PURPOSE: Complex metacarpophalangeal joint (MCPJ) dislocations typically require open reduction, but there is a paucity of literature exploring arthroscopic-assisted reduction. We present our experience in six cases of arthroscopic reduction of complex MCPJ dislocation, evaluating the success rate and clinical outcomes and delineating key technical pearls and intraoperative challenges.
METHODS: This retrospective series included patients with complex dorsal MCPJ dislocation without fractures or open injuries who were treated at a single institution from 2004 to 2025. After closed reduction fa

Splenic infarction typically appears on imaging as a peripheral wedge-shaped region of non-enhancement within the splenic parenchyma. Near-total infarction with liquefied transformation is uncommon and may mimic other cystic splenic lesions. We report a 55-year-old woman who presented with fever, vomiting, and left-sided chest pain radiating to the back. The patient had initially been evaluated at an outside facility and subsequently referred to our institution with a diagnosis of splenic vein thrombosis. She was admitted from 22 February 2026 to 24 February 2026 and managed conservatively wit

Emphysematous gastritis (EG) is a rare, potentially life-threatening condition. Extensive gastric pneumatosis and portal venous gas may suggest irreversible ischemia, although radiological severity and mucosal injury do not necessarily indicate transmural necrosis. A 70-year-old man presented with acute epigastric pain and vomiting. Computed tomography (CT) demonstrated extensive gastric and duodenal pneumatosis with portomesenteric venous gas but patent visceral arteries. Immediately after CT, he developed cardiac arrest, with return of spontaneous circulation after approximately 14 min. Emer

Pancreaticoduodenectomy is a technically demanding procedure associated with substantial surgical risk. Conventional laparoscopic approaches have several limitations, including restricted visualization, which can make exposure of the superior mesenteric artery and identification of vessels and lymph nodes challenging. In patients with vascular adhesion or tumor invasion, surgical dissection is particularly difficult and may increase the risk of intraoperative vascular injury and hemorrhage. In addition, poor delineation of anatomical planes may result in incomplete tumor dissection, inadequate

Background: Renal artery aneurysms (RAAs) are uncommon visceral arterial lesions whose management depends on symptoms, patient-specific rupture risk, and arterial anatomy. Markedly high origin renovisceral arteries may further complicate endovascular access and reconstruction. This report describes the management of a symptomatic RAA in a young woman with multifocal aneurysmal disease and unusual renovisceral arterial origins at the 11th and 12th thoracic vertebral levels (T11-T12). Case presentation: A 35-year-old woman presented with intermittent right loin pain. Computed tomography angiogra

Gastric ischemia is an uncommon entity, as the stomach has an extensive collateral arterial blood supply; clinically significant injury usually requires multivessel mesenteric compromise. When it occurs, it closely mimics peptic ulcer disease and gastric malignancy, and diagnosis is often delayed. A 77-year-old female patient with poorly controlled type 2 diabetes mellitus and dyslipidemia (non-compliant with metformin and atorvastatin) presented with a one-month history of severe postprandial epigastric pain, progressive food avoidance, and 10 kg of unintentional weight loss. She was admitted

Primary sclerosing cholangitis (PSC) is a chronic cholestatic liver disease characterized by progressive fibroinflammatory injury of intrahepatic and extrahepatic bile ducts that may lead to cirrhosis, hepatobiliary malignancy, and death, with liver transplantation representing the only effective intervention in end-stage disease. Rather than a single dominant pathomechanism, the data support a model in which four axes - bile acid metabolism, the intestinal microbiome and gut barrier, biliary epithelial dysfunction, and Th17-skewed immune dysregulation - are reciprocally coupled and converge o

BACKGROUND: Acute pancreatitis (AP) is a common acute abdominal inflammatory disorder characterized by premature activation of pancreatic enzymes, leading to pancreatic autodigestion, edema, hemorrhage, and even necrosis. The liver, as the first portal organ receiving pancreatic venous drainage, is one of the most frequently affected extra-pancreatic organs in AP. AP-associated liver injury (LI) is closely correlated with the intensity of systemic inflammation, the development of multiple organ dysfunction, and increased mortality. While machine learning (ML) models have been successfully appl

INTRODUCTION: Symptomatic chronic mesenteric artery occlusive disease requires timely revascularisation. Although endovascular management is guideline endorsed as first line treatment, inferior mesenteric artery (IMA) revascularisation lacks evidence based guidance. This report presents a case in which an open repair first approach was selected with the IMA as the primary revascularisation target, aiming to provide long term benefit while enabling concomitant superior mesenteric artery (SMA) revascularisation.
REPORT: A 60 year old woman, who was a smoker (40 pack/year), had a past medical hi

