensive understanding of both spinal and retroperitoneal vascular anatomy, including the relationship of the iliac

Comparative efficacy of nerve coagulation vs. cap implantation in neuroma prevention using the rat sciatic nerve model: A histological and neuroradiological insight.

INTRODUCTION: Peripheral nerve injuries pose a considerable challenge in reconstructive surgery due to the risk of neuroma formation, often resulting in severe pain and significant functional impairment. Various treatment modalities, including nerve coagulation and the use of nerve caps, have been explored to manage neuroma development. In this study, we compare the efficacy of chitosan and polylactide nerve caps to standard nerve coagulation in modulating neuroma morphology and promoting organized nerve regeneration. METHODS: Female Sprague Dawley rats (n = 40) were divided into four equal g

[Tibial nerve somatosensory evoked potentials localize the responsible level in tandem spinal stenosis: a case report].

An 84-year-old man had experienced unsteadiness while walking and coldness in both lower limbs for about 18 months, with gradual progression. Neurological examination revealed decreased tendon reflexes in the lower extremities, a positive Romberg sign, and a wide-based gait. MRI demonstrated spinal canal stenosis in the cervical and lumbar regions. Because concomitant peripheral neuropathy was suspected, electrophysiological studies were performed. Nerve conduction studies showed no evidence of peripheral neuropathy. Tibial nerve somatosensory evoked potentials (SEP) revealed prolonged N21-P38

Validation of Paw Skin Hyperspectral Imaging for Assessing Neuropathic Pain Severity in a Chronic Constriction Injury Model.

Neuropathic pain is a debilitating condition lacking objective and quantitative assessment tools, as current evaluations rely largely on subjective reports. Hyperspectral imaging (HSI) is a non-invasive technology that quantifies spatial and spectral tissue characteristics and has been applied in rheumatologic and metabolic disorders. This study investigated whether HSI-detected paw skin alterations correlate with graded nerve injury severity in a chronic constriction injury (CCI) model. Sprague-Dawley rats were assigned to sham or CCI groups with one to four sciatic nerve ligatures. Behaviora

Peripheral nerve transection predominantly drives sympathetic nerve sprouting in mouse dorsal root ganglia.

Sympathetic sprouting in dorsal root ganglia (DRG) is a feature of sympathetically maintained pain (SMP) after peripheral nerve injury, yet the factors determining its occurrence remain unclear. Here, we compare transection and crush injury models to determine whether injury type or site influence sympathetic remodeling and pain. Using tyrosine hydroxylase-immunoreactivity staining and Phox2b reporter mice to selectively label sympathetic fibers, we found that an L5 spinal nerve transection triggered robust sympathetic fiber sprouting and elevated norepinephrine (NE) levels in the DRG, correla

Manipulation of Wnt Signaling Pathway during Peripheral Nerve Regeneration.

PNS injury initiates transcriptional changes in Schwann cells, satellite glial cells and PNS neurons that facilitate regeneration. The signaling pathways that control these transcriptional changes are not fully understood. The canonical/β-catenin Wnt signaling pathway is active during early stages of PNS development, where it controls radial axonal sorting and the onset of PNS myelination. Upon PNS injury, the canonical Wnt signaling pathway is re-activated, suggesting that canonical Wnt signaling plays an important role in PNS regeneration. To explore the potential of the canonical Wnt signal

Mesenchymal stromal cells alleviate neuropathic pain in association with M2 macrophage polarization in dorsal root ganglia and peripheral nerve repair.

BACKGROUND: Neuropathic pain (NP) is a debilitating condition with limited effective treatments. Mesenchymal stromal cells (MSCs) have emerged as a potential therapeutic strategy due to their immunomodulatory properties. This study investigated whether intravenous administration of human umbilical cord-derived MSCs could alleviate NP in a rat chronic constriction injury (CCI) model and explored the associated changes in macrophage polarization and nerve structure. METHODS: Male Sprague Dawley rats were subjected to CCI of the sciatic nerve and randomly divided into Control, CCI, and CCI+MSCs

Hierarchical microtopology and phase-specific delivery functionally restore ultralong nerve continuity across species.

Repairing ultralong peripheral nerve defects remains a major clinical challenge, primarily due to the requirement for regenerative platforms to be capable of integrating spatial guidance with temporally resolved biochemical cues. To address this, we developed a clinically translatable, fully synthetic nerve conduit that aligns with native regenerative principles through hierarchical microtopological engineering and phase-specific molecular delivery. This design creates a proregenerative microenvironment by synchronizing structural cues with repair cascades. The neuroanatomically inspired core

A novel oxygen-releasing bilayer delivery system for cell survival and peripheral nerve regeneration.

Peripheral nerve injuries with long-gap defects pose a major regenerative challenge due to hypoxic microenvironments that impair cell survival and nerve repair. Here, we engineered a bilayer microsphere delivery system that encapsulates Perfluorotributylamine (PFTBA) within a PLGA core and an outer chitosan coating to achieve sustained oxygen release. In vitro, P-PLGA/CS markedly enhanced olfactory ensheathing cell proliferation and survival, reduced apoptosis, and boosted secretion of neurotrophic and angiogenic factors under hypoxic conditions. In vivo, a polycaprolactone conduit loaded with

Evaluation of neurotrophic therapies for sciatic nerve injury in mice.

Peripheral nerve injuries remain a major therapeutic challenge, frequently resulting in incomplete functional recovery and long-term disability. Although microsurgical repair techniques continue to advance, there is a clear unmet need for adjunctive pharmacological strategies capable of enhancing regenerative outcomes. In this preclinical study, we investigated the functional recovery potential of Calf Blood Hemodialysate, a deproteinized ultrafiltrate enriched with low-molecular-weight peptides and intermediary metabolites, and alpha-lipoic acid (ALA), a potent mitochondrial antioxidant and m