Celiac disease (CeD) is an acquired immune enteropathy for which the presence of two factors, i.e., predisposing HLA genes (HLA DQ2/DQ8) and gluten in diet are essential for induction of the pathogenic immune response. Acute symptoms/gut injury specifically depend on the amount of gluten ingested. The incidence and prevalence of medically diagnosed CeD is expected to rise in the coming decades, and most of these people are expected to be from Asia. Through publicly available datasets, we mapped the estimated proportion of population harboring HLA DQ2/DQ8 in different regions of Asia and the pe

We report the case of a 36-year-old man with neurosarcoidosis who developed multiple pseudoaneurysms of the superior mesenteric artery (SMA), infrarenal aorta, and bilateral iliac arteries. Fluorodeoxyglucose-positron emission tomography demonstrated no significant uptake within the aneurysmal segments. Given the complex vascular anatomy, a staged hybrid approach was undertaken. Initial treatment consisted of a hepatic artery-to-distal SMA bypass followed by plug embolization of the SMA aneurysms. Subsequently, aortoiliac reconstruction was performed with a left aortouni-iliac endograft, femor

The inferior phrenic arteries show considerable variation in their origins. We describe a rare origin of the right inferior phrenic artery (RIPA) from the anterior inferior pancreaticoduodenal artery (AIPDA) in a patient with ruptured hepatocellular carcinoma (HCC), identified by contrast-enhanced computed tomography (CT) and angiography, and discuss its anatomical and clinical significance. A woman in her 80s underwent emergency transcatheter arterial embolization for ruptured HCC. Preprocedural contrast-enhanced CT did not identify the origin of the RIPA at its usual sites but suggested that

Elbow synovectomy has traditionally been performed using direct visual observation. However, recent advancements in elbow arthroscopy have increased the popularity of arthroscopic synovectomies. This minimally invasive procedure has garnered attention because of its efficacy in halting joint deterioration while minimizing patient discomfort and invasiveness. Conversely, the technical demands of arthroscopic elbow synovectomy pose challenges, often resulting in notable complications, such as nerve damage. Considering the utility of real-time nerve visualization for injury prevention, we suggest

INTRODUCTION: Small bowel volvulus is a surgical emergency that may cause mesenteric ischemia; however, diagnosis may be difficult when bowel perfusion appears preserved.
CASE PRESENTATION: An 84-year-old man presented with recurrent postprandial abdominal pain and melena 6 months after a total pharyngolaryngectomy with a jejunal free flap reconstruction. Extensive endoscopic evaluations, including capsule endoscopy, revealed no bleeding source. Contrast-enhanced CT revealed the whirl sign at the root of the small-bowel mesentery with the narrowing of the superior mesenteric artery (SMA), whi

BACKGROUND: Splenic vein aneurysms (SVAs) are a rare vascular anomaly of the portal system that are often asymptomatic and found incidentally on imaging. Intervention is reserved for aneurysms that are enlarging, thrombosed, symptomatic, or ruptured to avoid catastrophic complications. This can range from endovascular to open, complex procedures. Here, we present the case of a massive, symptomatic, partially thrombosed SVA that required a hybrid approach involving both minimally invasive and open surgery.

Inferior vena cava (IVC) injury during thoracic endovascular aortic repair is exceedingly rare. We present the case of an 80-year-old woman who developed a fatal IVC perforation at the iliac confluence during large-bore (24F) sheath insertion for thoracic endovascular aortic repair. Despite successful thoracic endograft deployment, the patient developed acute hypotension and hemorrhagic shock. Emergent exploratory laparotomy revealed hemoperitoneum from an IVC tear, likely caused by shearing forces transmitted through heavily calcified iliac arteries. This case highlights the importance of pre

Concurrent chemoradiotherapy (CCRT) combined with pembrolizumab has become the standard treatment for high-risk locally advanced cervical cancer, increasing the need for prompt recognition of immune-related adverse events (irAEs). We report a rare case of severe gastrointestinal irAE complicated by hepatic portal venous gas (HPVG) during pembrolizumab-based CCRT. A 72-year-old woman with FIGO 2018 stage IIIC1r cervical squamous cell carcinoma developed persistent diarrhea and anorexia during treatment, followed by sudden hematemesis after the first brachytherapy session. Upper gastrointestinal

INTRODUCTION: The diagnosis of acute mesenteric ischemia (AMI) remains challenging due to nonspecific symptoms and signs that lead to a high mortality rate. Common causes include cardiac emboli and thrombosis of a mesenteric plaque.
OBJECTIVE: To present a clinical case of acute mesenteric ischemia that underwent successful endovascular revascularization.
CASE REPORT: 82-year-old female patient with a history of atrioventricular block, hypertension, and dementia. She was admitted to the emergency department for acute abdominal pain with onset 3 days before. Diagnosis of ischemic colitis seco
